O texto pretende ser um manifesto, em forma epistolar, sobre as confluências entre pesquisa, arte e vida, no sentido de refletir sobre a academia e as relações acadêmicas estabelecidas entre orientação e pessoa orientanda, visando lançar luz ao como se investiga e se instiga a investigar, observando os processos experimentados em direção à produção tal qual uma doula acompanha uma gestação, com rigor e responsabilidade tais que podem superar normas e modos tradicionais de se encarar uma pesquisa e seus resultados finais.

O presente dossiê apresenta, em meio a uma série de perguntas e questões que constituem as pesquisas dos integrantes de seu comitê editorial, um escopo rico e fértil de discussões acadêmico-científicas escritas por pesquisadores interessados em articular relações entre Corpo e(n)cena e (des)educação.

Objective: to investigate the association between intestinal constipation, lifestyle and eating habits in four to seven year old children. Methods: a cross-sectional study with a sample of 152 children aged four to seven years old. Intestinal constipation was characterized according to the Rome IV criteria. Excess weight was classified by the body mass index. Variables that could be associated with constipation, such as maternal constipation, lifestyle, food and water intake, were selected. In the analysis, the prevalence ratio using Poisson regression, with a robust variance and statistical significance was set at p0.05. Results: the prevalence of intestinal constipation was 32.2% (n = 49), of which 34.7% (n = 17) of the children were overweight. As for lifestyle, 73% of the children did not practice any regular physical activities and approximately 60% spent more than two hours a day in sedentary activities, such as watching television or playing electronic games. A statistically significant association was found between constipation and daily school time equal to or greater than 4.5 hours (p = 0.007), non-consumption of fruits (p = 0.003) and greens (p = 0.007) and regular intake of fried food (p = 0.003). Conclusion: an association between intestinal constipation and unhealthy habits, such as non-consumption of fruits and vegetables, and regular consumption of fried food was observed. Preventing intestinal constipation requires changes in the lifestyle and eating habits of the children and family.Objetivo: investigar la asociación entre estreñimiento intestinal, estilo de vida y hábitos alimentarios en niños de cuatro a siete años. Métodos: estudio transversal con muestra de 152 niños de cuatro a siete años. El estreñimiento intestinal se caracterizó según los criterios de Roma IV. El exceso de peso fue clasificado por el índice de masa corporal. Se seleccionaron variables que podrían estar asociadas al estreñimiento, como el estreñimiento materno, el estilo de vida, los hábitos alimentarios y el consumo de agua. En los análisis, la razón de prevalencia mediante regresión de Poisson con una varianza robusta y significación estadística se estableció0,05. Resultados: la prevalencia de estreñimiento intestinal fue del 32,2%, de la cual el 34,7% presentaba un exceso de peso. En cuanto al estilo de vida, el 71% de los niños no practicaba actividad física regular y aproximadamente el 60% pasaba más de dos horas diarias realizando actividades sedentarias, como ver la televisión y jugar a videojuegos. Se pudo constatar una asociación estadísticamente significativa entre el estreñimiento y un tiempo diario en la escuela igual o superior a 4,5 horas (p = 0,007), escaso consumo de frutas (p = 0,003) y verduras (p = 0,007) y consumo regular de frituras (p = 0,003). Conclusiones: se pudo observar una relación directa entre el estreñimiento intestinal y los hábitos no saludables, como no consumir frutas y verduras y el consumo regular de frituras. La prevención del estreñimiento intestinal requiere cambios en el estilo de vida y los hábitos alimentarios del niño y la familia.

Aims: To evaluate the capability of an electrolytes-enriched solution to prevent metabolic disorders during continuous veno-venous hemodiafiltration (CVVHDF). Methods: Serum biochemistry and clinical tolerance were compared during CVVHDF treatments with an electrolyte-enriched (Phoxilium) or standard solutions in 10 acute renal failure patients. Results: As compared to standard fluids, serum potassium and phosphate levels were maintained in the normal range with Phoxilium without any supplementation but total serum calcium levels were significantly lower. Bicarbonatemia was slightly higher (24-26 vs. 21.5-24.5 mmol/l, p < 0.05) with conventional solutions and was associated with a significant increased level of pH (>7.44). Despite the absence of glucose in the Phoxilium solution, blood glucose levels and glucose supplementation were similar between treatments. Clinical tolerance and efficiency of CVVHDF sessions were comparable. Conclusion: Phoxilium effectively prevented hypophosphatemia and hypokalemia during CVVHDF. It was, however, associated with a slight metabolic acidosis and hypocalcemia compared with conventional solutions.

