A transition towards a circular economy is a challenge. It is vital to know that circularity and sustainability are two different segments. So, circular economy can only be achieved in the long-term perspective. This study investigates accounting and accountability for circular economy and waste. Considering these principles and based on a critical review of the literature, economic gains from the transition toward a circular economy are measurable; the problems for corporations and governments are diverse; the way to handle the stakeholders who are losing control in the circular economy is considerable. Diffusion of innovation theory is used to conduct this study. It is essential that an organisational design built should help implement the circular model. Targeted questions responded by adopting a systematic literature review approach by applying PRISMA protocol. This study examines 78 publications in English between 2012 and 2021, which present a map of the circular economy-related knowledge published in web of science and Scopus. Besides, this study includes 03 European Commission reports, 01 Ellen Macarthur Foundation report, 01 Council for the Environment and Infrastructure report, 01 report from SUN IZA, 01 UN Global Compact and 01 the Brundtland Commission report. The results highlight how circular economy, waste management, sustainability, accountability, and management accounting practices help to develop an ecosystem and achieve sustainable development goals of the United Nations 2030 Agenda. Theoretical and practical implications are discussed.

This paper investigates how knowledge hiding (KH) contributes to individuals, groups, and the business processes of organizations, with regards to improving employee performance, strategic performance, and the organization’s overall knowledge management system (KMS), as well as the consequences and costs of KH in organizations. 117 English language research articles produced between 1988 and 2020 regarding KH are analyzed, and insights provided into science mapping and performance analysis of KH studies, by drawing evidence from publication activities, prominent themes, citation trends, and collaborations amongst contributors. The findings reveal that KH research has mainly focused on KH behavior, knowledge sharing (KS), and the KMS. Firms with KH practices are responsible for challenging employee creativity, motivation, and workplace environment. This study will help business managers and leaders hone cooperative behavior to achieve an innovative environment and desired goals. Knowledge hiding also has positive implications, where leaders may have to hide confidential information from external elements or internal subordinates to protect the enterprise's sovereignty and integrity.

Background: Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder that results from regurgitation of acid from the stomach into the esophagus. Treatment available for GERD includes lifestyle changes, antacids, histamine-2 receptor antagonists (H2RAs), proton pump inhibitors (PPIs), and anti-reflux surgery. Aim: The aim of this review is to assess the cost-effectiveness of the use of PPIs in the long-term management of patients with GERD. Method: We searched in PubMed to identify related original articles with close consideration based on inclusion and exclusion criteria to choose the best studies for this narrative review. The first section compares the cost-effectiveness of PPIs with H2RAs in long-term heartburn management. The other sections shall only discuss the cost-effectiveness of PPIs in 5 different strategies, namely, continuous (step-up, step-down, and maintenance), on-demand, and intermittent therapies. Results: Of 55 articles published, 10 studies published from 2000 to 2015 were included. Overall, PPIs are more effective in relieving heartburn in comparison with ranitidine. The use of PPIs in managing heartburn in long-term consumption of nonsteroidal anti-inflammatory drug (NSAID) has higher cost compared with H2RA. However, if the decision-maker is willing to pay more than US$174 788.60 per extra quality-adjusted life year (QALY), then the optimal strategy is traditional NSAID (tNSAID) and PPIs. The probability of being cost-effective was also highest for NSAID and PPI co-therapy users. On-demand PPI treatment strategy showed dominant with an incremental cost-effectiveness ratio of US$2197 per QALY gained and was most effective and cost saving compared with all the other treatments. The average cost-effectiveness ratio was lower for rabeprazole therapy than for ranitidine therapy. Conclusion: Our review revealed that long-term treatment with PPIs is effective but costly. To achieve long-term cost-effective approach, we recommend on-demand approach to treat heartburn symptoms, but if the symptoms persist, treatment with continuous step-down therapy should be applied.

