Libraries play an increasingly significant role in responding to the needs of the local economy. They have the resources to survey the business community to determine what these needs are. Libraries can also form partnerships with government agencies and nonprofits that can help to foster a thriving economy. Programming and events can be targeted to assist segments of the population that have traditionally been underserved or have only limited access to the resources needed for business creation. Richland Library in Columbia, SC, and the Baltimore County Public Library are cited for the community partnerships they have formed and for their innovative programming in support of small business. BACKGROUND: Small intestinal bacterial overgrowth (SIBO) is a common, yet underdiagnosed, gut condition caused by gut dysbiosis. A previous study has shown the potential of herbal therapy, providing equivalent results to rifaximin. OBJECTIVES: The objective of this study was to assess how the use of an oral botanical regimen may modulate the gut microbiome, facial erythema, and intestinal permeability in those with SIBO. METHODS: This was an open-label prospective study of adults that had lactulose breath test-confirmed SIBO. Participants received a 10-week oral supplementation of a Biocidin liquid tincture and GI Detox+. If participants were found to be non-responsive to treatment after 10 weeks with a persistently positive lactulose breath test, a third oral supplement, Olivirex, was administered for an additional 4 weeks. Lactulose breath tests were administered at baseline, weeks 6, 10, and 14 to assess for SIBO status. A high-resolution photographic analysis system was utilized to analyze changes in facial erythema. Stool sample collections and venipuncture were performed to analyze the gut microbiome and intestinal permeability. RESULTS: A total of 33 subjects were screened with breath testing, and 19 subjects were found to have SIBO. Three of the subjects withdrew during the screening period prior to baseline, and sixteen subjects enrolled. Four subjects dropped out after baseline. Hydrogen-dominant SIBO was the most common subtype of SIBO, followed by methane and hydrogen sulfide. The botanical regimen was most effective for hydrogen- and hydrogen sulfide-dominant SIBO, leading to negative breath test results at week 10 in 42.8% and 66.7% of participants, respectively. Compared to baseline, supplementation with the botanical regimen led to positive shifts in short-chain fatty acid-producing bacteria such as A. muciniphila, F. prausnitzii, C. eutectus, and R. faecis by 31.4%, 35.4%, 24.8%, and 48.7% percent at week 10, respectively. The mean abundance of Firmicutes decreased by 20.2%, Bacteroides increased by 30%, and the F/B ratio decreased by 25.4% at week 10 compared to baseline. At week 10, there was a trending 116% increase in plasma LPS/IgG (p = 0.08). There were no significant changes in plasma zonulin, DAO, histamine, DAO/histamine, LPS/IgG, LPS/IgA, or LPS/IgM. Facial erythema was not statistically different at week 6, but at week 10, there was a 20% decrease (p = 0.001) in redness intensity. Among the patients that extended to week 14, there was no statistical change in erythema. CONCLUSIONS: Supplementation with an antimicrobial botanical supplemental regimen may have therapeutic potential in hydrogen and hydrogen-sulfide subtypes of SIBO. Furthermore, the botanical supplemental regimen may reduce facial erythema, increase SCFA-producing bacteria, decrease the F/B ratio, and modulate markers of intestinal permeability.

A neural network was developed to detect and characterize bowel obstruction, a common cause of acute abdominal pain. In this retrospective study, 202 CT scans of 165 patients with bowel obstruction from March to June 2022 were included and partitioned into training and test data sets. A multi-channel neural network was trained to segment the gastrointestinal tract, and to predict the diameter and the longitudinal position (longitude) along the gastrointestinal tract using a novel embedding. Its performance was compared to manual segmentations using the Dice score, and to manual measurements of the diameter and longitude using intraclass correlation coefficients (ICC). ROC curves as well as sensitivity and specificity were calculated for diameters above a clinical threshold for obstruction, and for longitudes corresponding to small bowel. In the test data set, Dice score for segmentation of the gastrointestinal tract was 78 ± 8%. ICC between measured and predicted diameters was 0.72, indicating moderate agreement. ICC between measured and predicted longitude was 0.85, indicating good agreement. AUROC was 0.90 for detection of dilated bowel, and was 0.95 and 0.90 for differentiation of the proximal and distal gastrointestinal tract respectively. Overall sensitivity and specificity for dilated small bowel were 0.83 and 0.90. Since obstruction is diagnosed based on the diameter and longitude of the bowel, this neural network and embedding may enable detection and characterization of this important disease on CT.

Messenger RNA (mRNA) recruitment to the 40S ribosomal subunit is mediated by eukaryotic initiation factor 4F (eIF4F). This complex includes three subunits: eIF4E (m7G cap-binding protein), eIF4A (DEAD-box helicase), and eIF4G. Mammalian eIF4G is a scaffold that coordinates the activities of eIF4E and eIF4A and provides a bridge to connect the mRNA and 40S ribosomal subunit through its interaction with eIF3. While the roles of many eIF4G binding domains are relatively clear, the precise function of RNA binding by eIF4G remains to be elucidated. In this work, we used an eIF4G-dependent translation assay to reveal that the RNA binding domain (eIF4G-RBD; amino acids 682-720) stimulates translation. This stimulating activity is observed when eIF4G is independently tethered to an internal region of the mRNA, suggesting that the eIF4G-RBD promotes translation by a mechanism that is independent of the m7G cap and mRNA tethering. Using a kinetic helicase assay, we show that the eIF4G-RBD has a minimal effect on eIF4A helicase activity, demonstrating that the eIF4G-RBD is not required to coordinate eIF4F-dependent duplex unwinding. Unexpectedly, native gel electrophoresis and fluorescence polarization assays reveal a previously unidentified direct interaction between eIF4G and the 40S subunit. Using binding assays, our data show that this 40S subunit interaction is separate from the previously characterized interaction between eIF4G and eIF3. Thus, our work reveals how eIF4F can bind to the 40S subunit using eIF3-dependent and eIF3-independent binding domains to promote translation initiation.