The evaluation of clinical-nutrition status is essential to increase life quality and improve clinical outcomes of patients in hemodialysis (HD). In the absence of a gold standard, the goal of this integrative review was to present and discuss the latest scientific literature on the ability of clinical-nutritional indicators and inflammatory and oxidative stress markers to predict morbidity and mortality in HD. In this context, the lean and fat mass indexes have become good predictors of mortality in HD individuals, regardless of BMI. Subjective scoring systems have been more sensitive to malnutrition, and altogether anthropometric indicators may result in an early detection of mortality risk in this population. On the other hand, inflammation in HD, as assessed by C-reactive protein, is not only related to cardiometabolic alterations, but it is also one of the key-points in the development of malnutrition, exacerbated by the state of oxidative stress, which has been identified in this group by the increase of the serum levels of gamma-glutamyl transferase and malondialdehyde.

France has recently witnessed a nationwide outbreak of measles. Data on severe forms of measles in adults are lacking. We sought to describe the epidemiologic, clinical, treatment, and prognostic aspects of the disease in adult patients who required admission to an intensive care unit (ICU). We performed a retrospective analysis of a cohort of 36 adults admitted to a total of 64 ICUs throughout France for complications of measles from January 1, 2009, to December 31, 2011. All cases of measles were confirmed by serologic testing and/or reverse transcription polymerase chain reaction. The cohort consisted of 21 male and 15 female patients, with a median age of 29.2 years (25th–75th interquartile range [IQR], 27.2–34.2 yr) and a median Simplified Acute Physiology Score (SAPS II) of 13 (IQR, 9–18). Among the 26 patients whose measles vaccination status was documented, none had received 2 injections. One patient had developed measles during childhood. Underlying comorbid conditions included chronic respiratory disease in 9 patients, immunosuppression in 7 patients, and obesity in 3 patients, while measles affected 5 pregnant women. Respiratory complications induced by measles infection led to ICU admission in 32 cases, and measles-related neurologic complications led to ICU admission in 2 cases. Two patients were admitted due to concurrent respiratory and neurologic complications. Bacterial superinfection of measles-related airway infection was suspected in 28 patients and was documented in 8. Four cases of community-acquired pneumonia, 6 cases of ventilator-associated pneumonia, 1 case of tracheobronchitis, and 2 cases of sinusitis were microbiologically substantiated. Of 11 patients who required mechanical ventilation, 9 developed acute respiratory distress syndrome (ARDS). Among the patients with ARDS, extraalveolar air leak complications occurred in 4 cases. Five patients died, all of whom were severely immunocompromised. On follow-up, 1 patient had severe chronic respiratory failure related to lung fibrosis, and 2 patients had mild lower limb paraparesis along with bladder dysfunction, both of which were ascribable to measles-induced encephalitis and myelitis. Among the 5 pregnant patients, the course of measles infection was uneventful, albeit 1 patient underwent emergent cesarean delivery because of fetal growth restriction. Measles is a disease with protean and potentially deceptive clinical manifestations, especially in the immunocompromised patient. Measles-associated pneumonitis and its complications, and less commonly postinfectious encephalomyelitis, are the main source of morbidity and mortality. In contrast with the usually benign course of the disease in immunocompetent patients, measles occurring in immunocompromised patients gives rise to lethal complications including ARDS, with or without bacterial superinfection. Other patients potentially at high risk for severe measles are young adults and pregnant women. Measles pneumonitis may predispose to air leak disease in patients using mechanical ventilation. To date, vaccination remains the most potent tool to control measles infection.