Participants Of the 4203 Age-Related Eye Disease Study 2 (AREDS2) participants, 391 eyes (325 participants) had DPED without late AMD at the time of DPED detection. Genetic analyses included 120 white AREDS2 participants and 145 Age-Related Eye Disease Study (AREDS) participants with DPED. Methods Baseline and annual stereoscopic fundus photographs were graded centrally to detect DPED, a well-defined yellow elevated mound of confluent drusen ≥433 μm in diameter, and to evaluate progression rates to late AMD: geographic atrophy (GA) and neovascular (NV)-AMD. Five single nucleotide polymorphisms (CFH [rs10611670], C3 [rs2230199], CFI [rs10033900], C2/CFB [rs114254831], ARMS2 [rs10490924]) and genetic risk score (GRS) group were investigated for association with DPED development. Kaplan–Meier analyses and multivariable proportional hazard regressions were performed. Main Outcome Measures Progression rates to late AMD and decrease of ≥3 lines in visual acuity (VA) from the time of DPED detection; association of rate of DPED development with genotype. Results Mean (standard deviation [SD]) follow-up time from DPED detection was 4.7 (0.9) years. DPED was associated with increased risk of progression to late AMD (hazard ratio [HR], 2.36; 95% confidence interval [CI], 1.98–2.82; P Conclusions This study replicates the results of previous natural history studies of eyes with DPED including the high rates of progression to late AMD and vision loss (regardless of progression to late AMD). The genetic associations are consistent with genes associated with AMD progression.

Purpose To analyze the prevalence, incidence, and clinical characteristics of eyes with geographic atrophy (GA) in age-related macular degeneration (AMD), including clinical and genetic factors affecting enlargement. Design Prospective cohort study within a controlled clinical trial. Participants Age-Related Eye Disease Study 2 (AREDS2) participants, aged 50–85 years. Methods Baseline and annual stereoscopic color fundus photographs were evaluated for GA presence and area. Analyses included GA prevalence and incidence rates, Kaplan-Meier rates, mixed-model regression, and multivariable analysis of the square root of GA, area adjusted for covariates, including clinical/imaging characteristics and genotype. Main Outcome Measures (1) Presence or development of GA; (2) change in the square root of GA area over time. Results At baseline, 517 eyes (6.2%) of 411 participants (9.8%) had pre-existing GA (without neovascular AMD), with the following characteristics: 33% central, 67% noncentral; and the following configurations: 36% small, 26% solid/unifocal, 24% multifocal, 9% horseshoe/ring, and 6% indeterminate. Of the remaining 6530 eyes at risk, 1099 eyes (17.3%) of 883 participants developed incident GA without prior neovascular disease during mean follow-up of 4.4 years. The Kaplan-Meier rate of incident GA was 19% of eyes at 5 years. In eyes with incident GA, 4-year risk of subsequent neovascular AMD was 29%. In eyes with incident noncentral GA, 4-year risk of central involvement was 57%. GA enlargement rate (following square root transformation) was similar in eyes with pre-existing GA (0.29 mm/year; 95% confidence interval 0.27–0.30) and incident GA (0.28 mm/year; 0.27–0.30). In the combined group, GA enlargement was significantly faster with noncentrality, multifocality, intermediate baseline size, and bilateral GA (P Conclusions Analyses of AREDS2 data on natural history of GA provide representative data on GA evolution and enlargement. GA enlargement, which was influenced by lesion features, was relentless, resulting in rapid central vision loss. The genetic variants associated with faster enlargement were partially distinct from those associated with risk of incident GA. These findings are relevant to further investigations of GA pathogenesis and clinical trial planning.

Since the emergence of CoVID-19 pandemic in China in late 2019, scientists are striving hard to explore non-toxic, viable anti-SARS-CoV-2 compounds or medicines. We determined In Vitro anti-SARS-CoV-2 activity of oral formulations (syrup and capsule) of an Iodine-complex (Renessans). A monolayer of vero cells were exposed to SARS-CoV-2 in the presence and absence of different concentrations (equivalent to 50, 05 and 0.5 μg/ml of I2) of Renessans. Anti-SARS-CoV-2 activity of each of the formulation was assessed in the form of cell survival, SARS-CoV-2-specific cytopathic effect (CPE) and genome quantization. With varying concentrations of syrup and capsule, a varying rate of inhibition of CPE, cells survival and virus replication was observed. Compared to 0.5 μg/ml concentration of Renessans syrup, 5 and 50 μg/ml showed comparable results where there was a 100% cell survival, no CPEs and a negligible viral replication (ΔCT= 0.11 and 0.13, respectively). This study indicates that Renessans, containing iodine, may have potential activity against SARS-CoV-2 which needs to be further investigated in human clinical trials.

