Abstract

Actors of the agri-food chain lack reliable and robust information to meet consumer expectations in relation to the multiple aspects of intrinsic quality of livestock products from the various European livestock systems. The INTAQT project aims to assess the relationships between animal production systems and products quality in order to improve husbandry practices complying with high quality animal products and sustainability. This is the ‘One Quality’ concept. The project focuses on chicken meat, beef, and dairy products and applies a multi-actor participatory approach which involves all actors of the agri-food chain. The challenges are to: (1) develop comprehensive models to quantify the impact of livestock systems on product safety, nutritional value and sensory attributes; (2) propose, together with the agri-food chain actors, fast, easy and cost-effective analytical tools to predict the intrinsic quality of livestock products and authenticate the associated livestock systems; (3) propose together with the same actors multi-criteria scoring tools for the intrinsic quality of products; and (4) promote farming practices which can allow the production of safe, healthy and tasty animal products while ensuring a decent income to farmers and respecting animal welfare and the environment. Abstract

The quality of a food is generally reliant on the attributes that give it the ability to satisfy the stated or implicit needs of a user. The notion of quality has become increasingly complex, especially for animal-source foods. This evolution is paralleling and partially driven by concerns related to their impacts on human health and environment, to farming and food processing practices and to animal welfare. Such concerns, however, are set to play out in a sector marked by a mosaic of very different production and processing models. Consumption practices also change, which comes with a set of (sometimes conflicting) expectations. There is not only a clear demand for convenient ready-to-cook and ready-to-eat meals, which are often (but not always) highly processed, but also for quality labels (including organic), wholesome nutrition, etc. In addition, high levels of red meat and processed meat consumption have become a public health concern. Beyond their role in nutrition (macro- and micro-nutrients), recent public nutrition and health programmes (in UK and France, for instance) recommend limiting their consumption level. info:eu-repo/semantics/publishedVersion

Abstract

Background: The concept of the use of MRI for image-guided adaptive brachytherapy (IGABT) in locally advanced cervical cancer was introduced 20 years ago. Here, we report on EMBRACE-I, which aimed to evaluate local tumour control and morbidity after chemoradiotherapy and MRI-based IGABT. Methods: EMBRACE-I was a prospective, observational, multicentre cohort study. Data from patients from 24 centres in Europe, Asia, and North America were prospectively collected. The inclusion criteria were patients older than 18 years, with biopsy-proven squamous cell carcinoma, adenocarcinoma, or adenosquamous carcinoma of the uterine cervix, The International Federation of Gynecology and Obstetrics (FIGO) stage IB–IVA disease or FIGO stage IVB disease restricted to paraaortic lymph metastasis below the L1–L2 interspace, suitable for curative treatment. Treatment consisted of chemoradiotherapy (weekly intravenous cisplatin 40 mg/m2, 5–6 cycles, 1 day per cycle, plus 45–50 Gy external-beam radiotherapy delivered in 1·8–2 Gy fractions) followed by MRI-based IGABT. The MRI-based IGABT target volume definition and dose reporting was according to Groupe Européen de Curiethérapie European Society for Radiation Oncology recommendations. IGABT dose prescription was open according to institutional practice. Local control and late morbidity were selected as primary endpoints in all patients available for analysis. The study was registered with ClinicalTrials.gov, NCT00920920. Findings: Patient accrual began on July 30, 2008, and closed on Dec 29, 2015. A total of 1416 patients were registered in the database. After exclusion for not meeting patient selection criteria before treatment, being registered but not entered in the database, meeting the exclusion criteria, and being falsely excluded, data from 1341 patients were available for analysis of disease and data from 1251 patients were available for assessment of morbidity outcome. MRI-based IGABT including dose optimisation was done in 1317 (98·2%) of 1341 patients. Median high-risk clinical target volume was 28 cm3 (IQR 20–40) and median minimal dose to 90% of the clinical target volume (D90%) was 90 Gy (IQR 85–94) equi-effective dose in 2 Gy per fraction. At a median follow-up of 51 months (IQR 20–64), actuarial overall 5-year local control was 92% (95% CI 90–93). Actuarial cumulative 5-year incidence of grade 3–5 morbidity was 6·8% (95% CI 5·4–8·6) for genitourinary events, 8·5% (6·9–10·6) for gastrointestinal events, 5·7% (4·3–7·6) for vaginal events, and 3·2% (2·2–4·5) for fistulae. 39. Ergogenic effect of pre-exercise chicken broth ingestion on a high-intensity cycling time-trial

