Abstract

Summary: Background: Mucositis is a common and highly impactful side effect of conventional and emerging cancer therapy and thus the subject of intense investigation. Although common practice, mucositis assessment is heterogeneously adopted and poorly guided, impacting evidence synthesis and translation. The Multinational Association of Supportive Care in Cancer (MASCC) Mucositis Study Group (MSG) therefore aimed to establish expert recommendations for how existing mucositis assessment tools should be used, in clinical care and trials contexts, to improve the consistency of mucositis assessment. Methods: This study was conducted over two stages (January 2022–July 2023). The first phase involved a survey to MASCC-MSG members (January 2022–May 2022), capturing current practices, challenges and preferences. These then informed the second phase, in which a set of initial recommendations were prepared and refined using the Delphi method (February 2023–May 2023). Consensus was defined as agreement on a parameter by >80% of respondents. Findings: Seventy-two MASCC-MSG members completed the first phase of the study (37 females, 34 males, mainly oral care specialists). High variability was noted in the use of mucositis assessment tools, with a high reliance on clinician assessment compared to patient reported outcome measures (PROMs, 47% vs 3%, 37% used a combination). The World Health Organization (WHO) and Common Terminology Criteria for Adverse Events (CTCAE) scales were most commonly used to assess mucositis across multiple settings. Initial recommendations were reviewed by experienced MSG members and following two rounds of Delphi survey consensus was achieved in 91 of 100 recommendations. For example, in patients receiving chemotherapy, the recommended tool for clinician assessment in clinical practice is WHO for oral mucositis (89.5% consensus), and WHO or CTCAE for gastrointestinal mucositis (85.7% consensus). The recommended PROM in clinical trials is OMD/WQ for oral mucositis (93.3% consensus), and PRO-CTCAE for gastrointestinal mucositis (83.3% consensus). Interpretation: These new recommendations provide much needed guidance on mucositis assessment and may be applied in both clinical practice and research to streamline comparison and synthesis of global data sets, thus accelerating translation of new knowledge into clinical practice. Abstract

Abstract Background In November 2021, assisted dying (AD) became lawful in Aotearoa New Zealand. A terminally ill person may now request, and receive, pharmacological assistance (self-administered or provided by a medical practitioner/nurse practitioner) to end their life, subject to specific legal criteria and processes. Exploring the experiences of health providers in the initial stage of the implementation of the End of Life Choice Act 2019 is vital to inform the ongoing development of safe and effective AD practice, policy and law. Aim To explore the early experiences of health care providers (HCPs) who do and do not provide AD services seven months after legalisation of AD to provide the first empirical account of how the AD service is operating in New Zealand’s distinctive healthcare environment and cultural context. Design Qualitative exploratory design using semi-structured individual and focus group interviewing with a range of HCPs. Results Twenty-six HCPs participated in the study. Through a process of thematic analysis four key themes were identified: (1) Difference in organisational response to AD; (2) challenges in applying the law; (3) experiences at the coal face; and (4) functionality of the AD system. Conclusion A range of barriers and enablers to successful implementation of AD were described. Adoption of open and transparent organisational policies, ongoing education of the workforce, and measures to reduce stigma associated with AD are necessary to facilitate high quality AD service provision. Future research into the factors that influence responses to, and experience of AD; the impact of institutional objection; and the extent to which HCP perspectives evolve over time would be beneficial. In addition, further research into the integration of AD within Māori health organisations is required.

Abstract

Abstract Background New Zealand recently introduced law permitting terminally ill people to request and receive assisted dying (AD) in specified circumstances. Given the nature and complexity of this new health service, research is vital to determine how AD is operating in practice. Objective To identify research priorities regarding the implementation and delivery of AD in New Zealand. Methods Using an adapted research prioritisation methodology, the researchers identified 15 potential AD research topics. A mixed-methods survey of health professionals was undertaken where respondents were asked to rate the 15 topics according to the relative importance for research to be conducted on each issue. Respondents could also suggest additional research areas, and were invited to participate in a follow-up interview. Results One hundred and nineteen respondents completed the survey. 31% had some experience with AD. The highest rated research topic was the ‘effectiveness of safeguards in the Act to protect people’; the lowest rated topic was research into the ‘experiences of non-provider (e.g., administrative, cleaning) staff where assisted dying is being provided’. Respondents suggested 49 other research topics. Twenty-six interviews were conducted. Thematic analysis of interview data and open-ended survey questions was undertaken. Six research themes were identified: general factors related to the wider health system; the experiences of health care providers at the bedside; medico-legal issues; the impact of AD; experiences on the day of dying; and the overall effectiveness of the AD system. Key issues for stakeholders included safety of the AD service; ensuring access to AD; achieving equity for ‘structurally disadvantaged’ groups; and ensuring the well-being of patients, families/whānau, providers and non-providers. Conclusions Based on early experiences of the implementation of the AD service, health professionals provide important insights into what research should be prioritised post-legalisation of AD. These findings can be used to shape the research agenda so that research may inform law, policy and best practice.