Background: Remarkable achievements have been made in the last decade to reduce the child mortality worldwide. However, South Asia has one of the highest numbers of child deaths. Out of 10 child deaths worldwide, three occur in South Asia. The under-5 mortality rate is still very high with 51 deaths per 1000 live births. Moreover, the overall and country-specific risk factors associated with under-5 mortality in the region are largely unknown. This thesis aimed to study the difference in under-5 mortality in WHO South Asian countries and then to explore whether the associated risk factors are the same or different across the countries of South Asia. Methods: This study was based on Demographic and Health Survey (DHS), data collected from five South Asian countries (Bangladesh, India, Maladies, Nepal, and Pakistan). Data were obtained from the most recent live under-5 births from mothers within five years prior to the survey (n=570676). Under-5 mortality, death of the children from day of birth to fifth birthday of child was the outcome variable in this study. Association of under-5 mortality with risk factors including socio-demographic variables was studied using Cox Proportional hazard method. The estimates were presented as hazard ratio (HR) and their 95% confidence interval (CI). Survival Curves were used to explain the difference in survival of under-5 children in each country. Results: Overall prevalence of under-5 mortality in South Asian countries according to pooled data was 10%. Country-specific results showed that Nepal having the highest prevalence (11.1%) of under-5 mortality followed by India (10.3%) and Pakistan (10.2%) in South Asia. In a multivariable model in pooled data, older age of the women (HR 0.70, 95% CI 0.68-0.72), being employed (HR 1.09, 95% CI 1.07-1.12), having husband with higher education (HR 0.74, 95% CI 0.70-0.78) and having higher education (HR 0.36, 95% 0.32-0.40) were significantly associated with under-5 mortality. Among other maternal and child factors, being female child (HR 0.95, 95% CI 0.93-0.97), wanted no children (HR 0.92, 95% CI 0.87-0.97), no contraceptive use (HR 0.95, 95% CI 1.30-1.37), currently pregnant (HR 1.17, 95% CI 1.17-1.23), no smoking (HR 0.85, 95% CI 0.83-0.87), male sex of children was associated with under-5 mortality. Most of the studied risk factors were common across the countries, but some difference in the factors associated with under-5 were country-specific. Conclusion: The prevalence of under-5 mortality is still high in South Asia. Most of the socio-demographic factors are associated with under-5 mortality and are common risk factors for under-5 mortality across WHO South Asian countries. For improving the under-5 survival and achieving the Sustainable Development 2030 target, countries in South Asian region needs to put efforts on maternal and child health. Country-specific strategy should be focused on most prevalent risk factors. A multi-faceted approach that includes health and other related measures is needed to improve the child survival.

25. Age-dependent reliability analysis and development of a fuzzy based condition rating tool for timber utility poles

Timber is the most commonly used material for supporting utility power lines, with an estimated quantity of over 165 million across North America. Timber Poles provide a safe and cost effective mean to supply electricity and communication to vast majority of consumers, and are considered to be the most important asset by utility companies. Due to significantly large investment in timber poles across North America, there is need to investigate their structural reliability. Within the past few decades, different parts of the world have experienced significant climate changes. Specifically in North America, Hurricanes and strong winds have caused tremendous damage to infrastructure including Timber utility pole structures. Therefore, there is an urgent need to understand the performance of timber poles to mitigate damage during extreme climatic hazards. This research presents a fragility based methodology to assess and compare the vulnerability of timber poles exposed to wind hazards models for selected locations. Timber poles are designed as per both CSA 22.3 No.1 deterministic design wind loads and probabilistic wind loads. Wind hazard models for selected locations are developed using Extreme value analysis. Reliability of timber poles is determined through convolution of structural fragility models with the wind hazard models. Strength degradation with time due to decay was also taken into account for a holistic approach towards risk assessment of Timber poles. In addition to reliability analysis, a framework for development of a fuzzy logic based condition rating tool is also proposed in this research. Fuzzy synthetic evaluation technique, which is based on fuzzy logic theory has been utilized for the proposed framework. External decay, internal decay, wood pecker damage and mechanical damage were selected as performance indicators. A five tuple fuzzy linguistic evaluation set having levels of Very good, Good, Fair, Poor and Very Poor was used to assess the performance indicators. The proposed framework was validated through an illustrative example of ten timber poles. Analytical hierarchy process (AHP) was used to calculate weights for the four performance indicators. The proposed framework also provides the ability to prioritize timber poles according to their respective level of deterioration.

