Objective In the evaluation of oropharyngeal dysphagia, instrumental procedures, for example, flexible endoscopic evaluation of swallowing or videofluoroscopic swallowing study, are essential to improve diagnostic accuracy for salient findings such as penetration, aspiration, or pharyngeal residue. To date, it is unclear which of the 2 methods represents the diagnostic gold standard. The aim of this study, therefore, was to compare videofluoroscopy and swallowing endoscopy during a simultaneous swallowing examination in a large cohort of patients with oropharyngeal dysphagia. Designs Prospective observational study. Setting and participants In this study, 49 patients with oropharyngeal dysphagia (mean age 70.0 ± 10.8 years) were evaluated using simultaneous swallowing endoscopy and videofluoroscopy. Furthermore, the effect of narrow-band imaging in swallowing endoscopy on the assessment of penetration and aspiration was investigated in a subgroup of 19 patients. Measures The Penetration-Aspiration Scale and the Yale Pharyngeal Residue Severity Rating Scale were rated independently based on both modalities. Results Both modalities showed a high correlation between penetration, aspiration, and pharyngeal residue. Causes for a higher score on the Penetration-Aspiration Scale in videofluoroscopy were intradeglutitive events that were not visible in swallowing endoscopy or false-positive events because of the loss of the lateral dimension in videofluoroscopy. A typical reason for a higher score on this scale in swallowing endoscopy was the better visualization of the anatomical structures. Narrow-band imaging in swallowing endoscopy resulted in a higher score on the Penetration-Aspiration Scale for liquids and semisolids in individual cases, although overall there was no statistically significant difference between scores using white light or narrow-band imaging. Conclusions and Implications Videofluoroscopy and swallowing endoscopy may equally be considered as a diagnostic gold standard for oropharyngeal dysphagia regarding penetration, aspiration, and pharyngeal residue. Narrow-band imaging may increase the sensitivity for penetration and aspiration in individual cases.

Background and purpose Diagnosis of pharyngeal dysphagia caused by myasthenia gravis (MG) based on clinical examination alone is often challenging. Flexible endoscopic evaluation of swallowing (FEES) combined with Tensilon (edrophonium) application, referred to as the FEES-Tensilon test, was developed to improve diagnostic accuracy and to detect the main symptoms of pharyngeal dysphagia in MG. Here we investigated inter- and intra-rater reliability of the FEES-Tensilon test and analyzed the main endoscopic findings. Methods Four experienced raters reviewed a total of 20 FEES-Tensilon test videos in randomized order. Residue severity was graded at four different pharyngeal spaces before and after Tensilon administration. All interpretations were performed twice per rater, 4 weeks apart (a total of 160 scorings). Intra-rater test-retest reliability and inter-rater reliability levels were calculated. Results The most frequent FEES findings in patients with MG before Tensilon application were prominent residues of semi-solids spread all over the hypopharynx in varying locations. The reliability level of the interpretation of the FEES-Tensilon test was excellent regardless of the rater's profession or years of experience with FEES. All four raters showed high inter- and intra-reliability levels in interpreting the FEES-Tensilon test based on residue clearance (kappa = 0.922, 0.981). The degree of residue normalization in the vallecular space after Tensilon application showed the highest inter- and intra-rater reliability level (kappa = 0.863, 0.957) followed by the epiglottis (kappa = 0.813, 0.946) and pyriform sinuses (kappa = 0.836, 0.929). Conclusion Interpretation of the FEES-Tensilon test based on residue severity and degree of Tensilon clearance, especially in the vallecular space, is consistent and reliable.

Introduction Previous research has documented that pressure and duration of brainstem-generated pharyngeal swallowing can be cortically modulated. But there is a commonly held belief that the sequence of pharyngeal pressure remains constant. However, Huckabee et al. [19] reported a patient cohort who demonstrated reduced latency of peak pressure in the proximal and distal pharynx, disproportionate and sometimes inversely correlated with overall swallowing duration, suggesting independent timing of underlying muscle contraction within the overall pharyngeal response. This study examined if healthy adults can volitionally produce altered latency of pharyngeal closure in isolation following intensive training, thereby evaluating the capacity for pharyngeal adaptation in a healthy system. Method Six healthy participants were seen for intensive training, consisting of daily one-hour sessions over two weeks (10 days) using pharyngeal manometry as a visual biofeedback modality. The participants were instructed to produce simultaneous pressure in the pharyngeal sensors when swallowing. The temporal separation of peak proximal and distal pharyngeal pressure was measured with discrete-sensor pharyngeal manometry at baseline, during training with biofeedback, and following training without biofeedback. Results Following intensive training, participants were able to reduce temporal separation of peak pressure between the proximal and distal pharyngeal sensors from a baseline median of 188 ms (IQR = 231 ms) to 68 ms (IQR = 92 ms; p = 0.002). In contrast, there was no significant change in overall swallowing duration during training (p = 0.41). However, change in pharyngeal pressure latency was moderately correlated with both change in swallowing duration (r = 0.444) and amplitude (r = 0.571) during training, and there was a reduction in swallowing duration post-training (p = 0.03). Conclusion Given intensive manometric biofeedback training, participants substantially reduced temporal separation of peak proximal and distal pharyngeal pressure when volitionally swallowing. However, correlation with overall pressure and duration measures suggest the adaptation was one of modulating the cumulative pharyngeal response rather than altering discrete components of timing of pharyngeal pressure in isolation. This is inconsistent with the pattern of behaviour documented by Huckabee et al. [19] in the patient population. Further research on modulatory control over targeted aspects of the pharyngeal swallow is needed, and may provide avenues for rehabilitative treatment of patients with dysphagia.
