5. Cognitive effects of brief and intensive neurofeedback treatment in schizophrenia: a single center pilot study.

- Abstract
Background: Schizophrenia is characterized by significant cognitive impairments and affects up to 98% of patients. Neurofeedback (NF) offers a means to modulate neural network function through cognitive processes such as learning and memorization, with documented structural changes in the brain, most notably an increase in grey matter volume in targeted regions. Methods: The present 2-week, open-label, preliminary study aims to evaluate the efficacy on cognition of an adjunctive short and intensive (8 daily sessions lasting 30 minutes) alpha/theta NF training in a sample of subjects affected by schizophrenia on stabilized treatment with atypical antipsychotic drugs. The efficacy was measured at baseline and at the end of the study by the Brief Neuropsychological Examination 2 (ENB 2), the Mini Mental State Examination (MMSE), and the Stroop color-word interference test; the clinical symptoms were assessed using the Positive and Negative Syndrome Scale (PANSS). Results: A final sample of nine patients completed the study. Regarding the cognitive performance, at the final assessment (week 2), the NF treatment significantly improved the performance in the "Story Recall Immediate" (p = 0.024), "Story Recall Delayed" (p = 0.007), "Interference Memory 30 s" (p = 0.024), "Clock Test" (p = 0.014) sub-tests, and the ENB2 Total Score (p = 0.007). Concerning the clinical symptoms, no significant changes were observed in the PANSS subscales and the PANSS Total score. Conclusions: NF could represent an adjunctive treatment strategy in the therapeutic toolbox for schizophrenia cognitive symptoms. [ABSTRACT FROM AUTHOR]

Introduction: Higher percentages of gender dysphoria among autistic people has been recently observed. Previous studies showed that patients with autism spectrum disorders have difficulties in social communication and romantic relationships. Aim: In light of this, this manuscript aimed to study the quality of romantic relationship in autistic people and the possible role played by gender dysphoria. Methods: The search of the literature was conducted in this database: PubMed; we used these search terms: "autism spectrum disorder", "gender dysphoria", and "romantic relationship". We found a total of 74 citations. After excluding duplicates, 74 records were screened. 58 studies were excluded according to inclusion and exclusion criteria. A total of 16 studies has been finally assessed. Results: The findings of this work showed that autistic people have a difficulties in social and romantic relationships. A variety of difficulties in interpersonal and romantic relationships were detected, but to date, it is not clear how gender dysphoria could influence the possibility to initiating and maintaining romantic relationships as well as having a satisfying sexual life. Furthermore, different diagnostic instruments resulted in having been employed, and a general heterogeneity in the population under scrutiny was observed. Conclusions: Impaired social communication and interaction (SCI) and restricted, repetitive behavior (RRB) represent the two core symptom domains of ASD within the DSM-5. It is important to pay attention to these conditions, to manage the coming out about gender dysphoria in autistic people, and to develop strategies to help future social and romantic relationships. Furthermore, as different assessment tools resulted in having been used, in future research, greater attention should be paid to standardizing assessment tools and criteria for the autism spectrum, paying attention to the subjects presenting subthreshold traits as well. In general, future research should better investigate specific existential dimensions and/or difficulties met by people presenting both ASD and Gender Dysphoria.

The aim of this study is to explore the cognitive functions and processes through, also related to deficits, on the Rorschach test. The Rorschach is a projective method can be a valid predictor tool to identify neuropsychological levels, also many cognitive deficits. According to literature, the Rorschach test is most important in clinical practice, to improve the knowledge of patient’s personality, but also neuropsychological aspects, for assessing on perceptual and visuospatial abilities, verbal output skills, memory impairment, emotional control and impulses. In addition, the literature suggested the utility of the projective methods, in the study of brain lesions. In conclusion, the Rorschach task proves to be a valuable instrument including in clinical setting and assessment field of cognitive psychopathology. The method, usually considered in personality disease and mental health setting, improves understanding of the subject’s cognitive level and can be used on psychological diagnosis and assessment in neuropsychological field.

