Adenocarcinoma and lymphoma, potential complications of Crohn's disease (CD), may result in small intestinal perforations, particularly in those on immunosuppressive therapy. The ileum is typically the site of small intestinal perforations in CD, and the link between CD and lymphoma remains uncertain. This case report explores a long-term CD patient on immunosuppressive therapy who presented with acute abdominal pain. Imaging revealed signs of intestinal perforation, successfully managed with surgery. The final pathology report confirms the diagnosis of diffuse large B-cell lymphoma. Perforation of the gastrointestinal tract by ingested foreign body is an uncommon surgical emergency, most typically associated with the consumption of fish and chicken bones. We present an unusual case of a gentleman presenting emergently with an acute abdomen following ingestion of a meal containing frog meat. Emergent computed tomography (CT) revealed findings suggestive of jejunal perforation due to a foreign body. At laparotomy, a mid-jejunal site of perforation was noted due to a protruding piece of fractured frog bone. Background: Small bowel perforations are a rare diagnosis compared with esophageal, gastric, and colonic perforations. However, small bowel perforations can be fatal if left untreated. A classification of small bowel perforations or treatment recommendations do not exist to date. Methods: A retrospective, monocentric, code-related data analysis of patients with small bowel perforations was performed for the period of 2010 to 2019. Results: Over a 10-year period, 267 cases of small bowel perforation in 257 patients (50.2% male and 49.8% female; mean age of 60.28 years) were documented. Perforation's localization was 5% duodenal, 38% jejunal, 39% ileal, and 18% undocumented. Eight etiologies were differentiated: iatrogenic (41.9%), ischemic (20.6%), malignant (18.9%), inflammatory (8.2%), diverticula-associated (4.5%), traumatic (4.5%), foreign-body-associated (1.9%), and cryptical (1.5%) perforations. Operative treatment combined with antibiotics was the most commonly used therapeutic approach (94.3%). The mortality rate was 14.23%, with highest rate for patients with ischemic perforations. Discussion: An algorithm for diagnostic and therapeutic steps was established. Furthermore, it was found that small bowel perforations are rare events with poor outcomes. Intestinal tuberculosis (TB) is a frequently encountered pathology by surgeons all over India. There exists a vast body of knowledge about this disease; however, a detailed understanding of its presentation as well as surgical management is essential for every Indian surgeon, given its rampant nature. This report discusses the case of a 28-year-old female presenting with severe left upper abdominal pain, non-bilious vomiting, and fever, who was ultimately diagnosed with small bowel TB leading to perforations. Despite a history of pulmonary TB treated a year prior, the patient exhibited significant clinical and imaging findings, including pneumoperitoneum and peritonitis. Exploratory laparotomy revealed multiple tubercular perforations in the mid-jejunum and a stricture causing proximal jejunal dilatation. Surgical intervention involved resection of the affected segment and end-to-end anastomosis. Histopathological analysis confirmed TB as the cause. Objective: To determine the frequency of small bowel perforation in neonates with pneumoperitoneum. Study Design: Descriptive Cross-sectional study. Setting: Department of Pediatrics Surgery, National Institute of Child Health Karachi. Period: November 2018 to April 2019. Material & Methods: An observational study conducted to identify clinical presentation, risk factors, site of perforation and outcome of pneumoperitoneum in neonates. Data collected was entered and analyzed in the SPSS version 21. Mean with standard deviation was calculated for quantitative variables like age, gestational age, weight and duration of symptoms. Results: A total of 87 neonate presenting with abdominal distension and bilious vomiting presenting within 24 hours diagnosed as pneumoperitoneum with chest X-ray erect shows gas under diaphragm were included in this study. The average age of the neonate was 11.02±8.52 days. There were 48 (55.17%) male and 39(44.83%) female. Frequency of small bowel perforation in neonates with pneumoperitoneum was observed in 31.03 %(27/87). Conclusion: It is to be concluded rate of small bowel perforation was significantly high in those neonate who had below and equal to 2 kg weight and also high in those neonates who presented with in 12-24 hours duration of symptoms. Abstract Background True primary enterolithiasis is an uncommon condition, and nontraumatic perforation of the small intestine (NTPSI) is also an unusual entity. Therefore, NTPSI due to true primary enteroliths is an exceptionally rare complication. Moreover, enterolithiasis and radiation enteritis are also unique combinations. Herein, we present an exceedingly rare case of NTPSI induced by multiple true primary enteroliths associated with radiation enteritis. Case presentation A 92-year-old woman with acute abdominal pain was transferred to our hospital because a computed tomography (CT) scan performed by her family doctor revealed free air and fluid collection within her abdomen. Our initial diagnosis was upper gastrointestinal perforation, and we selected nonoperative management (NOM) with adnominal drainage. Although her general condition was stable, jejunal juice was drained continuously. Given that the CT performed 10 days after onset demonstrated perforation of the small intestine and adjacent concretion, we performed an emergency partial resection of the small intestine and jejunostomy. The resected bowel was 1 m in length and had many strictures that contained multiple enteroliths in their proximal lumens. The patient’s postoperative course was uneventful. The enteroliths were composed of deoxycholic acid (DCA). She was diagnosed with peritonitis due to NTPSI derived from multiple true primary enteroliths associated with radiation enteritis, as she had previously undergone hysterectomy and subsequent internal radiation therapy. Conclusions Clinicians should consider the rare entity of true primary enteroliths associated with radiation enteritis in NTPSI cases with unknown etiologies.