Abstract

Background: chicken meat extract is a popular functional food in Asia. It is rich in the bioactive compounds carnosine and anserine, two histidine-containing dipeptides (HCD). Studies suggest that acute pre-exercise ingestion of chicken extracts has important applications towards exercise performance and fatigue control, but the evidence is equivocal. This study aimed to evaluate the ergogenic potential of the pre-exercise ingestion of a homemade chicken broth (CB) vs a placebo soup on a short-lasting, high-intensity cycling exercise. Methods: fourteen men participated in this double-blind, placebo-controlled, crossover intervention study. Subjects ingested either CB, thereby receiving 46.4 mg/kg body weight of HCD, or a placebo soup (similar in taste without HCD) 40 min before an 8 min cycling time trial (TT) was performed. Venous blood samples were collected at arrival (fasted), before exercise and at 5 min recovery. Plasma HCD were measured with UPLC-MS/MS and glutathione (in red blood cells) was measured through HPLC. Capillary blood samples were collected at different timepoints before and after exercise. Results: a significant improvement (p = 0.033; 5.2%) of the 8 min TT mean power was observed after CB supplementation compared to placebo. Post-exercise plasma carnosine (p < 0.05) and anserine (p < 0.001) was significantly increased after CB supplementation and not following placebo. No significant effect of CB supplementation was observed either on blood glutathione levels, nor on capillary blood analysis. Conclusions: oral CB supplementation improved the 8 min TT performance albeit it did not affect the acid-base balance or oxidative status parameters. Further research should unravel the potential role and mechanisms of HCD, present in CB, in this ergogenic approach.

Abstract

Aim: to develop and psychometrically evaluate a skin tear knowledge assessment instrument (OASES). Design: Prospective psychometric instrument validation study. Method: the skin tear knowledge assessment instrument was developed based on a literature review and expert input (N = 19). Face and content validity were assessed in a two-round Delphi procedure by 10 international experts affiliated with the International Skin Tear Advisory Panel (ISTAP). The instrument was psychometrically tested in a convenience sample of 387 nurses in 37 countries (April–May 2020). Validity of the multiple-choice test items (item difficulty, discriminating index, quality of the response alternatives), construct validity, and test–retest reliability (stability) were analysed and evaluated in light of international reference standards. Results: a 20-item instrument, covering six knowledge domains most relevant to skin tears, was designed. Content validity was established (CVI = 0.90–1.00). Item difficulty varied between 0.24 and 0.94 and the quality of the response alternatives between 0.01–0.52. The discriminating index was acceptable (0.19–0.77). Participants with a theoretically expected higher knowledge level had a significantly higher total score than participants with theoretically expected lower knowledge (p <.001). The 1-week test-retest intraclass correlation coefficient (ICC) was 0.83 (95% CI = 0.78–0.86) for the full instrument and varied between 0.72 (95% CI = 0.64–0.79) and 0.85 (95% CI = 0.81–0.89) for the domains. Cohen's Kappa coefficients of the individual items ranged between 0.21 and 0.74. Conclusion: the skin tear knowledge assessment instrument is supported by acceptable psychometric properties and can be applied in nursing education, research, and practice to assess knowledge of healthcare professionals about skin tears. 4-5, Background Silicone adhesive multilayer foam dressings are used as adjuvant therapy to prevent hospital-acquired pressure ulcers (PUs). Objectives To determine whether silicone foam dressings in addition to standard prevention reduce the incidence of PUs of category 2 or worse compared with standard prevention alone. Methods This was a multicentre, randomized controlled medical device trial conducted in eight Belgian hospitals. At-risk adult patients were centrally randomized (n = 1633) to study groups based on a 1 : 1 : 1 allocation: experimental groups 1 (n = 542) and 2 (n = 545) – pooled as the treatment group – and the control group (n = 546). The experimental groups received PU prevention according to hospital protocol, and a silicone foam dressing on the relevant body sites. The control group received standard of care. The primary endpoint was the incidence of a new PU of category 2 or worse at the studied body sites. Results In the intention-to-treat population (n = 1605), PUs of category 2 or worse occurred in 4·0% of patients in the treatment group and 6·3% in the control group [relative risk (RR) 0·64, 95% confidence interval (CI) 0·41–0·99, P = 0·04]. Sacral PUs were observed in 2·8% and 4·8% of the patients in the treatment group and the control group, respectively (RR 0·59, 95% CI 0·35–0·98, P = 0·04). Heel PUs occurred in 1·4% and 1·9% of patients in the treatment and control groups, respectively (RR 0·76, 95% CI 0·34–1·68, P = 0·49). Conclusions Silicone foam dressings reduce the incidence of PUs of category 2 or worse in hospitalized at-risk patients when used in addition to standard of care. The results show a decrease for the sacrum, but no statistical difference for the heel and trochanter areas.

