Closed reduction used for management of ankle fractures usually result into delayed healing. It inhibits the healing of the ankle fractures. On the contrary, open reduction and internal fixation helps to preserve the supply of blood not only to soft tissues but also bones. This way it helps in proper and timely healing.To study outcome and complications of open reduction and internal fixation in ankle fractures A hospital based prospective study was carried out among 40 cases of ankle fractures. Detailed history, general examination were carried out as per the pre-tested, pre-designed study questionnaire. Once patient was found to be eligible for present study, surgical profile and pre-anesthetic check-up was done as per standard guidelines. All patients were treated with open reduction and internal fixation method. Most commonly affected age group was 31-40 years (40%). Males were affected more than females (4:1). Vehicular accident was most common cause (65%) of ankle fracture. 95% had closed fracture. Most common injury pattern (57.5%) was Pronation external rotation. K-wire was most commonly used (40%) operative technique. Among the variables, open type of fracture and other mode of injury were significantly associated with painful gait. There were no other complications recorded. 70% had excellent outcome and 25% had good outcome. Among all factors studied, only open type of fracture was found associated with functional outcome that is not excellent.Thus open reduction and internal fixation is the treatment of choice in case of fracture ankle joint, with very less change of complications

A field experiment was conducted during rabi 2015-16 at Research Farm, College of Horticulture, Dr.Y.S.R. Horticultural University, Anantharajupeta (Andhra Pradesh), India, to study the effect of different biofertilizers and micronutrients on growth, leaf yield and quality of coriander (Coriandrum sativum L.) cv. Sadhana. The experiment was conducted in a factorial randomized block design with biofertilizers and micronutrients. The biofertilizer treatments were seed inoculation with Azospirillum, Phosphate Solubilising Bacteria (PSB), Azospirillum + Phosphate Solubilising Bacteria (PSB) and control (without any biofertilizer), while the micronutrient treatments comprised of foliar sprays of Zinc sulphate, Copper sulphate, Ferrous sulphate each at @ 0.5% and control (without any micronutrient). The sixteen treatment combinations were replicated thrice. Among the treatments, seed inoculation with Azospirillum + PSB+ foliar spray of zinc sulphate @ 0.5% recorded maximum plant height, number of primary branches, leaf area, fresh leaf yield per plant, leaf yield per plot, leaf yield per hectare, dry matter production, protein content, ascorbic acid content and moisture content. While, the lowest days to germination and leaf chlorophyll contents were recorded with the seed inoculation of Azospirillum + PSB + foliar spray of ferrous sulphate @ 0.5%.

Greying of hair according to age is common phenomenon. But in modern era due to different kind of stress and changing life style Canities (Palitya) occur in early age. According to W.H.O in India, its incidence is high in the age group of 20-30 years. In Ayurveda premature greying of hair is called as Akala palitya (premature gray hair). According to Ayurveda grey hair is mainly due to the vitiation of Pitta dosha (biological entity related to exothermic reactions or transformation) & reference of Palitya is directly mentioned only in Pitta Prakruti (inherent constitution of an individual). The use of purification procedures of panchakarma (detoxifying therapy) has prime importance in the remedies mentioned for premature greying. Nasya karma (nasal medication) is one of the major rejuvenative treatments. Palliative external & internal medicaments measures like Shiro dhara (pouring oil on head), Shoro pichu (cloth dipped in oil kept on head), Shiro basti (oil is kept on head with an apparatus), Shiro lepa (application of medicated paste over head), Shiro abhyanga (oil massage to head) helps in speedy recovery. Sthaulya (Obesity) is a lifestyle and metabolic disorder. Ayurveda treated Sthaulya as a Santarpanottha Vikara (diseases due to excessive nutrition) which has been identified as one of the leading cause of morbidity and mortality worldwide in both developed and developing countries. Medodushti (disorders of fat metabolism) serves as one of the important etiological factor. In Ayurveda herbal drugs are being used to lower the levels of serum cholesterol for the treatment and prevention of this disorder. In this regard, an attempt has been made to review the drugs useful in Sthaulya specifically mentioned in Ganas (group of drugs) of Ayurvedic classical texts which are useful in prevention and management of conditions like Dyslipidemia and its complications.Â

