Abstract

The human tri-color vision process may be characterized as follows: 1. A requirement of three scalar quantities to fully define a color (for example, intensity, hue, and purity), with 2. These scalar measures linear in the intensity of the incident light, allowing in general any specific color to be duplicated by an additive mixture of light from three standardized (basis) colors, 3. The exception being that the spectral colors are unique, in that they cannot be duplicated by any positive mixture of other colors. These characteristics strongly suggest that human color vision makes use of Schwartz measures in processing color data. This hypothesis is subject to test. In this brief paper, the results of this hypothesis are shown to be in good agreement with measured data.

In this paper we investigate the role of the dependency tree in a named entity recognizer upon using a set of GCN. We perform a comparison among different NER architectures and show that the grammar of a sentence positively influences the results. Abstract

Dynamic modeling is used to design systems that are adaptive to their changing environment and is currently poorly understood in information retrieval systems. Common elements in the information retrieval methodology, such as documents, relevance, users and tasks, are dynamic entities that may evolve over the course of several interactions, which is increasingly captured in search log datasets. Conventional frameworks and models in information retrieval treat these elements as static, or only consider local interactivity, without consideration for the optimisation of all potential interactions. Further to this, advances in information retrieval interface, contextual personalization and ad display demand models that can intelligently react to users over time. This thesis proposes a new area of information retrieval research called Dynamic Information Retrieval. The term dynamics is defined and what it means within the context of information retrieval. Three examples of current areas of research in information retrieval which can be described as dynamic are covered: multi-page search, online learning to rank and session search. A probabilistic model for dynamic information retrieval is introduced and analysed, and applied in practical algorithms throughout. This framework is based on the partially observable Markov decision process model, and solved using dynamic programming and the Bellman equation. Comparisons are made against well-established techniques that show improvements in ranking quality and in particular, document diversification. The limitations of this approach are explored and appropriate approximation techniques are investigated, resulting in the development of an efficient multi-armed bandit based ranking algorithm. Finally, the extraction of dynamic behaviour from search logs is also demonstrated as an application, showing that dynamic information retrieval modeling is an effective and versatile tool in state of the art information retrieval research.

4. Associations among Professional Development, Teachers' Use of Naturalistic Language Strategies, and Preschoolers' Functional Communication

Abstract

Research Findings: Naturalistic instruction is the intentional use of strategies embedded within typical activities of the environment to create learning opportunities for children. We used multilevel modeling to analyze data from this single-case design study that focused on improving teachers' use of naturalistic language strategies and children's communication through professional development with bug-in-ear coaching. Our results showed that professional development was positively associated with children's communication during and after professional development. During generalization sessions in different types of activities, teachers' use of the naturalistic language strategies was positively associated with children's functional communication. We also found that children were more likely to communicate during child-led activities. Practice or Policy: Associations among professional development, teachers' use of evidence-based strategies, and children's outcomes have yet to be fully understood. Administrators and professional development providers should support teachers' use of naturalistic language strategies to create equitable exposure to language-learning opportunities for all children. Moreover, professional development efforts should support teachers' intentional use of child-led activities to embed language-learning opportunities based on children's interests. Finally, to strengthen children's generalization of skills across routines, professional development providers should identify methods to support teachers' generalized use of strategies within the typical activities of the setting.

Abstract

In 2003, a trial National Assessment of Educational Progress (NAEP) mathematics assessment was administered in Spanish to public school students at grades 4 and 8 in Puerto Rico. Based on preliminary analyses of the 2003 data, changes were made in administration and translation procedures for the 2005 NAEP administration in Puerto Rico. This report describes the content and administration of the trial NAEP mathematics assessments in Puerto Rico in 2003 and 2005, problems with item misfit in the 2003 data, results of a special validity analysis, and plans to integrate Puerto Rico into the national sample in future administrations. This report is one of a series of three on the administration and results of the 2003 and 2005 trial NAEP mathematics assessments in Puerto Rico: (1) "Mathematics 2003 and 2005 Performance in Puerto Rico: Highlights" [ED495982]; and (2) "Mathematics 2005 Performance in Puerto Rico: Focus on the Content Areas" [ED495981]. This, the third report, focuses on the technical considerations of the trial assessments and plans to include Puerto Rico as part of the national sample in future administrations. Abstract

