8. Paraneoplastic dermatomyositis associated with Merkel cell carcinoma.

- Abstract
A man in his 70s presented to a dermatology outpatient clinic with a new rash developing over 4 weeks, associated with weakness and swallowing difficulties. He had a background of Merkel cell carcinoma (MCC) diagnosed 2 years prior, with a recent CT suspicious for pancreatic metastasis. On examination, there were urticated plaques involving the face, neck, back and abdomen. Gottron sign and papules, Holster sign and periorbital erythema were also present. There was proximal muscle weakness and dysphagia, requiring urgent inpatient assessment and a multidisciplinary care approach. An autoantibody screen was positive for antitranscription intermediary factor 1 (TIF1)-γ. A diagnosis of paraneoplastic dermatomyositis (DM) was made, and the patient was commenced on intravenous methylprednisolone with good response to treatment. This case highlights the need for multidisciplinary care in the management of these two rare diseases: MCC and DM. We also discuss the autoantibody anti-TIF1-γ in paraneoplastic DM. Significant advances have been made towards building accurate automatic segmentation systems for a variety of biomedical applications using machine learning. However, the performance of these systems often degrades when they are applied on new data that differ from the training data, for example, due to variations in imaging protocols. Manually annotating new data for each test domain is not a feasible solution. In this work we investigate unsupervised domain adaptation using adversarial neural networks to train a segmentation method which is more invariant to differences in the input data, and which does not require any annotations on the test domain. Specifically, we learn domain-invariant features by learning to counter an adversarial network, which attempts to classify the domain of the input data by observing the activations of the segmentation network. Furthermore, we propose a multi-connected domain discriminator for improved adversarial training. Our system is evaluated using two MR databases of subjects with traumatic brain injuries, acquired using different scanners and imaging protocols. Using our unsupervised approach, we obtain segmentation accuracies which are close to the upper bound of supervised domain adaptation.

12. Efficient Multi-Scale 3D CNN with Fully Connected CRF for Accurate Brain Lesion Segmentation

We propose a dual pathway, 11-layers deep, three-dimensional Convolutional Neural Network for the challenging task of brain lesion segmentation. The devised architecture is the result of an in-depth analysis of the limitations of current networks proposed for similar applications. To overcome the computational burden of processing 3D medical scans, we have devised an efficient and effective dense training scheme which joins the processing of adjacent image patches into one pass through the network while automatically adapting to the inherent class imbalance present in the data. Further, we analyze the development of deeper, thus more discriminative 3D CNNs. In order to incorporate both local and larger contextual information, we employ a dual pathway architecture that processes the input images at multiple scales simultaneously. For post-processing of the network's soft segmentation, we use a 3D fully connected Conditional Random Field which effectively removes false positives. Our pipeline is extensively evaluated on three challenging tasks of lesion segmentation in multi-channel MRI patient data with traumatic brain injuries, brain tumors, and ischemic stroke. We improve on the state-of-the-art for all three applications, with top ranking performance on the public benchmarks BRATS 2015 and ISLES 2015. Our method is computationally efficient, which allows its adoption in a variety of research and clinical settings. - Abstract
Glucocorticoids modulate glucose homeostasis, acting on metabolically active tissues such as liver, skeletal muscle, and adipose tissue. Intracellular regulation of glucocorticoid action in adipose tissue impacts metabolic responses to obesity. ATP-binding cassette family C member 1 (ABCC1) is a transmembrane glucocorticoid transporter known to limit the accumulation of exogenously administered corticosterone in adipose tissue. However, the role of ABCC1 in the regulation of endogenous glucocorticoid action and its impact on fuel metabolism has not been studied. Here, we investigate the impact of Abcc1 deficiency on glucocorticoid action and high-fat-diet (HFD)-induced obesity. In lean male mice, deficiency of Abcc1 increased endogenous corticosterone levels in skeletal muscle and adipose tissue but did not impact insulin sensitivity. In contrast, Abcc1-deficient male mice on HFD displayed impaired glucose and insulin tolerance, and fasting hyperinsulinaemia, without alterations in tissue corticosterone levels. Proteomics and bulk RNA sequencing revealed that Abcc1 deficiency amplified the transcriptional response to an obesogenic diet in adipose tissue but not in skeletal muscle. Moreover, Abcc1 deficiency impairs key signalling pathways related to glucose metabolism in both skeletal muscle and adipose tissue, in particular those related to OXPHOS machinery and Glut4. Together, our results highlight a role for ABCC1 in regulating glucose homeostasis, demonstrating diet-dependent effects that are not associated with altered tissue glucocorticoid concentrations. 17. Investigating the relationship between glutamine deamidation and collagen breakdown in bone

