Background: The clinical effectiveness of coronavirus disease 2019 (COVID-19) vaccination in kidney transplant (KT) recipients is lower than in the general population., Methods: From April to October 2021, 481 KT recipients with COVID-19, included in the Spanish Society of Nephrology COVID-19 Registry, were analyzed. Data regarding vaccination status and vaccine type were collected, and outcomes of unvaccinated or partially vaccinated patients (n = 130) were compared with fully vaccinated patients (n = 351)., Results: Clinical picture was similar and survival analysis showed no differences between groups: 21.7% of fully vaccinated patients and 20.8% of unvaccinated or partially vaccinated died (P = 0.776). In multivariable analysis, age and pneumonia were independent risk factors for death, whereas vaccination status was not related to mortality. These results remained similar when we excluded patients with partial vaccination, as well as when we analyzed exclusively hospitalized patients. Patients vaccinated with mRNA-1273 (n = 213) showed a significantly lower mortality than those who received the BNT162b2 vaccine (n = 121) (hazard ratio: 0.52; 95% confidence interval, 0.31-0.85; P = 0.010)., Conclusions: COVID-19 severity in KT patients has remained high and has not improved despite receiving 2 doses of the mRNA vaccine. The mRNA-1273 vaccine shows higher clinical effectiveness than BNT162b2 in KT recipients with breakthrough infections. Background: Although the epidemiology of subjects with end-stage renal disease is well-known in Spain, the prevalence of mild to moderate chronic kidney disease (CKD) in the general population is unknown. In order to measure this, it is necessary to carry out studies in the general population including those who are not health service patients. During epidemiology studies, the method of measuring glomerular filtration rate can change significantly the measurements of the prevalence of mild to moderate CKD., Methods: Between 1997 and 2000, we performed a multi-phase descriptive polistratified epidemiological transversal study. The section of public chosen was between the ages of 15 and 85 living in the health area of western Valladolid (Spain). We calculated creatinine clearance using four methods: serum creatinine concentration, creatinine clearance using 24-hour urine samples adjusting the volume to the expected creatinuria with the Walser formulas, using the Cockcroft-Gault (CG) equation and applying the Modification of Diet in Renal Disease (MDRD) study abbreviated formula. We classified the level of kidney function, according to the National Kidney Foundation-Dialysis Outcomes Initiative (NFK-DOQI) guidelines., Results: The instances of stages 2 and 3 CKD rise with age and are more common in women than men. This tendency is apparent in middle age and persons of 65 and above. Using the CG method, almost half the old women had a stage 3 CKD as opposed to a third of the men. If the measurement is performed using the abbreviated MDRD study, there are very few differences between the sexes. The prevalence of stage 3 CKD is similar (around 8%) but the prevalence of stage 2 CKD rises to 60% as opposed to 36% calculated using the CG equation. After comparing the results obtained with those of the third National Health and Nutrition Examination Survey Study (see table VII), the measurements of stage 3 CKD using the CG formula or by means of expected creatininuria coincide relatively, although the prevalence of stage 2 CKD is higher in persons of 65 and over. If we use the abreviated MDRD study, the prevalences increase by more than 20%., Conclusions: The prevalence of stages 2 and 3 CKD is clearly influenced by the method of calculation used. The prevalence of stage 2 CKD affects at least a third of the general population while those affected by stage 3 CKD are between 3.3% and 8.5%.

Air-stable and readily available ruthenium benzylidene complexes of the general type [RuCl2(=CHPh)(L)(L')] (L, L' = PCy3 and/or N-heterocyclic carbene) constitute a new class of catalyst precursors for atom-transfer radical polymerization (ATRP) of methyl methacrylate and styrene, and provide an unprecedented example for the involvement of ruthenium alkylidenes in radical reactions. They promote the polymerization of various monomers with good to excellent yields, and in a controlled way with methyl methacrylate and styrene. Variations of their basic structural motif provide insights into the essential parameters responsible for catalytic activity. The ligands L (PCy3 and/or N-heterocyclic carbene) turned out to play a particularly important role in determining the rate of the polymerizations. A similarly pronounced influence is exerted by the substituents on the N-heterocyclic carbene. Our results indicate that the catalysts decompose quickly under ATRP conditions, and polymerizations are mediated by both [RuCl2(=CHPh)(L)(L')] complexes and ruthenium species bereft of the benzylidene moiety, through a pathway in which both tricyclohexylphosphane and/or N-heterocyclic carbene ligands remain bound to the metal center. Polymerization of n-butyl acrylate and vinyl acetate is not controlled and most probably takes place through a redox-initiated free-radical process.

Objective: Psoas abscess is currently an uncommon disease and is often difficult to diagnose because of its minor initial symptoms. We compare treatment by percutaneous drainage and surgery., Material and Methods: Five patients, all immunodepressed, which appears to be an important factor in the aetiology and pathogenesis of psoas abscess. Computed tomography appears to be the best diagnostic method., Results: Three subjects underwent open surgery and developed serious complications in contrast with two patients treated by CT-guided percutaneous drainage, preceded by antibiotics., Conclusion: First-line percutaneous drainage appears to be the best approach at the present time, reserving open surgery for very large abscesses.

A new set of ruthenium complexes such as 1 and 2 (the Grubbs ruthenium-carbene complex) has been shown to mediate the controlled atom-transfer radical polymerization (ATRP) of vinyl monomers with exceptional efficiency.
