Abstract

The novel coronavirus disease 2019 (COVID-19) pandemic has a detrimental impact on the physical and mental health of many children and their families worldwide. Although most children have mild disease, some develop severe complications, with multisystem inflammatory syndrome being the most well-described. Recent reports suggest that COVID-19 may not fully resolve even months after the end of the acute phase. There is a lack of harmonised longitudinal data on short- and long-term consequences of COVID-19 infection in children. Hence, an observational study is needed to evaluate the consequences of COVID-19 in the paediatric population. Methods and analysis This protocol describes a prospective, observational, longitudinal study in paediatric patients previously diagnosed with COVID-19 in hospital and/or primary care/ community settings. The aim of the study is to include children and adolescents (0 to 17 years old) at different sites internationally. Participants will be enrolled and followed up for up to 3 years. Basic (Tier 1) data will include the following: COVID-19 clinical characteristics during acute phase; child comorbidities; treatments received; child or caregiver-reported outcomes on health and wellbeing and impact of daily living; readmission rate. Additional symptom-specific (Tier 2) data may be collected in a subgroup of patients that are experiencing persistent problems. Ethics and dissemination This study will be conducted in accordance with the principles established by the 18th World Medical Assembly and all subsequent amendments and the guidelines for Good Epidemiology Practice. Each individual country joining this study assures that ethics approval are in place and local regulatory requirements met before commencing the study. The anonymised data may be disseminated via publications in peer-reviewed journals, presentations at workshops and conferences, and the public domain by press releases and social media.

Abstract

Background: The long-term sequelae of coronavirus disease 2019 (Covid-19) in children remain poorly characterised. This study aimed to assess long-term outcomes in children previously hospitalised with Covid-19 and associated risk factors.\ud \ud Methods: This is a prospective cohort study of children (≤18 years old) admitted with confirmed Covid-19. Children admitted to the hospital between April 2, 2020 and August 26, 2020, were included. Telephone interview using the International Severe Acute Respiratory and emerging Infection Consortium (ISARIC) Covid-19 Health and Wellbeing paediatric follow-up survey. Persistent symptoms (>5 months) were further categorised by system(s) involved.\ud \ud Findings: 518 of 853 (61%) of eligible children were available for the follow-up assessment and included in the study. Median age was 10.4 years (IQR, 3–15.2) and 270 (52.1%) were girls; median follow-up since hospital discharge was 256 (223–271) days. At the time of the follow-up interview 126 (24.3%) participants reported persistent symptoms among which fatigue (53, 10.7%), sleep disturbance (36, 6.9%,) and sensory problems (29, 5.6%) were the most common. Multiple symptoms were experienced by 44 (8.4%) participants. Risk factors for persistent symptoms were: older age “6–11 years” (odds ratio 2.74 (95% confidence interval 1.37 to 5.75) and “12–18 years” (2.68, 1.41 to 5.4); and a history of allergic diseases (1.67, 1.04 to 2.67).\ud \ud Interpretation: A quarter of children experienced persistent symptoms months after hospitalization with acute covid-19 infection, with almost one in ten experiencing multi-system involvement. Older age and allergic diseases were associated with higher risk of persistent symptoms at follow-up.

15. Does the Moon Spin?

Abstract

In this article, the authors explore the question, "Does the Moon spin?", and show how the question is investigated. They emphasise the importance of the process by which people work out what they know, by "learning from the inside out." They stress that those involved in science education have to challenge current conceptions and ideas, making new connections and reinforcing them strongly through practical experimentation and open discourse. The challenge for practitioners is twofold. First and foremost it is to create a classroom climate wherein individuals can regularly speak openly about their ideas and express them to adult-mentors and peer-groups alike, without the fear of anxiety or ridicule that often leads to disengagement. The second challenge is to put any erroneous thought observed to work, in order to help create useful knowledge. The efficiency with which this is done is enhanced by sharing with the learner what the new learning will be and where they stand in terms of achieving this.

29. Access to respite care for parents who have children with disabilities in B.C

Abstract

This study uses quantitative and qualitative methods to examine publicly funded respite care for parents who have children with disabilities in British Columbia and the current barriers to accessing this service. A survey was distributed to parents of such children to determine their attitudes towards respite care and the barriers they encounter. This survey data is supplemented by case studies of the respite care systems in Alberta and Manitoba. Analysis of these materials reveals that parents value publicly funded respite care highly and that significant barriers exist to accessing it. The policy alternatives developed through analysis of the data are elimination of the wait list, elimination of the cost to the user, revised eligibility criteria, and improving access to respite care workers. The study assesses the combinations of policies that should be implemented to improve access to respite care effectively and feasibly.

Abstract

Heart failure is the only cardiac syndrome still rising in prevalence. Even though heart failure severely affects quality of life and has a high mortality rate, patients with heart failure have less access to palliative care than patients with other life-threatening conditions such as cancer. To address this gap in care, the Heart Failure Supportive Care Clinic was introduced at St. Paul's Hospital in January 2011. The clinic's objective is to improve the quality of life for patients with advanced heart failure who are not candidates for cardiac transplantation or a ventricular assist device. Palliative care specialists can help patients and family members make well-informed and reasonable decisions about preferences for cardiopulmonary resuscitation and when to change to comfort care. Effective communication and a multidisciplinary, team-based approach are needed to ensure a smooth transition to palliative care. [ABSTRACT FROM AUTHOR]