Abstract

In the field of collaborative research in the didactic of languages, the actors of research-training projects postulate that collaboration meets the needs of both practitioners and researchers. Yet as it happens, the expected results do not reveal the same epistemology (practical and/or academic). However, if collaboration is possible, our premise is that it is thanks to the existence of triggers, which do not only include people skills, fortuitous encounters, or even shared ethics specific to collaborative researches. The basic principle is that collaboration is possible and even wanted as a benefit for each participant, pupils included. We will discuss from a collaborative research-training project in the area of the didactic of languages. The project ‘Schools, Pupils’ Languages and Plurilingualism: a collaborative research in the International French School of San Francisco – AEFE, USA (E2PRC_Francisco)’ is dealing with the didactic of oral language in a plurilingual context and gathers 9 teachers, 2 trainers for teachers, 4 researchers, 1 French inspector of education. We will more specifically focus on how researchers and trainers collaborate, through the double point of view of the co-pilots of the project who shared the writing. The article starts with a description of the context, and we will then demonstrate how the sociolinguistic data helped to achieve a shared approach of self-directed learning. We will finally detail the transformations and professional consequences initiated by such a conceptual framework for the trainers as well as for the co-pilots.

6. Thermal Impact of Cosmic Ray Interaction with an X‑Ray Microcalorimeter Array

Abstract

The X-ray Integral Field Unit (X-IFU) instrument on the Athena mission will be positioned at the Lagrangian point L2 and be subject to cosmic rays generated by astrophysics sources, primarily relativistic protons. Previous simulations have shown that particles of energy higher than 150 MeV will make it through the outer layers of the satellite. They will reach the detector wafer with a rate of 3 cts/sq. cm/s and a most probable energy deposited in the Si frame supporting the array at 150 keV. These events can affect the energy resolution of the detectors through the thermal fluctuations that they produce. This study assesses this potential problem and discusses two suggested design approaches to decrease the impact of cosmic ray in order to limit their effect to their allocation of 0.2 eV within the Athena/X-IFU energy-resolution budget. The first is the addition of a coating layer of high heat capacity material (e.g., Pd) and the second is the splitting of this coating into two thermal regions near the TES array to keep the heat away from the array. Implementing these two features is predicted to cause a decrease in the number of events above 1 μK by more than a factor 10 to ~ 1.5 cps when compared to an equivalent design without these features.

Abstract

Arrays of high-density microcalorimeters require careful heat sinking in order to minimize the thermal crosstalk between nearby pixels. For the array of microcalorimeters developed for the Athena X-ray Integral Field Unit instrument, which has more than 3000 pixels on a 275 µm pitch, it is essential to address this problem in order to meet the energy-resolution requirements. The instrument’s energy-resolution budget requires that the impact of the thermal crosstalk on the energy resolution be a contribution that, added in quadrature to other energy-resolution contributions, is less than 0.2 eV. This value results in a derived requirement that the ratio between the amplitude of the crosstalk signal to an X-ray pulse (for example at 6 keV) is less than 1 × 10(exp −3) (for the first neighbor), less than 4 × 10(exp −4) (for the diagonal neighbor) and less than 8 × 10(exp −5) (for the second nearest neighbor). We have measured the thermal crosstalk levels between pixels in various geometries and configurations. The results show a crosstalk ratio which is at least a factor of 4 lower than the derived requirement. We also developed a finite element (FEM) 2D thermal model to predict the thermal behavior of large-scale arrays. This model successfully simulates the measured data in terms of pulse amplitude and time constants.

Abstract

The Athena X-ray Integral Unit (X-IFU) is the high resolution X-ray spectrometer, studied since 2015 for flying in the mid-30s on the Athena space X-ray Observatory, a versatile observatory designed to address the Hot and Energetic Universe science theme, selected in November 2013 by the Survey Science Committee. Based on a large format array of Transition Edge Sensors (TES), it aims to provide spatially resolved X-ray spectroscopy, with a spectral resolution of 2.5 eV (up to 7 keV) over an hexagonal field of view of 5 arc minutes (equivalent diameter). The X-IFU entered its System Requirement Review (SRR) in June 2022, at about the same time when ESA called for an overall X-IFU redesign (including the X-IFU cryostat and the cooling chain), due to an unanticipated cost overrun of Athena. In this paper, after illustrating the breakthrough capabilities of the X-IFU, we describe the instrument as presented at its SRR, browsing through all the subsystems and associated requirements. We then show the instrument budgets, with a particular emphasis on the anticipated budgets of some of its key performance parameters. Finally we briefly discuss on the ongoing key technology demonstration activities, the calibration and the activities foreseen in the X-IFU Instrument Science Center, and touch on communication and outreach activities, the consortium organisation, and finally on the life cycle assessment of X-IFU aiming at minimising the environmental footprint, associated with the development of the instrument. Thanks to the studies conducted so far on X-IFU, it is expected that along the design-to-cost exercise requested by ESA, the X-IFU will maintain flagship capabilities in spatially resolved high resolution X-ray spectroscopy, enabling most of the original X-IFU related scientific objectives of the Athena mission to be retained. Abstract