Recently, ultra performance liquid chromatography (UPLC) has proven to be one of the most promising developments in the area of fast chromatographic separations, as it's been true to the objective of reducing analysis time and maintaining good efficiency. The present report describes development and validation of a new, rapid, and sensitive UPLC method with UV detection to quantify acyclovir in bulk sample using a Waters Acquity HSS T-3 (100 × 2.1 mm, 1.8 µm) column maintained at a temperature of 50 °C with a gradient elution consisting of 1.0% triethyl amine in water and acetonitrile at a flow rate of 0.4 mL/min and detected at 254 nm. Method was found to be selective, linear, accurate, and precise as per ICH guidelines. Detection and quantitation limits of drug were 3 and 8 ng/mL, respectively. Delivery of therapeutic proteins/peptides has received a considerable amount of attention over the last 10 years, but there are number of limitations to oral delivery of proteins. The barriers to peptide bioavailability after oral administration are intestinal membrane permeability, size, intestinal and hepatic metabolism and lastly solubility. A number of approaches have been used to overcome these limitations. Poor membrane permeabilities of hydrophilic peptides might be overcome by structurally modifying the compound, thus increasing their membrane partition characteristics and their affinity for carrier proteins. Another approach is site specific delivery of the peptides to the most permeable part of the intestine. Metabolism (hepatic and intestinal) of peptides might be controlled by co-administration of competitive enzyme inhibitors, structural modifications and administration of the compound as well as absorbed prodrug that is converted into therapeutically active agent after its absorption. Various delivery systems like prolease technology, nano-particulate and microparticulate delivery system, mucoadhesive delivery of peptides and microspheres have been developed for the delivery of proteins and peptides. Non-conventional delivery systems for proteins are biodegradable and non-biodegradable systems. Besides these, some other approaches for protein and peptide delivery are vector mediated delivery of proteins using adenovirus, macroflux transdermal patches, pulmonary delivery of proteins, delivery of proteins and peptides across blood brain barrier.

To evaluate the association of subretinal hyperreflective material (SHRM) with visual acuity (VA), geographic atrophy (GA), and scar in the Comparison of Age-Related Macular Degeneration Treatments Trials (CATT).Prospective cohort study within a randomized clinical trial.The 1185 CATT participants.Masked readers graded scar and GA on fundus photography and fluorescein angiography and graded SHRM on time-domain and spectral-domain (SD) optical coherence tomography (OCT) throughout 104 weeks. Measurements of SHRM height and width in the fovea, within the center 1 mm(2), or outside the center 1mm(2) were obtained on SD OCT images at 56 (n = 76) and 104 (n = 66) weeks.Presence of SHRM, as well as location and size, and associations with VA, scar, and GA.Among CATT participants, the percentage with SHRM at enrollment was 77%, decreasing to 68% at 4 weeks after treatment and to 54% at 104 weeks. At 104 weeks, scar was present more often in eyes with persistent SHRM than in eyes with SHRM that resolved (64% vs. 31%; P0.0001). Among eyes with detailed evaluation of SHRM at weeks 56 (n = 76) and 104 (n = 66), mean VA letter score was 73.5 (standard error [SE], 2.8), 73.1 (SE, 3.4), 65.3 (SE, 3.5), and 63.9 (SE, 3.7) when SHRM was absent, present outside the central 1 mm(2), present within the central 1 mm(2) but not the foveal center, or present at the foveal center (P = 0.02), respectively. When SHRM was present, the median maximum height under the fovea, within the central 1 mm(2) including the fovea and anywhere within the scan, was 86 μm, 120 μm, and 122 μm, respectively. Visual acuity was decreased with greater SHRM height and width (P0.05).In eyes with neovascular age-related macular degeneration (AMD), SHRM is common and often persists after anti-vascular endothelial growth factor treatment. At 2 years, eyes with scar were more likely to have SHRM than other eyes. Greater SHRM dimensions were associated with worse VA. In eyes with neovascular AMD, SHRM is an important morphologic biomarker.

