Summary: The African halfbeak Hyporhamphus picarti (Hemiramphidae) is one of the most abundant species within the ichthyoplankton community of the Sine Saloum estuary (Senegal). A year‐round occurrence of larvae suggests that the Sine Saloum is an important spawning habitat for this species. Annual fluctuations in water temperature, however, can have severe impacts on the survival probabilities of marine fish larvae. To determine whether temperature has an effect on the growth of H. picarti during its larval development, larval age at length and somatic growth rates were investigated for two contrasting spawning seasons in 2014: February (cold season, 20.8°C) and June (warm season, 26.4°C). In both months H. picarti larvae were sampled at the mouth of the Saloum River using neuston nets. Sagittal otoliths’ increments were counted to estimate the larva age at a given standard length (SL). The age of larvae ranged between 2 and 22 days, with SL of 3.86–21.68 mm, respectively. In order to describe larval age at length during the contrasting spawning seasons, two distinctive Gompertz functions were applied. Accordingly, specimens sampled in June (0.94 ± 0.17 mm per day) exhibited significantly higher somatic growth rates than those sampled in February (0.60 ± 0.06 mm per day). These findings suggest that water temperature is an important factor influencing larval growth in H. picarti. Information concerning the early life stages of H. picarti are scarce and the results of the present study may contribute to a better understanding of the species’ biology and ecology. Background: Splanchnic artery stenosis is common and mostly asymptomatic and may lead to gastrointestinal ischemia (chronic splanchnic syndrome, CSS). This study was designed to assess risk factors for CSS in the medical history of patients with splanchnic artery stenosis and whether these risk factors can be used to identify patients with high and low risk of CSS. Methods: All patients referred for suspected CSS underwent a standardized workup, including a medical history with questionnaire, duplex ultrasound, gastrointestinal tonometry, and angiography. Definitive diagnosis and treatment advice was made in a multidisciplinary team. Patients with confirmed CSS were compared with no-CSS patients. Results: A total of 270 patients (102 M, 168 F; mean age, 53 years) with splanchnic artery stenosis were analyzed, of whom 109 (40%) had CSS and 161 no CSS. CSS-patients more often reported postprandial pain (87% vs. 72%, p = 0.007), weight loss (85% vs. 70%, p = 0.006), adapted eating pattern (90% vs. 79%, p = 0.005) and diarrhea (35% vs. 22%, p = 0.023). If none of these risk factors were present, the probability of CSS was 13%; if all were present, the probability was 60%. Adapted eating pattern (odds ratio (OR) 3.1; 95% confidence interval (CI) 1.08-8.88) and diarrhea (OR 2.6; 95% CI 1.31-5.3) were statistically significant in multivariate analysis. Conclusions: In patients with splanchnic artery stenosis, the clinical history is of limited value for detection of CSS. A diagnostic test to detect ischemia is indispensable for proper selection of patients with splanchnic artery stenosis who might benefit from treatment. [ABSTRACT FROM AUTHOR]

Background: Endoscopic retrograde cholangiopancreatography (ERCP) with fully covered self-expandable metal stent (FCSEMS) placement is the preferred approach for biliary drainage in patients with suspected distal malignant biliary obstruction (MBO). However, FCSEMS placement is associated with a high risk of post-ERCP pancreatitis (PEP). Endoscopic sphincterotomy prior to FCSEMS placement may reduce PEP risk., Objective: To compare endoscopic sphincterotomy to no sphincterotomy prior to FCSEMS placement., Design: This multicentre, randomised, superiority trial was conducted in 17 hospitals and included patients with suspected distal MBO. Patients were randomised during ERCP to receive either endoscopic sphincterotomy (sphincterotomy group) or no sphincterotomy (control group) prior to FCSEMS placement. The primary outcome was PEP within 30 days. Secondary outcomes included procedure-related complications and 30-day mortality. An interim analysis was performed after 50% of patients (n=259) had completed follow-up., Results: Between May 2016 and June 2023, 297 patients were included in the intention-to-treat analysis, with 156 in the sphincterotomy group and 141 in the control group. After the interim analysis, the study was terminated prematurely due to futility. PEP did not differ between groups, occurring in 26 patients (17%) in the sphincterotomy group compared with 30 patients (21%) in the control group (relative risk 0.78, 95% CI 0.49 to 1.26, p=0.37). There were no significant differences in bleeding, perforation, cholangitis, cholecystitis or 30-day mortality., Conclusion: This trial found that endoscopic sphincterotomy was not superior to no sphincterotomy in reducing PEP in patients with distal MBO.
