Desde una aproximación a la escritura como práctica social, exploramos la construcción de identidades académicas en el proceso de elaboración de la tesis de pregrado en una universidad chilena. Se aplicó un cuestionario en línea y se realizaron entrevistas a estudiantes de cuatro áreas disciplinares. Los participantes reportan que el proceso de tesis significa un aprendizaje enriquecedor, pero también lo describen como un proceso solitario y poco motivante. Además, observamos tres formas de negociación identitaria que facilitaron u obstruyeron el involucramiento y la motivación hacia la tesis: la negociación de trayectorias diversas, de una voz propia y del campo disciplinar. A partir de estos hallazgos, se sugieren recomendaciones para orientar el desarrollo de programas de acompañamiento a la escritura de tesis. Finalmente, este estudio da cuenta de formas de negociación identitaria que juegan un rol central en el involucramiento de los estudiantes en su proceso de tesis.

PurposesMost molecular-based published studies on breast cancer do not adequately represent the unique and diverse genetic admixture of the Latin American population. Searching for similarities and differences in molecular pathways associated with these tumors and evaluating its impact on prognosis may help to select better therapeutic approaches.Patients and MethodsWe collected clinical, pathological, and transcriptomic data of a multi-country Latin American cohort of 1,071 stage II-III breast cancer patients of the Molecular Profile of Breast Cancer Study (MPBCS) cohort. The 5-year prognostic ability of intrinsic (transcriptomic-based) PAM50 and immunohistochemical classifications, both at the cancer-specific (OSC) and disease-free survival (DFS) stages, was compared. Pathway analyses (GSEA, GSVA and MetaCore) were performed to explore differences among intrinsic subtypes.ResultsPAM50 classification of the MPBCS cohort defined 42·6% of tumors as LumA, 21·3% as LumB, 13·3% as HER2E and 16·6% as Basal. Both OSC and DFS for LumA tumors were significantly better than for other subtypes, while Basal tumors had the worst prognosis. While the prognostic power of traditional subtypes calculated with hormone receptors (HR), HER2 and Ki67 determinations showed an acceptable performance, PAM50-derived risk of recurrence best discriminated low, intermediate and high-risk groups. Transcriptomic pathway analysis showed high proliferation (i.e. cell cycle control and DNA damage repair) associated with LumB, HER2E and Basal tumors, and a strong dependency on the estrogen pathway for LumA. Terms related to both innate and adaptive immune responses were seen predominantly upregulated in Basal tumors, and, to a lesser extent, in HER2E, with respect to LumA and B tumors.ConclusionsThis is the first study that assesses molecular features at the transcriptomic level in a multicountry Latin American breast cancer patient cohort. Hormone-related and proliferation pathways that predominate in PAM50 and other breast cancer molecular classifications are also the main tumor-driving mechanisms in this cohort and have prognostic power. The immune-related features seen in the most aggressive subtypes may pave the way for therapeutic approaches not yet disseminated in Latin America.Clinical Trial RegistrationClinicalTrials.gov (Identifier: NCT02326857).

Molecular profile of breast cancer in Latin-American women was studied in five countries: Argentina, Brazil, Chile, Mexico, and Uruguay. Data about socioeconomic characteristics, risk factors, prognostic factors, and molecular subtypes were described, and the 60-month overall cumulative survival probabilities (OS) were estimated. From 2011 to 2013, 1,300 eligible Latin-American women 18 years or older, with a diagnosis of breast cancer in clinical stage II or III, and performance status ≦̸1 were invited to participate in a prospective cohort study. Face-to-face interviews were conducted, and clinical and outcome data, including death, were extracted from medical records. Unadjusted associations were evaluated by Chi-squared and Fisher’s exact tests and the OS by Kaplan–Meier method. Log-rank test was used to determine differences between cumulative probability curves. Multivariable adjustment was carried out by entering potential confounders in the Cox regression model. The OS at 60 months was 83.9%. Multivariable-adjusted death hazard differences were found for women living in Argentina (2.27), Chile (1.95), and Uruguay (2.42) compared with Mexican women, for older (≥60 years) (1.84) compared with younger (≤40 years) women, for basal-like subtype (5.8), luminal B (2.43), and HER2-enriched (2.52) compared with luminal A subtype, and for tumor clinical stages IIB (1.91), IIIA (3.54), and IIIB (3.94) compared with stage IIA women. OS was associated with country of residence, PAM50 intrinsic subtype, age, and tumor stage at diagnosis. While the latter is known to be influenced by access to care, including cancer screening, timely diagnosis and treatment, including access to more effective treatment protocols, it may also influence epigenetic changes that, potentially, impact molecular subtypes. Data derived from heretofore understudied populations with unique geographic ancestry and sociocultural experiences are critical to furthering our understanding of this complexity.

