Abstract

Introduction The risk of bleeding associated with transjugular kidney biopsies is unclear, and which patients are the best candidates for this route is unknown. Methods This was a retrospective cohort study comparing proportion of bleeding associated with transjugular versus percutaneous native kidney biopsies in all patients in France in the 2010–2019 period. Major bleeding at day 8 (i.e., blood transfusions, hemorrhage/hematoma, angiographic intervention, nephrectomy) and risk of death at day 30 were assessed, and we used a bleeding risk score initially developed for the percutaneous route. Results Our analysis included 60,331 patients (transjugular route: 5305; percutaneous route: 55,026 patients). The observed proportion of major bleeding varied widely (transjugular vs. percutaneous): 0.4% versus 0.5% for the lowest risk scores (0–4) to 19.1% versus 30.8% for the highest risk scores (≥35). Transjugular was more frequently used than percutaneous route (39% vs. 24%) when the risk score was ≥20 (15,133/60,331; 25% of all patients). Transjugular was associated with a lower risk of major bleeding than percutaneous route in multivariate analyses (odds ratio [OR]: 0.88 [0.78–0.99]), especially for scores ≥20 (OR: 0.83 [0.72–0.96], (i.e., 25% of patients). Major bleeding was associated with an increased risk of death both for transjugular (OR: 1.77 [1.00–3.14]) and percutaneous (OR: 1.80 [1.43–2.28]) routes. Conclusions The transjugular route is independently associated with a lower risk of bleeding than the percutaneous route, especially in high-risk patients identified by a preprocedure risk score ≥20 (i.e., 25% of patients). 12. Optimization of multi-organ failure for successful coronary angioplasty in acute myocardial infarction after pneumonectomy

Abstract

Pneumonectomy is a procedure that possesses several chronic complications most commonly associated with cardiovascular and respiratory systems. Acute myocardial infarction in patients after pneumonectomy and with other comorbidities represents high risk of interventional complications and mortality. We present a case of effective percutaneous coronary angioplasty in a 75-year-old patient with acute non-STEMI infarction, previous pneumonectomy and multi-organ pathology. Choice of treatment strategy and clinical decision making were further complicated by the presence of multiple risk factors, including impaired respiratory function and advanced age of the patient. However, after the short term stabilization of cardiac and respiratory failure symptoms, the patient underwent successful angioplasty with implantation of a drug eluting stent on the right coronary artery. The patient demonstrated a significant improvement of the symptoms and multiorgan failure parameters after angioplasty. This report shows the effective management strategy of patient with acute myocardial infarction and concomitant multi organ failure with indication for percutaneous coronary intervention.

Abstract

Aims The 2019 report from the European Society of Cardiology (ESC) Atlas provides a contemporary analysis of cardiovascular disease (CVD) statistics across 56 member countries, with particular emphasis on international inequalities in disease burden and healthcare delivery together with estimates of progress towards meeting 2025 World Health Organization (WHO) non-communicable disease targets. Methods and results In this report, contemporary CVD statistics are presented for member countries of the ESC. The statistics are drawn from the ESC Atlas which is a repository of CVD data from a variety of sources including the WHO, the Institute for Health Metrics and Evaluation, and the World Bank. The Atlas also includes novel ESC sponsored data on human and capital infrastructure and cardiovascular healthcare delivery obtained by annual survey of the national societies of ESC member countries. Across ESC member countries, the prevalence of obesity (body mass index ≥30 kg/m2) and diabetes has increased two- to three-fold during the last 30 years making the WHO 2025 target to halt rises in these risk factors unlikely to be achieved. More encouraging have been variable declines in hypertension, smoking, and alcohol consumption but on current trends only the reduction in smoking from 28% to 21% during the last 20 years appears sufficient for the WHO target to be achieved. The median age-standardized prevalence of major risk factors was higher in middle-income compared with high-income ESC member countries for hypertension {23.8% [interquartile range (IQR) 22.5–23.1%] vs. 15.7% (IQR 14.5–21.1%)}, diabetes [7.7% (IQR 7.1–10.1%) vs. 5.6% (IQR 4.8–7.0%)], and among males smoking [43.8% (IQR 37.4–48.0%) vs. 26.0% (IQR 20.9–31.7%)] although among females smoking was less common in middle-income countries [8.7% (IQR 3.0–10.8) vs. 16.7% (IQR 13.9–19.7%)]. There were associated inequalities in disease burden with disability-adjusted life years per 100 000 people due to CVD over three times as high in middle-income [7160 (IQR 5655–8115)] compared with high-income [2235 (IQR 1896–3602)] countries. Cardiovascular disease mortality was also higher in middle-income countries where it accounted for a greater proportion of potential years of life lost compared with high-income countries in both females (43% vs. 28%) and males (39% vs. 28%). Despite the inequalities in disease burden across ESC member countries, survey data from the National Cardiac Societies of the ESC showed that middle-income member countries remain severely under-resourced compared with high-income countries in terms of cardiological person-power and technological infrastructure. Under-resourcing in middle-income countries is associated with a severe procedural deficit compared with high-income countries in terms of coronary intervention, device implantation and cardiac surgical procedures. Conclusion A seemingly inexorable rise in the prevalence of obesity and diabetes currently provides the greatest challenge to achieving further reductions in CVD burden across ESC member countries. Additional challenges are provided by inequalities in disease burden that now require intensification of policy initiatives in order to reduce population risk and prioritize cardiovascular healthcare delivery, particularly in the middle-income countries of the ESC where need is greatest.

