Abstract Objective Effective time management is crucial for the survival of all patients, particularly those with cardiovascular conditions. This is especially true in the context of pre-hospital emergency services, where prompt intervention can significantly impact outcomes. This study delves into the timeliness of emergency services and the subsequent outcomes for hospitalized cardiovascular patients in EMS center in Fasa University of Medical Sciences, southern Iran. Results A total of 4972 emergency calls related to cardiac diagnoses were received between 2020 and 2023. The transport time was significantly correlated with age, location of the mission, and type of mission. Of the total, 86 underwent angioplasty within the standard time of less than 90 min, of which 81 were discharged and 5 died. 51 patients underwent angioplasty after more than 90 min, of which 47 were discharged and 4 died. In addition, 124 of these patients experienced cardiopulmonary resuscitation, of which 63 were successful and 61 were unsuccessful.

Abstract Cancer, a multifaceted and diverse ailment, presents formidable obstacles to traditional treatment modalities. Nanotechnology presents novel prospects for surmounting these challenges through its capacity to facilitate meticulous and regulated administration of therapeutic agents to malignant cells while concurrently modulating the immune system to combat neoplasms. Bacteria and their derivatives have emerged as highly versatile and multifunctional platforms for cancer nanotherapy within the realm of nanomaterials. This comprehensive review delves into the multifaceted and groundbreaking implementations of bacterial nanotechnology within cancer therapy. This review encompasses four primary facets: the utilization of bacteria as living conveyors of medicinal substances, the employment of bacterial components as agents that stimulate the immune system, the deployment of bacterial vectors as tools for delivering genetic material, and the development of bacteria-derived nano-drugs as intelligent nano-medications. Furthermore, we elucidate the merits and modalities of operation pertaining to these bacterial nano-systems, along with their capacity to synergize with other cutting-edge nanotechnologies, such as CRISPR-Cas systems. Additionally, we offer insightful viewpoints regarding the forthcoming trajectories and prospects within this expanding domain. It is our deduction that bacterial nanotechnology embodies a propitious and innovative paradigm in the realm of cancer therapy, which has the potential to provide numerous advantages and synergistic effects in enhancing the outcomes and quality of life for individuals afflicted with cancer.

Abstract Objective Asthma, a chronic inflammatory disease with diverse pathomechanisms, presents challenges in developing personalized diagnostic and therapeutic approaches. This review aims to provide a comprehensive overview of the role of exosomes, small extracellular vesicles, in asthma pathophysiology and explores their potential as diagnostic biomarkers and therapeutic tools. Methods A literature search was conducted to identify recent studies investigating the involvement of exosomes in asthma. The retrieved articles were analyzed to extract relevant information on the role of exosomes in maintaining lung microenvironment homeostasis, regulating inflammatory responses, and their diagnostic and therapeutic potential for asthma. Results Exosomes secreted by various cell types, have emerged as crucial mediators of intercellular communication in healthy and diseased conditions. Evidence suggest that exosomes play a significant role in maintaining lung microenvironment homeostasis and contribute to asthma pathogenesis by regulating inflammatory responses. Differential exosomal content between healthy individuals and asthmatics holds promise for the development of novel asthma biomarkers. Furthermore, exosomes secreted by immune and nonimmune cells, as well as those detected in biofluids, demonstrate potential in promoting or regulating immune responses, making them attractive candidates for designing new treatment strategies for inflammatory conditions such as asthma. Conclusion Exosomes, with their ability to modulate immune responses and deliver therapeutic cargo, offer potential as targeted therapeutic tools in asthma management. Further research and clinical trials are required to fully understand the mechanisms underlying exosome‐mediated effects and translate these findings into effective diagnostic and therapeutic strategies for asthma patients.

Community detection is crucial for analyzing the structure of social networks and extracting hidden information from them. The goal is to find groups of nodes (communities) with high intra-group and low inter-group communications. This problem is NP-hard, and most existing algorithms are global with high computational complexity, especially for large networks. Recently, local methods with acceptable computational complexity have been developed, but many have low accuracy and are non-deterministic. This paper introduces a new local algorithm, LCD-SN, which identifies communities based on first- and second-degree neighbor nodes. Unlike other local algorithms, LCD-SN is highly accurate, definitive, and not dependent on initial seed nodes. Additionally, a new index is proposed to determine the importance of network nodes using their local characteristics (first- and second-degree neighbors). Using this index, LCD-SN first identifies important nodes, forms initial communities with these nodes and their first-degree neighbors, and then obtains final communities through post-processing. Experiments show that LCD-SN is effective in identifying communities in social networks.

The Internet of Vehicles (IoV) represents a paradigm shift in vehicular communication, aiming to enhance traffic efficiency, safety, and the driving experience by leveraging interconnected vehicles. Despite its promise, the IoV faces challenges such as efficient task offloading, energy management, and data security. Mobile Edge Computing (MEC) emerges as a solution to some of these challenges by bringing computational resources closer to the vehicular network’s edge, yet it raises critical concerns regarding resource management, service continuity, and scalability in dynamic vehicular environments. Addressing both IoV and MEC challenges necessitates robust and dynamic optimization mechanisms. In response to these challenges, our study introduces a multi-objective approach using Double Deep Q-Networks (DDQN). This algorithm combines the strengths of Deep Neural Networks (DNNs) and Deep Learning (DL) techniques, enabling dynamic decision-making that can adapt to changing conditions. By considering multiple objectives, the DDQN algorithm allows for a sophisticated trade-off analysis, efficiently balancing between the different objectives to optimize overall system performance. Through the use of Blockchain technology, known for its secure, decentralized structure, our model enhances the integrity of data, providing a reliable and efficient solution for IoV-MEC systems. We conducted a comparative analysis of our model against the standard Deep Q-Network (DQN) and Deep Deterministic Policy Gradient (DDPG) algorithms, which are prevalent in this field. Our model demonstrated significant improvements over these traditional methods: energy consumption was reduced by 26.4%, latency decreased by 6.87%, and the cost was minimized by 7.41%.

Abstract This study proposes the use of vinasse, an inexpensive and readily available waste biopolymer, as a fundamental component of a waste culture medium that can enhance the effectiveness and cost-efficiency of the microbial-induced calcite precipitation (MICP) method for sustainable soil improvement. Vinasse enriched with urea, sodium caseinate, or whey protein concentrate is employed to optimize bacterial growth and urease activity of Sporosarcina pasteurii (S. pasteurii) bacterium. The best culture medium is analyzed using Taguchi design of experiments (TDOE) and statistical analysis, considering the concentration of vinasse and urea as effective parameters during growth time. To test the best culture medium for bio-treated soil, direct shear tests were performed on loose and bio-treated sand. The results demonstrate a substantial cost reduction from $0.455 to $0.005 per liter when using the new culture medium (vinasse and urea) compared to the conventional Nutrient Broth (NB) culture medium. Additionally, the new medium enhances soil shear strength, increasing the friction angle by 2.5 degrees and cohesion to 20.7 kPa compared to the conventional medium. Furthermore, the recycling of vinasse as a waste product can promote the progress of a circular economy and reduce environmental pollution. As ground improvement is essential for many construction projects, especially those that require high shear strength or are built on loose soil, this study provides a promising approach to achieving cost-effective and sustainable soil microbial improvement using enriched vinasse.