Aim:In this study, we aimed to share our surgical experiences and clinical results in non-traumatic small bowel perforations.Method:Patients who underwent surgical treatment for non-traumatic small bowel perforation between 2009-2019 were included in the study. Patients were divided into two groups according to postoperative 90-day mortality status: Group 1 (no mortality), Group 2 (mortality). The demographic, clinical features, treatment methods and results of the patients were compared between the groups.Results:Forty-two patients participated in our study. Group 1 consisted of 25, and Group 2 consisted of 17 patients. Male sex was dominant in Group 2 (48% vs 76.5%, p=0.062). Mean age was higher in Group 2 (54 vs 61, p=0.218). American Anesthesiology Association score (ASA) was statistically significantly higher in Group 2 (12% vs 58.8%). Forty-two percent of patients had abdominal surgery and 30% had a history of malignancy. The presence of electrolyte imbalance was similar in the groups (56% vs 76.5%, p=0.049). The days between the first complaint and laparotomy were similar between the groups (6.40 vs. 5.70 p=0.699). In perforated areas, jejunum was dominant in Group 2 with 64.7%, and ileum in Group 1 with 68%. The multiple perforation rate was higher in Group 2, but was not statistically significant (12% vs 23.5%, p=0.284). From postoperative complications, anastomosis leakage was higher in Group 1, but it was not statistically significant (12% vs 5.9%, p=0.501).Conclusion:Morbidity and mortality of non-traumatic small bowel perforations is high. While the ASA score and hypoalbuminemia were associated with postoperative mortality in non-traumatic small bowel perforations, we did not find the localization of the perforation and the time between the first complaint and laparotomy to be related to mortality.

Abstract Background Diagnosis of intestinal tuberculosis poses a dilemma to physicians due to nonspecific symptoms like abdominal pain, fever, nausea, and a change in bowel habit. In particular, the distinction between inflammatory bowel disease and intestinal tuberculosis remains challenging. Case presentation A 27-year-old man from Colombia presented with fever, night sweats, and progressive lower abdominal pain. Computed tomography revealed a thickening of the bowel wall with a mesenterial lymphadenopathy, ascites ,and a pleural tumor mass. Histology of intestinal and pleural biopsy specimens showed a granulomatous inflammation. Although microscopy and polymerase chain reaction (PCR) for Mycobacterium tuberculosis (MTB) were negative, empirical MTB treatment was initiated on suspicion. Due to a massive post-stenotic atrophied intestinal bowel, MTB medications were administered parenterally in the initial phase of treatment to guarantee adequate systemic resorption. The complicated and critical further course included an intra-abdominal abscess and bowel perforation requiring a split stoma, before the patient could be discharged in good condition after 3 months of in-hospital care. Conclusions This case highlights the clinical complexity and diagnostic challenges of intestinal MTB infection. A multidisciplinary team of physicians should be sensitized to a timely diagnosis of this disease, which often mimics inflammation similar to inflammatory bowel disease, other infections, or malignancies. In our case, radiological findings, histological results, and migratory background underpinned the suspected diagnosis and allowed early initiation of tuberculostatic treatment.

