Abstract

ImportanceUrinary tract infection (UTI) is the second most common infection leading to hospitalization and is often associated with gram-negative multidrug-resistant organisms (MDROs). Clinicians overuse extended-spectrum antibiotics although most patients are at low risk for MDRO infection. Safe strategies to limit overuse of empiric antibiotics are needed.ObjectiveTo evaluate whether computerized provider order entry (CPOE) prompts providing patient- and pathogen-specific MDRO risk estimates could reduce use of empiric extended-spectrum antibiotics for treatment of UTI.Design, setting, and participantsCluster-randomized trial in 59 US community hospitals comparing the effect of a CPOE stewardship bundle (education, feedback, and real-time and risk-based CPOE prompts; 29 hospitals) vs routine stewardship (n = 30 hospitals) on antibiotic selection during the first 3 hospital days (empiric period) in noncritically ill adults (≥18 years) hospitalized with UTI with an 18-month baseline (April 1, 2017-September 30, 2018) and 15-month intervention period (April 1, 2019-June 30, 2020).InterventionsCPOE prompts recommending empiric standard-spectrum antibiotics in patients ordered to receive extended-spectrum antibiotics who have low estimated absolute risk (

ImportancePneumonia is the most common infection requiring hospitalization and is a major reason for overuse of extended-spectrum antibiotics. 10. From Entertainment to Empowerment: A Call for Social Media Literacy Education at an HBCU

Abstract

An analysis of 640 posts from the social media platforms of 14 students at a historically Black university revealed that entertainment accounted for 68% of their social media content, "uplift" 17%, and empowerment 14%. Educators worry that students may be squandering online resources that could help improve their lives when they choose entertainment over uplift and empowerment. The study examines why entertainment trumps uplift and empowerment, and more broadly, how it may serve as a catalyst to combat disinformation.

Abstract

Genome sequencing is a powerful tool for identifying SARS-CoV-2 variant lineages; however, there can be limitations due to sequence dropout when used to identify specific key mutations. Recently, ThermoFisher Scientific has developed genotyping assays to help bridge the gap between testing capacity and sequencing capability to generate real-time genotyping results based on specific variants. Over a 6-week period during the months of April and May 2021, we set out to assess the ThermoFisher TaqMan mutation panel genotyping assay, initially for three mutations of concern and then for an additional two mutations of concern, against SARS-CoV-2-positive clinical samples and the corresponding COVID-19 Genomics UK Consortium (COG-UK) sequencing data. We demonstrate that genotyping is a powerful in-depth technique for identifying specific mutations, is an excellent complement to genome sequencing, and has real clinical health value potential, allowing laboratories to report and take action on variants of concern much more quickly.

13. Designing a Process for Inclusive Innovation: A Radical Commitment to Equity. Version 1.0

Abstract

Under the current model of education research and development (R&D), solutions to the pressing challenges facing schools and districts across the country often fail to meet the needs of those who need them most: historically marginalized populations such as Black and Latinx students and students living in poverty. To address this problem, Digital Promise sought to understand the essential elements of equity-first R&D models and develop a prototype for education. This paper starts from the premise that missing from existing education R&D is a "radical commitment to equity." The paper presents "Inclusive Innovation," a model that reimagines authority, decision-making, and risk in the context of education R&D and provides an overarching framework for authentically engaging underrepresented stakeholders at the earliest stages and shifting their roles to leaders, participants, and beneficiaries. The power of "Inclusive Innovation" is that it does not just invite underrepresented voices and perspectives into the innovation ecosystem; it places them at the center of it.