RESUMEN En la presente investigación se reporta la abertura tricotomosulcada en cuatro de los 15 géneros de palmas que habitan en Cuba, así como una diferente configuración de la abertura monosulcada en el polen de Coccothrinax litoralis y Coccothrinax salvatoris. Los taxones que comparten granos de polen monosulcados y tricotomosulcados son: Pseudophoenix sargentii, Gastrococos crispa y Bactris cubensis, mientras que el polen de Acrocomia aculeata es exclusivamente tricotomosulcado. Se discute el significado palinilógico de la condición tricotomosulcada. Por otra parte, el uso de la microscopía electrónica revela marcadas variaciones en la estratificación de la exina; caracteres tales como: capa basal, estructura del infratecto y presencia de microcanales o microprocesos supratectales, investigados en siete especies de Arecaceae, corroboran que existen suficientes modificaciones del tecto y en la estructura de la exina, para ser evaluadas en la identificación de material fósil o melisopalinológico. Las variaciones estructurales de la pared en el polen de Coccothrinax crinita y Coccothrinax salvatoris, sugieren la continuación de investigaciones más detalladas en estosextensos géneros, presentes en Cuba. Palabras clave: Polen, Arecaceae, abertura tricotomosulcada, estratificación, exina ABSTRACT Trichotomosulcate pollen grains in fourth out of 15 genera of Cuban Arecaceae are reported in addition to a different aperture configuration in Coccothrinax litoralis and Coccothrinax salvatoris pollen grains. Taxa that share monosulcate and trichotomosulcate pollen are Pseudophoenix sargentii, Gastrococos crispa and Bactris cubensis whereas Acrocomia aculeata has only trichotomosulcate pollen grains. The palynological meaning of trichotomosulcate condition is discussed. On the other hand, the electron microscope techniques show outstanding variation in exine stratification, when tested in seven species of Arecaceae. Features such as basal layer, infratectum structure and microchannels or supretectum microprocesses corroborate that tectum modifications and exine structure exist to such extent as to be assessed in the determination of fossil or melissopalynological samples. Structure variations in pollen wall of Coccothrinax crinita and Coccothrinax salvatoris reveal that detailed research in large genera should be continued. The aim of this study was to evaluate and compare brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) levels as mortality prognosticator and predictor for myocardial dysfunction in severe sepsis and septic shock. Baseline clinical and biological variables were collected from 47 patients with severe sepsis or septic shock. Ventricular systolic function assessed by echocardiography was measured over a 5-day period. Both cTnI and BNP plasmatic levels were determined at intensive care unit (ICU) admission and during the following 15 days. At admission, cTnI and BNP levels were compared to those of 12 control critically ill nonseptic patients. The plasma levels of BNP and cTnI in patients with sepsis were elevated at admission and significantly higher than in the controls. Among patients with sepsis, BNP levels were significantly more elevated in nonsurvivors compared to survivors at admission and 1 day later. The cTnI levels were also significantly more elevated in nonsurvivors compared to survivors, but only at admission. From admission to day 5, patients with sepsis with left ventricular systolic dysfunction had higher BNP plasmatic concentrations than those without; differences were significant at days 3 and 4. In contrast, plasma cTnI levels were similar between the 2 groups. In critically ill patients, sepsis induces significant increase in BNP and cTnI levels. High BNP and cTnI plasma levels during ICU admission appear to be associated with poor outcome of sepsis. Time course of BNP levels seems helpful to discriminate between surviving and nonsurviving patients with sepsis and to detect myocardial dysfunction where troponin levels fail to do so.

Background/Aims: On-line hemodiafiltration (HDF) is not yet routinely used in ICUs given the potential risk of microbial contamination of dialysis fluids. We evaluated the safety and the tolerance of its use in our ICU. Methods: A weekly measurement of bacterial growth (CFU/ml) and endotoxin level (endotoxin units/ml) was performed in dialysis fluids over a 7-year period. Intradialytic hypotensive events and pyrogenic reactions were collected during 466 on-line HDF sessions. Results: A bacterial count Conclusion: The great majority of dialysis fluid samples were considered suitable with a 99% compliance rate. Use of on-line HDF, at a large scale of dialysate and infusate flows, is well tolerated and may be safely performed in critically ill.
