1. NIVOLUMAB EXPERIENCE IN PEDIATRIC MALIGNITIES

Abstract

Objective: Nivolumab is a human monoclonal antibody to programmed cell death receptor 1 (PD-1) that acts as an immune checkpoint inhibitor and is used in the immunotherapy of various types of advanced or metastatic cancer. The aim of this study is to evaluate the efficacy of nivolumab in pediatric patients with various highly malignant tumors and to share the experience of Ankara University Pediatric Oncology Department. Methodology: Eight patients were included in the study. Median age at diagnosis is 11.3 years (min 4.9- max 13.9). Treatment indications were malignant mesothelioma (1), rectal adenocarcinoma (1), malignant melanoma (1), ewing sarcoma (2), osteosarcoma (1), non-hodgkin lymphoma (1) and hodgkin lymphoma (1). Results: Four patients died due to progressive disease. Complete remission was achieved in four patients diagnosed with malignant mesothelioma, rectal adenocarcinoma, malignant melanoma and Hodgkin lymphoma. Conclusion: Immune checkpoint inhibitors are one of the greatest advances in oncological therapy and improve the overall survival of patients with advanced and resistant malignancies. More studies are needed to evaluate the efficacy of immune checkpoint inhibitors in pediatric tumors.

Abstract

Objective: Restriction of access to healthcare during COVID-19 pandemic is undoubtedly a major problem for patients with cancer. Although childhood cancers are highly curable, it is obvious that diagnostic and treatment disruptions will lead to poor Results. In this study we investigated the effects of pandemic on diagnosis and treatment delays of children with cancer along with their consequences. Methodology: We searched all pediatric patients treated for cancer between March 2020 and January 2022 for COVID-19 infection. Data were collected collected from medical files of patients diagnosed with COVID-19, confirmed by polymerase chain reaction (PCR), who received active antineoplastic treatment. Results: Fifty-eight patients developed COVID-19 infection at different stages of their anticancer treatment. Twenty-five had an asymptomatic COVID-19 infection, twenty-six had mild symptoms, three had moderate symptoms and four had severe disease. All of them recovered from COVID-19 infection. Chemotherapy courses were continued during active infection in four patients and interrupted in other patients. Conclusion: While strict measures are required to control the pandemic, patients with severe critical illness such as cancer should be carefully evaluated and treatment delays that may have vital consequences should be avoided. In pediatric patients with cancer whom infected by COVID-19, continuation of anticancer treatment may be considered by evaluating the clinical status of the patient.

Abstract

Case report In Langerhans cell histiocytosis thyroid involvement is rarely seen. Here, we would like to present a 12-year-old male patient with lung, external auditory canal skin and lymph node involvement in diagnosis. Disease relapse occurred with thyroid involvement 19 months after remission. In molecular analysis, BRAF p.N 486 _P490del was detected and he reccived MEK inhibitor Trametinib monotherapy. He is still in remission for 16 months.

Abstract

Case report Gastrointestinal stromal tumors (GISTs) occur exceedingly rare in children and adolescents. Eighty five percent of pediatric GISTs and 15 % of adult GISTs lack oncogenic mutations in KIT and PDGFRA. The results of tyrosine kinase inhibitor therapy in GIST cases with SDH deficiency are limited and controversial. Here, we would like to present a pediatric SDH deficient GIST case treated with surgery and Imatinib Mesylate. We obtained a good response with Imatinib Mesylate.

Abstract

INTRODUCTION: The aim of this study was to review our institutional experience with the clinical characteristics, imaging findings, and therapies in Neurofibromatosis type 1 (NF1) and defining the patients according to the USA- National Institute of Health (NIH) diagnostic criteria and comparing them with the literature. METHODS: Clinical and imaging findings of 18 children with NF1, treated at our hospital from January 2015 to January 2019, were retrospectively evaluated. RESULTS: The study group included 18 children diagnosed with NF1 according to NIH diagnostic criteria. Thirteen patients were female and 5 were male. The median age at admission was 8 years (1-17 years). The presenting symptoms were cafe-au-laits spots in 16 cases, abdominal pain in 1 case and jaw swelling in 1 case. All patients had cafe-au-lait spots, 9 (50%) axillary / inguinal freckles, 13 (72.22%) lisch nodules, 3 (16.66%) bone lesions, 1 (5.55%) neurofibromas and 1 (5.55%) plexiform neurofibromas. Short stature was detected in 3 (16.66%) cases. One of them was evaluated as growth hormone deficiency. Of the 10 patients attending school, 5 (27.77%) had poor school performance. Two of these cases (11.11%) were being diagnosed with ADHD. On magnetic resonance imaging, 6 (33.33%) cases had hamartomatous lesion and 1 (5.55%) case optic glioma. DISCUSSION AND CONCLUSION: We found that the clinical and radiological findings of the patient group were consistent with the literature. Multidisciplinary follow-up of patients throughout the lifetime is necessary for the early detection, follow-up and treatment of complications.