Background. The objective of this study is to establish the link between affective temperament traits and maladaptive personality traits, to verify whether the potential presence of elements related to emotional, affective and dysfunctional relational functioning can affect the couple satisfaction, modifying the well-being or discomfort condition. Materials and Methods. A data collection questionnaire was developed to investigate the factors associated with dysfunctional emotional, affective, and relational modes of functioning. The sample consisted of 473 subjects. Data were collected including the TEMPS-A questionnaire, The Dirty Dozen Italian Assessment and the Relationship Assessment Scale (RAS). Results. The findings of this study showed that the subscales of affective temperament were predictors of dark triad traits. The expressive, irritable and hyperthymic temperamental traits were found to be predictors of trait psychopathy; hyperthymic temperament is also a predictor of narcissistic traits and cyclothymic temperament is a predictor of lower couple satisfaction; men show higher scores than women in Dark triad. Conclusions. This study confirmed that temperamental traits can predict maladaptive personality traits belonging to the dark triad and confirms the importance of evaluating maladaptive personality traits to prevent forms of psychological violence in couple. Background: The interest of diagnosis in clinical psychology is growing, but psychological diagnosis is a complex construct to describe because it is not a static process. It can be subject to change due to continuous evolutions/involutions present in a person’s psychic life. This review aims to describe the concept of psychological-clinical diagnosis. Method: We have carried out, following PRISMA guidelines, a systematic review of literature on Pubmed. Data for this systematic review have been collected in compliance with the reporting elements used for systematic reviews and metanalysis. Search terms were "Psychological diagnosis” AND “Psychological formulation”. Results: Psychological diagnosis can be defined as a process through which clinicians aim to know people's psychic functioning to investigate conscious and unconscious, healthy or pathological psychic dimensions. Psychological diagnosis has great importance in the clinical setting. Conclusion: Psychological diagnosis can be carried out at different levels, according to the context in which it is applied, using a multi-instrumental and multidimensional to investigate the state of an individual's functioning. The literature about this topic is poor and often connected to psychiatric diagnosis, which reduces its particular features. In light of this, it is important to give an overview of the concept of psychological-clinical diagnosis.

Parental divorce is a traumatic experience for children, a stressful life event which may contributes to a future psychological vulnerability. The aim of this study is to explore psychological distress in children after parents’ separation. This research was based on a convenience sample of one-hundred children, divided in two subgroups: (1) 50 coming from a separated family (observational sample) with an age range spanning from 7 to 12 years [Mean age = 9.2 ± 1.5] and (2) 50 belonging to a nuclear family (control sample) with the same range of age [Mean age = 9.5 ± 1.6]. The sample resulted homogeneous for the gender: in particular, the observational group, is composed of 30 females (60%) and 20 males (40%). Showed several differences, highlighting the presence of negative affectivity, a well-known risk factor for the development of a depressive disorder, by reflecting the possible link between home environment and self-perception. In conclusion, this instrument, like any other projective proof, constitutes a privileged access to the psychological world in which the drawing remains the best communication channel and provides us precious informations about the emotional state of the children.

The aim of the present study was to explore the effectiveness of bibliotherapy in rehabilitation settings. The term bibliotherapy, coined in the 1930's by the psychiatrist William Menninger, conveys the importance of reading as a tool for cultural promotion and growth, as a means through which to acquire knowledge, enhance one's cognitive and emotional abilities, develop strategies for managing discomfort, as well as a psychoeducational and cognitive technique in the psychotherapeutic field. A study carried out on Pubmed showed that recent research on bibliotherapy, in particular experimental research, would be lacking and requires a humanistic medical approach, drawing on the knowledge and methods of psychology and cognitive literary studies, as well as clinical disciplines to broaden our understanding of how the dynamic processes of interpretation mediate between the textual and characteristic structures of mental health and illness. This narrative review was conducted on PubMed database and used two key terms "Bibliotherapy" AND "rehabilitation" AND "prison". Studies in line with this specific issue were included. The search of PubMed database provided a total of 470 items recruited online; of these 464 studies were excluded according to inclusion and exclusion criteria. After the screening, a total of 6 studies assessing the bibliotherapy in rehabilitation setting, met the inclusion criteria and were included in the narrative review. Bibliotherapy as a self-help approach also in psychotherapeutic treatments can activate the desire to seek changes in attitudes and behaviour, to acquire knowledge of symptoms, and the value of self-care. A psychological intervention self-administered through ACT bibliotherapy with minimal therapeutic support can improve the physical and emotional functioning of adults with chronic pain. If studies report the efficacy of parent-mediated bibliotherapy in situations of childhood anxiety, nocturnal fears, specific phobias, in adolescence chatbot-delivered self-help depression intervention was proven to be superior to the minimal level of bibliotherapy in terms of reduction on depression and anxiety. Bibliotherapy interventions may be helpful to fight against weight gain and potentially facilitate smoking cessation. [ABSTRACT FROM AUTHOR]