Plasticity among cell lineages is a fundamental, but poorly understood, property of regenerative tissues. In the gut tube, the small intestine absorbs nutrients, whereas the colon absorbs electrolytes. In a striking display of inherent plasticity, adult colonic mucosa lacking the chromatin factor SATB2 is converted to small intestine. Using proteomics and CRISPR-Cas9 screening, we identify MTA2 as a crucial component of the molecular machinery that, together with SATB2, restrains colonic plasticity. MTA2 loss in the adult mouse colon activated lipid absorptive genes and functional lipid uptake. Mechanistically, MTA2 co-occupies DNA with HNF4A, an activating pan-intestinal transcription factor (TF), on colonic chromatin. MTA2 loss leads to HNF4A release from colonic chromatin, and accumulation on small intestinal chromatin. SATB2 similarly restrains colonic plasticity through an HNF4A-dependent mechanism. Our study provides a generalizable model of lineage plasticity in which broadly-expressed TFs are retained on tissue-specific enhancers to maintain cell identity and prevent activation of alternative lineages, and their release unleashes plasticity.

BACKGROUND: Metastatic melanoma to the small bowel is an aggressive disease often accompanied by obstruction, abdominal pain, and gastrointestinal bleeding. With advancements in melanoma treatment, the role for metastasectomy continues to evolve. Inclusion of novel immunotherapeutic agents, such as checkpoint inhibitors, into standard treatment regimens presents potential survival benefits for patients receiving metastasectomy. CASE PRESENTATION: We report an institutional experience of 15 patients (12 male, 3 female) between 2014-2022 that underwent small bowel metastasectomy for metastatic melanoma and received perioperative systemic treatment. Median age of patients was 64 years (range: 35-83 years). No patients died within 30 days of their surgery, and the median hospital length of stay was 5 days. Median overall survival in these patients was 30.1 months (range: 2-115 months). Five patients died from disease (67 days, 252 days, 426 days, 572 days, 692 days postoperatively), one patient died of non-disease related causes (1312 days postoperatively), six patients are alive with disease, and three remain disease free. CONCLUSIONS: This case series presents an updated perspective of the utility of metastasectomy for small bowel metastasis in the age of novel immunotherapeutic agents as standard systemic treatment. Small bowel metastasectomy for advanced melanoma performed in conjunction with perioperative systemic therapy is safe and appears to promote long-term survival and enhanced quality of life.

PURPOSE: Intestinal obstruction caused by intestinal atresia is a surgical emergency in newborns. Outcomes for the jejunal ileal atresia (JIA), the most common subtype of atresia in low-income countries (LIC), are poor. We sought to assess the impact of utilizing the Bishop-Koop (BK) approach to JIA in improving outcomes. METHODS: A retrospective cohort study was performed on children with complex JIA (Type 2-4) treated at our national referral hospital from 1/2018 to 12/2022. The mechanism that triggers the progressive dysregulation of cell functions, inflammation, and breakdown of tissues during aging is currently unknown. We propose here a previously unknown mechanism due to tissue autodigestion by the digestive enzymes. After synthesis in the pancreas, these powerful enzymes are activated and transported inside the lumen of the small intestine to which they are compartmentalized by the mucin/epithelial barrier. We hypothesize that this barrier leaks active digestive enzymes (e.g. during meals) and leads to their accumulation in tissues outside the gastrointestinal tract. Using immune-histochemistry we provide evidence in young (4 months) and old (24 months) rats for significant accumulation of pancreatic trypsin, elastase, lipase, and amylase in peripheral organs, including liver, lung, heart, kidney, brain, and skin. The mucin layer density on the small intestine barrier is attenuated in the old and trypsin leaks across the tip region of intestinal villi with depleted mucin. The accumulation of digestive enzymes is accompanied in the same tissues of the old by damage to collagen, as detected with collagen fragment hybridizing peptides. We provide evidence that the hyperglycemia in the old is accompanied by proteolytic cleavage of the extracellular domain of the insulin receptor. Blockade of pancreatic trypsin in the old by a two-week oral treatment with a serine protease inhibitor (tranexamic acid) serves to significantly reduce trypsin accumulation in organs outside the intestine, collagen damage, as well as hyperglycemia and insulin receptor cleavage. These results support the hypothesis that the breakdown of tissues in aging is due to autodigestion and a side-effect of the fundamental requirement for digestion.

Translation is regulated mainly in the initiation step, and its dysregulation is implicated in many human diseases. Several proteins have been found to regulate translational initiation, including Pdcd4 (programmed cell death gene 4). Pdcd4 is a tumor suppressor protein that prevents cell growth, invasion, and metastasis. It is downregulated in most tumor cells, while global translation in the cell is upregulated. To understand the mechanisms underlying translational control by Pdcd4, we used single-particle cryo-electron microscopy to determine the structure of human Pdcd4 bound to 40S small ribosomal subunit, including Pdcd4-40S and Pdcd4-40S-eIF4A-eIF3-eIF1 complexes. The structures reveal the binding site of Pdcd4 at the mRNA entry site in the 40S, where the C-terminal domain (CTD) interacts with eIF4A at the mRNA entry site, while the N-terminal domain (NTD) is inserted into the mRNA channel and decoding site. The structures, together with quantitative binding and in vitro translation assays, shed light on the critical role of the NTD for the recruitment of Pdcd4 to the ribosomal complex and suggest a model whereby Pdcd4 blocks the eIF4F-independent role of eIF4A during recruitment and scanning of the 5' UTR of mRNA.