Abstract

© 2020 European Society of Cardiology. All rights reserved., Atrial fibrillation (AF) poses significant burden to patients, physicians, and healthcare systems globally. Substantial research efforts and resources are being directed towards gaining detailed information about the mechanisms underlying AF, its natural course and effective treatments (see also the ESC Textbook of Cardiovascular Medicine: CardioMed) and new evidence is continuously generated and published. The complexity of AF requires a multifaceted, holistic, and multidisciplinary approach to the management of AF patients, with their active involvement in partnership with clinicians. Streamlining the care of patients with AF in daily clinical practice is a challenging but essential requirement for effective management of AF. In recent years, substantial progress has been made in the detection of AF and its management, and new evidence is timely integrated in this third edition of the ESC guidelines on AF. The 2016 ESC AF Guidelines introduced the concept of the five domains to facilitate an integrated structured approach to AF care and promote consistent, guideline-adherent management for all patients. The Atrial Fibrillation Better Care (ABC) approach in the 2020 ESC AF Guidelines is a continuum of this approach, with the goal to further improve the structured management of AF patients, promote patient values, and finally improve patient outcomes.

Abstract

We present two cases of acute myocardial infarction in young patients with asymptomatic COVID‐19 infection and ST‐elevation myocardial infarction (STEMI), complicated by severe acute heart failure and ventricular fibrillation, resulting cardiopulmonary resuscitation and mechanical ventilatory support. Urgent primary percutaneous coronary intervention with further complex treatment was effective in both cases with critical cardiovascular state and co‐morbid COVID‐19 infection. This report illustrates the challenges in clinical severity of STEMI with COVID‐19 infection, despite of young age and absence of clinical symptoms and chronic co‐morbidities. STEMI patients with even asymptomatic COVID‐19 infection may be presented with significantly higher rates of severe acute heart failure. [ABSTRACT FROM AUTHOR]

30. Effect of selective heart rate reduction through sinus node Ifcurrent inhibition on severely impaired left ventricular diastolic dysfunction in patients with chronic heart failure

Abstract

AimsThe aim of the study was the assessment of heart rate reduction effect through selective If current inhibition on several left ventricular (LV) diastolic function parameters and left atrial (LA) volumetric parameters in patients with systolic heart failure (HF).Methodsand results In total 54 patients with systolic HF and pseudonormal/restrictive type of diastolic dysfunction were randomly allocated to either the ivabradine (27) or control group (27) during a 3-month period. Left atrial volume index (LAVI) and the following diastolic function parameters were determined by transmitral pulsed wave and mitral annulus tissue Doppler study before and after the therapy period: deceleration time (DT), peak early to late mitral inflow wave velocity ratio (E/A) and the ratio of mitral peak velocity of early filling (E) to early diastolic mitral annular velocity (Em) (E/Em). Parametric variables were compared by a two-set analysis of variance with a repeated measurement for the time factor. Ivabradine treatment was accompanied by increase of DT (30.19 ± 30.21 msec in ivabradine, versus 4.07 ± 16.93 msec in controls, 95% CI, P= 0.00001), decrease of E/A ratio (-0.83 ± 1.2, in ivabradine versus –0.05 ± 0.42 in controls, P= 0.0001) and decrease of E/Em ratio (-5.16 ± 3.9 in ivabradine versus 0.99 ± 4.34, P= 0.0002 in controls). Ivabradine also improved LAVI (-6.14 ± 5.17 ml/m² versus 0.65 ± 4.29 ml/m² in controls, P< 0.0001). After 3 months of treatment heart rate was significantly reduced in the ivabradine group (P <0.0001).ConclusionTreatment with ivabradine improves LV diastolic function through reducing E/A ratio, E/Em ratio and increasing DT. These changes may contribute to the improvement of intracardiac haemodynamics with decrease of LAVI and improvement of LV filling.

