Na de brand bij Chemie-Pack in 2011 startte op 1 september 2015 de bodemsanering van de ondergrond. In een bodemlaag van circa 2 meter in een gebied van 7 hectare is een cocktail aan stoffen aanwezig. De gekozen gefaseerde saneringsaanpak met per fase specifieke saneringsdoelen is het resultaat van een proces van onderzoek en karakterisatie van stoffen.

Als gevolg van de grote brand op 5 januari 2011 bij het chemicaliën opslag- en mengbedrijf Chemie-Pack kwam een grote hoeveelheid en variëteit aan chemicaliën vrij. Verspreiding over een oppervlakte van acht hectare vond onder andere plaats door het bluswater. De stoffen zijn in de bodem én in de omliggende sloten terecht gekomen. In T-cell acute lymphoblastic leukemia (T-ALL), transcription factors are known to be deregulated by chromosomal translocations, but mutations in protein tyrosine kinases have only rarely been identified(1-3). Here we describe the extrachromosomal (episomal)(4) amplification of ABL1 in 5 of 90 (5.6%) individuals with T-ALL, an aberration that is not detectable by conventional cytogenetics. Molecular analyses delineated the amplicon as a 500-kb region from chromosome band 9q34, containing the oncogenes ABL1 and NUP214 (refs.(5,6)). We identified a previously undescribed mechanism for activation of tyrosine kinases in cancer: the formation of episomes resulting in a fusion between NUP214 and ABL1. We detected the NUP214-ABL1 transcript in five individuals with the ABL1 amplification, in 5 of 85 (5.8%) additional individuals with T-ALL and in 3 of 22 T-ALL cell lines. The constitutively phosphorylated tyrosine kinase NUP214-ABL1 is sensitive to the tyrosine kinase inhibitor imatinib(7,8). The recurrent cryptic NUP214-ABL1 rearrangement is associated with increased HOX expression(1) and deletion of CDKN2A(9), consistent with a multistep pathogenesis of T-ALL. NUP214-ABL1 expression defines a new subgroup of individuals with T-ALL who could benefit from treatment with imatinib.

In de tweede helft van de jaren negentig heeft de Stichting Sanering Gasfabrieksterreinen (SSG) in Noord-brabant, opgericht en gefinancierd door de Provinciale Energiemaatschappij Noord-Brabant (PNEM, thans Essent), elf voormalige gasfabrieksterreinen overgenomen van de provincie, en via een clustergewijze en projectmatige aanpak versneld gesaneerd. Uitleg over de projectorganisatie, de financiering, en de saneringsdoelstellingen. Het ging om een voorloper van het 'functiegerichte saneren': via de 'Methodiek Rendementsverbetering Bodemsanering' werd het milieurendement van diverse saneringsvarianten beoordeeld op basis van vracht- en risicoreductie in relatie tot de saneringskosten en de gebruiksfunctie

Background: Despite advances in the treatment of primary limb osteosarcoma, the outcome of patients with primary metastatic and axial skeletal disease remains poor. The European Osteosarcoma Intergroup have assessed a combination chemotherapy regimen consisting of ifosfamide (IFOS) 3 g/m(2)/d1-2, doxorubicin (DOX) 25 mg/m(2)/d1-3 i.v. bolus and cisplatin (CDDP) 100 mg/m(2)/d1. Patients and methods: One hundred nine previously untreated patients with primary osteosarcoma were registered. Eligibility was confirmed in 103. At presentation, 45 eligible patients had metastatic disease, 15 axial skeletal primary tumours and 43 non-metastatic limb tumours. Results: The major toxicities were myelosuppression (90%, grade 3 or 4) and nausea and vomiting (74%, grade 3 or 4). Overall mean relative dose intensity (RDI) was 80% (88% CDDP, 75% IFOS, 81% DOX). Clinical response as measured by reduction in tumour volume occurred in 36% (95% confidence interval (95% CI): 27%-47%) of primary tumours. Response of pulmonary metastases to chemotherapy was seen in 33% (95% CI: 19%-49%). Good histological response (greater than or equal to 90% necrosis of the tumour) occurred in 33% (95% CI: 22%-45%) of resected tumours. Five-year survival was 62% in limb-non-metastatic, 41% in axial skeletal and 16% in limb metastatic patients. Conclusions: This regimen is active in osteosarcoma but does not appear to be more active than the two-drug CDDP-DOX regimen currently recommended by EOI.

Purpose: A total of 2,185 patients with advanced soft tissue sarcomas who had been treated in seven clinical trials investigating the use of doxorubicin- or epirubicin-containing regimens as first-line chemotherapy were studied in this prognostic-factor analysis. Patients and Methods:: Overall survival time (median, 51 weeks) and response to chemotherapy (26% complete response or partial response) were the two end paints. The cofactors were sex; age; performance status; prior therapies; the presence of locoregional or recurrent disease; lung, liver and bone metastases at the rime of entry onto the trial: long time period between the initial diagnosis of sarcoma and entry onto the study; and histologic type and grade. Results: Univariate analyses showed (a) a significant, favorable influence of good performance status, young age, and absence of river metastases on both survival time and response rate, (b) a significant, favorable influence of low histopathologic disease grade on survival time, despite a significantly lower response rate, (c) increased survival time for patients with a long time period between the initial diagnosis of sarcoma and entry onto the study, despite equivalent response rates, and (d) increased survival time with liposarcoma or synovial sarcoma, a decreased survival time with malignant fibrous histiocytoma, a lower response rare with leiomyosarcoma, and a higher response rate with liposarcoma (P < .05 for all log-rank and chi(2) tests). The Cox model selected good performance status (P < .0001), absence of liver metastases (P = .0001), low histopathologic grade (P = .0002), long time lapse since initial diagnosis (P = .0004), and young age (P = .0045) as favorable prognostic factors of survival time. The logistic model selected absence of liver metastases (P .0001), young age(P = .0024), high histopathologic grade (P = .0051), and liposarcoma (P = .0065) as favorable prognostic factors of response race. Purpose: Before recognizing mantle-cell lymphoma (MCL) as a distinct entity, these patients were grouped into low-grade (LG) or intermediate-/high-grade categories (IGHG) according to the Working Formulation and received various therapies. This was a unique opportunity to evaluate characteristics, behavior, response to treatment, and outcome of patients with MCL from two phase III trials conducted by the European Organization for the Research and Treatment of Cancer (EORTC): EORTC 20855 IGHG and EORTC 20856 LG. Patients and Methods: After histologic review, 64 diagnosed MCL patients (29 IGHG and 35 LG) were compared with other patients in their respective trials. In the IGHG group, patients received cyclophosphamide, doxorubicin, teniposide (VM(26)), prednisone, vincristine, and bleomycin (CHVmP-VB) or modified doxorubicin, cyclophosphamide, etoposide (VP16), mechlorethamine, vincristine, procarbazine, and prednisone (ProMACE-MOPP). in the LG group, after receiving cyclophosphamide, vincristine, and prednisone (CVP) induction, patients were randomized between maintenance treatment with interferon alfa-2a (IFN) or no further treatment. Results: MCL patients compared with IGHG subtypes showed a similar overall survival and response rate, but shorter duration of response and progression-free survival. Comparing with LG patients, their response rate, duration of response, and progression-free survival showed no difference, while their overall survival wets nearly twice shorter. MCL patients treated with CHVmP-VB had the longest survival. No treatment showed any significant improvement in terms of progression-free survival. Conclusion: These data confirm that MCL represents a clinicopathologic entity, In terms of survival, it behaves like IGHG subtypes, while in terms of progression-free survival, it behaves like LG lymphoma.