Abstract Background Epilepsy remains a significant public health concern in Tanzania, with affected individuals enduring stigma, whether through actions or perceptions. Myths, misunderstandings, and misconceptions about epilepsy have persisted due to a multitude of factors. Here, we assessed attitudes and perceptions toward epilepsy in Mahenge. Methods A cross-sectional study utilising a mixed-methods approach was undertaken in eight villages in the Ulanga district of Mahenge, integrating a semi-structured questionnaire and focus group discussions (FGDs). The questionnaire involved 778 community members, and 15 FGDs were conducted (seven groups with people with epilepsy and eight without). Descriptive statistics, chi-square, and logistic regression were used for quantitative analysis, while we used NVivo version 14 for thematic analysis of qualitative data. Results Of 778 participants, over half were women (425, 54.6%) with a median age of 41 years (IQR: 30–55) and most had completed primary education (79.9%). The majority of participants were aware of epilepsy (96.8%), yet they displayed low knowledge (51%), negative attitudes (45.5%), and perceptions (42.1%) towards the disorder. A low level of understanding was significantly associated with negative attitudes (Adjusted Odds Ratio [AOR] = 1.89, 95%CI: 1.41–2.53) and perceptions (AOR = 3.22, 95%CI: 2.05–5.04) towards epilepsy. In the qualitative analysis, often hereditary factors and infections were named as causes of epilepsy, along with misconceptions involving witchcraft and divine punishment. There was also a misconception about the contagiousness of epilepsy. Traditional healers were often the initial point of treatment. Epilepsy-related stigma was evident, with individuals with epilepsy facing derogatory labels, social isolation, and barriers to education. Lastly, there was a lack of understanding regarding a possible association between epilepsy and onchocerciasis. Conclusions Despite high awareness of epilepsy, there is insufficient understanding, negative attitudes, and perceptions, including misconceptions and stigma about this neurologic condition. Community-based education programmes are essential for promoting proper healthcare-seeking behaviour and dispelling myths.

Abstract

Purpose To describe a pharmacist-led transitional care clinic (TCC) for high-risk patients who were recently hospitalized or seen in the emergency department (ED). Summary The Memphis Veterans Affairs Medical Center (VAMC) established a pharmacist-led face-to-face and telephone follow-up TCC to improve posthospitalization follow-up care through medication optimization and disease state management, particularly for veterans with high-risk disease states such as chronic obstructive pulmonary disease (COPD) and heart failure (HF). The clinic’s clinical pharmacy specialists (CPSs) ordered diagnostic and laboratory tests, performed physical assessments, and consulted other providers and specialty services in addition to performing medication reconciliation, compliance assessment, and evaluation of adverse drug events. TCC patients were typically seen within 2 weeks of discharge and subsequently referred back to their primary care provider or a specialty care provider for continued management. A retrospective review of 2016 TCC data found that 7.8% of patients seen in the TCC were readmitted within 30 days of discharge; readmission rates for COPD and HF were reduced to 13% and 10%, respectively, compared to hospital-wide readmission rates of 17% and 24%. A separate observational analysis found that 30-day readmissions for COPD and HF were reduced in TCC patients, with pharmacists documenting an average of 6.2 interventions and 3.3 medication-related problems per patient. To reduce clinic appointment no-shows, the CPSs worked with inpatient providers and schedulers to emphasize to patients the importance of clinic attendance; also, TCC services were expanded to include telehealth appointments to increase access for rural and/or homebound patients. Conclusion A pharmacist-led TCC effectively reduced readmissions and prevented medication-related problems for high-risk patients who were hospitalized or seen in the ED.

