Objective: Efforts to improve physician well-being have focused on gratitude, which predicts health and happiness. Despite reported benefits, expressions of gratitude in healthcare can seem infrequent. Here, we describe Gratitude-Grams, an intervention to cultivate expressions of gratitude throughout a department., Methods/approach: Piloted in our Department of Surgery and adopted by others, Gratitude-Grams employs a web-based platform (Qualtrics). Program feedback was solicited during teaching conferences using an anonymous department survey., Results: Gratitude-Grams streamlines and encourages expressions of gratitude while minimizing maintenance, cost, and time. The platform has been highly utilized and well-received in our Department of Surgery., Conclusion: Expressing and receiving gratitude has been shown to be critical for well-being. Gratitude-Grams is a highly utilized, simple, and attainable system to support expressions of gratitude and is ready for rapid implementation., (Copyright © 2023 Association of Program Directors in Surgery. Perianal skin lesions, such as skin tags, can be an early presenting sign of Crohn's disease. Surgical intervention on these lesions may increase the risk of fistula development and lead to worse outcomes. This case series examined 8 patients who underwent surgical intervention on what appeared to be benign perianal skin lesions, only to reveal fistulas leading to the diagnosis of Crohn's disease. This patient population comprised 20% of all pediatric patients with Crohn's disease who had perianal fistula present at diagnosis. The initial type of perianal lesion varied from case to case and included skin tags, hemorrhoids, and perianal abscesses. All of the patients had other presenting features that, in retrospect, may have been attributed to Crohn's disease. None presented solely with a perianal lesion. Four patients had weight loss or growth failure. Most of the remainder had abnormal laboratory test results. These findings should raise the awareness of primary care providers that perianal lesions can be the first presenting sign of possible Crohn's disease in otherwise healthy appearing children. Such children should undergo a thorough evaluation for Crohn's disease before surgical intervention on perianal lesions because surgical procedures may be associated with worse outcomes., (Copyright © 2016 by the American Academy of Pediatrics.)

Salt-sensitive hypertension is common in the aged population. Increased fruit and vegetable intake reduces hypertension, but its effect on eventual diastolic dysfunction is unknown. This relationship is tested in the Dahl Salt-Sensitive (Dahl-SS) rat model of salt-sensitive hypertension and diastolic dysfunction. Table grape powder contains phytochemicals that are relevant to human diets. For 18 weeks, male Dahl-SS rats were fed one of five diets: low salt (LS), a low salt + grape powder (LSG), high salt (HS), a high salt + grape powder (HSG), or high salt + vasodilator hydralazine (HSH). Compared to the HS diet, the HSG diet lowered blood pressure and improved cardiac function; reduced systemic inflammation; reduced cardiac hypertrophy, fibrosis, and oxidative damage; and increased cardiac glutathione. The HSH diet similarly reduced blood pressure but did not reduce cardiac pathogenesis. The LSG diet reduced cardiac oxidative damage and increased cardiac glutathione. In conclusion, physiologically relevant phytochemical intake reduced salt-sensitive hypertension and diastolic dysfunction.

Elevated plasma lipids, glucose, insulin, and fatty liver are among components of metabolic syndrome, a phenotypic pattern that typically precedes the development of Type 2 diabetes. Animal studies show that intake of anthocyanins reduces hyperlipidemia, obesity, and atherosclerosis and that anthocyanin-rich extracts may exert these effects in association with altered activity of tissue peroxisome proliferator-activated receptors (PPARs). However, studies are lacking to test this correlation using physiologically relevant, whole food sources of anthocyanins. Tart cherries are a rich source of anthocyanins, and whole cherry fruit intake may also affect hyperlipidemia and/or affect tissue PPARs. This hypothesis was tested in the Dahl Salt-Sensitive rat having insulin resistance and hyperlipidemia. For 90 days, Dahl rats were pair-fed AIN-76a-based diets supplemented with either 1% (wt:wt) freeze-dried whole tart cherry or with 0.85% additional carbohydrate to match macronutrient and calorie provision. After 90 days, the cherry-enriched diet was associated with reduced fasting blood glucose, hyperlipidemia, hyperinsulinemia, and reduced fatty liver. The cherry diet was also associated with significantly enhanced hepatic PPAR-alpha mRNA, enhanced hepatic PPAR-alpha target acyl-coenzyme A oxidase mRNA and activity, and increased plasma antioxidant capacity. In conclusion, physiologically relevant tart cherry consumption reduced several phenotypic risk factors that are associated with risk for metabolic syndrome and Type 2 diabetes. Tart cherries may represent a whole food research model of the health effects of anthocyanin-rich foods and may possess nutraceutical value against risk factors for metabolic syndrome and its clinical sequelae.