Objective: Rare disease Background: Small bowel transplantation (SBT) is a rare but life-saving surgery. However, successful full-term pregnancies in individuals with SBT are exceedingly rare due to the nutritional and immunosuppression challenges this transplant poses for pregnancy. Therefore, clear guidelines for treating pregnant SBT recipients are unavailable. Here, we report the second case of a successful pregnancy in an individual with a triple organ transplant, including SBT, highlighting the need for careful immunosuppressive management and multidisciplinary care. Case Report: A 20-year-old woman in the third trimester of pregnancy with a history of small bowel, liver, and pancreas transplantation at age 1 year presented with elevated liver function test results. She had been taking tacrolimus, sirolimus, and prednisone before pregnancy, with no signs of organ rejection. While sirolimus and prednisone was discontinued upon conception, laboratory test results at presentation revealed low serum tacrolimus levels. The patient had an acute kidney injury and pulmonary edema during her hospitalization and received a diagnosis of preeclampsia. She underwent a successful cesarean delivery, due to labor induction complications; however, about 1 month after hospital discharge, the patient experienced elevated liver enzymes, which was treated with high-dose steroids and adjusted tacrolimus. Sirolimus was restarted, and the patient's liver enzymes have been normalized to date. Conclusions: Comprehensive multidisciplinary care, as well as monitoring and optimizing immunosuppression, are essential for pregnant SBT recipients throughout the prenatal, perinatal, and postpartum periods to mitigate risks, prevent graft rejection, and ensure positive maternal and fetal health outcomes. Purpose Methods Results Conclusions To compare vitrectomy with and without internal limiting membrane (ILM) peeling in small idiopathic macular holes.Retrospective multicentre study including consecutive eyes with ≤250 μm idiopathic macular hole treated with vitrectomy. The primary outcome was hole closure rate. Best‐corrected visual acuity (BCVA) change, closure patterns on optical coherence tomography, rates of external limiting membrane (ELM) and ellipsoid zone (EZ) recovery, and rate of complications were also investigated.In total, 693 eyes were included. Hole closure rate was 98% in the peeling and 85% in the no‐peeling group (p < 0.001). At 12 months, mean BCVA change was 0.38 ± 0.22 logMAR in the peeling and 0.45 ± 0.21 logMAR in the no‐peeling group (p = 0.02); 66% versus 80% of eyes had a U‐shaped morphology, respectively; EZ recovery rate was 75% and 93%, respectively (p = 0.02). In the no‐peeling group, eyes with a vitreomacular traction (VMT) showed a 96% closure rate, comparable to the peeling group (p = 0.40). The incidence of adverse events was similar except for dissociated optic nerve fibre layer (55% in the peeling vs. 9% in the no‐peeling group, p < 0.001).In small idiopathic macular holes, ILM peeling provides a higher closure rate compared to no‐peeling; however, if a VMT is present closure rates are comparable. Gallstone ileus results in a mechanical small bowel obstruction when an itinerant gallstone tumbles downstream and obstructs the bowel lumen. Associated proximal intestinal injury with perforation is rare, and concomitant perforation of multiple segments of bowel in the setting of gallstone ileus has never been reported in the literature. We are reporting the case of a 67-year-old female patient who had gallstone ileus causing perforation of multiple segments of small bowel. At operation, she was found to have a 3.2-cm gallstone lodged at the terminal ileum, perforation of both the mid ileum and mid jejunum, and gross enteric spillage. She underwent removal of the gallstone and small bowel resection × 2. She was initially left in discontinuity with an open abdomen. She returned to the operating room 2 days later for bowel anastomosis and abdominal closure. She recovered well after surgery and was discharged home. [ABSTRACT FROM AUTHOR]

Small, Medium and Micro Enterprises (SMMEs) are crucial to South Africa's economy, significantly contributing to job creation and innovation. However, they face challenges such as limited access to finance, skills shortages, and difficulties in adopting emerging technologies, which hinder their growth and sustainability. This study aims to explore the role of innovative entrepreneurship and technology in enhancing SMME performance. Through a systematic literature review and thematic analysis, the research identifies twelve key themes that underscore how innovative entrepreneurship can drive adaptability, improve operational efficiency, and facilitate market access. Findings indicate that innovative business models and practices are essential for SMMEs to overcome financial and operational barriers, enabling them to respond effectively to market demands. The study recommends that the South African government and private sector collaborate to enhance financial accessibility, develop targeted training programmes, and improve infrastructure. By fostering a supportive environment for innovative entrepreneurship, SMMEs can leverage emerging technologies more effectively, ultimately driving economic growth and social development in the country. To mediate critical host-microbe interactions in the human small intestine, Paneth cells constitutively produce abundant levels of α-defensins and other antimicrobials. We report that the expression profile of these antimicrobials is dramatically askew in human small intestinal organoids (enteroids) as compared to that in paired tissue from which they are derived, with a reduction of α-defensins to nearly undetectable levels. Murine enteroids, however, recapitulate the expression profile of Paneth cell α-defensins seen in tissue. WNT/TCF signaling has been found to be instrumental in the regulation of α-defensins, yet in human enteroids exogenous stimulation of WNT signaling appears insufficient to rescue α-defensin expression. By stark contrast, forkhead box O (FOXO) inhibitor AS1842856 induced the expression of α-defensin mRNA in enteroids by >100,000-fold, restoring DEFA5 and DEFA6 to levels comparable to those found in primary human tissue. These results newly identify FOXO signaling as a pathway of biological and potentially therapeutic relevance for the regulation of human Paneth cell α-defensins in health and disease.