Abstract

Background: α-1 adrenergic antagonists are commonly prescribed, but there is question regarding their safety in patients at increased fall risk. Objective: The purpose of the FRAGILE study was to determine the risk for developing adverse drug events (ADEs) in veterans prescribed α-1 blockers. Methods: A single-center, retrospective, observational cohort analysis was conducted of veterans newly initiated on α-1 antagonists. Veterans were categorized into at-risk (patients who met at least 1 of 2 criteria: age 65 or older or high initial dose of α blockade) or control (veterans without either risk factor) groups. The primary outcome was the composite all-cause ADEs, including hospitalizations or emergency department (ED) visits. Secondary outcomes included number of fall-related ADEs and medication discontinuation rates with follow-up for 12 months. Results: A total of 300 veterans were evaluated. There was no significant difference in the composite outcome of all-cause ED visits between at-risk (n = 169) versus control (n = 131) groups (0.81 vs 1.17, P = 0.09) or all-cause hospitalizations (0.28 vs 0.39, P = 0.25). Seventy-three veterans in the at-risk group experienced an all-cause ADE versus 64 in the control group ( P = 0.36). No significant differences in secondary outcomes were found. Fall-related side effects occurred in 8% of the total cohort. Conclusion and Relevance: Rates of all-cause or fall-related ADEs were not significantly different. An 8% discontinuation rate resulting from fall-related ADEs and high rates of coadministered medications that could increase fall risk. Pharmacists can play a key role in optimizing α-1 blocker administration.

Abstract

Abstract Background Exploration of client identity negotiations during treatment for Anorexia Nervosa (AN) is a relatively new area of research. Research suggests that difficulties with identity negotiations may present as a barrier to treatment. This study sought to explore individuals’ identity negotiations during therapy sessions using Compulsive Exercise Activity Therapy (LEAP) combined with Cognitive Behaviour Therapy for Anorexia Nervosa (CBT-AN). Analysis focused on moments in therapy where individuals’ identities were dominated or defined by AN and where alternative identities could be generated. Method 40 in-session transcripts from sessions at early, mid and end points of the CBT-AN (with LEAP) treatment were qualitatively analysed for nine of the 78 participants in the original randomised control trial. Through a constructivist framework, thematic analysis was used to identify surface and latent meanings and discursive material participants used to negotiate their identities in the context of therapy sessions. Results Analysis of in-therapy transcripts generated two themes pertaining to identity negotiations: (1) troubled identities and (2) rebuilding identities and lives outside of AN. Early therapy sessions explored fragmented and AN dominated identities, including how AN was troubling to participants’ sense of self, contributed to conflicted identities, positioned them outside of normality, and was associated with isolated and othering identities. Within therapy sessions, participants engaged in a recursive process of shifting relationships with AN and themselves and building identities and lives outside of the AN identity. This included generating hopes for recovery and the future more frequently in mid- to late- therapy sessions. Conclusion Identity negotiations evident in the therapeutic conversations aligned with the key components of the CBT-AN intervention, including addressing (1) the characterisation of oneself as ‘an anorexic’ and (2) the diversification of roles and activities to broaden and enhance self-concepts. Future developments of therapeutic interventions for AN would benefit from greater consideration of ways to assist individuals to more comprehensively address problematic identities, including uncovering identities hidden by the AN identity and generating preferred identities. Abstract

Reduction of copper(II) chloride using sodium ascorbate in the presence of pure sodium 5-nitro-tetrazolate (NaNT) forms copper(I) 5-nitrotetrazolate – a known initiatory explosive (DBX-1) – and the novel mixed-ligand copper(I) chloride 5-nitrotetrazolate coordination polymer Cu3Cl(N4C-NO2)2, as well as mixtures of both. The reaction is controlled by the presence of seed crystals and transition metal compounds other than CuCl2. Cu3Cl(N4C-NO2)2 is obtained as a wine-red, air stable, water-insoluble, crystalline and highly sensitive explosive material with a greater crystal density, lower thermal stability and a higher sensitivity toward hydrolysis and shock than DBX-1. Efforts to obtain the stable and pure starting material are improved by crystallisation of NaNT as a tetrahydrate. Cu3Cl(N4C-NO2)2 and Na(H2O)4(NT) were characterised by single crystal and powder XRD, IR spectroscopy, magnetic and thermal measaurements, elemental analysis, particle size measurements, mass spectrometry, and by drop weight testing.

