Abstract

The archive contains the results of research on BNC bacterial biocellulose, which was modified in two ways. When BNC was synthesized, it was subjected to in situ modification involving the addition of hyuloronic acid to the bacterial medium. The second way of ex situ modification consisted of soaking the BNC in water and then convectionally drying it at 25 C and 105 C and also freeze-drying it. XRD, nanoindentation and also Raman spectra were performed, and pore distribution and size were determined. The relationship between the structure of modified and unmodified BNC and its mechanical properties was demonstrated. The studies were performed in the context of BNC applications for cardiovascular implants.

30. The Hyperbaric Protective Tube: A housing for a left ventricular assist device (LVAD) in a multiplace hyperbaric chamber.

Abstract

Introduction: During a hyperbaric oxygen therapy (HBOT) session, every medical device that is used within the hyperbaric chamber is exposed to several hazards, including an increased ambient pressure and partial pressure of oxygen. In Europe, all medical devices marketed and/or sold for use in hyperbaric conditions must be tested by the manufacturer and marked 'CE' if approved. At the moment, no left ventricular assist device (LVAD) has been formally approved and CE-marked for HBOT. Case: A 65-year-old male was referred to our Hyperbaric Centre for HBOT due to a persistent life-threating soft tissue infection of the non-removable wire connecting the external controller with the pump implanted into the left ventricle of the heart (Heartware LVAD). The aim of the intervention reported here was to safely conduct HBOT sessions with this non- CE marked medical device. After risk analysis, the decision was made to isolate the external part of the LVAD (controller and batteries) from the ambient conditions in the hyperbaric chamber by placing it in a pressure-resistant housing that was vented to the external atmosphere. The housing, a 'Hyperbaric Protective Tube' was built and tested, and the resulting operating procedures were practiced by personnel involved in the patient's care. Thirty uneventful HBOT standard sessions were conducted with subsequent clinical improvement of the soft tissue infection, resulting in an extended timeframe for awaiting heart transplantation. Conclusion: An isolation housing that vents into the dumping system of the hyperbaric chamber allows for the safe use of critical medical devices without prior testing for their compatibility with the hyperbaric environment. 36. A new diagnostic method for evaluation of cardiosurgery wound healing

Abstract

One of important and still unsolved problems in medicine is the question of objective and quantitative evaluation of post-surgery wound healing. The aim of this research is to analyse possibility and value of using the newly elaborated infrared imaging procedure – Active Dynamic Thermography (ADT) – for quantitative description of wound healing processes in cardiosurgery. Both, classical thermal figures of merit as well as ADT descriptors are compared from the point of view of objective, quantitative estimation of wound state. Temporal properties of thermal transients are proposed for objective quantitative description of a tested wound and for classification of healing processes as normal or suspected, requesting a special care. Algorithms enabling evaluation of surgical wound healing process are discussed in terms of possible implementation of the method into clinical practice for proper classification of patients for discharging from a hospital and further treatment at home or for prolonged treatment at the hospital.

37. Arm temperature distribution in thermographic pictures after radial artery harvesting for coronary bypass operation.

Abstract

The radial artery (RA) beside the left internal mammary artery and veins is often used for coronary artery bypass grafting. The aim of this study was to assess arm temperature changes after removal of the radial artery and determine whether there is an effect on hand function. Between April and December 2005, fifty patients underwent myocardial revascularization employing the pedicle RA as one of the coronary grafts. The study protocol included thermographic and clinical examination one day before the surgery and on the sixth day after the procedure. An Agema 900 Thermovision System was used for thermographic evaluation. Thermovision examinations were carried out at rest and after a handgrip test before and after radial artery harvesting. Temperature was measured on each finger and on the forearm (separately on the radial and ulnar side of the forearm). All examinations were performed in the same conditions. Hand endurance was compared with the 2 min handgrip test before and after the surgery. A comparison of hand temperature changes during the handgrip test before and after radial artery harvesting showed no significant difference (P>0.05). Before the surgery a significant increase in temperature over the ulnar artery was observed after the handgrip test. There were no clinical symptoms of acute or subacute hand ischemia. No clinical evidence of hand claudication was detected. There was no significant difference in hand endurance after radial artery harvesting. This study suggests that removal of the radial artery changes the hand temperature distribution but does not affect hand function in the short-term. In the study group, radial artery harvesting did not cause clinically relevant changes in blood support of the arm.

Abstract

We present a 77-year-old female with recent myocardial infarction complicated by antero-apical ventricular septal defect (VSD) and quick development of low cardiac output. Using interrupted horizontal mattress sutures VSD closure was successfully performed on the beating heart. Transthoracic echocardiography at the four-month follow-up did not reveal residual VSD.

Abstract

Coronary artery disease is a frequent comorbidity in patients undergoing major thoracic surgery. Simultaneous operations eliminate the necessity of a second operation and, more importantly, minimize the delay in compulsory postoperative oncological therapy. We describe a relaxing incision in the contralateral pericardium, which allows for simple displacement of the heart. This maneuver improves exposure of the pulmonary hilum and middle mediastinum on the side of resection.

Abstract

To confirm the sensitivity of the polymerase chain reaction (PCR) technique (versus blood cultures) and to gain a better understanding of the incidence of true- and false-positive results when using this technique, one hundred randomly chosen patients treated operationally because of valve defects were examined. In our studies we found that PCR techniques using universal primers complementary to the bacterial 16S rDNA showed promise as being more sensitive than conventional blood culture (BC) techniques. From the time that a blood culture is positive, conventional methods of culture and antibiotic susceptibility testing require at least 64 h for the detection of infection or colonization. Rapid identification of bacteria from blood using PCR technique accelerates the microbiological diagnosis.

Abstract

Unlabelled: Although the incidence of wound complications after median sternotomy is less than 1%, it remains a serious complication in patients undergoing cardiac procedures. We suggest that the combination of hyperbaric oxygen therapy and aggressive surgical approach improves clinical outcomes in these patients. Between August 1997 and May 2002, 55 patients with postoperative sternal wound infection and/or mediastinitis were qualified for hyperbaric oxygen treatment in connection to surgical management. Surgical procedure included wound debridment and/or sternum rewiring, omental pedicle flap plasty or sternectomy. Hyperbaric oxygen therapy consisted of 20 to 40 expositions per patient and was carried before and after the surgery., Results: There was no in-hospital death. The total time between the admission and discharge from the hospital varied from 2 to 24 weeks (average 8 weeks). The infection has been cured in all patients treated for postoperative sternal wound infection. That has been confirmed by negative bacteriological tests, stabilization of the sternum and complete wound healing., Conclusions: The combination of surgical treatment and hyperbaric oxygen therapy may improve clinical outcome in patients with sterno-mediastinis and poststernotomy wound infection after cardiac surgery.
