Abstract

Implementation of change in an organisation through culture can elicit a wide array of reactions from organisational members, spanning from acceptance to resistance. Drawing on Hatch's cultural dynamics model and on Wegner's social theory of learning, this paper dwells on an underdeveloped area in the extant literature, namely understanding change from the perspective of the change recipient, and then analysing how this is linked to the broader framework of organisational change. It is suggested that in order to drive meaningful change strategy, change agents need to learn that sensegiving imparted by them needs to be closely aligned to the sensemaking of change recipients at three levels of analysis. Abstract

Leukemia cutis is the infiltration of neoplastic leukocytes or their precursors into the epidermis, the dermis, or the subcutis, resulting in clinically identifiable cutaneous lesions. We describe a case of CML who presented with extensive cutaneous manifestations at the time of second blast crisis with multiple subcutaneous skin nodules over the face and trunk with extensive violaceous papules and plaques over all four limbs and the trunk, with scalp showing extensive crusting and scaling with foul smelling discharge.

Abstract

Human epidermal growth factor receptor 2 (HER2)-positive is an aggressive subtype of breast cancer and has historically been associated with poor outcomes. The availability of various anti-HER2 therapies, including trastuzumab, lapatinib, pertuzumab, and trastuzumab emtansine (TDM-1), has remarkably improved the clinical outcomes in patients with HER2-positive metastatic breast cancer (mBC). However, there is a need to optimize treatment within this population, given the wide variability in clinical presentation. Additionally, geographical and socio-economic considerations too need to be taken into account. To clarify and collate evidence pertaining to HER2-positive metastatic breast cancer, a panel of medical and clinical oncologists from across India developed representative clinical scenarios commonly encountered in clinical practice in the country. This was followed by two meetings wherein each clinical scenario was discussed in detail and relevant evidence appraised. The result of this process is presented in this manuscript as evidence followed by therapeutic recommendations of this panel for management of HER2-positive mBC in the Indian population.

Abstract

Background: The "anatomical" and "mathematical" methods are the two main types of adult stature estimation methods available. Till date, most of the studies on stature estimation have used the length of bones such as femur, humerus, radius, etc. Very limited work has been done to estimate stature from finger length, as there is very little literature available for the same. Aim: Measurement of ring finger to estimate the stature in young adul ts. Material and Methodology: A correlation study was conducted with the approval of institutional Ethical Committee during the period of May 2017 to October 20147among 500 adults. All the right handed males and females in the age group of 22 to 40 years were included in the study. Individuals having congenital defects on right upper limb, congenital defects on right lower limb, pregnant woman, previous history of fracture in upper limb or lower limb or any bony malformation due to rickets, osteomalacia was excluded from the study. Data analysis was done using SPSS 17. Results: In the study 49.8% of the participants were males with mean age of 31.18 ± 5.54 and females with mean age of 32.14 ± 6.36. Mean stature among males was 162.51 ± 7.56 cm and in females it was 153.59 ± 5.21 cm. The mean value of ring finger length in males and females were 7.02 ± 0.42 cm and 6.40 ± 0.67 cm respectively. Abstract

Background: COPD usually is diagnosed late in its natural course because patients may adapt to the condition or doctors may not notice the symptoms. Spirometry is however only one way of interpreting COPD disease severity. Quantitative assessment of symptoms like dyspnoea, measurement of PEFR and exercise test like 6-minute walk test (6-MWT), which are cheaper modes of diagnosis, can be considered to substitute the spirometry at places where it is not available. Therefore, this study aims to find out correlation of Six Minutes' Walk Test with Spirometric and Clinical parameters in COPD patients. Materials & Methods: A hospital based prospective study was carried out in COPD patients at Government (RVRS) Medical College & Attached groups of Mahatma Gandhi Hospital, Bhilwara, Rajasthan. A total of 50 patients were included in the study, reporting to the OPD or admitted in the wards. At the time of test, the patient's heart rate, blood pressure and oxygen saturation were measured. The 6MWT was performed according to the ATS guidelines. The SPSS 16.0 software package was used for statistical analyses, and a value of p < 0.05 was considered significant. Results: Our study showed that the mean value of six-minute walk distance (6MWD) was 373.5±67.82 in stage I COPD, 266.7±50.88 in stage II, 199.3±63.12 in stage III & 152.2±50.5 in stage IV, which was statistical significant (P<0.0001). It means inverse relationship in six-minute walk distance (6MWD) and severity of COPD. The mean 6MWD was 237.9 ± 86.92 meters (range 88-500 m) and correlated positively to weight, BMI and negatively correlated to breathlessness (mMRC grade) (P<0.0001). Conclusion: Our results suggested that 6MWT can be used as an additional tool in combination with other physiological parameters in assessing the lung function in COPD patients.