Abstract Background Cholesterol crystal embolism (CCE) following transcatheter arterial chemoembolization (TACE) is rare. Case presentation A 71-year-old man underwent TACE for recurrence of hepatocellular carcinoma (HCC). On postoperative day (POD) 5, he developed abdominal pain and fever. Computed tomography revealed intraperitoneal free air. The patient was diagnosed with gastrointestinal perforation with peritonitis, for which partial intestinal resection and covering ileostomy were performed. Histological examination revealed perforation of the small intestine caused by CCE. The patient made a satisfactory recovery and was discharged on POD 30. The patient showed no recurrence of cholesterol crystal embolism or HCC for 2 years after surgery. Conclusion We reported a successfully treated case of ischemic small bowel perforation due to cholesterol crystal embolism following transcatheter arterial chemoembolization for recurrent HCC.

Abstract Undifferentiated abdominal pain is a common presentation often requiring immediate medical or surgical intervention. Providing an accurate diagnosis involves a detailed patient history and thorough physical exam. Point of care ultrasound is gaining acceptance as a rapid diagnostic tool that can be used to accurately detect life-threatening conditions while potentially avoiding unnecessary radiation exposure and facilitating rapid treatment. Detection of pneumoperitoneum with point-of-care ultrasound is a simple procedure that relies heavily on the experience of the investigating practitioner. Standard technique involves placing a high-frequency linear-array transducer in the right upper quadrant, where abdominal free air is most likely to accumulate. Detection of the ‘gut point’, which is the transition of abdominal wall sliding to lack thereof in a single image, is the pathognomonic finding of pneumoperitoneum. If visualization is difficult, moving the patient to the left lateral decubitus position or using the scissors technique can provide additional image views. This representative case report and review highlights the use of abdominal POCUS for the diagnosis of pneumoperitoneum. Ultrasound should continue to be explored by clinicians to narrow the differential diagnosis of acute abdominal pain.

Incisional hernia which if not repaired at the earliest might increase in size and eventually end up in complications like incarceration or strangulation. Perforation of the bowel loop in case of incarcerated or strangulated incisional hernia without any precipitating events like trauma is very rare. Intestinal perforation, if identified is a life-threatening condition that requires emergency surgical management. Perforation of the small bowel along its mesenteric border is a rare entity with only very few cases being reported. Authors hereby, present a case of a 55-year-old female with long standing incisional hernia who was diagnosed as an incarcerated incisional hernia after Computed Tomography (CT) and was proceeded with an exploratory laparotomy, which on further exploration revealed perforation of the incarcerated ileum along its mesenteric border for which resection and anastomosis was done.

Bowel perforation is an emergency condition. Common causes of bowel perforation include infection, infarction, radiation enteritis, Crohn's disease, and cancer. Ingested foreign body causing bowel perforation is rare. Wooden toothpick-related injuries are uncommon. We report a case of ileal perforation caused by ingested wooden toothpick preoperatively diagnosed by ultrasound and computed tomography.

Ehlers-Danlos Syndrome (EDS) is a rare connective tissue disorder characterized by mutation in genes that encode or modify collagen. Clinical findings in these patients include skin hyperextensibility, hypermobility of joints, and tissue fragility. Vascular EDS (vEDS) is an autosomal dominant disease typically caused by a mutation in COL3A1 , which encodes type III collagen. Presenting signs in the majority of vEDS patients include arterial rupture, uterine rupture, and sigmoid colon perforation. We report a rare case of a 77-year-old man with a known left inguinal hernia presenting with groin pain following a blunt trauma of the left leg. Diagnosis of small bowel perforation away from the hernia was obtained only in surgery. Difficulty in preoperative diagnosis, rarity of histologic pattern, and surgical challenges make this case very interesting for surgeons and radiologists. Our conclusion upon dealing with the situation is that the diagnosis of small bowel perforation following blunt injury to a non-abdominal trauma is rare and difficult. The association between inguinal hernia and non-abdominal trauma may result in small bowel injuries that normally do not appear. Therefore, clinicians should be cautious in treating non-abdominal trauma patients with inguinal hernias.