32. The effect of trimetazidine on left ventricular systolic function and physical tolerance in patients with ischaemic cardiomyopathy

Abstract

Objective—Trimetazidine (TMZ) is the first of novel antianginal drugs with a cardioprotective effect, selectively inhibiting mitochondrial long-chain 3-ketoacyl coenzyme A thiolase. This study tested the hypothesis that the cytoprotective beneficial effect of this agent can lead to the improvement of left ventricular (LV) systolic function and tolerance to physical activity in patients with ischaemic cardiomyopathy.Methods and results— In 82 consecutive patients with ischaemic cardiomyopathy, a subgroup of patients (n = 42) was assigned to receive a modified form of TMZ (35 mg twice daily) in addition to the conventional therapy for the duration of three months.All patients underwent clinical, echocardiography examination and a six-minute walk test at baseline and after a three-month treatment. The therapy with TMZ significantly improved the functional class in these patients. Left ventricular ejection fraction (LVEF) increased by 3.5 ± 6.72% (from 34.5 ± 3.8% to 38.0 ±4.8%) in the TMZ group vs. 0.8 ± 8.06% (from 32.4 ± 5.6% to 33.2 ± 5.8%) in the control group (P = 0.05). The tolerance to physical activity improved by 30.0 ± 20.7 m in the TMZ group (from 215 ± 17.5 m to 245 ± 20.7 m) vs. 2.0 ± 18.85 m (from 208.2 ±12.4 m to 210.2 ±14.2 m) in the control group (P < 0.001).Conclusions— A therapeutic intervention with TMZ in conjunction with the standard therapy, over a three-month period, is associated with an increase in LVEF and improved tolerance to physical activity in patients with ischaemic cardiomyopathy.

Abstract

ObjectivePulmonary congestion is the main cause of hospital admission in patients with heart failure (HF). Lung ultrasound (LUS) is a useful tool to identify subclinical pulmonary congestion. We evaluated the usefulness of LUS in addition to physical examination (PE) in the management of outpatients with HF.MethodsIn this randomised multicentre unblinded study, patients with chronic HF and optimised medical therapy were randomised in two groups: ‘PE+LUS’ group undergoing PE and LUS and ‘PE only’ group. Diuretic therapy was modified according to LUS findings and PE, respectively. The primary endpoint was the reduction in hospitalisation rate for acute decompensated heart failure (ADHF) at 90-day follow-up. Secondary endpoints were reduction in NT-proBNP, quality-of-life test (QLT) and cardiac mortality at 90-day follow-up.ResultsA total of 244 patients with chronic HF and optimised medical therapy were enrolled and randomised in ‘PE+LUS’ group undergoing PE and LUS, and in ‘PE only’ group. Thirty-seven primary outcome events occurred. The hospitalisation for ADHF at 90 day was significantly reduced in ‘PE+LUS’ group (9.4% vs 21.4% in ‘PE only’ group; relative risk=0.44; 95% CI 0.23 to 0.84; p=0.01), with a reduction of risk for hospitalisation for ADHF by 56% (p=0.01) and a number needed to treat of 8.4 patients (95% CI 4.8 to 34.3). At day 90, NT-proBNP and QLT score were significantly reduced in ‘PE+LUS’ group, whereas in ‘PE only’ group both were increased. There were no differences in mortality between the two groups.ConclusionsLUS-guided management reduces hospitalisation for ADHF at mid-term follow-up in outpatients with chronic HF.

34. Optimization of multi-organ failure for successful coronary angioplasty in acute myocardial infarction after pneumonectomy.

Abstract

Pneumonectomy is a procedure that possesses several chronic complications most commonly associated with cardiovascular and respiratory systems. Acute myocardial infarction in patients after pneumonectomy and with other comorbidities represents high risk of interventional complications and mortality. We present a case of effective percutaneous coronary angioplasty in a 75-year-old patient with acute non-STEMI infarction, previous pneumonectomy and multi-organ pathology. Choice of treatment strategy and clinical decision making were further complicated by the presence of multiple risk factors, including impaired respiratory function and advanced age of the patient. However, after the short term stabilization of cardiac and respiratory failure symptoms, the patient underwent successful angioplasty with implantation of a drug eluting stent on the right coronary artery. The patient demonstrated a significant improvement of the symptoms and multiorgan failure parameters after angioplasty. This report shows the effective management strategy of patient with acute myocardial infarction and concomitant multi organ failure with indication for percutaneous coronary intervention., Competing Interests: None., (AJCD Copyright © 2020.)
