5. Vitamin D metabolites in idiopathic infantile hypercalcaemia.

Abstract

Metabolites of vitamin D were measured in plasma from 83 patients with idiopathic infantile hypercalcaemia syndrome who were mentally handicapped but had normal calcium values at the time of the study. No significant difference was detected in the mean plasma concentrations of 25-hydroxyvitamin D2, 1,25-dihydroxyvitamin D, 24,25-dihydroxyvitamin D3, or 25,26-dihydroxyvitamin D3 between patients and age matched controls. The mean plasma concentration of 25-hydroxyvitamin D3 was significantly lower in patients than controls but this may be a secondary phenomenon related to less sunlight exposure. In addition, two hypercalcaemic patients with this syndrome were studied during the first year of life, and were found to have normal concentrations of vitamin D metabolites. These findings do not support a role for abnormal vitamin D metabolism in the pathogenesis of this syndrome. [ABSTRACT FROM AUTHOR]

6. Idiopathic infantile hypercalcaemia--a continuing enigma.

Abstract

Seventy six children with documented Fanconi-type idiopathic infantile hypercalcaemia were studied and compared with 41 with the Williams-Beuren syndrome. Clinical comparison showed, as expected, very close similarities but also considerable differences, particularly in the severity of feeding problems and the degree of failure to thrive. The estimated incidence of idiopathic infantile hypercalcaemia alone has remained constant for the past 20 years, at approximately 18 cases per year in the United Kingdom (1 per 47 000 total live births). Long term morbidity in these children is mainly due to mental handicap and arteriopathy, but hypertension (29%), kyphoscoliosis (19%), hyperacusis (75%), and obesity (50%) may be added complications. In one child, hypercalcaemia recurred during adolescence but this seems to be excessively rare. More detailed investigation before treatment is required to discover the aetiology of hypercalcaemia in this condition. [ABSTRACT FROM AUTHOR]

Abstract

(1) Of 71 infants fed on reconstituted dried or evaporated cow's milk, 31 showed a fall in plasma calcium between the 1st and 6th days of life, whereas in 35 breast-fed infants this occurred in only 5. (2) Those artificially-fed infants who had shown a rise in plasma calcium over this period had significantly lower plasma creatinine values and significantly higher excretion of creatinine than those infants who showed a fall in calcium levels. (3) Artificially-fed infants who had shown a rise in calcium had significantly lower plasma osmolality and significantly higher osmolar excretion in the urine than those infants who showed a fall in plasma calcium. (4) It is suggested that a delay in the normal increase in glomerular filtration rate during the first week of life in some infants leads to phosphate retention. This, together with a higher dietary intake of phosphate, leads to a decrease of the plasma calcium to hypocalcaemic levels. [ABSTRACT FROM AUTHOR]

Abstract

This article presents information on fructosemia. Many factors influence the course and presentation of fructosemia. One major critical factor is the age at which frutose ingestion begins. Fructosemia may develop in the first week of life if fructose-containing substances are in the formula. Vomiting is considered the earliest of all symptoms and may be severe. It begins on the first day of fructose ingestion, and usually persists as long as the diet continues to include it. The treatment of fructosemia invlves the withdrawal of all fructose containing substances from the diet.

18. Insulin Disappearance after Intravenous Injection and its Effect on Blood Glucose in Diabetic and Non-Diabetic Children and Adults

Abstract

1. Porcine insulin (0·1 i.u./kg) was injected intravenously into diabetic and non-diabetic subjects. 2. In each group insulin produced a fall in blood glucose within 5 min which was exponential and which continued until approx. 99% of the insulin had left the plasma. 3. In the diabetic subjects both the insulin values and blood glucose fell more slowly than in the controls, when expressed as a proportion of the fasting value. However, both insulin and glucose continued to fall for a longer period, with the result that there was a total fall of blood glucose of 67% in the diabetics and 70% in the non-diabetics. Though in absolute terms considerably more glucose disappeared in diabetics than in the normal subjects, this could be related to the higher initial glucose concentrations in the former. 4. It is suggested that not only does insulin act very rapidly, but it is also rapidly inactivated.

19. STEROID EXCRETION AND BIOSYNTHESIS WITH SPECIAL REFERENCE TO C19-Δ16-STEROIDS IN AN INFANT WITH A VIRILIZING ADRENOCORTICAL CARCINOMA

Abstract

A case of a 15-month old girl with a virilizing adrenocortical carcinoma is described. Three C19-Δ16-steroids, 3α-hydroxy-5α-androst-16-diene, 3α-hydroxy-5β-androst-16-ene and 3β-hydroxy-androsta-5,16-diene, have been obtained from the glucuronide fraction of the urine and identified by column and gas-liquid chromatographic techniques. Estimation of these compounds, not normally present in infants' urine, showed that adult female amounts were being excreted, although post-operatively very little was detected. Androsterone, aetiocholanolone and especially dehydroepiandrosterone were present in excessive amounts preoperatively; approximately one-third of the dehydroepiandrosterone was excreted as glucuronide, the rest as sulphate. The adrenal tumour metabolized [4-14C] pregnenolone and [4-14C] progesterone to 4.7% and 0.15% respectively of 3β=hydroxy-androsta-5.16-diene. The evidence for biosynthesis of this compound from progesterone supports that obtained by other workers for the production of Δ5- from Δ4-steroids. Labelled androstenedione was also obtained from both substrates and 1 1β-hydroxyandrostenedione from progesterone. Only small quantities of radioactivity were recovered in the glucuronide and sulphate fractions from the tissue incubations.

Abstract

This article presents suggestions for the recognition of seizures in individual newborn patients. The recognition of seizures in newborn infants, suggestively, differ qualitatively from those seen in older infants. According to the author, these events are often only observed by members of the nursing staff, who may be inexperienced in distinguishing them from "jitteriness" or "tremulousness." Neonatal seizures are rarely fatal in themselves. An earnest search, suggestively, must be made for the cause of the seizures rather than too easily ending enquiry by labeling them "idiopathic." Based on findings of a recent research, it is suggested that, the prognosis of neonatal seizures depends mainly upon the initial cause. Pyridoxine-dependent seizures respond within two minutes to intravenous pyridoxine. Isolated hypocalcemia carries a virtual 100% certainty for normal neurological development, although dental abnormalities seem to be almost inevitable. Treatment of seizures, suggestively, should first be directed at establishing a cause as quickly as possible and taking appropriate action.