The X-IFU (X-ray Integral Field Unit) onboard the large ESA mission Athena (Advanced Telescope for High ENergy Astrophysics), planned to be launched in the mid 2030s, will be a cryogenic X-ray imaging spectrometer operating at 55 mK. It will provide unprecedented spatially resolved high-resolution spectroscopy (2.5 eV FWHM up to 7 keV) in the 0.2-12 keV energy range thanks to its array of TES (Transition Edge Sensors) microcalorimeters of more than 2k pixel. The detection chain of the instrument is developed by an international collaboration: the detector array by NASA/GSFC, the cold electronics by NIST, the cold amplifier by VTT, the WFEE (Warm Front-End Electronics) by APC, the DRE (Digital Readout Electronics) by IRAP and a focal plane assembly by SRON. To assess the operation of the complete readout chain of the X-IFU, a 50 mK test bench based on a kilo-pixel array of microcalorimeters from NASA/GSFC has been developed at IRAP in collaboration with CNES. Validation of the test bench has been performed with an intermediate detection chain entirely from NIST and Goddard. Abstract

Future X-ray astrophysics experiments require multiplexed readout of high fill-factor, kilo-pixel arrays of transition-edge sensors (TESs), with very high spectral resolution over a broad range of energies. In this paper we report on a prototype kilo-pixel array of Mo/Au TESs readout with 8-column by 32-row time-division multiplexing (TDM). This system is being used to demonstrate the critical detector and readout technology for ESA's Athena X-IFU, and when complete will be used in laboratory astrophysics experiments. Our array and TDM readout have demonstrated a combined full-width-at-half-maximum energy resolution, including > 200 pixels, of: 1.95 eV for Ti-Kα (4.5 keV), 1.97 eV for Mn-Kα (5.9 keV), 2.16 eV for Co-Kα (6.9 keV), 2.33 eV for Cu-Kα (8 keV), 3.26 eV for Br-Kα (11.9 keV). The 1 sigma statistical errors are ≤0.01 eV for all spectra. These results meet the broad-band resolution requirements for X-IFU with margin.

Abstract

Multiple myeloma is the most common haematological malignancy accounting for ten percent of all haematological cancers. Treatment of myeloma has evolved in recent years leading to improved survival. Lesions related to myeloma are frequently observed within the oral cavity and jawbone. In addition, many of the therapeutic agents have side effects with implications for provision of dental treatment. This case series aims to highlight some of these presentations to remind dental practitioners to be vigilant. Observation of suspicious lesions within the oral cavity or jawbone may warrant further investigation.

Abstract

Temporomandibular disorder is a broad term encompassing pain and/or dysfunction of the masticatory musculature and the temporomandibular joints. Pain arising from a temporomandibular disorder is a common reason for seeking dental care. It is essential that dental practitioners are able to accurately diagnose and manage this condition. Identifying people at highest risk of developing a temporomandibular disorder and knowing which procedures are more likely to initiate or exacerbate a temporomandibular disorder, are important to reduce the likelihood of its acute and chronic presentation. The aim of this paper is to provide the dental practitioner with a clinical guideline for reference including practical tools to examine, diagnose and manage patients with temporomandibular disorder. In addition the risk profile of patients and procedures is explored to help minimize the occurrence of temporomandibular disorders and mitigate its symptoms. The scope of the dental practitioner in the management of acute and chronic temporomandibular disorders is presented, with guidance about when referral to a specialist is indicated.

Simple Summary Myeloma is a common cancer involving the bone marrow. Some of the medications used in the treatment of myeloma, including those that reduce the risk of bone fractures, can increase the chance of side effects occurring in the jawbone. The most serious complication in the jawbone is called medication-related osteonecrosis, meaning part of the jawbone dies. The aim of this review is to highlight some of the medications that are implicated and other risk factors that can contribute to osteonecrosis. Suggestions to prevent this complication from occurring are described. Conventional methods of treating established medication-related osteonecrosis of the jawbone are outlined as well as emerging new treatments. Abstract Myeloma is a common haematological malignancy in which adverse skeletal related events are frequently seen. Over recent years, treatment for myeloma has evolved leading to improved survival. Antiresorptive therapy is an important adjunct therapy to reduce the risk of bone fractures and to improve the quality of life for myeloma patients; however, this has the potential for unwanted side effects in the oral cavity and maxillofacial region. Osteonecrosis of the jaw related to antiresorptive medications and other myeloma therapies is not uncommon. This review serves to highlight the risk of osteonecrosis of the jaw for myeloma patients, with some suggestions for prevention and management.