Stem cells are undifferentiated cells capable of self - renewal and multilineage differentiation. The use of cells as mesenchymal dental stem cells, which has immunosuppressant properties, high proliferation, and capacity to differentiate into osteoblasts, odontoblasts, cementoblasts and other cells, suppose a good perspective in clinical dentistry. Also, these cells need a scaffold that facilitate their integration, differentiation, matrix synthesis and promote multiple specific interactions between cells. To date, different human dental stem cell progenitors have been identified and isolated - bone marrow stromal cells (BMMSC), human pulp tissue (DPSC), exfoliated deciduous teeth (SHED), periodontal ligament (PDLSC), apical papilla (SCAP), dental follicle (DFSC). Also included in this article are the potential applications of stem cell therapy in periodontics. The tissue engineering triad of cells, signaling molecules, scaffolds plays a significant role in achieving regeneration.

Background & Aims We investigated the effects of a diet low in fermentable oligo-, di-, and monosaccharides and polyols (FODMAPs) vs traditional dietary recommendations on health-related quality of life (QOL), anxiety and depression, work productivity, and sleep quality in patients with irritable bowel syndrome and diarrhea (IBS-D). Methods We conducted a prospective, single-center, single-blind trial of 92 adult patients with IBS-D (65 women; median age, 42.6 years) randomly assigned to groups placed on a diet low in FODMAPs or a modified diet recommended by the National Institute for Health and Care Excellence (mNICE) for 4 weeks. IBS-associated QOL (IBS-QOL), psychosocial distress (based on the Hospital Anxiety and Depression Scale), work productivity (based on the Work Productivity and Activity Impairment), and sleep quality were assessed before and after diet periods. Results Eighty-four patients completed the study (45 in the low-FODMAP group and 39 in the mNICE group). At 4 weeks, patients on the diet low in FODMAPs had a larger mean increase in IBS-QOL score than did patients on the mNICE diet (15.0 vs 5.0; 95% CI, –17.4 to –4.3). A significantly higher proportion of patients in the low-FODMAP diet group had a meaningful clinical response, based on IBS-QOL score, than in the mNICE group (52% vs 21%; 95% CI, –0.52 to –0.08). Anxiety scores decreased in the low-FODMAP diet group compared with the mNICE group (95% CI, 0.46–2.80). Activity impairment was significantly reduced with the low-FODMAP diet (–22.89) compared with the mNICE diet (–9.44; 95% CI, 2.72–24.20). Conclusions In a randomized, controlled trial, a diet low in FODMAPs led to significantly greater improvements in health-related QOL, anxiety, and activity impairment compared with a diet based on traditional recommendations for patients with IBS-D. Primary sclerosing cholangitis and Ulcerative colitis are caused by progressive inflammation of the bile duct and large intestine respectively. The existence of any plausible association between Primary sclerosing cholangitis and Ulcerative colitis remains highly elusive. Little is known about the incidence and prevalence of primary sclerosing cholangitis with concomitant Ulcerative colitis in the Indian subcontinent. We report a case of Primary sclerosing cholangitis with long standing Ulcerative colitis which later also developed Primary biliary cirrhosis.

This paper proposes modifications to the Multi-resolution RASTA (MRASTA) feature extraction technique for the automatic speech recognition (ASR). By emulating asymmetries of the temporal receptive field (TRF) profiles of auditory mid-brain neurons, we obtain more than $13\% $ relative improvement in word error rate on OGI-Digits database. Experiments on TIMIT database confirm that proposed modifications are indeed useful.

We present a framework to apply Volterra series to analyze multilayered perceptrons trained to estimate the posterior probabilities of phonemes in automatic speech recognition. The identified Volterra kernels reveal the spectro-temporal patterns that are learned by the trained system for each phoneme. To demonstrate the applicability of Volterra series, we analyze a multilayered perceptron trained using Mel filter bank energy features and analyze its first order Volterra kernels.