Abstract

The aim of this study was to estimate genetic parameters for pork intramuscular fatty acid (FA) composition and indices for desaturase and elongase activities involved in n-3 and n-6 PUFA metabolism. The LM of 437 slaughter pigs was analyzed for FA composition (expressed as g/100 g of FA). Indices for enzyme activities were calculated from product to precursor FA ratios. Genetic parameters were estimated with single-and multi-trait animal models. The total FA content, reflecting the intramuscular fat content, was either included or not in the model. Results from the models without total FA content showed relatively high heritability estimates, generally above 0.50, for the proportion of the most important MUFA and PUFA, compared with much smaller values for the SFA. When total FA content was included in the models, heritability values decreased (P < 0.001) for most individual FA and for all sums of FA groups, except for C18:0, C18:3n-6, and C18:3n-3. Heritability estimates for the ratios C20:4n-6/C18:2n-6 and C22:6n-3/C18:3n-3, reflecting the overall conversion in the n-6 and n-3 PUFA pathway, respectively, were 0.29 and 0.35, respectively, with total FA content in the model and increased to 0.38 and 0.49, respectively, if total FA content was not in the model. Heritabilities for other more specific indices were of the same order. Genetic correlations between PUFA proportions and indices for enzyme activities with ADG were mostly negative, whereas the correlations with carcass lean meat percentage were mostly positive. It was concluded that there is meaningful genetic variation for long-chain PUFA metabolism that is only partly dependent on the carcass and muscle fat content. Abstract

In this experiment, the effect of duration and time of feeding n-3 PUFA sources on the fatty acid composition and oxidative stability of the longissimus thoracis (LT) muscle was investigated. Linseed (L) and fish oil (F), rich in [alpha]-linolenic acid and eicosapentaenoic and docosahexaenoic acid (EPA and DHA), respectively, were supplied equivalent to a level of 1.2% oil (as fed), either during the whole fattening period or only during the first (P1; 8 wk) or second (P2; 6 to 9 wk until slaughter) fattening phase. All diets were based on barley, wheat, and soybean meal and were fed ad libitum. Crossbred pigs (n = 154; Topigs 40 x Pietrain) were randomly allotted to the 7 feeding groups. In the basal diet (B), only animal fat was used as the supplementary fat source. Three dietary groups were supplied the same fatty acid source during both fattening phases (i.e., group BB, LL, and FF). For the other 4 dietary groups, the fatty acid source was switched after the first phase (groups BL, BF, LF, and FL; the first and second letter indicating the diet in P1 and P2, respectively). Twelve animals per feeding group were selected based on average live BW. The LT was analyzed for fatty acid composition; lipid stability (thiobarbituric acid-reactive substances) and color stability (a* value, % of myoglobin pigments) were determined on the LT after illuminated chill storage for up to 8 d. The [alpha]-linolenic acid, EPA, and docosapentaenoic acid incorporation was independent of the duration of linseed feeding (1.24, 0.54, and 0.75% of total fatty acids, respectively, for group LL). Supplying fish oil during both phases resulted in the greatest EPA and DHA proportions (1.37 and 1.02% of total fatty acids; P < 0.05), but the content of docosapentaenoic acid was not affected. The proportion of DHA was greater when fish oil was administered during P2 compared with P1 (P < 0.05). There was no effect of diet on meat ultimate pH and drip loss or on lipid or color oxidation.
