Abstract

Received: February 4th, 2021 ; Accepted: April 24th, 2021 ; Published: April 29th, 2021 ; Correspondence: giuseppe.rossi@unifi.it Buildings have a relevant impact on the environment, and building materials cause environmental impacts during all life cycle stages: production, utilization, management and demolition. The global request for more efficient buildings with less environmental impacts has grown during the last years. Among various technologies, thermal insulation has proven to be helpful in reducing emissions by increasing energy conservation. This paper intends to show how the Building Performance Analysis (BPA) supports the decision-making process in many areas where common insulation materials are not available and there is a general reluctance to use local natural materials. A building located in the city of Al Chubaish in Dhi Qar Province in Iraq is examined as a case study. The construction is designed for processing buffalo milk. It was built in the first decade of the century, during the Iraqi conflict, using only the materials available at that time, most of which, concrete bricks, mortar and plaster. Currently, this dairy factory is a very inefficient structure in terms of energy saving. But because its elementary form, it is a perfect example to investigate how a simple exterior wall insulation can improve building performance in extreme environmental conditions. Accordingly, two different models have been created. One is the replica of the real building without any upgrading. The second instead presents a thermal insulation realized with reed bio-based material locally available. Through advanced simulation engines and building performance analysis data integrated into Autodesk Revit, each model has been tested to identify significant improvements in terms of energy savings in this particular stressed background.

Abstract

Applicare appieno i principi del gender main-streaming, in accordo alle indicazioni della Comunità Europea, alla tutela della salute e sicurezza nei luoghi di lavoro significa in pratica rivedere tutti gli aspetti della prevenzione sotto l'ottica di genere, vale a dire tenendo in considerazione le differenze di sesso e di genere tra lavoratori e lavoratrici non solo come determinanti della salute, ma ancor più nel caso specifico come determinanti del rischio. In questo passaggio evolutivo il ruolo del medico competente è particolarmente importante. In questo lavoro vengono proposti alcuni strumenti operativi che possono concretamente implementare l'ottica di genere nei diversi ambiti della prevenzione, nei quali il medico competente ha un ruolo di primo piano, se non esclusivo, quali la valutazione dei rischi, la formazione e la sorveglianza sanitaria.

Abstract

E' stata condotta una revisione della letteratura per evidenziare quali tra le patologie lavoro-correlate esprimano differenze di sesso/genere negli outcome di salute e focalizzare i principali limiti degli studi in tale ambito. La ricerca, effettuata su PubMed mediante stringa specifica, ha identificato 4828 articoli (periodo 1997-2017) di cui 381 eleggibili ai fini della revisione (4-22% a seconda della patologia). Tra questi, il 68% hanno evidenziato differenze di sesso/genere negli outcome di salute che nella maggior parte dei casi sembrano dovute ad una diversa esposizione e/o ad una segregazione lavorativa, piuttosto che a differenze biologiche. Pochi studi tuttavia pongono tale valutazione tra gli obiettivi della ricerca e i risultati non vengono quasi mai discussi e raramente vengono formulate ipotesi sulle eventuali differenze osservate. Appare necessario utilizzare metodologie di ricerca e disegni degli studi in grado di rilevare e spiegare la complessità descritta e utili a definire adeguate strategie preventive.

Abstract

Summary Healthcare workers who use or may be exposed to needles are at risk of needlestick injuries, which can lead to serious infections by bloodborne pathogens. These injuries can be avoided by eliminating the unnecessary use of needles and using safety devices. The present study was aimed at evaluating the impact of a safety-engineered device, with passive fully automatic needlestick protection, on the rate of needlestick injuries among healthcare workers. The setting of the study was a network of five public healthcare institutions situated in a Northern Italian Region. Data on the type of device, the number of employees and the number of catheter devices used per year were collected through regular meetings with healthcare workers over a period of five years. The most notable result of this study was the huge risk reduction associated with safety devices. Indeed, the risk of needlestick injuries due to conventional devices was found to be 25-fold higher than that observed for safety devices. However, it is noteworthy that a considerable part of this excess can be explained by the different background number of devices used. Moreover, descriptive analysis suggested that individuals with a poor/moderate training level had a lower risk than those with good/high training, though the difference was not statistically significant. In conclusion, there is convincing evidence of a causal connection between the introduction of safety devices and the reduction in needlestick injuries. This consideration should prompt the introduction of safety devices into daily clinical practice.