5. How do students write in engineering and the humanities? Intertextuality and metadiscourse in undergraduate dissertations written in Spanish

This article aims to contrast metadiscourse and intertextuality in 40 undergraduate dissertations written in Spanish in engineering and the humanities at a Chilean university. Results show that guidance on dissertations’ goals and stages is common across disciplines, especially in introductions, although engineering signals goals more often. All students graduate statements, especially in conclusions, but the frequency of graduation doubles in the humanities. Humanities students prefer hedging over boosting, while boosting is more common in engineering. Self-mentions, especially plural authorial, are frequent in the humanities but do not occur in engineering. Citations are five times more frequent and usually integral in the humanities, while engineering only uses non-integral citations. Indirect speech predominates across disciplines, but direct and mixed speech are also relatively common in the humanities. La displasia broncopulmonar (DBP) es la enfermedad crónica más frecuente del recién nacido prematuro. Los avances en su prevención y tratamiento han permitido una mayor sobrevida de prematuros más pequeños, pero su incidencia se ha mantenido estable en el tiempo, con una fisiopatología y presentación clínica que abarca un amplio espectro y que difiere de la DBP descrita originalmente hace más de 50 años. Aún existen controversias en su definición, la que se ha establecido en base al tratamiento, específicamente al requerimiento de soporte respiratorio. Las definiciones más utilizadas son el requerimiento de oxígeno por 28 días y a las 36 semanas de edad gestacional corregida (EGC). Recientemente se ha propuesto definirla en base al requerimiento de ventilación mecánica a las 36 semanas de EGC, lo que identificaría a los prematuros con DBP más grave y mayor probabilidad de complicaciones respiratorias y neurológicas en los 2 primeros años de vida. Nuestro objetivo en la comisión de Neo-SOCHINEP es el de recomendar la definición y clasificación que nos parece más adecuada para identificar a los prematuros portadores de DBP, considerando los aspectos fisiopatológicos, del compromiso de la función pulmonar y consecuencias prácticas de la definición en nuestro medio. También proponemos la definición del requerimiento de oxígeno en el prematuro cuando esta en neonatología, las condiciones e interpretación de la saturometría contínua cuando está pronto al alta y el seguimiento de la oxigenoterapia posterior al alta.

10. An Infant with Persistent Respiratory Failure Associated with Refractory Pulmonary Hypertension: Pulmonary Interstitial Glycogenosis

Pulmonary interstitial glycogenosis (PIG) is a disease of unknown etiology. It is part of the interstitial lung diseases, corresponding to the compartment of the fetal pulmonary interstitium. It typically presents within the first week of life as refractory respiratory distress with tachypnea and persistent hypoxemia, and it is not associated with glycogen deposition in other organs. Usually, there is a clinical improvement and good prognosis after steroid therapy unless there are associated conditions such as congenital heart disease, pulmonary hypertension, or genetic disorders. We report a case diagnosed by lung biopsy at 4 months of age in a male preterm born, small for gestational age infant, who developed refractory hypoxemia and pulmonary hypertension with fatal outcome. There was no response to steroids and hydroxychloroquine. He was not candidate for extracorporeal membrane oxygenation. PIG should be considered in the differential diagnosis of persistent respiratory distress and hypoxemia despite standard treatment, even after the first month of life.