Paneth cells (PCs), a specialized secretory cell type in the small intestine, are increasingly recognized as having an essential role in host responses to microbiome and environmental stresses. Whether and how commensal and pathogenic microbes modify PC composition to modulate inflammation remain unclear. Using newly developed PC-reporter mice under conventional and gnotobiotic conditions, we determined PC transcriptomic heterogeneity in response to commensal and invasive microbes at single cell level. Infection expands the pool of CD74+ PCs, whose number correlates with auto or allogeneic inflammatory disease progressions in mice. Similar correlation was found in human inflammatory disease tissues. Infection-stimulated cytokines increase production of reactive oxygen species (ROS) and expression of a PC-specific mucosal pentraxin (Mptx2) in activated PCs. A PC-specific ablation of MyD88 reduced CD74+ PC population, thus ameliorating pathogen-induced systemic disease. A similar phenotype was also observed in mice lacking Mptx2. Thus, infection stimulates expansion of a PC subset that influences disease progression.

The Fourth Cell Stress Society International workshop on small heat shock proteins (sHSPs), a follow-up to successful workshops held in 2014, 2016 and 2018, took place as a virtual meeting on the 17-18 November 2022. The meeting was designed to provide an opportunity for those working on sHSPs to reconnect and discuss their latest work. The diversity of research in the sHSP field is reflected in the breadth of topics covered in the talks presented at this meeting. Here we summarise the presentations at this meeting and provide some perspectives on exciting future topics to be addressed in the field.

The aim of this paper is to examine the use of project management practices in Slovenian micro and small firms (MSFs) and to identify project success factors (SF) and project success criteria (SC).