Abstract

A literature review was performed to highlight which work-related diseases express sex/gender differences in health outcomes and focus the main limits of studies in this field. The research, carried out on PubMed by specific search string, identified 4828 articles (1997-2017period) of which 381 are eligible for review (4-22%, depending on the disease). Among them, 68% reported sexlgender differences in health outcomes, which in most cases appear to be due to different exposure and/or work segregation rather than to biological differences. However, few studies place this assessment among the research goals and results are almost never discussed and hypothesis are seldom formulated about any observed differences. It seems necessary to use research methodologies and study design Thai can detect and explain the described complexity and useful in defining appropriate preventive strategies.

International audience; Background: In Great Britain, sheep scrapie cases have been detected through active and passive surveillance programmes which do not conform to the characteristics usually associated with classical scrapie. These cases, designated as ‘atypical scrapie’, demonstrate a weak positive PrPres reaction in Bio-Rad ELISA and in histopathology produce a weak immunohistochemistry signal and demonstrate a different PrPSc distribution pattern compared to classical scrapie. Other features, such as an absence of vacuolation at the obex and a characteristic PrPres Western blotting profile are also apparent. Aim: The aim of this Defra-funded study was to determine the transmissibility of atypical scrapie cases from Britain to transgenic PrP and conventional mice, and to examine the biochemical and histopathological characteristics of the transmitted agent(s). Methods: Atypical scrapie isolates were selected from sheep carrying a range of PrP genotypes and inoculated into tg338, C57BL/6 and VM mice. Inoculated mice were clinically monitored and at termination brain samples were examined by Western blotting, lesion profiling and immunohistochemistry. Results: Eighteen atypical scrapie isolates derived from sheep of different genotypes transmitted successfully to tg338 mice at VLA and INRA. Incubation periods, lesion profiles, PrPSc distribution and Western blot profiles of the examined cases were similar, suggesting the isolation of a single strain. Furthermore, the biochemical and histopathological characteristics produced were indistinguishable from those of Nor 98 in the same transgenic mouse line. Bioassay in wild-type mice is ongoing with no TSE positive mice identified up to 431 days post-inoculation. Conclusion: Our findings suggest that the atypical scrapie cases from GB represent infection resulting from a single strain of TSE agent that is indistinguishable from Nor98.

46. Beneficial effects of biventricular pacing in patients with a 'narrow' QRS.

Abstract

Congestive heart failure (CHF) patients with LBBB and QRS duration > 150 ms are considered the best candidates to biventricular pacing (Biv-P). However, patients with a narrow (120-150 ms) QRS may also benefit from Biv-P since true ventricular dyssynchrony may be underestimated by considering only QRS enlargement. From October 1999 to April 2002, 158 CHF patients (121 men, mean age 65 years, mean LVEF 0.29, mean QRS width 174 ms) underwent successful Biv-P implantation and were then followed for a mean time of 11.2 months. According to basal QRS duration, patients were divided in two groups, with wide QRS (> or = 150 ms, 128 patients, 81%) and with narrow QRS (< 150 ms, 30 patients, 19%). In the wide QRS group, LVEF improved from 29% to 39% (P < 0.0001), 6-minute walk test from 311 to 463 m (P < 0.0001), while NYHA Class III-IV patients decreased from 86% to 8% (P < 0.0001). In the narrow QRS group LVEF improved from 30% to 38% (P < 0.0001), 6-minute walk test from 370 to 506 m (P < 0.0001), and NYHA Class III-IV patients decreased from 60% to 0% (P < 0.0001). The data showed that in wide and narrow QRS patients, Biv-P significantly improved clinical parameters (NYHA class, 6-minute walk test, quality-of-life, and hospitalization rate) and main echocardiographic indicators. Furthermore, narrow QRS patients had a better survival rate, rapidly regained left ventricular function, and only a few patients remained in a higher NYHA class during follow-up.