Background: The perception of time passing depends on various variables such as the age, culture, relationships, and emotions of each human being. During Covid-19 outbreak, the perception of time was altered by a variety of variables: objective such as, Lockdown, vaccine, fear of infections, etc. and subjective, for example, temperament and personality traits. As showed by previous studies, patients with depressive disorder have a compromised evaluation of time. In light of this, the objectives of this study were to explore the possible correlation between the perception of time, the affective temperament and the fear of covid-19 infection in patients with depressive disorders. Method: We studied 70 patients, through Preliminary personal data sheet dedicated to the collection of socio-demographic and clinical data and information about any previous Covid-19 infection and any vaccination. The psychological instruments regard Zimbardo Time Perspective Inventory (ZTPI); Irritability, Depression, Anxiety Scale (IDAS); Temperament Evaluation of Memphis (TEMPS-A); Fear of Covid-19 Scale (FCV-19S). Results: The results of this study showed that anxious temperament correlates negatively with the hyperthymic temperament, and anxious temperament correlates negatively in women with the irritable temperament. There are not significant correlations in relation to the fear of Covid-19 infection. - Abstract
Psychological pain or psychache is intolerable and unacceptable mental pain. This construct plays a key role in the risk of suicide in adolescence. This is a frequent condition in adolescents with depression; a variety of studies propose a bidirectional relationship between these two circumstances. Adolescents with psychological pain are exposed to the risk of suicidal ideation, and behavior. This review aimed to explore psychological pain in adolescents, and the correlation to the risk of suicide. We have conducted, following PRISMA guidelines, a systematic review for the literature on PubMed. Search terms were "Suicide risk in adolescents" AND "Mental Pain in adolescence" OR "Psychological Pain in adolescence". An initial search identified publications 1189. A total of 15 studies met the predefined inclusion criteria and were analyzed. Mental pain is a stronger factor of vulnerability of suicidal ideation and behavior. Intense negative emotions, such as guilt, shame, hopelessness, may become a psychological pain in adolescence, and many studies showed a relation between psychological pain and capability for suicide. Background. Cyberchondria is a term used to refer to excessive surfing the web looking for health care information, excessive checking behavior being related to health-related anxiety. This period of quarantine for the Covid-19 pandemic is increasing the pathological use of the internet, and the excessive surfing the web looking for health care information. Another dimension related to the Covid-19 outbreak refers to uncertainty intolerance, for this reason being necessary for the healthcare professionals to provide clear and linear information. Aim. The aim of this review is to identify the psychological correlations connected to cyberchondria in the quarantine period. Methods. Following the PRISMA guidelines, we carried out a systematic review of the literature on PubMed. The terms used for the search were "Cyberchondria" OR "Anxiety" AND "Quarantine". Results. As resulting from the reviewed literature, there is a relationship between anxiety for one's own state of health and cyberchondria, with negative psychological effects of quarantine, including post-traumatic stress symptoms, depression, anxiety, low mood, irritability, insomnia, uncertainty, emotional exhaustion, this condition being associated with hypervigilance, and catastrophic misinterpretation of bodily signs. Conclusion. In the light of this and according to the literature, it would be desirable that research can further explore the factors influencing the increase in cyberchondria in the future. Parental divorce is a traumatic experience for children, a stressful life event which may contributes to a future psychological vulnerability. The aim of this study is to explore psychological distress in children after parents’ separation. This research was based on a convenience sample of one-hundred children, divided in two subgroups: (1) 50 coming from a separated family (observational sample) with an age range spanning from 7 to 12 years [Mean age = 9.2 ± 1.5] and (2) 50 belonging to a nuclear family (control sample) with the same range of age [Mean age = 9.5 ± 1.6]. The sample resulted homogeneous for the gender: in particular, the observational group, is composed of 30 females (60%) and 20 males (40%). Showed several differences, highlighting the presence of negative affectivity, a well-known risk factor for the development of a depressive disorder, by reflecting the possible link between home environment and self-perception. In conclusion, this instrument, like any other projective proof, constitutes a privileged access to the psychological world in which the drawing remains the best communication channel and provides us precious informations about the emotional state of the children.

Objective: The domain of the clinic assessment in psychology, shares the same theoretical and methodological assumptions with the traditional psychometric methods. In this case, the authors will focus on a clinimetric approach, different from the traditional psychometric ones, usually used in assessment practices. Methods: the authors analysed book by Per Bech “Klinisk Psycometri” and the Italian version titled “l’Assessment in Clinica psicologica: la psicodiagnostica clinimetrica”, edited by Giovanni Fioriti in 2018. Results: The book gives an innovative approach in understanding the clinical use of psychological rating scales, in line with a dimensional approach. Conclusion: The book offers a view on the importance of an assessment in the psychological clinic, which takes into account the nosography conditions of the pathological clinical conditions and the related quality of life and health. Background:Many experts agree that the Covid-19 pandemic and lockdown restriction have had an impact on relationships and eating habits in couples, but this hypothesis lacks evidence and empirical studies. Aim:The aim of the study was to investigate the relationship between satisfaction with the couple's relationship, body self-esteem, and eating behaviors during the Covid-19 lockdown. Method:Three hundred and eighty-one subjects, aged between 18 and 60 years (M = 26.88; SD= 9.22), of whom 89.8% women, participated in the survey. The online assessment included: the Relationship Assessment Scale; the Multidimensional Self Concept Scale and the Eating Disorder Examination Questionnaire. Results:The results showed that the relationship and the levels of couple's satisfaction are independent of respect to body experience and eating behavior. On the contrary, bodily experience is negatively correlated with diet, weight, body shape, and restriction attempts. However, the eating style of couple showed a change during the quarantine period, both in healthy and at-risk of ED subjects. Conclusion:The psychological impact of Covid-19 and its lockdown has had profound repercussions on the subjective relationship with body and food but have apparently preserved stable and satisfying bonds. The importance of the study confirmed the primary relation between self and body's satisfaction in subjective dimensions of life.