Un Evento de Aparente Amenaza de la Vida o ALTE (del inglés: Apparent Life Threatening Event) corresponde a un episodio agudo que ante los ojos del observador pone en riesgo la vida de un lactante menor de 1 año. Debe presentar la combinación de 2 o más de los criterios siguientes: apnea, cambio de color, alteración del tono, atoro e implicar la necesidad de algún tipo de maniobras para reanimar. En el presente consenso sobre el manejo de un ALTE se revisaron la evidencia internacional y nacional respecto al enfoque diagnóstico, estudio etiológico, criterios y duración de hospitalización y las indicaciones de monitorización domiciliaria.

- Abstract
La contaminación ambiental afecta a más del 90% de la población mundial, y es considerado un factor de morbi-mortalidad respiratoria desde mediados de 1900. Sin embargo, al día de hoy las políticas públicas no han logrado mejorar en forma sustancial la calidad del aire. Siendo causante de más de 3.7 millones de muertes anuales según datos OMS. Los niños son los principales afectados debido a diferencias anatómicas, fisiológicas y de estilos de vida. Dentro de las complicaciones más frecuentes se encuentran las infecciones respiratorias, pero además puede verse afectado el desarrollo neurológico, patologías oncológicas, enfermedades crónicas (diabetes y asma), alteraciones cardiovasculares, malformaciones congénitas y salud mental entre otros. En el siguiente artículo se revisará la evidencia científica en relación a la contaminación ambiental, como afecta al sistema inmune y el microbioma, generando un aumento en las infecciones respiratorias de los niños.

14. How do students write in engineering and the humanities? Intertextuality and metadiscourse in undergraduate dissertations written in Spanish

This article aims to contrast metadiscourse and intertextuality in 40 undergraduate dissertations written in Spanish in engineering and the humanities at a Chilean university. Results show that guidance on dissertations’ goals and stages is common across disciplines, especially in introductions, although engineering signals goals more often. All students graduate statements, especially in conclusions, but the frequency of graduation doubles in the humanities. Humanities students prefer hedging over boosting, while boosting is more common in engineering. Self-mentions, especially plural authorial, are frequent in the humanities but do not occur in engineering. Citations are five times more frequent and usually integral in the humanities, while engineering only uses non-integral citations. Indirect speech predominates across disciplines, but direct and mixed speech are also relatively common in the humanities. This study can help to understand undergraduate students’ authorial voices written in Spanish, depict discipline-specific writing choices, and supply data for writing instruction initiatives.

15. Valores de Oximetría de Pulso en Prematuros a las 34-36 semanas post-concepcional

La oximetría de pulso (SpO2) es fundamental para orientar la oxigenoterapia en prematuros. Existen escasas publicaciones que muestren valores de SpO2 previo al alta. Objetivo: Obtener valores de SpO2 en prematuros asintomáticos a las 34, 35 y 36 semanas post-concepcionales. Sujetos y Método: Estudio longitudinal multicéntrico realizado entre mayo 2018 y mayo 2019, en tres unidades de neonatología de Santiago, Chile (altura 579 m). Se incluyeron recién nacidos ≤ 32 semanas de edad gestacional, clínicamente estables, sin morbilidad respiratoria al momento de la evaluación y hasta el alta. Se analizaron los siguientes parámetros: SpO2 y DS acumulados medios, valor mínimo de SpO2, porcentaje de tiempo de SpO2 < 90%, porcentaje de tiempo de SpO2 < 80%, ID4 e ID80. Se obtuvo SpO2 continua nocturna con oxímetro Masimo Radical-7 o Radical-8 (USA), tiempo válido de registro (TVR) ≥ 6 horas. Resultados: se obtuvieron 101 registros de SpO2: 44, 33 y 24 estudios a las 34, 35 y 36 semanas post-concepcionales respectivamente. Estos pertenecían a 62 prematuros, 28 (45%) varones, mediana (rango) de edad gestacional al nacer 30 (26-32) semanas, mediana (rango) peso al nacer 1480 (785-2700) g. Variables de oximetría: TVR, media (± DE) 8,6 (± 1,5) horas; SpO2 promedio, mediana (rango) 96,9% (93,3-99,3%); SpO2 mínima, mediana (rango) 74% (51-89%); tiempo de SpO2 < 90%, mediana (rango) 2% (0-10,6%); tiempo de SpO2 < 80%, mediana (rango) 0,1% (0-1,3%); evento de desaturación ≥ 4% (ID4) ≥ 10 segundos por hora muestreada, mediana (rango) 15 (3,5–62,5); evento de desaturación < 80% (ID80), mediana (rango) 0,58 (0-10,8). No hubo diferencias entre valores de SpO2 entre las distintas semanas post-concepcionales. Conclusiones: Describimos valores de SpO2 en prematuros extremos asintomáticos a las 34-36 semanas post- concepcionales. Estos valores podrían utilizarse como referencia para orientar oxigenoterapia.

