Background: The importance of assessing and monitoring the health status of a population has grown in the last decades. Consistent and high quality data on the morbidity and mortality impact of a disease represent the key element for this assessment. Being increasingly used in global and national burden of diseases (BoD) studies, the Disability-Adjusted Life Year (DALY) is an indicator that combines healthy life years lost due to living with disease (Years Lived with Disability; YLD) and due to dying prematurely (Years of Life Lost; YLL). As a step towards a comprehensive national burden of disease study, this study aims to estimate the non-fatal burden of cancer in Belgium using national data. Methods: We estimated the Belgian cancer burden from 2004 to 2019 in terms of YLD, using national population based cancer registry data and international disease models. We developed a microsimulation model to translate incidence-into prevalence-based estimates, and used expert elicitation to integrate the long-term impact of increased disability due to surgical treatment. Results: The age-standardized non-fatal burden of cancer increased from 2004 to 2019 by 6 and 3% respectively for incidence- and prevalence-based YLDs. In 2019, in Belgium, breast cancer had the highest morbidity impact among women, followed by colorectal and non-melanoma skin cancer. Among men, prostate cancer had the highest morbidity impact, followed by colorectal and non-melanoma skin cancer. Between 2004 and 2019, non-melanoma skin cancer significantly increased for both sexes in terms of age-standardized incidence-based YLD per 100,000, from 49 to 111 for men and from 15 to 44 for women. Important decreases were seen for colorectal cancer for both sexes in terms of age-standardized incidence-based YLD per 100,000, from 105 to 84 for men and from 66 to 58 for women. Conclusions: Breast and prostate cancers represent the greatest proportion of cancer morbidity, while for both sexes the morbidity burden

Advancements in medicine and biostatistics have already resulted in a better access to insurance for people diagnosed with cancer. This materializes into the “right to be forgotten” adopted in several EU member states, granting access to insurance after a waiting period of at most 10 years starting at the end of the successful therapeutic protocol. This paper concentrates on insurance covers on a market where such a right has been implemented. Stand-alone products are considered, as well as guarantees included as a rider in an existing package. The cost of offering standard premium rates to all applicants in mortgage insurance related to property loans is also evaluated. The 3-state (healthy–ill–dead) Semi-Markov hierarchical model developed in Denuit et al. [4] for long-term care insurance is adopted here for actuarial calculations. Semi-Markov transition intensities are estimated from cancer cases recorded by the Belgian Cancer Registry. The obtained results suggest that a new offer could develop, targeting the particular needs of cancer patients.

29 p., ill., Le Centre fédéral d’Expertise des Soins de santé (KCE) publie un ensemble de 15 indicateurs de qualité pour la prise en charge du cancer de l’ovaire. Cette démarche s’inscrit dans un processus continu d’amélioration de la qualité des soins en oncologie en Belgique et a déjà été appliquée avec fruit pour d’autres types de cancers. Les résultats des 15 indicateurs ont été calculés au niveau national, et tous les hôpitaux recevront ensuite un rapport d'évaluation individuel envoyé par la Fondation Registre du Cancer. Cela leur permettra d'identifier leurs points forts et leurs points faibles et d'améliorer leurs pratiques si nécessaire. Dans une seconde partie, l’étude a démontré que les chances de survie sont nettement plus élevées dans les hôpitaux qui traitent un grand nombre de patientes chaque année. Le KCE plaide donc pour une concentration de la prise en charge du cancer de l’ovaire dans un nombre limité de centres de référence.