In Malaysia, the B40 segment, particularly low-income women entrepreneurs, faces distinct challenges in adopting technology within their businesses. This qualitative study explores the low uptake of technology adoption among these entrepreneurs to understand the underlying dynamics. By addressing the "what" and "why" of low technology adoption, the research aims to provide insights for fostering inclusivity and empowerment within Malaysia's entrepreneurial landscape. The study involved an in-depth focus group discussion with 25 women entrepreneurs from four states across Malaysia, utilizing thematic analysis with Atlas TI. Key findings reveal issues related to technology adoption based on TAM theory. The findings indicate that most entrepreneurs are integrating technology into their business operations, either through social media marketing, e-commerce platforms, cashless payment methods, online delivery services, and machines, regardless of whether they are located in rural or metropolitan areas. The majority of the respondent are utilizing technology through their social media marketing. These situations have arisen as a result of the impact of COVID-19, which has compelled all entrepreneurs to adopt technology, even at a minimal level. The majority use social media platforms like Facebook and WhatsApp for marketing and adopt online banking and cashless payment methods such as DuitNow QR Code, Touch 'n Go, Boost, Shopee Pay, and Sarawak Pay. Despite using online food delivery services like Food Panda and Grab Food, trust issues such as payment delays, non-payment by drivers, and high commission fees make these platforms less appealing. Additionally, entrepreneurs prefer traditional methods over machinery to maintain quality and struggle to meet e-commerce demand due to their small-scale production, limiting their ability to fully use technology for business growth and sustainability. The findings support the theoretical insights of the TAM model and suggest actionable strategies for fostering technology adoption and empowerment among low-income women entrepreneurs in Malaysia. This study aims to provide a comprehensive overview of Micro, Small, and Medium Enterprises (MSMEs) in Davao de Oro by employing descriptive quantitative research. The focus is on demographics and operational characteristics. Twelve registered MSMEs from five municipalities were included using convenience sampling. Our analysis revealed a dynamic entrepreneurial landscape among respondents aged 26-30 years old, with a significant presence of female entrepreneurs. Maragusan and New Bataan emerged as central hubs, reflecting diverse geographical distribution. The duration of operations indicated a mix of newer and established enterprises, and a substantial portion of surveyed businesses operated with moderate monthly expenses. Perceptions regarding the impact of lockdown measures were consistent, with a significant proportion of respondents perceiving severe negative effects, particularly at Level 5. This study underscores the challenges faced by MSMEs due to the COVID-19 pandemic, emphasizing the need for tailored policies to support diverse business needs. Policymakers are urged to leverage these insights for evidence-based interventions, considering the urgency of addressing economic repercussions and promoting resilient recovery in Davao de Oro. Suggested strategies include financial assistance, business continuity programs, and capacity-building initiatives to alleviate hardships and foster economic recovery post-COVID-19. Simple Summary: Single small primary liver cancers are curable with treatments such as surgical resection, ablation, and liver transplant; however, many patients initially receive trans-arterial chemo-embolisation (TACE), which is generally not considered a curative treatment in itself, and often go on to receive further follow-up treatments. Little is known regarding the outcomes of such patients compared to those who receive upfront ablation. Our real-world multi-centre study demonstrates that key survival outcomes are similar between those initially undergoing TACE and those receiving ablation after controlling for other key clinical variables and allowing for subsequent individualised treatment selection. Percutaneous ablation is recommended in Barcelona Clinic Liver Cancer (BCLC) stage 0/A patients with HCC ≤3 cm as a curative treatment modality alongside surgical resection and liver transplantation. However, trans-arterial chemo-embolisation (TACE) is commonly used in the real-world as an initial treatment in patients with single small HCC in contrast to widely accepted clinical practice guidelines which typically describe TACE as a treatment for intermediate-stage HCC. We performed this real-world propensity-matched multi-centre cohort study in patients with single HCC ≤ 3 cm to assess for differences in survival outcomes between those undergoing initial TACE and those receiving upfront ablation. Patients with a new diagnosis of BCLC 0/A HCC with a single tumour ≤3 cm first diagnosed between 1 January 2016 and 31 December 2020 who received initial TACE or ablation were included in the study. A total of 348 patients were included in the study, with 147 patients receiving initial TACE and 201 patients undergoing upfront ablation. After propensity score matching using key covariates, 230 patients were available for analysis with 115 in each group. There were no significant differences in overall survival (log-rank test p = 0.652) or liver-related survival (log-rank test p = 0.495) over a median follow-up of 43 months. While rates of CR were superior after ablation compared to TACE as a first treatment (74% vs. 56%, p < 0.004), there was no significant difference in CR rates when allowing for further subsequent treatments (86% vs. 80% p = 0.219). In those who achieved CR, recurrence-free survival and local recurrence-free survival were similar (log rank test p = 0.355 and p = 0.390, respectively). Our study provides valuable real-world evidence that TACE when offered with appropriate follow-up treatment is a reasonable initial management strategy in very early/early-stage HCC, with similar survival outcomes as compared to those managed with upfront ablation. Although advanced deep learning techniques have significantly improved SAR ship detection, accurately detecting small ships remains challenging due to their limited size and the few appearance and geometric clues available. In order to solve this problem, we propose several small object detection strategies. The backbone network uses space-to-depth convolution to replace stride and pooling. It reduces information loss during down-sampling. The neck integrates multiple layers of features globally and injects global and local information into different levels. It avoids the inherent information loss of traditional feature pyramid networks and strengthens the information fusion ability without significantly increasing latency. The proposed intersection-of-union considers the center distance and scale of small ships specifically. It reduces the sensitivity of intersection-of-union to positional deviations of small ships, which is helpful for training toward small ships. During training, the smaller the localization loss of small ships, the greater their localization loss gains are. By this, the supervision of small ships is strengthened in the loss function, which can make the losses more biased toward small ships. A series of experiments are conducted on two commonly used datasets, SSDD and SAR-Ship-Dataset. Paraduodenal hernias are a rare but important clinical entity, as their presentation can be life-threatening if not properly diagnosed and managed appropriately. Additionally, this entity is defined by a unique and complex congenital anatomical abnormality which dictates a specific treatment algorithm. Here we present the cases of three patients from our clinical experience who all presented with either acute or chronic small bowel obstructions secondary to paraduodenal hernias. Two were left-sided paraduoenal hernias and one was right-sided. All three patients were successfully managed with surgical intervention. Saudi Arabia's obesity prevalence is 19.2% among men and 21.4% among women. Treatment includes lifestyle modifications, medication, and bariatric surgery. Procedures reach up to 1200 annually in our center. Porto-mesenteric venous thrombosis associated with venous mesenteric ischemia and bowel necrosis is a rare complication that necessitates an early workup and management. A 29-year-old Saudi male underwent laparoscopic sleeve gastrectomy. Post-surgery, he experienced abdominal pain, nausea, and vomiting, exacerbated by eating and smoking. Abdomen computed tomography scans revealed engorged portal veins, congested mesenteric veins, and small bowel thickening. He underwent exploratory laparoscopy shifted to laparotomy with resection of an infarcted omentum and 1 m of jejunal small bowel loop, and was discharged postoperatively after 7 days. Porto-mesenteric venous thrombosis is a rare complication after laparoscopic sleeve gastrectomy, requiring early diagnosis and appropriate treatment. The primary goal of the article is to analyse whether the benefits of implementing AI-based solutions in the area of cybersecurity may be a sufficient stimulus for Polish Small and Medium-Sized Enterprises (SMEs) to decide for their adoption. With this view in mind, the author of the paper analyses recent legislative changes in the domain of data protection such as General Data Protection Regulation. Additionally, the need for legislative changes is highlighted in the light of ever increasing adoption of AI-based solutions in all types of organisations. Then, the research focus pertains to contemporary threats to data security in Polish SMEs, with a particular stress on the latest AI-based attacks. For this purpose data from national cybersecurity institutions are analysed to determine the most serious attacks aimed at Polish organisations. This is followed by the analysis of the defence potential of the investigated enterprises in terms of financial funds they spend on cybersecurity as well as persons in these organisations responsible for handling data protection issues. The next part of the paper is an analysis of the benefits stemming from application of AI-based tools in the cybersecurity domain. Its main objective is to find out the answer to the research question: What benefits stemming from application AI-based solutions can encourage Polish SMEs to make investments into these cybersecurity tools given their size and budget capacities)? Managing a modern enterprise requires the use of appropriate IT technologies. Currently, an interesting option for companies is the possibility of using IT technologies provided through cloud computing. Cloud Computing (CC) offers enterprises the ability to remotely use various IT resources in the form of services. Nowadays, many small and medium-sized enterprises (SMEs) utilize CC solutions, with many of them having several years of experience in their implementation and operation. The article presents the results of research on future trends in the application of cloud computing as indicated by SMEs in Poland. After presenting the theoretical aspects of cloud computing, various aspects related to the use of CC (such as the period of use, models, and types of services used) in the surveyed enterprises are discussed. Subsequently, the effects and changes brought about by the introduction of CC solutions, as well as the place of CC in the business strategy of SMEs, are examined. The subsequent sections present anticipated changes in the use of CC in the surveyed enterprises. The article concludes with forecasts of the popularity of CC solutions as indicated by respondents from SMEs in Poland. According to respondents from medium-sized enterprises, the most likely changes, in order, will be: many cloud services from different providers have started to be used, more auxiliary processes have been moved to the cloud, and the type of cloud used or the cloud service has changed or other modifications have been made to CC services. Conversely, respondents from small enterprises foresee that the first changes will involve: more auxiliary processes being moved to the cloud, more main and critical processes being moved to the cloud, and the type of cloud used or the cloud service has changed or other modifications made to CC services. According to the surveyed enterprises, the services expected to gain the most popularity over the next five years include file storage and sharing, email, archiving and storing data, documents and databases, and office applications and document flow support. The objective of the study is to present original research tool for measuring the information effects of the entrepreneur's personal network based on factors related to the enterprise resource base and access to opportunities. The study focuses on the description of the method of preparing and verifying the tool and indicating the results of the analyses. This tool was constructed based on the analysis of the specificity of information provided by entrepreneurs' personal networks, with particular emphasis on information about resources and opportunities. Since the information effects of the entrepreneur's personal network were considered a directly unobservable variable, constituting a multidimensional construct, the scales used for measurement were subjected to factor analysis and internal consistency assessment using the Cronbach's alpha coefficient. In the process of assessing the tool and determining its suitability for measurement purposes, discriminatory power analysis was used. The tool designed to measure information effects of the entrepreneur's personal network meets the assumed criteria, and its use to measure these effects in the case of the research sample in question can be considered legitimate. The results of the analysis provide useful tool for managers and researchers to measure entrepreneurial network information effects. Micro, Small, and Medium Enterprises (MSMEs) playing a crucial role in the upliftment of poverty in India. MSMEs contribute more than 29 percent to the Gross Domestic Product (GDP0 and are responsible for 50 percent of the country's total exports, but these days these are facing different constraints for their smooth functioning like financial, marketing, operational and technological. For this study primary as well as secondary data was collected from different MSMEs in India. Major obstacles that prevent MSMEs from taking advantage of government initiatives include difficult application processes, low awareness, and insufficient support networks. Adherence to financial reporting guidelines promotes openness and aids companies in showcasing their financial well-being to investors, lenders, and stakeholders. The impediment to enhancing productivity and competitiveness is the high expense of adopting new technologies, along with low technical understanding and limited awareness. The study investigates the critical importance of destination resilience in enhancing tourism, particularly in the wake of challenges such as the COVID-19 pandemic. The primary aim of the research is to analyze how effective governance, stakeholder engagement, resource management, and risk assessment contribute to the resilience of tourist destinations and micro, small, and medium enterprises (MSMEs). To achieve this, the study employs a comprehensive literature review methodology, synthesizing existing research on destination resilience strategies and their implications for tourism promotion. Precision agriculture is the use and application of new technologies in big and small surface units, taking into account the diversity of the soil, the environmental environment and the needs of the plants in order to manage and optimize the application of inputs (seeds, fertilizers, phytosanitary products, irrigation...) to obtain profitable, quality and environmentally friendly production. About 20 years ago or so, applications of the GPS system (Global Positioning System) were achieved in agriculture to obtain the position using coordinates and the production obtained at each of those points. With the preparation of this data, the first yield maps were represented, and the use of these new systems began to be called "Precision Agriculture." In recent years, with technological advances in computing and new communication networks, very important advances have been obtained, being able to control positioning with errors of less than 1 meter, through DGPS and GIS (Geographic Information Systems). This technology in Cuba is in a completely experimental phase, but it is a fact that performance maps are being obtained and this information can be used by equipment that can be mounted on tractors. This paper provides a systematic review of the literature on big data capabilities, business model innovation, firm performance, and environmental uncertainty. It aims to establish a foundation for theoretical modeling, research proposition refinement, and the overall research framework by meticulously examining the theoretical backgrounds of existing studies and identifying research gaps. An initial search yielded 1,360 articles, which were filtered to remove duplicates and irrelevant studies, resulting in 475 articles for final analysis. These articles were classified into three main categories: the relationship between big data capabilities and business model innovation, the impact of business model innovation on firm performance, and the integrated relationship involving environmental uncertainty. Additionally, it examines the mediating role of business model innovation on firm performance as well as the moderating effect of environmental uncertainty on these relationships. Finally, the paper formulates research hypotheses and discusses identified research gaps, establishing a solid groundwork for methodological discussions in future research and contributing to the advancement of knowledge in the field. Tissue resident memory CD8+ T (TRM) cells offer rapid and long-term protection at sites of reinfection1. Tumour-infiltrating lymphocytes with characteristics of TRM cells maintain enhanced effector functions, predict responses to immunotherapy and accompany better prognoses2,3. Thus, an improved understanding of the metabolic strategies that enable tissue residency by T cells could inform new approaches to empower immune responses in tissues and solid tumours. Here, to systematically define the basis for the metabolic reprogramming supporting TRM cell differentiation, survival and function, we leveraged in vivo functional genomics, untargeted metabolomics and transcriptomics of virus-specific memory CD8+ T cell populations. We found that memory CD8+ T cells deployed a range of adaptations to tissue residency, including reliance on non-steroidal products of the mevalonate-cholesterol pathway, such as coenzyme Q, driven by increased activity of the transcription factor SREBP2. This metabolic adaptation was most pronounced in the small intestine, where TRM cells interface with dietary cholesterol and maintain a heightened state of activation4, and was shared by functional tumour-infiltrating lymphocytes in diverse tumour types in mice and humans. Enforcing synthesis of coenzyme Q through deletion of Fdft1 or overexpression of PDSS2 promoted mitochondrial respiration, memory T cell formation following viral infection and enhanced antitumour immunity. In sum, through a systematic exploration of TRM cell metabolism, we reveal how these programs can be leveraged to fuel memory CD8+ T cell formation in the context of acute infections and enhance antitumour immunity.