Bone can survive in the burial environment for millions of years and can provide direct information about individuals, during their lifetime and post mortem. However it is not currently possible to directly date bone material beyond the limits of radiocarbon dating ~50,000 years before present(BP). It was suggested by van Doorn et al., (2012), that deamidation in bone collagen may be related to thermal age. However, a number of bones investigated from the same site resulted in a large range of glutamine deamidation measurements. This thesis therefore explores in detail the potential of glutamine deadmidation as a dating tool for bones older than 50,000 yeas BP. The work presented in this thesis investigates a number of potential causes for the observed variability. It was found that the pre-treatment of bone with HCl not only increased levels of deamidation, but also caused significant levels of swelling and breakdown of the collagen fibril structure. Areas of bone exhibiting localised macroscopic degradation were shown to be correlated with an increase in the levels of glutamine deamidation. This indicates that the measurement of deamidation may be used as a method by which to investigate the preservation state of bone. The analysis of synthetic peptide mixtures, containing glutamine or glutamic acid showed that an ionisation bias occurs when using MALDI, with product peptide E consistently underestimated. However, analysis of the same peptide mixtures using ESI showed no ionisation bias between the two peptide products and is therefore preferable for the measurement of glutamine containing peptides. Kinetic heating experiments performed at high temperatures (80 ᵒC -140 ᵒC), have proven unsuitable for the measurement of deamidation within the buffer soluble collagen fraction. Deamidation in the buffer soluble collagen fraction was successfully measured at low temperature (65 ᵒC), and resulted in an increase in deamidation over time. However levels of deamidation in the mineral bound fraction extracted from the same bone chips heated at 65 ᵒC did not appear to undergo deamidation over time; indicating that glutamine deamidation in this fraction is slower and therefore may be suitable for the analysis of preservation of older bone from the Late Pleistocene Period. Levels of deamidation in bones analysed from nine sites ranging in age from ~ 8000 - 30,000 years BP showed a correlation with levels of glutamine deamidation and thermal age. However, using the current methodology, it is not possible to identify the age of the sites using only the measured levels of deamidation. An adapted extraction method has been suggested to further investigate the relationship between deamidation and the thermal age of bone. The work carried out in this thesis has directly contributed to the broader knowledge of how collagen degrades in bone over time, and thus has wide reaching implications within the archaeological scientific community.

Objective: Endocrine systems are disrupted in acute illness, and symptoms reported following coronavirus disease 2019 (COVID‐19) are similar to those found with clinical hormone deficiencies. We hypothesised that people with severe acute COVID‐19 and with post‐COVID symptoms have glucocorticoid and sex hormone deficiencies. Design/Patients: Samples were obtained for analysis from two UK multicentre cohorts during hospitalisation with COVID‐19 (International Severe Acute Respiratory Infection Consortium/World Health Organisation [WHO] Clinical Characterization Protocol for Severe Emerging Infections in the UK study), and at follow‐up 5 months after hospitalisation (Post‐hospitalisation COVID‐19 study). Measurements: Plasma steroids were quantified by liquid chromatography–mass spectrometry. Steroid concentrations were compared against disease severity (WHO ordinal scale) and validated symptom scores. Data are presented as geometric mean (SD). Results: In the acute cohort (n = 239, 66.5% male), plasma cortisol concentration increased with disease severity (cortisol 753.3 [1.6] vs. 429.2 [1.7] nmol/L in fatal vs. least severe, p <.001). In males, testosterone concentrations decreased with severity (testosterone 1.2 [2.2] vs. 6.9 [1.9] nmol/L in fatal vs. least severe, p <.001). In the follow‐up cohort (n = 198, 62.1% male, 68.9% ongoing symptoms, 165 [121–192] days postdischarge), plasma cortisol concentrations (275.6 [1.5] nmol/L) did not differ with in‐hospital severity, perception of recovery, or patient‐reported symptoms. Male testosterone concentrations (12.6 [1.5] nmol/L) were not related to in‐hospital severity, perception of recovery or symptom scores. Conclusions: Circulating glucocorticoids in patients hospitalised with COVID‐19 reflect acute illness, with a marked rise in cortisol and fall in male testosterone. These findings are not observed 5 months from discharge. The lack of association between hormone concentrations and common post‐COVID symptoms suggests steroid insufficiency does not play a causal role in this condition. [ABSTRACT FROM AUTHOR]

27. Systematic review and longitudinal analysis of implementing Artificial Intelligence to predict clinical deterioration in adult hospitals: what is known and what remains uncertain.