22. What solutions exist for developmental delays facing indigenous children globally? A co-designed systematic review

Abstract

Early childhood is important for future cognitive and educational outcomes. Programs overcoming barriers to engagement in early education for Indigenous children must address family cultural needs and target developmental delays. This systematic review identifies culturally adapted programs to improve developmental delays among young children, in response to an identified priority of a remote Indigenous community. Five databases (the Cochrane Library, Embase, Medline, Scopus and CINAHL) were searched for English language papers in January 2018. Study quality was assessed, and findings were analysed thematically. Findings were presented to the community at an event with key stakeholders, to determine their inclusion and face validity. Seven relevant studies, published between 1997 and 2013, were identified by the researchers and each study was supported by the community for inclusion. Three studies included on Native American children and four studies included children from non-Indigenous disadvantaged backgrounds. Findings were reported narratively across four themes: storytelling to improve educational outcomes; family involvement improved development; culturally adapted cognitive behavioural therapy to reduce trauma; rewards-based teaching to improve child attention. Limited published research on culturally adapted and safe interventions for children with developmental delays exists but these four themes from seven studies identify useful components to guide the community and early childhood program development.

Abstract

Trypanosomatid parasites are causative agents of important human and animal diseases such as sleeping sickness and leishmaniasis. Most trypanosomatids are transmitted to their mammalian hosts by insects, often belonging to Diptera (or true flies). With resistance to both vector-targeted pesticides and trypanocidal drugs being reported, there is a need for novel transmission blocking strategies to be developed. Studies using the blood-feeding vectors themselves are not broadly accessible, as such, new model systems are being developed to unpick insect-trypanosmatids interactions. In this thesis the interactions between model dipteran Drosophila melanogaster and its own natural trypanosomatid Herpetomonas muscarum are investigated from the perspective of the parasite. This work describes the sequencing and annotation of the H. muscarum genome, which was shown to be highly syntenic to disease-causing trypanosomatids, especially with those of the Leishmania genus. Infections of D. melanogaster were shown to be confined to adults, where H. muscarum persisted in the crop and midgut of the flies for 3-4 days. RNA-sequencing analysis of infected flies showed that rapid, large-scale transcriptomic changes, associated with autophagy, nutrient acquisition and cell surface remodelling, are triggered in H. muscarum after ingestion by the fly. Much of the transcriptomic changes observed reflect what is known for disease-causing trypanosomatids. As such, several of the identified surface proteins are of interest as putative transmission blocking targets. Further characterisation of these genes will be greatly aided by genetic manipulation methods developed for H. muscarum as part of this work. This thesis has described multiple, evolutionarily conserved, trypanosomatid responses to ingestion by dipterans by using a Drosophila-Herpetomonas model. Future studies of these conserved responses will inform development of novel, broad-spectrum transmission blocking strategies.

40. A lag model for turbulent boundary layers developing over rough bleed surfaces

Abstract

Boundary layer mass removal (bleed) through spanwise bands of holes on a surface is used to prevent or control separation and to stabilize the normal shock in supersonic inlets. The addition of a transport equation lag relationship for eddy viscosity to the rough wall algebraic turbulence model of Cebeci and Chang was found to improve agreement between predicted and measured mean velocity distributions downstream of a bleed band. The model was demonstrated for a range of bleed configurations, bleed rates, and local free stream Mach numbers. In addition, the model was applied to the boundary layer development over acoustic lining materials for the inlets and nozzles of commercial aircraft. The model was found to yield accurate results for integral boundary layer properties unless there was a strong adverse pressure gradient.

Abstract

The 2009 novel H1N1 influenza A virus (H1N1) became a global pandemic. Data on antiviral prescriptions by location from 2008 to 2010 have not been reported. The purpose of this study was to assess antiviral geographic trends and correlation with influenza-like illness (ILI) over 3 years. Percent of outpatient ILI visits and antiviral prescriptions from 1 January 2008 to 31 December 2010 were included. Linear regression was used to assess correlation. In total, 14 million antivirals were dispensed during this period. A 115% increase was observed in 2009 compared with prescriptions dispensed in 2008, and an 84% decrease was observed in 2010 compared with 2009. The rate of antivirals was 1.32 prescriptions/100 persons in 2008, 2.85/100 persons in 2009 and 0.435/100 persons in 2010. 2009 regional growth was observed in most states and was highest in the West (293%) and the Northeast (272%). A positive correlation was observed between antivirals and ILI visits (R(2)=0.7853; P0.0001). With the 2009 H1N1 pandemic, antivirals increased compared with 2008 or 2010. Without the concern of H1N1, antivirals decreased in 2010 to levels lower than 2008. Geographic trends were also observed, which may be a result of the different intensity of influenza transmission and difference practice patterns. ILI diagnoses correlate with influenza antiviral prescription use in the USA.

