Tonsilloliths, also known as tonsil stones or tonsillar calculi are clusters of calcified material that form in crypts of palatine tonsil. They are usually of small size. Large or giant tonsilloliths are rare. We report a case of giant tonsillolith in left palatine tonsil and literature is reviewed. Keywords: Tonsillolith, Tonsil stone.

To compare surgical outcomes with different meatoplasty techniques without removal of a cartilage piece in canal wall down mastoidectomy. Total 61 patients of canal wall down mastoidectomy included in the study where either inferior based flap technique or division in middle technique meatoplasty performed and 2 groups formed. Group A consisted of 33 patients and the inferior based flap technique of meatoplasty used in these patients. Group B consisted of 28 patients and the division in middle technique of meatoplasty used in these patients. Granulations, discharge or stenosis of canal were observed in less than 8% of cases in both the groups. Meatoplasty done without incision or excision of a piece of cartilage from pinna can be achieved with good success rates with either inferiorly based flap technique or division in middle technique.

The current study aims at observing the technique and results of doing lateral tucking during tympanoplasty surgery. It is intended to provide additional support to the graft and prevent displacement as well. 98 cases included in the study and the graft uptake rate of 97.96% noted. Average pre operative air bone gap (ABG) was 31.12 db and average post operative ABG observed to be 14.01 db. Gain of 17.11 ± 7 db achieved by the procedure. Lateral tucking is a concept that requires to be inculcated in routine tympanoplasty surgeries, especially where canal skin incisions are used. It is easy to practice and the results are good and complications are minimal.

The current study compares outcomes of modified cartilage shield tympanoplasty (CST) with temporalis fascia tympanoplasty in type I procedures in Indian patients. Graft uptake rates are better with the CST technique and hearing results are almost equivalent with both techniques except at 8000 Hz where improvement in hearing was found better with the use of temporalis fascia alone. The CST technique used in the study is unique.

The current study aims at observing effects of sacrificing the tensor tympani tendon when manubrium of malleus is foreshortened or retracted on graft uptake, hearing improvement, and occurrence of complications if any during type I tympanoplasty surgery for central perforations. 42 patients were included in group A where the tensor tendon was sectioned and 42 patients were included in group B where the tensor tympani tendon was retained and kept intact. Graft uptake rates are very good in both groups but hearing improvement was found significantly better in group A than group B. No unusual or undesired complications were seen in any of the cases. Sectioning of tensor tympani tendon is safe and effective procedure in cases where manubrium is foreshortened.

To compare the results, in terms of graft uptake and hearing improvement, of modified cartilage shield technique of tympanoplasty using either partial thickness tragal cartilage or full thickness tragal cartilage for type I procedures. 35 patients were included in group A where a partial thickness tragal cartilage was used and 27 patients included in group B where a full thickness tragal cartilage was used for modified cartilage shield tympanoplasty. Audiometry done at 4 months after surgery and the results compared. The graft take up rate for both these techniques is excellent. There had been no statistically significant difference in hearing gain between these two groups, except at 4,000 Hz, where hearing gain had been more in group A than group B with P value being 0.027. The modified cartilage shield tympanoplasty is a good technique for closure of tympanic membrane perforations. Hearing gain is very much similar between thin and thick cartilage groups, except at 4,000 Hz.

Aim and Objective: To study various erosion patterns related to incus and observe results of various methods of ossiculoplasty using autologous incus or cartilage. Materials and Methods: A total 82 cases found suitable for the study having some abnormalities of incus with inclusion criteria being presence of intact and normal malleus and stapes and where canal wall preserved. Cases were divided into four groups with group A having minimum erosion of lenticular process and group D includes those cases where incus found absent. Results: Appropriate methods of ossiculoplasty performed using remnant of incus or tragal cartilage and hearing results compared. More than 75% of cases had a postoperative air-bone gap (ABG)

Aim and Objective: To compare surgical outcomes with different canal wall incisions used in common practice for tympanoplasty for large perforations. Materials and Methods: Totally 88 patients included in the study and three groups formed. Group A consisted of 35 patients and the vascular strip incision with anterior tucking (VSAT) technique used in these patients. Group B consisted of 31 patients and the tympanometal flap with anterior tucking (TMFAT) technique used in these patients. Group C had 22 patients and the near full cuff (NFC) technique used in them. Results: Success rate of 97.15% observed for Group A, 93.55% observed for Group B and 86.36% observed for Group C patients. Most of the patients in Group A had healing time

Aim: Use of cartilage for repair of tympanic membrane is recommended by many otologists. The current study aims at evaluating results of cartilage shield tympanoplasty in terms of graft take up and hearing outcomes. Material and Methods: In the current study, cartilage shield tympanoplasty(CST) is used in ears with high risk perforations of the tympanic membrane. A total of 40 ears were selected where type I CST was done in 30 ears and type III CST was done in 10 ears. Results: An average of 37.08 dB air bone gap(ABG) was present in pre operative time and an average of 19.15 dB of ABG was observed at 6 months after the surgery with hearing gain of 17.28 dB on average was observed. Graft take up rate of 97.5% was observed. The technique is modified to make it easier and to minimize chances of lateralization of graft. Conclusion: The hearing results of this technique are comparable to other methods of tympanic membrane repair.