Abstract

Case report: Actinomyces spp. are gram-positive bacilli found in humans as a common flora of the oropharynx, gastrointestinal tract, and urogenital tract. We describe a case of Actinomyces odontolyticus bacteremia in an Ewing sarcoma and febrile neutropenic girl. This is the first time that bacteremia due to A. odontolyticus has been reported in a pediatric cancer patient. . This case suggests that A. odontolyticus should be regarded as a possible cause of bacteremia in neutropenic pediatric cancer patients.

8. Clinical and Prognostic Characteristics in Childhood Osteosarcoma: A Single-Center Experience in Türkiye

Abstract

Objective: In our study, we aimed to share the clinical experiences of our center regarding osteosarcoma cases, the most common primary malignant bone tumor in children and adolescents. Materials and Methods: With approval from the Clinical Research Ethics Committee of our center, the data of 59 pediatric patients who were followed up in our center with the diagnosis of osteosarcoma between 2007 and 2021 were evaluated retrospectively. Results: The mean time between the onset of symptoms and diagnosis was 3 months. Although not statistically significant, patients with a diagnostic delay of 3 months or less had a higher rate of recurrence and mortality. 59.3% of patients had metastatic disease, and the presence of metastases was associated with higher rates of recurrence and mortality. Significant number of patients had multiple surgical operations. Amputation as the first operation and the need for multiple surgeries were associated with higher mortality. Pathologically poor response to chemotherapy is associated with mortality. 42.4% of patients died, and the 5-year overall and disease-free survival rates were 47.5% and 30.5%, respectively. Survival rates were highest in non-metastatic and non-relapsed patients, and lowest in metastatic patients and patients with poor response to chemotherapy. Renal problems and cardiotoxicity were most frequently treatment-related complications. Conclusion: Significant improvements have been achieved in the survival and quality of life in osteosarcoma cases compared to previous years; however, there is still a long way to go, and more multicenter and multidisciplinary studies are needed on osteosarcoma.

Abstract

Objective: The role of cord milking as an alternative to delayed cord clamping is an area that requires more research. Purpose of this clinical trial was to investigate the impact of umbilical cord milking on the absolute neutrophil counts (ANCs) and the neutropenia frequency of preterm infants.Methods: Fifty-eight pregnant women were randomly assigned to one of the umbilical cord milking and control groups. A total of 54 preterm infants (gestational age ≤32 weeks) were enrolled into the study. The umbilical cords of 25 infants were clamped immediately after birth, and in 29 infants, umbilical cord milking was performed first.Results: The ANCs were statistically significantly lower in the cord milking group compared with the control group on days 1, 3 and 7. The frequency of neutropenia was higher in the cord milking group compared with the control group.Conclusion: In our study, ANCs were lower in the cord milking group and the frequency of neutropenia was higher. Abstract

Bevacizumab is a monoclonal human antibody that is effective in the treatment of cancers in both adults and children due to inhibition of vascular endothelial growth factor. Throbotic microangiopathy (TMA) is well-known complication of bevacizumab. Herein, we report a case of desmoplastic small round cell tumor in a 17-year-old male presenting with TMA after bevacizumab treatment. Eculizumab was administered as second line agent for treatment of thrombotic microangiopathy and partial remission was achieved. With this case report we would like to emphasize the renal effects of bevacizumab and discuss the role of eculizumab in the treatment of bevacizumab related TMA.

Abstract

Introduction:The neutrophil-lymphocyte ratio (NLR) and the platelet-lymphocyte ratio (PLR) are accepted as the indicators of systemic inflammatory response. The aim of this study was to evaluate the value of the changes in NLR and PLR values as a biomarker in patients presenting with febrile seizure.Methods:Patients with a diagnosis of febrile seizure were retrospectively evaluated. A total of 175 febrile seizure patients and 150 healthy children constituting the control group were included in the study. The febrile seizure type was simple in 113 and complicated in 62 subjects.Results:The NLR value was higher in the febrile seizure group (3.36±3.28) than in the control group (1.82±2.21) with statistical significance (p=0.000). The PLR value was also higher in the febrile seizure group (135.10±80.44) than in the control group (123.43±67.06) but without statistical significance (p>0.05). There was no statistically significant relationship between the NLR or PLR values and age. The main characteristic influencing the NLR and PLR values in the febrile seizure group was found to be the type of febrile seizure.Conclusion:Febrile seizure diagnosis and typing is done clinically, if the examination is performed for any reason, the NLR and PLR values can provide guidance in the differentiation of simple and complex febrile seizures.