Abstract

This paper discusses a helical auxetic structure which has a diverse range of practical applications. The mechanical properties of the system can be determined by particular combinations of geometry and component material properties; finite element analysis is used to investigate the static behavior of these structures under tension. Modeling criteria are determined and design issues are discussed. A description of the different strain-dependent mechanical phases is provided. It is shown that the stiffnesses of the component fibers and the initial helical wrap angle are critical design parameters, and that strain-dependent changes in cross-section must be taken into consideration: we observe that the structures exhibit nonlinear behavior due to nonzero component Poisson’s ratios. Negative Poisson’s ratios for the helical structures as low as -5 are shown. While we focus here on the structure as a yarn our findings are, in principle, scaleable. [ABSTRACT FROM AUTHOR]

Mutations in the leucine-rich repeat kinase II (LRRK2) gene cause autosomal-dominant Parkinson’s disease (PD) which is indistinguishable from sporadic forms of the disease. However, the mechanism by which LRRK2 mutation promotes PD pathology and the functions of the LRRK2 protein itself are currently unknown. Transgenic animal models, expressing PD-linked genes, have been crucial in developing a clearer picture of the mechanisms behind PD progression. Bacterial artificial chromosome (BAC) transgenics, in particular, allow for physiologically relevant disease models to be generated, encompassing entire gene sequences and using endogenous promoters, which result in an accurate recapitulation of spatiotemporal gene expression. We have, therefore, developed BAC transgenic rats expressing human LRRK2. These lines express either human wild-type, G2019S mutant (the most common LRRK2 mutation) or R1441C mutant (the LRRK2 mutation leading to a more aggressive pathology) LRRK2. These rats display LRRK2 transgene expression patterns in the brain which highly resemble human LRRK2 expression. We subsequently aged these lines for the purposes of behavioural and molecular analysis. Additionally, we have performed a number of in vitro studies in neurons cultured from these rats to further investigate the effects of pathological forms of LRRK2. Mutant LRRK2 transgenic rats displayed progressively impaired striatal dopaminergic release along with corresponding behavioural deficits, both motor and cognitive. These motor deficits could be rescued by treatment with levodopa, implying a similar underlying pathological process to PD. These phenotypes occurred in the absence of any overt degeneration of the dopaminergic neurons of the substantia nigra pars compacta or other PD-associated neuropathological markers. We also detected a number of molecular alterations associated with mutant LRRK2 expression including altered LRRK2 phosphorylation and in vitro evidence of altered autophagic processes. In conclusion, these rats recapitulate a number of the key features of Parkinson’s disease and provide clues as to the progression of pathology in PD.

2-Methylbenzoic acid phenylhydrazide was polylithiated with excess lithium diisopropylamide, and the resulting polylithiated intermediate was condensed with methyl 4-dimethylaminobenzoate or methyl 4-methoxybenzoate to afford C-acylated intermediates that were immediately acid cyclized to afford 3-(4-(dimethylamino)-phenyl)-2-(phenylamino)isoquinolin-1(2H)-one C23H21N3O or 3-(4-methoxyphenyl)-2-(phenylamino)isoquinolin-1(2H)-one C22H18N2O2. The polylithiated intermediate underwent N-acylation when it was condensed with methyl 4-methylbenzoate to give 2-methyl-N′-(4-methylbenzoyl)-N′-phenylbenzohydrazide C22H20N2O2. Abstract

