Abstract

Background: Pulmonary artery (PA) dilatation is often seen in pulmonary hypertension (PH) and is considered a long-term consequence of elevated pressure. The PA dilates over time and therefore may reflect disease severity and duration. Survival is related to the stage of the disease at the time of diagnosis and therefore PA diameter might be used to predict prognosis. This study evaluates the outcome of patients with pulmonary arterial hypertension (PAH) and chronic thrombo-embolic pulmonary hypertension (CTEPH) and investigates whether PA diameter at the time of diagnosis is associated with mortality. Methods: Patients visiting an outpatient clinic of a tertiary centre between 2004 and 2018 with a cardiac catheterisation confirmed diagnosis of PAH or CTEPH and a CT scan available for PA diameter measurement were included. PA diameter and established predictors of survival were collected (New York Heart Association (NYHA) class, N‑terminal pro-brain natriuretic peptide (NT-proBNP) level and 6‑min walking distance (6MWD)). Results: In total 217 patients were included (69% female, 71% NYHA class ≥III). During a median follow-up of 50 (22–92) months, 54% of the patients died. Overall survival was 87% at 1 year, 70% at 3 years and 58% at 5 years. The mean PA diameter was 34.2 ± 6.2 mm and was not significantly different among all the diagnosis groups. We found a weak correlation between PA diameter and mean PA pressure (r = 0.23, p < 0.001). Male sex, higher age, shorter 6MWD and higher NT-proBNP level were independently associated with mortality, but PA diameter was not. Conclusion: The prognosis of PAH and CTEPH is still poor. Known predictors of survival were confirmed, but PA diameter at diagnosis was not associated with survival in PAH or CTEPH patients.

Abstract

TenneT, a leading European electricity transmission system operator (TSO) is planning to install a 3.4 km long underground superconducting 110 kV cable as part of the Dutch electricity grid, in the city of Enschede. HTS cables have already been demonstrated on a relatively small scale in other countries, but they are usually not part of the meshed high-voltage grid and the length of the relevant cable section generally does not exceed 1 km. In 2009, a 600-meter section of HTS cable was installed in New York, and in 2014 a 1-km long section was taken in operation in Essen, Germany to replace a 10 kV AC medium-voltage line. In the Supernet NL program, TenneT is working together with several leading knowledge institutes including University of Twente, Delft University of Technology, the Institute of Science and Sustain- able Development (IWO), HAN University of Applied Sciences and RH Marine. These institutes have been investigating control engineering aspects and the requirements the cable must meet. In the meantime, the tender process has been started which consists of two phases. In the fi rst phase (summer 2017) appropriate candidates are selected directly followed by a call for tender in August. Receipt of the best and fi nal offer is scheduled for the end of November. In the presentation, the project will be introduced and requirements will be discussed, specifi cally focusing on the cryogenic aspects.

Abstract

Aims: In the past few years, promising results were described in targeting the arrhythmogenic substrate of the epicardial right ventricular outflow tract (RVOT) region in patients with Brugada syndrome (BrS). In this report, we describe our experience with endo- and epicardial substrate mapping and ablation in a series of highly symptomatic BrS patients. Methods: This case series consists of seven patients with clinical BrS diagnosis who underwent catheter ablation in two Dutch hospitals (Isala hospital Zwolle; and Amsterdam University Medical Centre, location AMC, Amsterdam) and Hamad Heart Hospital in Qatar between 2013 and 2017. All patients had an ICD and recurrent ventricular arrhythmia (VA) episodes. All patients underwent endo-and epicardial mapping of the RVOT region. Elimination of all abnormal potentials and disappearance of BrS ECG pattern during the ablation procedure was the aimed endpoint. Results: The study group consisted of seven patients with mean age 45.6 ± 16.9 years. Five patients had SCN5A mutations. One patient was excluded from analysis, since ablation could not be performed due to a very large low-voltage area and was later diagnosed with arrhythmogenic right ventricular cardiomyopathy, associated with an SCN5A mutation. One patient underwent both endo- and epicardial ablation to eliminate VA. During a mean follow-up of 3.6 ± 1.5 years, 5/6 patients remained VA free with two patients continuing quinidine. Conclusion: In patients with BrS and drug-refractory VA, ablation of the arrhythmogenic substrate in the RVOT region was associated with excellent long-term VA-free survival. Abstract

Intestinal epithelial stress or damage may contribute to allergic sensitization against certain food antigens. Hence, the present study investigated whether impairment of intestinal barrier integrity by the mycotoxin deoxynivalenol (DON) contributes to the development of whey-induced food allergy in a murine model. C3H/HeOuJ mice, orally exposed to DON plus whey once a week for 5 consecutive weeks, showed whey-specific IgG1 and IgE in serum and an acute allergic skin response upon intradermal whey challenge, although early initiating mechanisms of sensitization in the intestine appeared to be different compared with the widely used mucosal adjuvant cholera toxin (CT). Notably, DON exposure modulated tight-junction mRNA and protein levels, and caused an early increase in IL-33, whereas CT exposure affected intestinal γδ T cells. On the other hand, both DON- and CT-sensitized mice induced a time-dependent increase in the soluble IL-33 receptor ST2 (IL-1R1) in serum, and enhanced local innate lymphoid cells type 2 cell numbers. Together, these results demonstrate that DON facilitates allergic sensitization to food proteins and that development of sensitization can be induced by different molecular mechanisms and local immune responses. Abstract

This paper discusses partial discharge (PD) measurements performed on turbogenerators using the VHF PD detection technique. With this technique it is possible to perform PD measurements on turbogenerators without interruption of their operation. In addition to PD in any particular phase, the recorded patterns can show crosstalk from other phases and disturbances. The membership of each signal source (phase's own PD pattern, crosstalk, disturbances) to the recorded pattern depends on the selected center frequency. By examination of some typical examples, the relationship between the center frequency and recorded patterns is discussed. Furthermore, PD measurements performed over time on the same generator are studied to discuss possible changes in the spectral responses of particular phases of a turbogenerator. By visual comparison of PD patterns as well as computer-based discrimination tools it was found that several center frequencies are suitable to tune the VHF PD detection technique. However, these center frequencies can change in the course of time for a particular phase of a turbogenerator. As a result, in order to avoid a misjudgment on the insulation condition of turbogenerators, the analysis of on-line recorded PD should not be restricted to just one single center frequency.