- Abstract
Background This study investigated the prevalence of and the factors associated with under-5 mortality across five South Asian countries. Methods Cross-sectional pooled data from 1999 through 2014 collected from the Demographic and Health Survey (DHS) in five South Asian countries (Bangladesh, India, Maldives, Nepal and Pakistan) were used. Associations of under-5 mortality with sociodemographic characteristics and maternal and child factors were studied using the Cox proportional-hazard method. The estimates were presented as hazard ratios (HR) and their 95% CIs. Kaplan–Meier survival curves were used to describe time-to-event of under-5 survival patterns. Results Overall, 15-year prevalence of under-5 mortality in South Asian countries was 10%, with Nepal having the highest prevalence (11.1%) and the Maldives the lowest (5%). In a multivariable model in pooled data, older age of mother (HR 0.70, 95% CI 0.68–0.72), being employed (HR 1.09, 95% CI 1.07–1.12), having a higher level of education (HR 0.36, 95%, CI 0.32–0.40) and having a husband with higher level of education (HR 0.74, 95% CI 0.70–0.78) were significantly associated with under-5 mortality. Factors associated with under-5 mortality were mostly common across countries. Conclusion The prevalence of under-5 mortality is still high in South Asia. Most of the studied sociodemographic factors were associated with under-5 mortality and were common across South Asian countries. [ABSTRACT FROM AUTHOR]

The objective of the present study is to develop microspheres for celecoxib to enhance its bioavailability by increasing its gastric residence time. Four different polymers-polyethylene oxide, Eudragit S, cellulose acetate, and Eudragit RL-were used to form the floating microspheres using an emulsion-solvent diffusion technique. The use of two different solvents (dichloromethane and ethanol) that differed in the rate of diffusion led to formation of a hollow core in the microspheres, which was partially responsible for the flotation ability. The formulation was optimized on the basis of in vitro buoyancy and in vitro release in simulated gastric fluid at pH 3. Scanning electron microscopy revealed differences between the formulations in terms of their topography. X-ray diffractometry and differential scanning calorimetry examination showed the amorphous nature of the drug. Microspheres prepared with polyethylene oxide:Eudragit S:celecoxib (2:2:1) gave the best in vitro percentage release and was taken as the optimized formulation. By fitting the data into zero order, first order, and Higuchi model, it could be concluded that the release followed first-order release kinetics. The correlation coefficient (R2 value) was obtained upon fitting the data to Higuchi equation, which signifies that the mechanism of release of celecoxib from the microspheres was diffusion rate-limited.