In the pediatric emergency department, dehydrated children are one of the most frequent causes for consultation, however, the coexistence of hyponatremia with hypochloremia and metabolic alkalosis is rare. The presence of metabolic alkalosis due to chloride depletion has been reported as a form of presentation of Cystic Fibrosis (CF).to describe a case of cystic fibrosis of unusual presen tation in a pediatric patient.we report a 3-month-old previously healthy male infant who presented with internal environment abnormalities consisting of metabolic alkalosis, hypona tremia, hypokalemia, and extreme hypochloremia associated with septic shock due to mixed viral- bacterial pneumonia (Rhino/enterovirus, Streptococcus pneumoniae, and Staphylococcus aureus). Cys tic fibrosis (CF) was suspected, thus the diagnosis was corroborated by sweat test and genetic study which showed the pathogenic variants c.2834CT (p.Ser945Leu) and c.3484CT (p.Arg1162X), both heterozygous.special attention should be paid to the existence of hypochloremia with metabolic alkalosis and hyponatremia associated or not with pulmonary disease, suspecting CF as the first option. This consideration becomes more relevant in those countries where the neonatal screening test is not widely available.

Background: Oxygen supplementation is an important component for preterm infants neonatal care. Pulse oximetry (SpO2) is essential to guide oxygen therapy. Evidence on SpO2 values in premature infants previous to discharge is limited. Objectives: To establish SpO2 values in asymptomatic premature infants at 34, 35, and 36 weeks postmenstrual age (PMA). Methods: Longitudinal, multicentric study. From May 2018 to May 2019 premature infants born ≤32 weeks gestational age, from three level III NICUs in Santiago, Chile (altitude 579mt), were enrolled. Healthy children without current apnea of prematurity were included. Continuous SpO2 was obtained with Masimo-Radical 7/8 (USA), averaging time 2-4 seconds. Results: 101 SpO2 recordings (n = 44, 33 and 24 at 34, 35 and 36 weeks PMA respectively) from 62 infants. Twenty eight (45%) male, median (range) gestational age at birth 30 (26-32) weeks, median (range) birth weight 1480 (785-2700) g. Cada vez es más frecuente la atención médica en la Unidad de Cuidados Intensivos (UCI) de niños o adolescentes inmigrantes como también de aquellos nacidos en nuestro país con padres en tal condición. Esto ha ocasionado, en la actualidad, que el equipo de salud se deba enfrentar con problemas diagnósticos derivados del escaso conocimiento de condiciones genéticas propias de esta población y/o el desarrollo de diversas patologías infrecuentes en nuestro país, algunas resultantes de su condición sanitaria. En esta revisión se abordan diversos aspectos de la patología hematológica, infecciosa, parasitaria, respiratoria y cardiovascular, todos tópicos relevantes de conocer durante su estadía en la UCI. Es un deber del equipo de salud actualizarse sobre patologías de baja prevalencia en nuestro país, algunas de ellas muy poco conocidas hasta hace una década, pero que, actualmente, están cada vez más presentes en las UCI del sistema de salud público chileno.