23. Teacher education in England undone : developing teacher educator agency through theory and practice

UK government rhetoric and action has progressively altered the landscape of teacher education in England and marginalised the role of the university. This has impacted the professional lives of university-based teacher educators in particular ways. Significantly, they have needed to adjust their practice in partnerships with schools in relation to a shifting professionalism within the field of teacher education. The thesis provides a critical application of various theoretical lenses to one university teacher educator’s professional journey through this landscape over a 20 year period. It researches the question of how she has developed agency to effect positive change in teacher education in the policy context. In so doing, articles first published by the author as university teacher educator are re-examined using readings in a contemporary setting to reflect upon thinking and practice during successive policy enactments. The discussion begins with a retrospective consideration of the use of principles of reflective practice in student teacher development and raises the question of social theorising and a psychoanalytical approach for players in teacher education. Particular focus is given to a critical discussion of the author’s earlier use of Bourdieu’s concept of cultural capital in social relations governing prescribed subject knowledge development of student teachers in the field; an apparent disconnect between use of the concept and explicit psychoanalytical approaches based on the work of Lacan is revealed. Significantly, the author’s later professional experience of tutoring and researching on the employment-based Graduate Teacher Programme is explored specifically in relation to Lacan’s four speech discourses. These are used to develop theoretical understanding about the positioning of student teachers and the university teacher educator in teacher education. It is argued that professional agency derives from the intersect of informed academic, analytical action and response between players engaged in the field. Furthermore, such professional agency is required to provide sustainable teacher education of quality able to serve schools and their wider communities in troubled times.

24. Supporting and Scaling Change: Lessons from the First Round of the Investing in Innovation (i3) Program

Abstract

It is widely acknowledged that innovation will be necessary to dramatically improve public services in America. But innovation in the public sector doesn't happen in a vacuum; innovation happens at the nexus of policy, research, capital, and practice. This project looks at one case study--education--by analyzing some of the key aspects of an emerging ecosystem for innovation in public education in the US, including the flow of investment capital for such efforts, the uptake of innovations by buyers and users, federal efforts to stimulate and scale innovation, and ways that technology could facilitate innovation investment and practice. Drawing on surveys, interviews, and working groups, the project highlights recent efforts to fuel and steer more innovation, and frames the remaining challenges that lie ahead for the public, private, and philanthropic sectors. This project culminates in an analysis of the lessons and insights drawn from the recent experience of US public education in comparison to the way leaders are using innovation to address similar intractable social problems in other fields and in other countries. 25. Steering Capital: Optimizing Financial Support for Innovation in Public Education

This paper revisits the central question of how to improve the provision of capital for entrepreneurial change in public education, but emphasizes the innovation ecosystem that surrounds the capital markets. The authors consider capital as one of the most important levers individuals need to align in this innovation ecosystem, but as a force that can both influence the way innovation takes hold--and can in turn be influenced by others in the wider ecosystem, including public policy. In this paper, the authors briefly define what they mean by innovation and the cycle of constant learning it requires, and lay out three contributing trends relevant to this context: social entrepreneurs, social purpose investors and social purpose market steering. They then consider the five elements they believe are necessary to enable effective capital market dynamics to support innovation, including: (1) Clarity and agreement on the problems, goals and metrics for success; (2) An effective research and development (R&D) system; (3) A culture that is evidence-based, with incentives and infrastructure aligned for continuous improvement; (4) Data that are transparent, available, comparable and useful; and (5) Robust, diverse and aligned investment capital. The authors conclude by offering some recommendations about how they might work together across the sectors to mobilize public, private and philanthropic capital in ways that harness the ingenuity and energy of entrepreneurial change agents in service of ambitious public education goals. 26. Anxiety, Knowledge and Help: A Model for How Black and White College Students Search for HIV/AIDS Information on the Internet

Using the "think aloud" protocol, which allows for the collection of data in real time, the researcher audio taped comments from 13 white college students from a predominately white university in the Southeastern United States and 15 black students from a predominately black university, as they explained how they searched for HIV/AIDS information on the Internet. A grounded theory analysis of the tapes revealed a three-stage model that students progressed through as they searched for HIV/AIDS information on the Internet. That model also revealed that all of the white students searched for general information about HIV/AIDS on the Internet, while all black students searched for general and specific information about how the disease affected the African-American community. Eighty percent of students regardless of race did not know how to properly search for online health information. The researcher discusses the need for online health information literacy training, the theories that might explain why black students searched the way that they did, and the challenges to providing culturally-sensitive online health information literacy training for African-Americans who have been historically suspicious of the United State's health care system. 28. Expanding the Supply of High-Quality Public Schools