Objective To observe the clinical effect of small-serrated flap technique in improving facial scar. Methods From October 2020 to October 2022, 90 patients with facial scar width < 0.5 cm with or without depression admitted to the Department of Burn and Plastic Surgery, Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were retrospectively analyzed. A zigzag incision with a margin of < 0.3 cm was designed. After removing the scar, the skin margin was fully freed and then sutured. Tension was controlled locally with a tension reduction tape for 6 months after surgery. Evaluate patient satisfaction. Results Ninety patients were followed up for 6 months after operation, the facial scar of the patients improved significantly and the satisfaction was high. Ectopic pancreatic tissue (EPT) is a rare condition in which pancreatic tissue is situated outside its normal position. It is commonly found in the stomach and small bowel, typically asymptomatic, and is usually discovered incidentally during histopathological analysis. Although fewer than 40 cases of ectopic pancreatic tissue in the gallbladder have been reported, its significance relies on the risk of malignant transformation, highlighting the need for a thorough pathological study. This case report describes the presence of EPT in the gallbladder incidentally found during the pathological examination following a laparoscopic cholecystectomy of a 37-year-old female due to chronic cholecystitis. Most processing of the human diet occurs in the small intestine. Metabolites in the small intestine originate from host secretions, plus the ingested exposome1 and microbial transformations. Here we probe the spatiotemporal variation of upper intestinal luminal contents during routine daily digestion in 15 healthy male and female participants. For this, we use a non-invasive, ingestible sampling device to collect and analyse 274 intestinal samples and 60 corresponding stool homogenates by combining five mass spectrometry assays2,3 and 16S rRNA sequencing. We identify 1,909 metabolites, including sulfonolipids and fatty acid esters of hydroxy fatty acids (FAHFA) lipids. We observe that stool and intestinal metabolomes differ dramatically. Food metabolites display trends in dietary biomarkers, unexpected increases in dicarboxylic acids along the intestinal tract and a positive association between luminal keto acids and fruit intake. Diet-derived and microbially linked metabolites account for the largest inter-individual differences. Notably, two individuals who had taken antibiotics within 6 months before sampling show large variation in levels of bioactive FAHFAs and sulfonolipids and other microbially related metabolites. From inter-individual variation, we identify Blautia species as a candidate to be involved in FAHFA metabolism. In conclusion, non-invasive, in vivo sampling of the human small intestine and ascending colon under physiological conditions reveals links between diet, host and microbial metabolism.

