3. MRI evaluation of the anti-adhesion molecule antibody Natalizumab and the blood-brain barrier in Multiple Sclerosis

Abstract

As Blood-brain barrier (BBB) breakdown is central to inflammatory lesion formation, it presents a potential target in the formulation of putative therapeutic agents in MS. The action of natalizumab, a monoclonal antibody acting at the BBB, is investigated through a phase III monotherapy trial (AFFIRM) and associated substudies. Subtle BBB disruption from non-inflamed lesions, which could contribute to axonal damage through leakage of inflammatory cells and associated mediators into surrounding parenchyma, is also studied. Introductory chapters (1-3) provide a brief overview of MS, clinical trials, magnetic resonance imaging (MRI), the BBB and natalizumab. Chapter four describes MRI results of AFFIRM- a 2 year multi-centre trial involving 942 patients. Compared with placebo, natalizumab reduced number of gadolinium (Gd)- enhancing lesions by 92%, new/enlarging T2-hyperintense lesions by 83%, and new T1- hypointense lesions by 76%. Chapter five describes a 57 patient AFFIRM trial substudy in which the influence of natalizumab on segmental atrophy was investigated. Atrophy was predominant in grey matter (GM) and was independent of lesion load. Fluctuations in white matter (WM) volume followed changes in inflammatory lesion load. Atrophy was not influenced by natalizumab. The effect of natalizumab on subtle BBB disruption (inferred by measuring the post-Gd %change in T1 weighted signal intensity) is studied in chapter 6. This AFFIRM substudy involved 40 patients (27 on natalizumab, 13 on placebo.) Although subtle BBB leakage was consistently detected in non-visibly enhancing lesions, natalizumab did not influence the degree of leakage. Chapter 7 describes a cross-sectional study which utilised post-Gd change in R1 (1/T1) as a marker BBB leakage. 19 patients (10 RRMS, 9 SPMS) were involved in this study. The subtle leakage observed from non-visibly enhancing lesions was distinct from leakage from visibly enhancing lesions. This was sustained over 60 minutes, greater in smaller lesions and in size-adjusted T1 hypointense lesions.

5. REMOTELY SENSEDC IMAGE COMPRESSION BASED ON WAVELET TRANSFORM

Abstract

In this paper, we present an image compression algorithm that is capable of significantly reducing the vast mount of information contained in multispectral images. The developed algorithm exploits the spectral and spatial correlations found in multispectral images. The scheme encodes the difference between images after contrast/brightness equalization to remove the spectral redundancy, and utilizes a two-dimensional wavelet trans-form to remove the spatial redundancy. The transformed images are than encoded by hilbert-curve scanning and run-length-encoding, followed by huffman coding. We also present the performance of the proposed algorithm with KITSAT-1 image as well as the LANDSAT MultiSpectral Scanner data. The loss of information is evaluated by peak signal to noise ratio (PSNR) and classification capability.

Abstract

Thc CCD earth image experiment(CEIE) is one of the main payload of the KITSAT-1. Since it was launched on Age. 11, 1992, the CEIE has taken more than 500 images on the earth surface world-wide so far. An image from the space is very different from a feature on the real Earth surface due to various radiometric and geometric distortions. Preprocessing to remove those distortions has to take place before the image data are processed and analyzed further for various applications. This paper summarizes the result of the operation of the CEIE and describes the procedure to perform preprocessing including radiometric and geometric correction.

17. Psychological Evaluation of Korean School Children: A Preliminary Report.

Abstract

Observations were made while psychologically evaluating Korean school children in the greater Chicago area. The observations were drawn from work with Korean children who were referred for psychological evaluation because of learning or behavioral/emotional problems. Initial demographic data was taken describing the children's age, length of time in the U.S., sex, language spoken at home, and reason for referral. Results for intelligence, perceptual-motor, projective, and achievement tests were obtained. On the intelligence test both the Mean Full Scale and Verbal IQ scores were slightly below equal to the American norms. Performance on the perceptual-motor test was also similar to American norms. Tentative conclusions were drawn from the projective test for personality and emotional assessment, which was given to a small number of children who revealed a limited verbal capacity and limited creative resources. On the achievement test children were on the whole three or four years behind in reading, and spelling, and two years behind in arithmetic. Recommendations were made on the basis of these test results and solutions to Korean children's language and acculturation problems were offered. (Author/APM)

18. Cognitive and Affective Adaptation of Korean American School Children in Chicago: Service and Research Priorities.

Abstract

The purposes of this paper are to: (1) identify learning and adaptive behavior problems that Korean-American students encounter in schools by reviewing psychological evaluation data collected from them; (2) evaluate the extent to which these problems are addressed; and (3) recommend service and research needs in relation to these problems. One hundred seventy-nine Korean-American children from Chicago and surrounding suburban schools were psychologically evaluated between 1978 and 1986. The data indicate that learning problems were well identified, and that children were provided appropriate educational services to remediate these problems, but psychological and emotional problems were unidentified by teachers and were not treated by the schools or community health agencies. Training teachers and counselors to identify psychological problems and instituting primary mental health activities in the schools need to be given high priority, and valid, reliable, and sensitive cognitive and personality assessment techniques for future research should be developed. Abstract