Background: Chronic, disease-associated oxidative stress induces myocardial peroxynitrite formation that may lead to nitrosative inhibition of the calcium cycling protein sarcoplasmic endoreticular calcium adenosine triphosphatase subtype 2a (SERCA2a). The current study was designed to test the hypothesis that the acute oxidative stress associated with lung resection also induces myocardial nitrosative stress and alters SERCA2a activity., Methods: Ventricular myocardium from 16 swine was studied; 11 animals had undergone left upper lobectomy (n = 7) or sham thoracotomy (n = 4) 3 days before harvest, and 5 were nonoperated controls. Tissue peroxynitrite was assessed by measurement of 3-nitrotyrosine incorporation into proteins. SERCA2a activity was determined from indo-1 uptake by isolated sarcoplasmic reticular membranes. Expression of SERCA2a and its regulatory protein phospholamban were determined by Western blotting, as was the phospholamban phosphorylation state (when dephosporylated, phospholamban inhibits SERCA2a). Mechanical significance of changes in SERCA2a activity was assessed from the force-frequency relation of isometric myocardial trabeculae., Results: Relative to both the control and sham groups, lobectomy animals exhibited a greater than twofold higher myocardial 3-nitrotyrosine incorporation and an approximately 50% lower SERCA2a activity, but no difference in SERCA2a or phospholamban expression or phospholamban phosphorylation. Concomitantly, whereas the trabecular force-frequency relation of control animals was positive, that of lobectomy animals was negative, consistent with impaired calcium cycling., Conclusions: These data indicate that oxidative/nitrosative stress associated with lung resection influences SERCA2a activity independent of any influence on protein expression or phospholamban phosphorylation. The findings link an acute event with a subcellular process primarily described for chronic illness and suggest a biochemical basis for perioperative changes in myocardial mechanical reserve.

Caloric restriction extends longevity and reduces the onset of chronic disease in many animal models. Recently, caloric restriction was shown in humans to be associated with lower blood pressure, decreased systemic inflammation, and improved cardiac diastolic parameters. However, the causation and mechanisms of caloric restriction were obscured by the varied diet composition of the participants. The Dahl salt-sensitive rat which develops gradual, hypertension-associated diastolic dysfunction was used in this study to assess the impact of caloric restriction upon decompensated pressure-overload hypertrophy. Male Dahl salt-sensitive rats were provided either a low-salt diet or a high-salt diet to initiate heart failure progression. A further subset of high-salt rats underwent 15% calorie restriction, with salt load held constant. Parameters measured included serial systolic blood pressure, body weight, and changes of left ventricular systolic and diastolic parameters and ventricular geometry by echocardiography. After 18 weeks, fasting glucose, blood lipids, heart weight, kidney weight, lung weight, plasma interleukin-6 and TNF-alpha, and cardiac lipid peroxidation were measured. Low-salt rats did not develop heart failure. While high-salt rats displayed features of decompensated pressure-overload hypertrophy, moderate calorie restriction remarkably reduced morbidity. Compared to the high-salt fed group, the high-salt, calorie-restricted group showed reduced blood pressure, delayed onset of cachexia, lower fasting hyperlipidemia, lower cardiac, renal and lung weight, less plasma IL-6 and TNF-alpha, less cardiac oxidative damage, and improved diastolic chamber function and cardiac index. Modest calorie restriction, independent of salt intake, reduced pathogenesis in this well described model of decompensated pressure-overload hypertrophy.

Although leukemic invasion of bowel is frequently seen on pathologic examination, it has rarely been described in the imaging literature. Previous radiographic reports have relied mostly on non-cross-sectional imaging techniques such as barium enema and have detected abnormalities only in the most advanced stages of disease when prognosis is uniformly poor. We describe a case of direct leukemic invasion seen on computed tomography which may offer the advantage of earlier detection and more favorable prognosis.

Background: This study was conducted to determine if pelvic computed tomography (CT) should routinely be appended to abdominal CT in the workup of patients with breast carcinoma., Methods: The abdominal-pelvic CTs of 139 breast carcinoma patients (195 exams) were reviewed. Scans were grouped by indication and whether pelvic pathology was known before CT. Pelvic CT results were correlated with their effect on patient management., Results: Among the 119 patients without pre-CT evidence of pelvic disease, a nonosseous pelvic metastasis was identified in only 1; this patient also had liver metastases and management was not changed. No unsuspected pelvic CT finding altered therapy for breast carcinoma. However, three patients underwent surgery for asymptomatic masses discovered on pelvic CT; all were benign., Conclusions: Pelvic CT is unlikely to affect the management of patients with breast carcinoma by detecting occult metastatic disease.