This study conducted biostatistical multivariate analyses on 23 craniodental morphological measurements from 209 specimens to study interspecific variations amongst 15 bat species of the genus Myotis in Vietnam. Univariate and multivariate analyses demonstrated that the studied species can be divided into four groups as follows: extra-large-sized species (M. chinensis), large-sized species (M. pilosus, M. indochinensis and M. annectans), medium-sized species (M. altarium, M. hasseltii, M. montivagus, M. horsfieldii, M. ater, M. laniger and M. muricola) and small-sized species (M. annamiticus, M. aff. siligorensis, M. rosseti and M. alticraniatus). Our data revealed that the main craniodental features contributing to the variations in distinguishing Myotis species are the width of the anterior palatal, least height of the coronoid process, length of the upper and lower canine-premolar, zygomatic width and width across the upper canines and lower premolar-molar length. Based on patterns of morphological differences, we conducted comparisons between morphometrically closely resembling species pairs and further discussed additional characteristics that are expected to support the taxonomy and systematics of Vietnamese Myotis bats.

Abstract Background Chronic enteropathy associated with SLCO2A1 gene (CEAS) is a unique type of inflammatory bowel disease. CEAS is monogenic disease and is thought to develop from childhood, but studies on pediatric CEAS are scarce. We analyzed characteristics of pediatric CEAS. Methods Eleven patients diagnosed with CEAS at Seoul National University Children’s Hospital were identified and analyzed. Clinical data of patients were collected. Sanger sequencing of SLCO2A1 was performed on all patients. Results Patients were diagnosed at a median age of 16.0 years (IQR 11.0 ~ 20.0), and the median age at symptoms onset was only 4.0 years (IQR 2.5 ~ 6.0). Growth delay was observed at the time of diagnosis. Patients showed multiple ulcers or strictures in the small intestine, while the esophagus and colon were unaffected in any patients. Almost half of the patients underwent small intestine resection. The major laboratory features of pediatric CEAS include iron deficiency anemia (IDA), hypoalbuminemia, and near-normal levels of C-reactive protein (CRP). Two novel mutations of SLCO2A1 were identified. The most prevalent symptoms were abdominal pain and pale face. None of the immunomodulatory drugs showed a significant effect on CEAS. Conclusions Pediatric CEAS typically develop from very young age, suggesting it as one type of monogenic very early onset inflammatory bowel disease. CEAS can cause growth delay in children but there is no effective treatment currently. We recommend screening for SLCO2A1 mutations to pediatric patients with chronic IDA from a young age and small intestine ulcers without elevation of CRP levels.

This research came to shed light on small industries in the center of Zagazig, to identify the geographical distribution of industrial sites and small industries in the center of Zagazig. The researcher also focused on explaining the role of factors affecting industries, which were represented in (raw materials, labor, energy and fuel, capital, market).The study focuses on clarifying the small industrial sectors and their geographic characteristics in the center, in addition to the economic and human factors on distribution, identifying the problems of small industries, solutions and recommendations to address them, and highlighting the role of small industries in economic development.The study used the descriptive analytical approach to study the natural and human factors affecting settlement, and to study the factors of settlement, in addition to the historical approach to study the development of small industries in the period from 2010-2020. The researcher also relied on the field study, and the use of the cartographic method in representation, in addition to the statistical method in Analysis of table numbers.The research reached the spatial disparity of industrial sites, which was reflected in the industrial concentration in two areas: Bardin and Al-Aslouji. Each of the influencing factors took its role in its impact on small industries. The study suggested the establishment of youth guarantor companies and training centers for small industries.

The enteric nervous system (ENS) consists of glial cells (EGCs) and neurons derived from neural crest precursors. EGCs retain capacity for large-scale neurogenesis in culture, and in vivo lineage tracing has identified neurons derived from glial cells in response to inflammation. We thus hypothesize that EGCs possess a chromatin structure poised for neurogenesis. We use single-cell multiome sequencing to simultaneously assess transcription and chromatin accessibility in EGCs undergoing spontaneous neurogenesis in culture, as well as small intestine myenteric plexus EGCs. Cultured EGCs maintain open chromatin at genomic loci accessible in neurons, and neurogenesis from EGCs involves dynamic chromatin rearrangements with a net decrease in accessible chromatin. A subset of in vivo EGCs, highly enriched within the myenteric ganglia and that persist into adulthood, have a gene expression program and chromatin state consistent with neurogenic potential. These results clarify the mechanisms underlying EGC potential for neuronal fate transition.