The impact of para-aortic field radiation therapy upon survival was studied among 26 patients with para-aortic nodal metastases from carcinoma of the endometrium. Seventeen of these 26 patients received postoperative radiation therapy to the para-aortic field as a part of their primary therapy. Sixteen of the 17 also received adjuvant hormonal therapy. Nine of 17 patients (53%) are alive without evidence of disease (18-55 months) with a median survival time of 27 months. Of the remaining eight patients, six (35%) died of endometrial cancer at 6-38 months, with a median survival time of 14.5 months. Five of these patients had distant disease. Two of the 17 patients (12%) died of intestinal obstruction felt to be secondary to radiation enteritis, one of whom was disease free. No difference in survival was detected in patients treated with radiation therapy with microscopic versus macroscopic nodal involvement. Of the nine patients who did not receive para-aortic radiation, eight were treated with hormonal therapy (n = 6) or chemotherapy (n = 2). Seven patients died of disease from 5-28 months, with a median survival time of 13 months. One patient is alive at 12 months. Survival in the 17 patients treated with para-aortic radiation was better than the eight patients not treated with para-aortic radiation (p = 0.004). This survival difference remained significant for patients with microscopic but not macroscopic nodal disease. Para-aortic field radiation appears to improve survival, but has a significant complication rate, and should be reserved for patients with histologic evidence of para-aortic metastases.

44. Effect of exenatide on the pharmacokinetics and pharmacodynamics of warfarin in healthy asian men.

Abstract

Exenatide, a treatment for type 2 diabetes, slows gastric emptying as part of its pharmacologic action and may alter the absorption of concomitant oral drugs. This open-label, 2-period, fixed-sequence study evaluated the influence of exenatide coadministration on the pharmacokinetics and pharmacodynamics of warfarin, a narrow therapeutic index drug, in healthy men (N = 16). A single, 25-mg oral dose of warfarin, with a standardized breakfast, was administered alone in period 1 and concomitantly with 10 mug exenatide subcutaneous twice daily in period 2. Exenatide did not produce significant changes in R- or S-warfarin pharmacokinetics. Although there were minor reductions in warfarin anticoagulant effect, the ratios of geometric means for the area under the international normalized ratio (INR)-time curve from dosing until the time of the last measurable INR value or maximum-observed INR response being 0.94 (0.93-0.96) and 0.88 (0.84-0.92), respectively, the magnitude and direction of these changes do not suggest a safety concern from this interaction. 46. Prevalence of Helicobacter pylori Infection in Korean Children: Inverse Relation to Socioeconomic Status Despite a Uniformly High Prevalence in Adults.

Abstract

The prevalence of Helicobacter pylori infection in US adults was shown to be inversely correlated with the socioeconomic status of the family during childhood, and it was suggested that this was additional evidence of transmission occurring in childhood. The present study of H. pylori infection was conducted in South Korea, which has emerged as a developed country in the last two decades. The authors attempted to determine whether there was a difference in prevalence of H. pylori infection in Korean children of different socioeconomic classes despite the high prevalence of infection in childbearing adults. The authors also attempted to identify the factors responsible for the different patterns of transmission by estimating the age-specific prevalence of H. pylori infection in 413 healthy 1- to 75-year-old asymptomatic volunteers who resided in Seoul. H. pylori status was evaluated using an enzyme-linked immunosorbent assay for antiH. pylori immunoglobulin G. Demographic data were obtained from each individual, and socioeconomic class was assessed by the education level of the adults and of the children's parents as well as family income.H. pylori infection was present in 75% of adults and 22% of children, and its prevalence increased with age (p < 0.001). In adults, the rate of infection was high and independent of socioeconomic class. In children, it was inversely related to the socioeconomic class of the child's family: 12% among upper socioeconomic class, 25% among the middle class, and 41% among the lowest class (p = 0.016). No associations were found between prevalence of H. pylori infection and any factor tested including sex, smoking, and alcohol consumption. In addition, type of housing, whether owned or rented, number of family members living in the same household, water source, and type of community in which a child grew up were not found to be risk factors influencing H. pylori infection prevalence. The prevalence of H. pylori infection in Korea appears to be changing with markedly lower prevalence in children of families of higher socioeconomic status. The factor(s) responsible for the break in the pattern of transmission in children of the higher socioeconomic class was not discovered. Abstract

A consistent disparity has been found between interview and self-report data on East Asian mental health. Two hypotheses, incomparability of item content and cultural differences in the social desirability of items, were raised as plausible explanations for the disparity. An empirical test rejected the hypothesis of cultural differences in the social desirability of questionnaire items. Abstract

In a 2-year, placebo-controlled trial (the Natalizumab Safety and Efficacy in Relapsing Remitting Multiple Sclerosis AFFIRM study), involving 942 patients with relapsing multiple sclerosis (MS), natalizumab significantly reduced the relapse rate by 68% and progression of sustained disability by 42% vs placebo. We report the effect of natalizumab on MRI measures from the AFFIRM study.