- Abstract
Objective To identify factors influencing implementation of machine learning algorithms (MLAs) that predict clinical deterioration in hospitalized adult patients and relate these to a validated implementation framework. Materials and methods A systematic review of studies of implemented or trialed real-time clinical deterioration prediction MLAs was undertaken, which identified: how MLA implementation was measured; impact of MLAs on clinical processes and patient outcomes; and barriers, enablers and uncertainties within the implementation process. Review findings were then mapped to the SALIENT end-to-end implementation framework to identify the implementation stages at which these factors applied. Results Thirty-seven articles relating to 14 groups of MLAs were identified, each trialing or implementing a bespoke algorithm. One hundred and seven distinct implementation evaluation metrics were identified. Four groups reported decreased hospital mortality, 1 significantly. We identified 24 barriers, 40 enablers, and 14 uncertainties and mapped these to the 5 stages of the SALIENT implementation framework. Discussion Algorithm performance across implementation stages decreased between in silico and trial stages. Silent plus pilot trial inclusion was associated with decreased mortality, as was the use of logistic regression algorithms that used less than 39 variables. Mitigation of alert fatigue via alert suppression and threshold configuration was commonly employed across groups. Conclusions : There is evidence that real-world implementation of clinical deterioration prediction MLAs may improve clinical outcomes. Various factors identified as influencing success or failure of implementation can be mapped to different stages of implementation, thereby providing useful and practical guidance for implementers. 32. A Case Study on Enrichment Seminar and Gifted Adolescents

- Abstract
The purpose of this qualitative case study was to explore gifted alumni perceptions of how participating in an Enrichment Seminar course met their socioemotional needs as they related to identity formation, and whether or not their academic achievement was influenced. The researcher used interviews, narrative questions, and surveys to gather data. The research questions focused on the perspectives of gifted students regarding how Enrichment Seminar courses during high school met their socioemotional needs in relation to identity formation, and their perspectives on how the seminar course affected their academic achievement. The population of interest was gifted alumni who had participated in an Enrichment Seminar course at a public high school in the southwestern United States between the years of 2007-2011. The sample consisted of 30 participants who were surveyed using The Gifted Program Evaluation Survey, which included narrative questions. The interview sample consisted of 15 participants who came from the population of 30. The participants for this study were from both genders, were of various ethnicities and were between the ages of 19 and 23 years old. The results reflect that participants perceived the Enrichment Seminar positively influenced their identity formation through self-knowledge provided by the curriculum, and relationship building. The participants also perceived that participation in the Enrichment Seminar course positively influenced their academic achievement through college and career preparation.

- Abstract
Osteoclasts play a central role in cancer-cell-induced osteolysis, but the molecular mechanisms of osteoclast activation during bone metastasis formation are incompletely understood. By performing RNA sequencing on a mouse breast carcinoma cell line with higher bone-metastatic potential, here we identify the enzyme CYP11A1 strongly upregulated in osteotropic tumor cells. Genetic deletion of Cyp11a1in tumor cells leads to a decreased number of bone metastases but does not alter primary tumor growth and lung metastasis formation in mice. The product of CYP11A1 activity, pregnenolone, increases the number and function of mouse and human osteoclasts in vitrobut does not alter osteoclast-specific gene expression. Instead, tumor-derived pregnenolone strongly enhances the fusion of pre-osteoclasts via prolyl 4-hydroxylase subunit beta (P4HB), identified as a potential interaction partner of pregnenolone. Taken together, our results demonstrate that Cyp11a1-expressing tumor cells produce pregnenolone, which is capable of promoting bone metastasis formation and osteoclast development via P4HB.

46. Using the Common Core: Differentiating a Middles Grades Reading Standard

- Abstract
The two authors of this manuscript are professors at a university in Georgia, and work closely with preservice and in-service teachers. In addition to teaching courses on literacy, the authors also offer professional development opportunities to schools around the state on how to properly differentiate instruction. What we have noticed is that many teachers at the middle grade level (4th – 8th grades) struggle with differentiated instruction, even more so, than their elementary counterparts. Part of this struggle is because the teachers simply never learned how to do this properly. Part of this is because they have many more students than their elementary colleagues,and are overwhelmed by the thought of individualizing lessons for 150 students. This paper seeks to propose a model that is seeing success with middle grades teachers in Georgia. There is a chance that this model, which is actually a lesson plan format, will be helpful to other teachers who are looking to differentiate instruction in their classrooms.