Tissue-resident memory CD8+ T (TRM) cells are a subset of memory T cells that play a critical role in limiting early pathogen spread and controlling infection. TRM cells exhibit differences across tissues, but their potential heterogeneity among distinct anatomic compartments within the small intestine and colon has not been well recognized. Here, by analyzing TRM cells from the lamina propria and epithelial compartments of the small intestine and colon, we showed that intestinal TRM cells exhibited distinctive patterns of cytokine and granzyme expression along with substantial transcriptional, epigenetic, and functional heterogeneity. The T-box transcription factor Eomes, which represses TRM cell formation in some tissues, exhibited unexpected context-specific regulatory roles in supporting the maintenance of established TRM cells in the small intestine, but not in the colon. Taken together, these data provide previously unappreciated insights into the heterogeneity and differential requirements for the formation vs. maintenance of intestinal TRM cells.

The enteric nervous system (ENS) is an extensive network of neurons and glia within the wall of the gastrointestinal (GI) tract that regulates many essential GI functions. Consequently, disorders of the ENS due to developmental defects, inflammation, infection, or age-associated neurodegeneration lead to serious neurointestinal diseases. Despite the prevalence and severity of these diseases, effective treatments are lacking as they fail to directly address the underlying pathology. Neuronal stem cell therapy represents a promising approach to treating diseases of the ENS by replacing the absent or injured neurons, and an autologous source of stem cells would be optimal by obviating the need for immunosuppression. We utilized the swine model to address key questions concerning cell isolation, delivery, engraftment, and fate in a large animal relevant to human therapy. We successfully isolated neural stem cells from a segment of small intestine resected from 1-month-old swine. Enteric neuronal stem cells (ENSCs) were expanded as neurospheres that grew optimally in low-oxygen (5%) culture conditions. Enteric neuronal stem cells were labeled by lentiviral green fluorescent protein (GFP) transduction, then transplanted into the same swine from which they had been harvested. Endoscopic ultrasound was then utilized to deliver the ENSCs (10,000-30,000 neurospheres per animal) into the rectal wall. At 10 and 28 days following injection, autologously derived ENSCs were found to have engrafted within rectal wall, with neuroglial differentiation and no evidence of ectopic spreading. These findings strongly support the feasibility of autologous cell isolation and delivery using a clinically useful and minimally invasive technique, bringing us closer to first-in-human ENSC therapy for neurointestinal diseases.

Fluorescence imitating brightfield imaging (FIBI) is a novel microscopy method that allows for real-time, nondestructive, slide-free tissue imaging of fresh, formalin-fixed, or paraffin-embedded tissue. The nondestructive nature of the technology permits tissue preservation for downstream analyses. The objective of this observational study was to assess the utility of FIBI compared with conventional hematoxylin and eosin (H&E)-stained histology slides in feline gastrointestinal histopathology. Formalin-fixed paraffin-embedded full-thickness small intestinal tissue specimens from 50 cases of feline chronic enteropathy were evaluated. The ability of FIBI to evaluate predetermined morphological features (epithelium, villi, crypts, lacteals, fibrosis, submucosa, and muscularis propria) and inflammatory cells was assessed on a 3-point scale (0 = FIBI cannot identify the feature; 1 = FIBI can identify the feature; 2 = FIBI can identify the feature with more certainty than H&E). H&E and FIBI images were also scored according to World Small Animal Veterinary Association (WSAVA) Gastrointestinal Standardization Group guidelines. FIBI identified morphological features with similar or, in some cases, higher confidence compared with H&E images. The identification of inflammatory cells was less consistent. FIBI and H&E images showed an overall poor agreement with regard to the assigned WSAVA scores. While FIBI showed an equal or better ability to identify morphological features in intestinal biopsies, its ability to identify inflammatory cells is currently inferior compared with H&E-based imaging. Future studies on the utility of FIBI as a diagnostic tool for noninflammatory histopathologic lesions are warranted.

This paper aimed to accentuate concerns related to the quality of data that would typically constitute business intelligence for Small and Medium Enterprises (SME's) domiciled in Nigeria. Additionally, it proposed best practice recommendations to address the accentuated concerns. To determine relevant data quality concerns, reference is made to generically acknowledged data quality dimensions. These dimensions are widely regarded as aspects of data which necessitate assessments required to arrive at data quality judgements. Particular attention is paid to data quality dimensions that pertain to structured data, given the invaluable role that structured data plays in every profit-oriented establishment. To ensure that the determined data quality concerns are relevant to the experienced peculiarities of Nigerian based SME's a thorough literature review is conducted on data quality related challenges faced by Nigerian SME's. The findings from the literature review informs a systematic comparative analysis of twenty-five text files - which suggest best practice recommendations for addressing the related data quality challenges. The findings suggest that in order for the proposed best practice recommendations to be adequately applied, Nigerian based SME's must be sensitized on the importance of data capture, as well as pay attention to quality aspects of any data captured. Furthermore, it is revealed that key stakeholders of Nigerian owned SME's must undergo capacity building exercises to improve their proficiency levels in the use of related digital tools required to capture and maintain business intelligence related data at its optimum quality. The rapid development of small and medium-sized logistics enterprises has also revealed potential credit risk issues. Based on this, this paper constructs a system dynamics model to simulate the key factors affecting the credit risk of small and medium-sized logistics enterprises. Firstly, the paper analyzes the factors affecting the credit risk of small and medium-sized logistics enterprises from multiple aspects such as industry characteristics and sources of risk, and constructs an index system for these factors. Secondly, the paper uses the system dynamics method to build a mechanism model of credit risk for small and medium-sized logistics enterprises, and deeply analyzes the factors affecting the mechanism of these enterprises. Finally, through simulation analysis of examples, the paper identifies key risk factors and proposes effective control measures to improve the overall credit risk situation of small and medium-sized logistics enterprises.
