Acne inversa (Ai) ist eine chronische, entzundliche Hauterkrankung mit hoher Krankheitslast und eingeschrankten Behandlungsmoglichkeiten. In einer fruheren Studie untersuchten wir die Wirksamkeit einer lokalen Behandlung mit der LAight®(LENICURA GmbH, Wiesbaden Deutschland)-Therapie, einer Kombination aus intensiv gepulstem Licht (IPL) und Radiofrequenz (RF) bei Acne inversa. Es erfolgte der Vergleich der Versorgung von Ai-Patienten, die LAight-Therapie erhielten, mit der Regelversorgung vor Inanspruchnahme der Therapie. In die retrospektive Studie wurden 111 Patienten eingeschlossen, die zwischen Januar 2014 und Marz 2017 mindestens 5‑mal mit LAight behandelt wurden. Primarer Endpunkt war die Veranderung der Anzahl chirurgischer Eingriffe. Zu den sekundaren Endpunkten gehorten weitere Selbsteinschatzungen der Patienten z. B. zur Lebensqualitat. Hidradenitis suppurativa (HS) is a chronic, inflammatory skin disease with high burden for patients and limited treatment options. We recently analyzed the efficacy of local treatment with LAight therapy, a combination of intense pulsed light (IPL) and radiofrequency (RF).The aim of the present survey was to compare care of HS patients under LAight with standard care prior to therapy.The retrospective study included 111 patients who were treated with LAight at least 5 times between 01/2014 and 03/2017. Primary endpoint was the change in surgical interventions. Secondary endpoints included additional patient-reported outcomes, e.g., effects on their quality of life.In all, 50 completed surveys were available for evaluation. Under LAight therapy, the number of surgical interventions decreased from 3.2 to 0.3 per year (p 0.001). Secondary endpoints were also significantly improved: The days of sick leave/year due to HS decreased from 30.4 to 7.2 (p = 0.002), and the number of doctor appointments due to HS flare ups was altered from 7.3 to 1.5 (p 0.001). In line, patient-reported pain levels, general wellbeing and quality of life were also improved.This retrospective assessment of patient-reported treatment outcomes of 50 HS cases after treatment with LAight revealed a significant reduction of required surgical interventions and sick-days along with improved quality of life. Thus, it appears that IPL + RF treatment not only improves disease activity, but also secondary care aspects. Entsprechend verbesserten sich auch das vom Patienten berichtete Schmerzlevel, das allgemeine Wohlbefinden und die Lebensqualität.Die retrospektive Erfassung ergab eine signifikante Verringerung der erforderlichen chirurgischen Eingriffe und der Krankheitstage bei gleichzeitig verbesserter Lebensqualität unter LAight-Therapie. Es scheint, dass diese ambulante Behandlungsoption nicht nur die Krankheitsaktivität, sondern auch Aspekte der Sekundärversorgung verbessert. Es sind allerdings weitere Studien nötig, um diese Ergebnisse in einem kontrollierten Umfeld zu bestätigen.

Background Malignant melanoma is an immunogenic skin cancer with an increasing global incidence. Advanced stages of melanoma have poor prognoses. Currently, there are no reliable parameters to predict a patient's response to immune checkpoint inhibitor (ICI) therapy. Methods This study highlights the relevance of a distinct immune signature in the blood for response to ICI therapy and overall survival (OS). Therefore, the immune cell composition in the peripheral blood of 45 melanoma patients prior to ICI therapy was analyzed by flow cytometry and complete blood count. Results Responders to ICI therapy displayed an abundance of proliferating CD4+ T cells, an increased lymphocyte‐to‐monocyte ratio, a low platelet‐to‐lymphocyte ratio, low levels of CTLA‐4+ Treg, and (arginase 1+) polymorphonuclear myeloid‐derived suppressor cells (PMN‐MDSC). Nevertheless, non‐responders with similar immune cell compositions also benefited from therapy displaying increased long‐term OS. Conclusions Our study demonstrated that the observed immune signature in the peripheral blood of melanoma patients prior to treatment could identify responders as well as non‐responders that benefit from ICI immunotherapies., In this study, we offer insight into how immune cell composition in the peripheral blood before beginning immune checkpoint inhibitor (ICI) therapy differs in responders and non‐responders to ICI. Notably, non‐responders with an immune cell composition comparable to responders displayed a prolonged overall survival (OS). This newly characterized immune signature aids in improving the identification of patients (responders and non‐responders) that would benefit from ICI treatment.

Hidradenitis suppurativa (HS also named as acne inversa) is a chronic skin disease characterized by relapsing formation of abscesses, inflammatory nodules, and fistulas. In moderate-to-severe disease, HS leads to the formation of scarring and thus irreversible tissue destruction. In the past few years, two new treatment options became available: adalimumab, the first biologic therapy approved for HS (Humira®, AbbVie), and a noninvasive, device-based treatment utilizing a combination of intense pulsed light and radiofrequency (LAight® therapy, LENICURA, Germany). Here, we report a case of a Hurley stage III patient where the positive effect of adalimumab could be enhanced by simultaneously applying LAight® therapy. Moreover, long-term symptom control could be achieved under monotherapy with LAight® after adalimumab was terminated.