Purpose: Define factors for proper diagnosis and treatment of small intestinal injury during procedures with percutaneous renal access, thus optimizing favorable outcomes and avoiding complications and death during conservative or surgical approaches. Materials and methods: Bibliographic review of case reports available in the literature and presentation of data from an additional case have been carried out. Results: Percutaneous nephrolithotripsy was the procedure that most frequently caused injury of the small intestine. Time for diagnosis of the lesion took up to 5 days after the intraoperative phase. When occurring in the intraoperative phase, perforation was identified by direct endoscopic visualization; a catheter was then placed inside the intestinal lumen and a conservative approach to the derived fistula was adopted, which led to successful outcomes in all cases. Abdominal pain was the most common symptom in cases diagnosed during the postoperative phase (75%). In the presence of signs of peritonitis, surgical intervention was performed, with favorable evolution in all cases. Conclusions: Conservative management of small intestine injuries is possible when there is no peritoneal contamination. Its success factors include intraoperative diagnosis and non-transfixing lesions, which is more common in duodenal involvement. Laparotomy to clean the cavity associated with a corrective approach (enterorrhaphy or enterectomy with primary anastomosis) was successfully indicated in cases of late diagnosis with signs of peritonitis, a situation that is most commonly found in transfixing lesions of ileum and jejunum. Although rare, small bowel perforation post pregnancy related dilation and curettage presents a serious complication. Herein, we reported a case of 34-year-old female patient presenting for uterine-ileal perforation post pregnancy related dilatation and curettage managed successfully by laparoscopic small bowel resection and primary anastomosis.

A ventriculoperitoneal (VP) shunt is a standard treatment option for the treatment of hydrocephalus. Small bowel perforation is a rare complication of VP shunt placement. We describe a case and image findings of a 15-year-old male with VP shunt who had an asymptomatic small bowel perforation. He had a history of constipation and occasional abdominal pain. The imaging findings were confirmed surgically. The results of abdominal complications of VP shunts are excellent when diagnosed and treated early. Mortality and morbidity decrease significantly with early diagnosis and treatment, especially in asymptomatic bowel perforations. We also provide an overview of the current literature discussing previously reported cases, clinical features, and treatment.

Percutaneous access for treatment of renal pathologies is a minimally invasive modality, although it can present complications. Small bowel lesions are rare but correct diagnosis and management are essential to prevent major complications. A patient submitted to an uncomplicated percutaneous nephrolithotomy presented jejunal transfixing perforation with a stable clinical progression. It was first managed conservatively unsuccessfully. Therefore, a laparotomy with enterectomy was necessary, with a favorable outcome. In transfixing lesions of the small bowel, diagnosis may be difficult and delayed. This contributes to conservative management failures and the requirement of laparotomy with enterectomy in order to reduce further complications.

A 74-year-old man visited our hospital complaining of abdominal pain. An abdominal computed tomography scan showed multiple wall thickness of the small bowel. Capsule endoscopy was recommended for further evaluation, and patency capsule examination was performed. Eighteen hours after patency capsule ingestion, he experienced small bowel perforation with severe peritonitis caused by intestinal pressure rising because of the patency capsule trapped in his terminal ileum. An ileocolic resection was performed, including the removal of the sclerotic ileum as an emergency surgery. A pathological examination showed transmural inflammation and multiple ulcers with perforation of the small intestine, consistent with Crohn’s disease. Here, we report a rare and valuable case of novel tag-less AgileTM patency capsule (Given Imaging Ltd., Yoqneam, Israel) retention leading to small bowel perforation.