Mutations in leucine-rich repeat kinase 2 (LRRK2) lead to late-onset, autosomal dominant Parkinson's disease, characterized by the degeneration of dopamine neurons of the substantia nigra pars compacta, a deficit in dopamine neurotransmission and the development of motor and non-motor symptoms. The most prevalent Parkinson's disease LRRK2 mutations are located in the kinase (G2019S) and GTPase (R1441C) encoding domains of LRRK2. To better understand the sequence of events that lead to progressive neurophysiological deficits in vulnerable neurons and circuits in Parkinson's disease, we have generated LRRK2 bacterial artificial chromosome transgenic rats expressing either G2019S or R1441C mutant, or wild-type LRRK2, from the complete human LRRK2 genomic locus, including endogenous promoter and regulatory regions. Aged (18-21 months) G2019S and R1441C mutant transgenic rats exhibit L-DOPA-responsive motor dysfunction, impaired striatal dopamine release as determined by fast-scan cyclic voltammetry, and cognitive deficits. In addition, in vivo recordings of identified substantia nigra pars compacta dopamine neurons in R1441C LRRK2 transgenic rats reveal an age-dependent reduction in burst firing, which likely results in further reductions to striatal dopamine release. These alterations to dopamine circuit function occur in the absence of neurodegeneration or abnormal protein accumulation within the substantia nigra pars compacta, suggesting that nigrostriatal dopamine dysfunction precedes detectable protein aggregation and cell death in the development of Parkinson's disease. In conclusion, our longitudinal deep-phenotyping provides novel insights into how the genetic burden arising from human mutant LRRK2 manifests as early pathophysiological changes to dopamine circuit function and highlights a potential model for testing Parkinson's therapeutics.

Abstract

Summary: Background: Identification of convulsive epilepsy in sub-Saharan Africa relies on access to resources that are often unavailable. Infrastructure and resource requirements can further complicate case verification. Using machine-learning techniques, we have developed and tested a region-specific questionnaire panel and predictive model to identify people who have had a convulsive seizure. These findings have been implemented into a free app for health-care workers in Kenya, Uganda, Ghana, Tanzania, and South Africa. Methods: In this retrospective case-control study, we used data from the Studies of the Epidemiology of Epilepsy in Demographic Sites in Kenya, Uganda, Ghana, Tanzania, and South Africa. We randomly split these individuals using a 7:3 ratio into a training dataset and a validation dataset. We used information gain and correlation-based feature selection to identify eight binary features to predict convulsive seizures. We then assessed several machine-learning algorithms to create a multivariate prediction model. We validated the best-performing model with the internal dataset and a prospectively collected external-validation dataset. We additionally evaluated a leave-one-site-out model (LOSO), in which the model was trained on data from all sites except one that, in turn, formed the validation dataset. We used these features to develop a questionnaire-based predictive panel that we implemented into a multilingual app (the Epilepsy Diagnostic Companion) for health-care workers in each geographical region. Findings: We analysed epilepsy-specific data from 4097 people, of whom 1985 (48·5%) had convulsive epilepsy, and 2112 were controls. From 170 clinical variables, we initially identified 20 candidate predictor features. Eight features were removed, six because of negligible information gain and two following review by a panel of qualified neurologists. Correlation-based feature selection identified eight variables that demonstrated predictive value; all were associated with an increased risk of an epileptic convulsion except one. The logistic regression, support vector, and naive Bayes models performed similarly, outperforming the decision-tree model. We chose the logistic regression model for its interpretability and implementability. The area under the receiver operator curve (AUC) was 0·92 (95% CI 0·91–0·94, sensitivity 85·0%, specificity 93·7%) in the internal-validation dataset and 0·95 (0·92–0·98, sensitivity 97·5%, specificity 82·4%) in the external-validation dataset. Similar results were observed for the LOSO model (AUC 0·94, 0·93–0·96, sensitivity 88·2%, specificity 95·3%). Interpretation: On the basis of these findings, we developed the Epilepsy Diagnostic Companion as a predictive model and app offering a validated culture-specific and region-specific solution to confirm the diagnosis of a convulsive epileptic seizure in people with suspected epilepsy. The questionnaire panel is simple and accessible for health-care workers without specialist knowledge to administer. This tool can be iteratively updated and could lead to earlier, more accurate diagnosis of seizures and improve care for people with epilepsy.