This case report of severe gastrointestinal (GI) bleeding demonstrates the utility of computed tomography (CT) in localizing such hemorrhages when all other diagnostic modalities have been exhausted. All previous studies in this case were nondiagnostic. However, abdominal CT performed without the benefit of oral contrast but immediately following negative angiography was useful in directing the surgeon to the region of hemorrhage by documenting the presence of focally dense intraluminal contrast within the small bowel loops.

Background: Determining the nature of an adrenal mass is often a clinical challenge., Objective: To determine if unenhanced computed tomographic (CT) scanning can differentiate benign adenomas from metastases., Methods: Twenty-four pathologically proven adrenal masses were retrospectively correlated with their appearance on unenhanced CT scanning., Results: Metastases were significantly larger than adenomas and had higher attenuation coefficients. A sensitivity-to-specificity ratio of 33:100 was achieved at a threshold of 0 Hounsfield units (HU), while a threshold of 10 HU produced a ratio of 58:92. A threshold size of 2.5 cm produced a ratio of 58:100. Attenuation and size were the only useful criteria for differentiating adenomas from metastases., Conclusion: Measuring the size and attenuation of adrenal masses can help identify benign adenomas. Lesions exceeding specific thresholds may still be benign and may require biopsy. We advocate documenting lesion stability for longer than is usually done, as one metastatic lesion remained without significant change in appearance for 18 months.

Forty patients without evidence of liver, kidney, or significant cardiac disease were prospectively divided into two groups of 20, receiving either iohexol-240 or iohexol-300. A contrast load of 150 ml was administered in conjunction with a rapid scanning technique at a preselected, fixed level to include liver, renal cortex, and aorta. Peak enhancement was calculated as change in Hounsfield units (HU) over baseline for each area of interest. Mean peak enhancement and standard deviation were calculated for each organ, and the difference between the means for the two contrast agents was compared using the Student's t test. Differences were not statistically significant with all p values greater than 0.05. Our results suggest iohexol-240 is preferred to iohexol-300 for body computed tomography (CT) due to its lower cost and iodine load without statistically significant change in diagnostic quality of the examination.

All testicular sonograms performed over a 2.5-year period were retrospectively reviewed, yielding eight patients with pathologically proven lesions consisting primarily of tubular sclerosis and interstitial fibrosis. Only two patients (25%) had a palpable abnormality. A variety of sonographic patterns was found, including focal hypoechoic or hyperechoic lesions and diffuse heterogeneity of the testicular parenchyma. The clinical and sonographic findings prompted open biopsy or orchiectomy in all cases. In the same time period, nine pathologically proven testicular malignancies were evaluated sonographically and displayed either well-defined hypoechoic or diffusely heterogeneous echo patterns. All but two of these patients (78%) had palpable abnormalities. This study demonstrates a significant overlap in the sonographic appearance of benign fibrotic lesions and testicular malignancies. When careful palpation of a sonographically heterogeneous or focal hypoechoic lesion fails to reveal a mass and serum tumor markers are negative, an open biopsy with frozen section analysis should be considered rather than proceeding directly to orchiectomy. Homogeneously hyperechoic masses can be considered benign and do not require surgery.

The frequency and degree of visualization of medullary pyramids in a normal population, aged 10-29 years, was analyzed. Hypoechoic pyramids were visualized in 42% of right kidneys in subjects aged 10-18 years and in 27% of subjects aged 19-29 years. Prominently hypoechoic pyramids, mimicking the appearance of neonatal kidneys, were seen in an additional 34% of subjects aged 10-18 years and in 16% aged 19-29 years. Prominent pyramids were present in 50% of subjects with renal cortical echogenicity (RCE) equal to liver, but also in 21% of subjects with RCE less than liver. Our study expands the age at which prominently hypoechoic medullary pyramids can be considered a normal finding. This may relate to recent improvements in ultrasound technology.

Many malignant processes cause abdominal lymphadenopathy, and computed tomography (CT) has become the primary modality for its detection. Diagnosis of lymphadenopathy is facilitated by optimal imaging techniques and a knowledge of the various nodal chains, their complex interconnections, and preferential pathways of spread. Optimal techniques include imaging after oral administration of adequate amounts of barium suspension and dynamic scanning after intravenous administration of contrast material with an infusion pump. Although such techniques help prevent misdiagnoses due to normal and anomalous vascular structures, other benign diseases can mimic the CT appearance of malignant lymphadenopathy. The authors emphasize a regional approach for the diagnosis of lymphadenopathy, according to the groupings of retrocrural, retroperitoneal, gastrohepatic ligament, porta hepatis, celiac and superior mesenteric artery, pancreaticoduodenal, perisplenic, mesenteric, and pelvic lymph nodes. Lymphadenopathy is defined as retrocrural nodes greater than 6 mm in short axis, upper abdominal nodes greater than 10 mm, and pelvic nodes greater than 15 mm.