Abstract

The Bridgespan Group, NewSchools Venture Fund, and the Bill & Melinda Gates Foundation have been working individually and collectively with a wide range of school development organizations to increase the number of high-performing centers of learning. Through this work, we have sought answers to persistent questions of how to replicate the successes and raise the quality of U.S. public education. In the process, we have identified two levers that play critically important roles in determining how quickly and consistently successful schools and design models can be replicated. One is the degree of managerial responsibility, support, and control the organization chooses to exercise. The other is related to specificity of school design. Leadership's choices about each will affect not only how quickly the model can be deployed, but also the human and financial capital required--and ultimately the likelihood of achieving consistent high-quality results. This paper examines the school development landscape in the context of these levers, with examples of organizations that have chosen different paths with different tradeoffs and outcomes. 31. 'Strands of the sixties' : a cultural analysis of the design and consumption of the new London West End hair salons, c. 1954-1975

Abstract

This thesis is a study of the under-researched subject of British hairdressing, focussing on the growth of London’s West End hair salons from 1954 to 1975. It challenges dominant historical accounts that have focussed on Paris and examines developments in London, leading to its centrality as a centre for hairdressing creativity in the 1960s. It culturally contextualises these shifts in the consumption of hair dressing in Britain from 1954 to 1975 by analysing the leading trade paper, the Hairdressers’ Journal signalling how salon design and management, and hair dressing’s fashionable consumption during this era related to wider socio-economic and cultural developments. This study is divided into four chapters. Chapter One examines the emergence of the public ladies’ hair salon in the late nineteenth century and the developments in its interior design up to 1950s evaluating the hair salon as a gendered public space in Mayfair, the heart of elite West End hairdressing. Chapter Two explains why Mayfair became established as a place of luxury and elitism and how this was manifest in the style of the salons and hairdressing performed there and through its perception as such in British provinces. Chapter Three identifies the major innovations in cutting and colouring techniques which elevated London to its position as a world leader in these practices. Furthermore, Black hairdressing and its professionalization as a result of mass-immigration, is analysed. Chapter Four investigates why smaller, intimate spaces including hair salons attracted fashionable youth audiences and it examines the salon’s suitability as economically viable entrepreneurial space aimed at young consumers. It contends that economic changes coupled with more informal social attitudes led to the formation of unisex salons. My conclusion argues that these developments in British hairdressing and hair salon design from 1954 to 1975 evidence an important transitional moment in hairdressing history and in its consumption. It maintains that while West End hairdressing was an elite part of the national hairdressing trade in Britain, nevertheless, it was keenly responsive to broader socio-cultural and economic changes, which directed and shaped its practices and consumption patterns and its international standing.

35. Sliding Subject Positions: Knowledge and Teacher Educators

Abstract

In England, adjustments to policy in teacher education have had implications for how subject knowledge is understood and for how job descriptions are defined. That is, the interface between teacher educator and subject knowledge representation has been changing. This paper reports on a wider study that considers the experience of university teacher educators adjusting to new academic and operational conditions. On the one hand, the teacher educators are confronted with their subject specialism being set according to new learning objectives and to new time and curriculum constraints. On the other hand, their professional identity is reshaped in response to structural changes to teacher education where earlier job definitions had been reconfigured or removed. The paper analyses resultant conceptions of subject knowledge and of teacher education emerging through this changing interface and how these conceptions are variously located across staffing arrangements. A Lacanian model of subjectivity provides a theoretical approach to depicting teacher educator and pre-service teacher identification with subject knowledge. The paper provides a theoretical account of how the teacher educator/trainee interface has been reshaped in line with the market-led terminology that governs current practices. Specifically, the analytical tools enable us to dismantle and restructure the prevalent symbolic order guiding current teacher practice and understanding, particularly our entrapment within specific discourses and identity constructs that shape our interactions with subject knowledge.