To compare baseline characteristics, treatment frequency, visual acuity (VA), and morphologic outcomes of eyes with50% of the lesion composed of blood (B50 group) versus all other eyes (Other group) enrolled in the Comparison of Age-Related Macular Degeneration Treatments Trials (CATT).Prospective cohort study within a multicenter randomized clinical trial.CATT patients with neovascular age-related macular degeneration (AMD).Treatment for the study eye was assigned randomly to either ranibizumab or bevacizumab and to 3 different dosing regimens over a 2-year period. Reading center graders evaluated baseline and follow-up morphology in color fundus photographs, fluorescein angiography (FA), and optical coherence tomography (OCT). Masked examiners tested VA.Morphologic features and VA at 1 and 2 years.The B50 group consisted of 84 of 1185 (7.1%) patients enrolled in CATT. Baseline lesion characteristics differed between groups. In the B50 group, choroidal neovascularization size was smaller (0.73 vs 1.83 disc areas [DA]; P0.001), total lesion size was greater (4.55 vs 2.31 DA; P0.001), total retinal thickness was greater (524 vs 455 μm; P = 0.02), and mean VA was worse (56.0 vs 60.9 letters; P = 0.002). Increases in mean VA were similar in the B50 and Other groups at 1 year (+9.3 vs +7.2 letters; P = 0.22) and at 2 years (9.0 vs 6.1 letters; P = 0.17). Eyes treated PRN received a similar number of injections in the 2 groups (12.2 vs 13.4; P = 0.27). Mean lesion size in the B50 group decreased by 1.2 DA at both 1 and 2 years (primarily owing to resolution of hemorrhage) and increased in the Other group by 0.33 DA at 1 year and 0.91 DA at 2 years (P 0.001). Leakage on FA and fluid on OCT were similar between groups at 1 and 2 years.In CATT, the B50 group had a visual prognosis similar to the Other group. Lesion size decreased markedly through 2 years. Eyes like those enrolled in CATT with neovascular AMD lesions composed of50% blood can be managed similarly to those with less or no blood.

The aim of the present study was to develop a delivery system wherein the retention of ofloxacin could be achieved for increased local action in gastric region against Helicobacter pylori infection. The formulation was optimized on the basis of in vitro buoyancy and in vitro release in citrate phosphate buffer (pH 3). The hydrodynamically balanced capsules were prepared by physical mixing of various grades of HPMC and poly(ethylene oxide) (PEO) alone as well as in combinations. Cellulose acetate pthalate, liquid paraffin, and ethyl cellulose were used as release modifiers so as to maintain release of drug over a period of 12 h. The capsules prepared with PEOWSR 60K and drug coated with 2.5% ethyl cellulose gave the best in vitro percentage release and were taken as the optimized formulations. Various grades or Eudragit and PEO were used in combination for formulating floating microspheres using solvent diffusion technique for preparation of multiple unit system. The use of two different solvents (dichloromethane and ethanol) that differed in the rate of diffusion led to formation of a hollow core in the microspheres, which was partially responsible for the flotation ability. The in vitro release of the floating capsules and microspheres was found to be 96.02% and 95.83% in 12 h, respectively. Both the dosage forms follow Higuchi model for release from formulations. By fitting the in vitro release data of single unit dosage form into zero-order, first-order, and Higuchi model, it could be concluded that the release followed Higuchi model, as the correlation coefficient (R 2 value) was higher than those in the other two release models. In both cases of single and multiple unit dosage form, R 2 values for Higuchi model were found to be good, showing that drug release followed non-Fickian diffusion mechanism.

Prior research conducted on budding female entrepreneurship did not highlight the transition nations such as Asia experiencing relatively faster economic growth and socioeconomic transformation. The role of contextual environment and motivation type in female entrepreneurship has not been adequately researched. This exploratory study of the association between the cognitive, social, and normative determinants of nascent female entrepreneurship activities in the two transition economies of India and Vietnam. This comparative study used the Global Entrepreneurship Monitor (GEM) database of 2015 for data collection. Data analysis of individual surveys from the database was conducted for selected categorical variables by using nonparametric tests of association for cross-tabulation (Chi-square test). The results show that the cognitive, social, and normative factors have a significant role in nascent female entrepreneurial levels in India and Vietnam. Socio-cultural influences play a dominant role in influencing and shaping women's entrepreneurial career choices to start a business. Moreover, women's entrepreneurial intentions is a product of the dynamic interaction of cognitive, social, and cultural factors at the individual (micro), social (meso), and cultural (macro) levels. This comparative country study of two middle-income transition South Asian economies creates insight into the gender and motivation specific contextualized roles of cognitive creates insight into the gender and motivation specific contextualized roles of cognitive and normative factors associated with nascent entrepreneurship at the country level. No prior other study has evaluated the contextualized role of women entrepreneurship's cognitive and social determinants by comparing two countries with similar economic and socio-cultural environments. The findings of the study can be used by policymakers to make better-informed decisions to promote women entrepreneurs by curating contextspecific policies.