Abstract

Microcalorimeters have the potential to provide line shapes well described by a single Gaussian broadening term of few-eV width. This attribute makes the detectors especially well suited for x-ray astrophysics observations; however, low-level non-Gaussian broadening terms are expected and must be characterized. These terms depend on the composition of the x-ray absorber, the detailed x-ray absorption physics, the device thermalization processes, and the incident x-ray energy. Here we present the first measurements targeted at understanding the extended line-spread function (LSF) of x-ray microcalorimeter pixels under development for the X-ray Integral Field Unit on the Athena X-ray Observatory. These pixels are composed of Mo/Au transition-edge sensors with overhanging electroplated Au/Bi absorbers. We have measured the line shapes using monochromatic x-ray sources with $ -eV width at several x-ray energies (0.85, 0.93, 1.25, 1.5, 5.4, and 8.0 keV) across the instrument bandpass (0.3–12 keV) and have modeled the line profiles. These results are compared to the extended LSF of the Hitomi microcalorimeter pixels that used HgTe absorbers.

Abstract

We present a test platform for the Athena X-IFU detection chain, which will serve as the first demonstration of the representative end-to-end detection and readout chain for the X-IFU, using prototypes of the future flight electronics and currently available subsystems. This test bench, housed in a commercial two-stage ADR cryostat, includes a focal plane array placed at the 50 mK cold stage of the ADR with a kilopixel array of transition-edge sensor microcalorimeter spectrometers and associated cold readout electronics. Prototype room temperature electronics for the X-IFU provide the readout, and will evolve over time to become more representative of the X-IFU mission baseline. The test bench yields critical feedback on subsystem designs and interfaces, in particular the warm readout electronics, and will provide an in-house detection system for continued testing and development of the warm readout electronics and for the validation of X-ray calibration sources. Abstract

The Viking Graben area in the Central North Sea was the initial focus of a series of early 3D GeoStreamer dual-sensor surveys between 2009 and 2012 (Figure 1). These MultiClient surveys immediately demonstrated significant image quality improvements achieved with the dual-sensor towed streamer seismic in an area characterized by significant subsurface complexity. After more than 50 years of active exploration, the Viking Graben is widely regarded as a mature basin whose petroleum system is thought to be well understood. Many of the classic hydrocarbon targets of the region, including the large rotated Jurassic fault blocks of Gullfaks and Oseberg, and the Paleocene and Eocene fan systems of the Heimdal and Frigg fields, have already been exploited. Challenges remain to stratigraphic interpretation and mapping of sands within the Upper Jurassic and it is therefore important to have data which has sufficient penetration and accurate positioning of reflectors to continue to explore these levels. Determining the exploration opportunities beyond these classic targets also requires optimization of the seismic data. The Eocene sands have been extensively mapped, but the Paleocene fans are less well known. Although the play is proven, the more subtle sands of the Ty and Maureen formations have been masked by artefacts caused by shallow events. In addition, several of the classic plays have now become the subject of re-evaluation. Better resolution and deeper signal penetration with dual-sensor data has provided detailed insight into many of these plays and also improved the understanding of the overall connectivity of several highly compartmentalized reservoirs that are often comprised of a variety of barriers and baffles. In order to improve the understanding of the geology within this established area and to be able to exploit the fields to their full potential, modern seismic data need to be consistently re-evaluated and subjected to new processing techniques. The remobilized and injected reservoir sands of the Volund field were considered unique at the time of its discovery in 1994, however, many other recent discoveries are also now considered to be the result of remobilized sands. Potentially, known fields such as Balder, with its steep sided reservoir, may also be re-categorized as an injectite rather than a deep marine fan reservoir. Utilizing a depth migrated seismic dataset with reduced artefacts from shallow anomalies may validate this and enable additional fields to be discovered.

Abstract

Multi-azimuth data acquisition has shown benefits in noise reduction and structure illumination, which leads generally to improved seismic images. However the current time processing, which is based on independent processing of the data from the different predominant azimuths, may lead to destructive interferences and deterioration of the final stacked image when the earth model is anisotropic or laterally heterogeneous. In order to solve this problem a prestack time migration for anisotropic, weakly heterogeneous media has been recently introduced and tested on synthetic data. In this work, we ran a first field data application on a multi-azimuth data set. From data in three different acquisition azimuths we performed migration velocity analysis to build azimuth-dependent migration operators. We processed the data in parallel also with conventional narrow azimuth prestack time migration in order to assess the performance of the new approach.
