4. Prevalence of ruptured small and very small aneurysms: a retrospective single-centre study.

Abstract

Aim of study. To retrospectively assess the occurrence and consequences of subarachnoid haemorrhages (SAH) caused by ruptured intracranial aneurysms (RIA), particularly focusing on the treatment outcomes of small aneurysms treated with either endovascular embolisation or surgical intervention. Material and methods. We retrospectively analysed data from 408 patients (144 males and 264 females) who were hospitalised between 2013 and 2022 at the Department of Neurosurgery and Neurology in University Hospital Nr 2 in Bydgoszcz, Poland. Clinical conditions at admission, assessed using the Glasgow Coma Scale, Hunt-Hess scale (H-H), modified Rankin scale (mRS), as well as age and sex, were recorded. Additionally, aneurysm data including size, localisation, and the method and outcome of endovascular or surgical treatment were examined. Results. Among the 408 patients hospitalised due to SAH, the most common localisation of the 375 RIAs was the anterior communicating artery (AcomA) complex (111 cases, 29.6%), followed by the medial cerebral artery (MCA) (95 cases, 25.3%), internal carotid artery (ICA) (94 cases, 25%), and the vertebrobasilar complex (consisting of the basilar artery (BA) -- 25 cases, vertebral artery (VA) -- 13 cases, anterior inferior cerebellar artery (AICA) -- one case, and posterior inferior cerebellar artery (PICA) -- four cases), which accounted for 43 SAH cases (11.46%). In 33 cases, neither RIA nor a haemorrhage source was identified, or arteriography showed no cerebral arteries contrast flow. Among the examined group of 375 RIAs, 45 (12%) were microaneurysms (≤ 3 mm), 35 (9%) were small aneurysms (3 ≤ 5 mm), 89 (24%) were medium-sized (5 ≤ 7 mm), and 151 (40%) were large aneurysms (> 7 mm), serving as the source of SAH. A better outcome was significantly associated with lower initial H-H grade (p < 0.001), higher GCS (p < 0.001), lower mRS at admission (p < 0.001), younger age (p < 0.001), smaller size (≤ 3 mm) (p < 0.001), and endovascular treatment (p < 0.001). Conclusions. In this series, over 21% of patients suffered from SAH resulting from ruptured small aneurysms (≤ 5 mm), with 12% specifically attributed to ruptured very small aneurysms (≤ 3 mm), despite large aneurysms (> 7 mm) being the most prevalent source of bleeding in 40% of cases. A worse prognosis was primarily associated with the severity of SAH, reflected in poorer clinical status at admission and older age. Abstract

Bilateral cingulotomy is a procedure applied to patients with obsessive–compulsive disorder (OCD). This report presents the structural changes occurring within the forceps minor and arcuate fascicles nerve fibers after a successful bilateral anterior cingulotomy in the patient with refractory OCD. Cingulotomy mainly affects the values of FA, MD, and ADC in the treatment of the examined nerve bundles. This structural reorganization coexists with a good clinical effect. However, it is necessary to expand the study group and to investigate the correlation between the parameters of diffusion and anisotropy and the patient's clinical condition (Y-BOCS scale). Abstract

Background: For about 20 years we have been observing the development of three-dimensional (3D) printing. The aim of this study was to systematize the current knowledge on the use of 3D printing technology in neurosurgery and to attempt an outline the future paths of its development. Material and methods: The analysis was based on English-language literature from 2017-2021 indexed in the Mendeley and Scopus databases. Results: The application of 3D printing in neurosurgery concerns: 1) teaching students, 2) training of residents neurosurgeons, 3) individualized surgery planning, 4) dedicated cranial and spinal implants, 5) the future of 3D printing in neurosurgical implantology. There were 5 main neurosurgery subtopics in which 3D printing was used: "vascular neurosurgery" (31%), "skull, cranial neurosurgery" (22.4%), "neuro-oncology" (19.3%), "spine" (14.3%) and "others" (13%). The number of published articles has been steadily increasing by 11-33% annually. Conclusion: 3D printing has an enormous potential for clinical use and in the we will continue to observe its dynamic development. In neurosurgery 3D prints are currently most commonly used for didactic purposes as detailed anatomical models, for training residents and young surgeons and by specialists for the simulation of complex or innovative surgeries. The future of the use of additive 3D printing in neurosurgery lies in the biological 3D printing, the creation of artificial organs and the development of biological implants in tissue engineering. 44. Tibial nerve stimulation with a miniature, wireless stimulator in chronic peripheral neuropathic pain.

Abstract

Peripheral neuropathic pain (PNP) and complex regional pain syndrome (CRPS) can be effectively treated with peripheral nerve stimulation. In this clinical trial report, effectiveness of novel, miniature, wirelessly controlled microstimulator of tibial nerve in PNP and CRPS was evaluated. In this pilot study the average preoperative visual analog scale (VAS) score in six patients was 7.5, with 1, 3 and 6 months: 2.6 (p=0.03), 1.6 (p=0.03), and 1.3 (p=0.02), respectively. The mean average score in the six patients a week preceding the baseline visit was 7.96, preceding the 1, 3 and 6 month visits: 3.32 (p=0.043), 3.65 (p=0.045), and 2.49 (p=0.002), respectively. The average short-form McGill pain score before surgery was 23.8, and after 1, 3 and 6 months it was 11.0 (p=0.45), 6.3 (p=0.043), and 4.5 (p=0.01), respectively. Applied therapy caused a reduction of pain immediately after its application and clinical improvement was sustained on a similar level in all patients for six months. No complications of the treatment were observed. Intermittent tibial nerve stimulation by using a novel, miniature, wirelessly controlled device can be effective and feasible in PNP and CRPS. It is a safe, minimally invasive, and convenient neuromodulative method. [ABSTRACT FROM AUTHOR]