- Abstract
Psychological pain or psychache is intolerable and unacceptable mental pain. This construct plays a key role in the risk of suicide in adolescence. This is a frequent condition in adolescents with depression; a variety of studies propose a bidirectional relationship between these two circumstances. Adolescents with psychological pain are exposed to the risk of suicidal ideation, and behavior. This review aimed to explore psychological pain in adolescents, and the correlation to the risk of suicide. We have conducted, following PRISMA guidelines, a systematic review for the literature on PubMed. Search terms were “Suicide risk in adolescents” AND “Mental Pain in adolescence” OR “Psychological Pain in adolescence”. An initial search identified publications 1189. A total of 15 studies met the predefined inclusion criteria and were analyzed. Mental pain is a stronger factor of vulnerability of suicidal ideation and behavior. Intense negative emotions, such as guilt, shame, hopelessness, may become a psychological pain in adolescence, and many studies showed a relation between psychological pain and capability for suicide.

Summary: Clinical psychology based on gender medicine is a core topic of this Special Issue. In general, consideration of women's mental health is limited; however, it is important to consider subjective wellbeing factors connected with cultural, environmental, epigenetic and personality aspects. Many factors, such as the roles assigned to women nowadays in social and work contexts, can act as predisposing conditions in the etiology of the psychopathological frame, particularly affective disorders. Furthermore, in the developmental life of a woman, important risk factors can be highlighted, such as the vulnerability to psychological distress in women and couples. In particular, the topic addresses the individual maternal requirements for successful transition to healthy motherhood and innovative programs based on gender medicine in the life cycle considering student and elderly experiences. The connection of psychological vulnerability to the environment and repercussions for relationships have been studied in connection with the COVID-19 lockdown, induced changes in women's psychological distress and research regarding sexual arousal, self-image and mental wellbeing. Psychological and emotional forms of violence in couples, such as IPV, is another point highlighting new trend of assessments (i.e., Intimate Partner Violence EAPA-P) and ad hoc treatment in emotional regulation and resilience. Psychological support for women is central to the prevention of psychopathology, especially in relation to subthreshold traits; finally, the topic offers an overview of ad hoc treatments in clinical contexts.

40. Lifetime Psychiatric Comorbidity and Diagnostic Trajectories in an Italian Psychiatric Sample.

- Abstract
Objective: Comorbidity in psychiatric patients has been widely examined in the literature, enucleating the role in misinterpretation of symptom's root in a multi-disease background, as well as the impact on the quality of life, outcome, and health-care effects. This research aimed to examine, in an Italian population of psychiatric patients, the diagnostic continuum in the context of lifetime psychiatric comorbidity, assessing possible differences related to the onset disorder., Method: A retrospective analysis of medical records of 458 subjects, in which various psychiatric diagnoses were represented and categorized in 16 nosographic classes, was conducted., Results: Results showed that "Bipolar disorder" (22.06%) was the most frequent diagnosis, "Eating disorder" had the earliest age onset (Mean age years = 16 ± 1.41), and "Schizophrenia" showed the longest disease duration (Mean years = 24.20±12.76). Moreover, 54,4% of the final sample presented at least one psychiatric comorbidity in disease history, while "Other personality disorders" was the most comorbidity-associated diagnosis, representing 29% of all the cases with more than 3 past diagnoses. Heterotypic transition was observed in fairly all considered onset diagnoses, exception made for "Schizophrenia" with 75% of the subjects showing homotypic progression., Conclusions: Our results suggest a tendency to make multiple diagnoses over psychiatric patients' lifetime in the majority of cases, often escaping from the original onset nosographic domain. More generally, our findings agree with a broad consensus that describes psychiatric symptomatic dimensions rather overlapped and correlated with each other, leading to a more transdiagnostic clinical approach., Competing Interests: Competing interests: None., (© 2020 Giovanni Fioriti Editore s.r.l.)