La deshidratación es una causa frecuente de consulta en emergencia pediátrica, siendo inhabitual la coexistencia de alcalosis metabólica hipoclorémica y deshidratación hiponatrémica. La presencia de alcalosis metabólica por depleción de cloro se ha reportado como una forma de presentación de Fibrosis Quística (FQ). Objetivo: Describir una forma de presentación inhabitual de fibrosis quística. Caso Clínico: Lactante masculino de 3 meses de edad quien presentó anormalidades del medio interno consistentes en alcalosis metabólica, hiponatremia, hipokalemia e hipocloremia extrema asociadas a shock séptico secundario a etiología mixta (Rhino/enterovirus, Streptoccus pneumoniae y Staphyloccus aureus). Se sospechó FQ, corroborándose el diagnóstico con medición de electrolitos en sudor y estudio genético, donde se evidenciaron dos variantes patogénicas; S945L (c.2834C>T o p.Ser945Leu) y R1162X (c.3484C>T o p.Arg1162X), ambas heterocigotas. Conclusión: Se debe prestar especial atención ante la existencia de alcalosis metabólica hipoclorémica y deshidratación hiponatrémica asociado o no a compromiso pulmonar, sospechando FQ como primera opción. Esta consideración toma mayor relevancia en aquellos países donde el test de tamizaje neonatal no está ampliamente disponible.

It is increasingly common to provide medical care in the Intensive Care Unit (ICU) for immigrant children and adolescents as well as those born in Chile with parents in such condition. Currently, this has caused that the health team has to face diverse infrequent pathologies in our country and/ or diagnostic problems derive from the poor knowledge of genetic conditions of this population, some resulting from their health conditions. This review addresses several aspects of hematological, infectious, parasitic, respiratory, and cardiovascular pathologies, all relevant topics to know during their stay in the ICU. It is a duty of the health team to be updated on pathologies of low prevalence in our country, some of them very little known until a decade ago, but which are currently increasingly present in the ICUs of the Chilean public health system.

El síndrome de Down es la alteración cromosómica más frecuente en los recién nacidos, con una alta incidencia en Chile. Esta condición presenta aspectos fisiológicos únicos, los cuales pueden afectar al niño durante su estadía en una Unidad de Cuidados Intensivos, posterior al período neonatal. En esta revisión abordamos aspectos actuales de la patología respiratoria, cardiovascular, infecciosa y neurológica, así como también consideraciones anestésicas y de analgesia postoperatoria, destete de la ventilación mecánica, inestabilidad columna cervical y pronóstico del niño críticamente enfermo portador de síndrome de Down. La evaluación de todas estas condiciones debe ser realizada cuando el paciente es ingresado a la Unidad de Cuidados Intensivos. El objetivo de la presente actualización es profundizar el conocimiento del diagnóstico y tratamiento de las potenciales complicaciones del niño con síndrome de Down durante su estadía en la unidad de paciente crítico.

The aim is to analyse the different stages and problems through which the initial formation of teachers has passed until the creation of University Education Departments, profound institutional transformations framed in a particular socio-political context in each historical moment, that glimpses the progress and setbacks, the ups and downs of the initial formation of the teachers in Spain for over two centuries of existence. It is under study the innovative nature of the reforms, their professional nature and their educational repercussions, as well as the identity of the teachers corresponding to this level of education and the image of the prospective students who project their expectations and motivations in an imprecise and blurred professional trajectory.

Down syndrome is the most common chromosomal abnormality in newborns, with a high incidence in Chile. This condition presents unique physiological aspects that should be known, which can affect the child during their stay in an Intensive Care Unit, beyond the neonatal period This review is focused on the respiratory, cardiovascular, infectious and neurological disorders. Anesthetic management and postoperative analgesia considerations, weaning from mechanical ventilation, cervical spine instability and prognosis of the critically ill child with Down syndrome are also analyzed. The evaluation of these conditions should be performed when the patient is admitted to the intensive care unit. The purpose of this update is to update the knowledge of the diagnosis and treatment of potential complications of children with Down syndrome during their stay in the unit of critical patient.

