Highlights • Mandibular arteriovenous malformation (AVM) are very unusual condition. • They remain frequently undiagnosed until a severe bleeding occurs during an oral surgical procedure. • This case was a real diagnostical challenge: clinical presentation and initial radiographic examination was atypical. • Curently there is no guideline to manage this kind of pathology., Introduction Arteriovenous malformations (AVMs) are very rare. Only 5% of them occurs in the jaws but they can manifest with dramatic bleeding and be life-threatening. Presentation of case We report the case of a 11-year-old healthy girl who presented a massive hemorrhage after extraction of the right mandibular first primary molar. This patient received a blood transfusion and was hospitalized in pediatric intensive care unit. CT angiography highlighted an AVM of the mandible. Treatment consisted in selective embolization. Discussion A review of the literature shows that the majority of AVMs of the jaws are often unknown until severe bleeding occurs during dental surgery. The low specificity of radiological signs on panoramic radiography makes the diagnosis particularly challenging. Their management requires an interdisciplinary approach. Selective embolization has a place of choice in the treatment of these complex pathologies. Conclusion Although AVMs of the jaws are rare, they are frequently revealed through a massive hemorrhage during tooth extraction. Dentists have to suspect them when young patients present some clinical features, as spontaneous gingival bleeding, unexplained dental mobility, or facial asymmetry.

International audience; Introduction. The causes of postoperative dissatisfaction in orthognathic surgery are difficult to grasp. The aims of our study are to analyze the effects of orthognathic surgery on self-esteem, body image, psychological morbidity, and quality of life. We also want to assess the combined effects of these factors on postoperative dissatisfaction, and to study the interest of personality assessment (especially neuroticism) as a predictive factor of dissatisfaction. Method. Three hundred patients candidates for maxillo-mandibular osteotomy will be included in the study. They will answer a questionnaire assessing self-esteem, body image, psychological morbidity, quality of life, and personality. The evaluation will be conducted preoperatively and postoperatively at 3 months and at 1 year. The degree of satisfaction will be measured postoperatively. Expected results. The results should help evaluate the psychological effects of orthognathic surgery and identify predictors of postoperative dissatisfaction, and especially the role of neuroticism.; Introduction. Les causes d’insatisfaction postopératoire en chirurgie orthognathique sont difficiles à appréhender. Nous envisageons de réaliser une étude visant à analyser les effets de la chirurgie orthognathique sur l’estime de soi, l’image du corps, la morbidité psychique et la qualité de vie. Nous voulons aussi évaluer les effets combinés de ces facteurs sur l’insatisfaction postopératoire et étudier l’intérêt de l’évaluation de la personnalité (notamment du névrosisme) comme un facteur prédictif de cette insatisfaction. Nous présentons la méthodologie de cette étude.Méthode. Trois cents patients candidats à une ostéotomie maxillo-mandibulaire seront inclus dans l’étude. Ils répondront à un questionnaire évaluant l’estime de soi, l’image du corps, la morbidité psychique, la qualité de vie et la personnalité. L’évaluation sera réalisée en préopératoire, et à trois mois et à un an postopératoires. Le degré de satisfaction sera mesuré en postopératoire. Résultats attendus. Les résultats devraient permettre d’évaluer les effets psychologiques de la chirurgie orthognathique et de cerner les facteurs prédictifs d’insatisfaction postopératoire, et plus particulièrement le rôle du névrosisme.

The clinical experience confirms this study when it comes to distribute tissues but also when a tissular growthmust be obltained. This procedure, though of high utility in our series for the limbs or body, remains contestable for the face where classical or microsurgical procedures shall remain the rule., L'extension tissulaire, basée sur l'aptitude qu'a la peau de se laisser distendre lorsqu'on lui applique une force de traction, est une propriété essentielle à la vie. Si l'extension pluri-axiale est bien connue en chirurgie, sa forme uni-axiale, après de nombreuses controverses notamment sous forme de pré-suture, semble intéressante lorsqu'on l'applique sous forme progressive. Après étude de la tolérance de la peau à subir une traction d'intensité croissante, force est de constater qu'une contrainte d'intensité modérée permet, par répétition des phénomènes de contrainte et de relaxation, d'obtenir un gain cutané non négligeable. L'expérience clinique confirme cette étude lorsqu'il s'agit de redistribuer les tissus mais également lorsqu'une croissance tissulaire doit être obtenue. Ce procédé, bien que fort utile dans notre série au niveau des membres et du tronc, reste discutable au niveau du visage où les techniques classiques et microchirurgicales doivent rester la règle.