Abstract

Neurotrophic tyrosine receptor kinase (NTRK) gene rearrangements have been recently identified and developed as one of the biomarkers that have been utilized as new targets for cancer therapy. NTRK gene fusions have taken their place in individualized targeted therapy by being used as a predictive (diagnostic) biomarker as well as a treatment target. Selective inhibitors of NTRK fusion proteins have potent efficacy in the treatment of NTRK fusion-positive solid tumors. Detection of these fussions have become important since the finding of new drugs for which U.S. Food and Drug Administration (FDA) granted approval are used on the treatment of patients who has NTRK fussions positive cansers. Clinical trials have shown that first generation tyrosine receptor kinase (TRK) inhibitors, larotrectinib (Vitrakvi, Bayer HealthCare Pharmaceutical Inc, New Jersey, U.S.) and entrectinib (Rozlytrek, Genentech Inc, California, U.S.), have potent efficacy in the treatment of NTRK fusion positive cancers. In the future, with the increase in the number of comprehensive studies on these drugs further information will become available and beneficial.

During the 6-year study using the Centers for Disease Control and Prevention's (CDC) U.S. National Healthcare Safety Network (NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 605,310 patients hospitalized in the INICC's ICUs for an aggregate of 3,338,396 days. Although device utilization in the INICC's ICUs was similar to that reported from ICUs in the U.S. in the CDC's NHSN, rates of device-associated nosocomial infection were higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infection in the INICC's ICUs, 4.9 per 1,000 central line days, is nearly 5-fold higher than the 0.9 per 1,000 central line days reported from comparable U. S. ICUs. The overall rate of ventilator-associated pneumonia was also higher (16.8 vs 1.1 per 1,000 ventilator days) as was the rate of catheter-associated urinary tract infection (5.5 vs 1.3 per 1,000 catheter days). Frequencies of resistance of Pseudomonas isolates to amikacin (42.8% vs 10%) and imipenem (42.4% vs 26.1%) and Klebsiella pneumoniae isolates to ceftazidime (71.2% vs 28.8%) and imipenem (19.6% vs 12.8%) were also higher in the INICC's ICUs compared with the ICUs of the CDC's NHSN. Abstract

The prognosis for relapsing or refractory neuroblastoma (NB) remains dismal, with a five-year disease-free survival of20%, and no effective salvage treatment has been identified so far. 131I-metaiodobenzylguanidine (131I-MIBG) has come to play an essential role in the imaging and therapy of NB over the past 30 years. The role of 131I-MIBG in the treatment of NB is continually expanding. 131I-MIBG treatment together with cumulative doses of other alkylating agents has potential serious late side effects such as myelodysplasia and leukemia, although rare. We describe a secondary acute myeloid leukemia case with complex karyotypic anomalies that included monosomy 5, monosomy 7 and translocation (1;10) in a child with relapsed NB who received therapeutic 131I-MIBG.

Abstract

Case report: Primary lymphoma of bone (PLB) is a rare malignant condition with lymphocytic infiltration of the bone; it accounts for 2–3% of all primary bone tumours in adults and children .Here we report a little girl with isolated PLB of B cell lineage focussing on diagnosis, evaluation and treatment strategy. Our case can help to get acquaintance with PBL,it should be taken into consideration as a different diagnosis for osteolytic lesions of bone.

Case report: Nasopharyngeal carcinoma is a rare tumor that accounts for 1-3% of all childhood malignancies. A 16-year-old patient with refractory nasopharyngeal carcinoma, whose treatment has to be interrupted due to COVID-19 positivity.After 6 weeks because of disease progression, we started his chemotherapy altough he is still COVID-19 positive. We didn't see any complication. Prolonged COVID-19 positivity is thought to be associated with the infection of immortal malignant cells located in the nasopharynx

Abstract

Case report: The majority of neuroblastomas are sporadic and not correlated with any specific constitutional germline chromosomal abnormality, inherited predisposition, or associated congenital anomalies. We report here a 1.5-year-old girl with a diagnosis of 21 hydroxylase deficiency and neuroblastoma. Neuroblastoma in a known case of congenital adrenal hyperplasia has rarely been reported. Based on our literature review, this is the fifth case report of congenital adrenal hyperplasia and neuroblastoma.