Pancreatic pseudocysts are a known complication of pancreatitis. There are a multitude of complications which have been described in relation to pancreatic pseudocysts. Perforation of a gastrointestinal hollow viscus is rare but has previously been reported. We report a case of a 72-year-old female with the development of three pancreatic pseudocysts four weeks post gallstone pancreatitis. The patient deteriorated and was found to have a duodenal perforation due to compression forces from the underlying pseudocysts. Presently, the advantage of laparoscopic surgery is widely recognized and it is gaining popularity not only in elective abdominal surgeries but also in various kinds of emergent abdominal surgeries. This report describes two patients diagnosed with ileal perforation due to an ingested foreign body (FB)—a rare abdominal emergency—who were treated laparoscopically. Both patients were brought in by an ambulance to our hospital, with complaints of increasing lower abdominal pain. Computed tomography scan revealed a small bowel perforation due to an ingested FB in both patients, and laparoscopy-assisted partial ileal resection was immediately performed. Their postoperative recoveries were uneventful. Both patients with this rare abdominal emergency were successfully treated with laparoscopic surgery. Its feasible diagnostic and therapeutic abilities and decreased invasiveness contributed to their uneventful and fast recoveries. Extracorporeal handling of the affected small bowel via mini-laparotomy was useful in this setting as it was safe, fast, and cost-effective. Laparoscopic surgery is effective, even for this rare abdominal emergency. We present a rare case of a focal perforation of the jejunum after a high-speed motor vehicle crash. A 60-year-old restrained rear seat passenger presented with severe abdominal pain. She was hemodynamically stable and underwent the traditional trauma workup. CT scan of the abdomen showed large-volume free intraperitoneal air and L4/L5 compression fractures. Given the peritoneal physical exam finding and free air on CT scan she was taken emergently to the operating room. Operative exploration revealed free intraperitoneal air upon entry into the abdominal cavity as well as murky fluid throughout the mid abdomen. A focal perforation was discovered on the antimesenteric surface of a segment of jejunum. The perforation was repaired primarily in two layers and the abdomen was closed. Postoperative course was uncomplicated. Antibiotics were continued for 4 days. Focal perforation of the small bowel from high-speed blunt trauma is a rare isolated injury. Close attention to physical exam and radiologic findings allows for early diagnosis and treatment of these injuries. [ABSTRACT FROM AUTHOR]

Bowel perforation is a surgical emergency. There are many causes of bowel perforation commonest being infection, infarction, radiation enteritis, crohn's disease and carcinomas. Bowel perforation secondary to tooth pick are not so common. Tooth picks are used after food intake for cleaning purpose. They are made from bamboo and wood and have hard material with sharp end. Tooth pick are usually ingested un-intentionally. Patient without adequate history of ingestion pose difficulty in diagnosis. Patients with less developed palatal sensations are prone to accidental ingestion. A ventriculoperitoneal (VP) shunt is a standard treatment option for the treatment of hydrocephalus. Small bowel perforation is a rare complication of VP shunt placement. We describe a case and image findings of a 15-year-old male with VP shunt who had an asymptomatic small bowel perforation. He had a history of constipation and occasional abdominal pain. The imaging findings were confirmed surgically. The results of abdominal complications of VP shunts are excellent when diagnosed and treated early. Mortality and morbidity decrease significantly with early diagnosis and treatment, especially in asymptomatic bowel perforations. This article reviews the various aetiologies of small bowel perforations and their management. In addition to the well-known aetiologies such as trauma, inflammation and circulatory disorders, several new causes of small bowel perforation have been described in recent years. The spectrum reaches from iatrogenic perforations during laparoscopic surgery or enteroscopies to drug-induced perforations with new anticancer agents. The management of small bowel perforations requires a concept consisting of the safe revision of the leaking bowel and the treatment of the peritonitis. Depending on the local situation and the condition of the patient, several treatment options are available. The surgical management of the bowel leak can range from a simple primary closure to a delayed restoration of bowel continuity. When the condition of the bowel or patient is frail, the risk of a failure of a closure or anastomosis is too high, and the exteriorization of the bowel defect as a primary measure is a safe option. The treatment of the peritonitis is also dependent on the condition of the patient and the local situation. Early stages of peritonitis can be treated by a simple peritoneal lavage, either performed by laparoscopy or laparotomy. Severe forms of peritonitis with multi-organ failure and an abdominal compartment syndrome need repeated peritoneal revisions. In such cases, the abdomen can only be closed temporarily. Different technical options are available in order to overcome the difficult care of these patients.