Abstract

Mitigation of SARS-CoV-2 transmission from international travel is a priority. We evaluated the effectiveness of travellers being required to quarantine for 14-days on return to England in Summer 2020. We identified 4,207 travel-related SARS-CoV-2 cases and their contacts, and identified 827 associated SARS-CoV-2 genomes. Overall, quarantine was associated with a lower rate of contacts, and the impact of quarantine was greatest in the 16-20 age-group. 186 SARS-CoV-2 genomes were sufficiently unique to identify travel-related clusters. Fewer genomically-linked cases were observed for index cases who returned from countries with quarantine requirement compared to countries with no quarantine requirement. This difference was explained by fewer importation events per identified genome for these cases, as opposed to fewer onward contacts per case. Overall, our study demonstrates that a 14-day quarantine period reduces, but does not completely eliminate, the onward transmission of imported cases, mainly by dissuading travel to countries with a quarantine requirement., We thank members of the COVID-19 Genomics Consortium UK and Test and Trace contact tracers for their contributions to generating the data used in this study. 47. Rethinking Research in Teacher Education

Abstract

The expansion of school-based teacher training is impacting onthe practice of universities, schools and trainees. University tutors and managers were interviewed on how they experienced working in partnership with schools and how this impacted on the composition of their work. They variously reported on how their sense of professional purpose had been challenged as a result of changing expectations. Their involvement in research is used as a barometer for these changes. The teacher educators are depicted as wavering between governmental regulation (master discourse) and professional imperatives (university discourse), where the latter comprise an uneasy alliance of expertise in school and academic rigour. Through depicting the unsettlement of practice and accounts of it (hysteric discourse), the study points to possible resolutions that might be achieved through more systematic resistance to external demands (analytic discourse). The university teacher educator identity results from attempted resolution of these conflicting demands.

49. The Discursive Production of Classroom Mathematics

Abstract

School mathematics is a function of its discursive environment where the language being used formats mathematical activity. The paper explores this theme through an extended example in which the conduct of mathematical teaching and learning is restricted by regulative educational policies. It considers how mathematics is discursively produced by student teachers within an employment-based model of teacher education in England where there is a low university input. It is argued that teacher reflections on mathematical learning and teaching within the course are patterned discursively in line with formal curriculum framings, assessment requirements and the local demands of their placement school. Both teachers and students are subject to regulative discourses that shape their actions and as a consequence this regulation influences the forms of mathematical activity that can take place. It is shown how university sessions can provide a limited critical platform from which to interrogate these restrictions and renegotiate them.

Abstract

Social media platforms (i.e., Twitter, Tik Tok, YouTube, Snapchat) transmit traumatic imagery of racist encounters that dehumanize racially marginalized Americans. Behavioral and social sciences have long documented the adverse effects of media on young adults which include negative physical and mental health outcomes. However, evidence is sparse on the health-related impact of viewing gender-specific, racially motivated violence via social media. Through an interdisciplinary approach, with theories from psychology, sociology, and communications, we propose a novel framework called the Vicarious Intersectional Trauma (VIT) Conceptual Model. The proposed model situates the Racial Encounter Coping Appraisal and Socialization Theory (RECAST) as a foundational theory understanding the pathways by which traumatic racial encounters impact health among Black Americans. We expand on RECAST by integrating Agenda Setting Theory, Cultivation Theory, and Intersectionality Theory, to facilitate a better understanding of the health-related implications of consuming intersectional violence on social media. Our interdisciplinary work contributes to literature promoting health equity by expanding an existing theory to address the intersections of gender and race on the perception of traumatic imagery and how gendered-racial socialization affects subsequent coping mechanisms.