We have determined different patterns of fissurization in Broca's area, the gyrus of Heschl, the planum temporale, the inferior parietal lobe, and the cingulate sulcus. Such patterns were asymmetrically distributed, indicating increased folding on the left side in most cases. More folding can sometimes be related to a larger cortical area, resulting in increased processing capacities in the respective brain region. Furthermore, the brain regions associated with the asymmetrical sulci are involved in lateralized functions. Of special interest are the asymmetries observed in regions corresponding to the inferior parietal lobe (the accessory postcentral sulcus and the intraparietal sulcus), which, according to recent studies, is involved in linguistic working memory. We did not detect a tendency of distinct fissurization patterns in a given brain region to be associated with specific patterns in other fissures, indicating that the different fissure types develop independently in each brain region and can therefore be determined by local processes. These descriptions are of relevance to imaging studies that intend to establish correspondences between gross morphology and functional parameters such as behavior and brain activation.

Adenovirus infection in a children´s hospital with chronic pulmonary diseases Introduction: Intrahospital adenovirus infections spread rapidly in closed environments causing outbreaks associated with high morbidity and mortality. Objectives: To identify the attack rate (AR) of adenovirus in a hospital treating children with Chronic Respiratory Diseases (CRD) and to evaluate associated factors that facilitate infection. Patients and Methods: Fifty children with CRD were evaluated between June 2010 and October 2010 at Josefina Martinez Hospital. They were exposed to patients with acute lower respiratory infection during winter. Patients with CRD symptoms (fever and change in basal nasal mucus) underwent Viral Direct Immunofluorescence (DIF). The case was defined as "symptomatic patient with positive DIF for Adenovirus". Primary, secondary and global ARs were evaluated. Variables such as gender, age, tracheostomy and gastrostomy were analyzed looking for associations with the cases. Results: 44 % of patients were infants; 68% of children had tracheostomy and 54 % had gastrostomy. The first patient with Adenovirus was admitted in June and the primary case was reported two weeks later. 25 cases presented secondary AR corresponding to 51%. Global AR was 52% overall. No mortality was reported. The risk was three times higher in infants than older children (OR 3.31 [IC95% 1.02-10.72; p = 0.046]). No significant associations with tracheostomy, gastrostomy and gender were found. Introduccion: El Programa Chileno de Ventilacion No Invasiva en domicilio (AVNI) extendio su cobertura utilizando generadores de flujo con presion bi-nivelada (BiPAP) en ninos con ventilacion mecanica prolongada (VMP) y traqueostomia (TQT). Objetivo: Reportar la experiencia de esta estrategia describiendo criterios de seleccion, modalidades de uso y tecnologias complementarias. Pacientes y Metodos: Estudio descriptivo, transversal y prospectivo durante un ano desde Junio 2006, en 20 pacientes manejados en el Hospital Josefina Martinez, otros centros de la red asistencial del Ministerio de Salud y en domicilio. Se registraron las caracteristicas clinicas, modos ventilatorios, evolucion y complicaciones. Resultados: La mediana de edad fue de 3,5 anos (rango 3 meses a 17 anos). La duracion de la VMP fue lm a 5a, 6 pacientes (30%) se encuentran en domicilio y 14 (70%) hospitalizados. La principal causa para VMP fue enfermedad neuromuscular (14, 70%). No hubo mortalidad y 4 pacientes tuvieron complicaciones menores. Conclusion: El BiPAP a traves de TQT, usado con criterios estrictos de seleccion, es un metodo de VMP que puede ser factible. Se requieren estudios comparativos para definir costos, beneficios y riesgos de estos equipos comparandolos con ventiladores licenciados para soporte vital.
