Alternatively, a modified landmark approach might be used to mitigate the amount of immortal time bias, which assumes exposure initiation at a predefined landmark time. In the context of an expected positive association between adjuvant chemotherapy (ACT) and overall survival among resected pancreatic cancer (PCa) patients, this study aims to empirically assess the performance of this approach relative to the Mantel–Byar method.Patients and methods: Data from resected PCa patients diagnosed between 2003 and 2014 and registered in the national cancer registries of Belgium, the Netherlands, and Slovenia were used to estimate the association between ACT and overall survival using a Cox proportional hazards model by country and overall. Results derived from the immortal time bias (misclassifying the time to ACT initiation), Mantel–Byar method, and conventional and modified landmark analyses with assumed cutoff times of ACT initiation at 9, 12 and 15 weeks post-diagnosis were compared.Results: In total, 5,668 patients with a total of 10,921 person-years of follow-up were eligible. All analytical approaches showed a significant survival benefit for resected PCa patients who received ACT, but immortal time bias analyses led to strong overestimation of ACT benefits compared to the Mantel–Byar method (immortal time bias: overall HR [95% CI] 0.68 [0.62–0.75] vs Mantel–Byar method: 0.82 [0.71–0.93]), whereas the conventional landmark approach generally provided rather conservative estimates (0.86 [0.75–1.00], 15 weeks landmark). HRs derived from modified landmark analyses depended on the cutoff time, but were similar compared to the Mantel–Byar method at 15 weeks (0.81 [0.70–0.94]).Conclusion: A modified landmark approach might be a valid alternative to the Mantel–Byar method if no time of treatment information is available. Abstract

Foreign and native populations differ in terms of breast cancer outcomes. Studies rarely distinguish between premenopausal and postmenopausal breast cancer, although the risk profile is different; nor between migrants of the first and second generation (FG and SG), which is crucial to examine genetic and environmental influences on breast cancer. This research fills these gaps by investigating patterns in breast cancer incidence and survival in different migrant groups by menopausal and migrant generational status, taking various risk factors into account. To this end, individually linked data from the 2001 census, the Belgian Cancer Registry and the Crossroads Bank for Social Security are used. Age‐standardised incidence rates and incidence rate ratios are calculated by migrant background group, stratified according to ages 30–50 (premenopausal) and 50–70 (postmenopausal). Incidence rate ratios are examined with and without taking reproductive factors and socioeconomic position (SEP) into account. Relative survival percentages and relative excess risks of dying among premenopausal and postmenopausal patients are computed with and without controlling for the stage at diagnosis and SEP. Premenopausal breast cancer is further examined by migrant generational status. Breast cancer incidence is lower among non‐European migrants compared to Belgians. Keeping SEP and known risk factors constant reduces much, but not all of the observed discrepancies. A risk convergence between SG migrants and Belgians for the development of premenopausal breast cancer is observed. Premenopausal breast cancer survival is worse among Moroccan patients due to a higher stage at diagnosis. This disadvantage is concentrated in the FG.

Abstract

Aim: A fair to moderate concordance in grading of the total mesorectal excision (TME) surgical specimen by local pathologists and a central review panel has been observed in the PROCARE (Project on Cancer of the Rectum) project. The aim of the present study was to evaluate the difference, if any, in the accuracy of predicting the oncological outcome through TME grading by local pathologists or by the review panel. Method: The quality of the TME specimen was reviewed for 482 surgical specimens registered on a prospective database between 2006 and 2011. Patients with a Stage IV tumour, with unknown incidence date or without follow-up information were excluded, resulting in a study population of 383 patients. Quality assessment of the specimen was based on three grades including mesorectal resection (MRR), intramesorectal resection (IMR) and muscularis propria resection (MPR). Using univariable Cox regression models, local and review panel histopathological gradings of the quality of TME were assessed as predictors of local recurrence, distant metastasis and disease-free and overall survival. Differences in the predictions between local and review grading were determined. Results: Resection planes were concordant in 215 (56.1%) specimens. Downgrading from MRR to MPR was noted in 23 (6.0%). There were no significant differences in the prediction error between the two models; local and central review TME grading predicted the outcome equally well. Conclusion: Any difference in grading of the TME specimen between local histopathologists and the review panel had no significant impact on the prediction of oncological outcome for this patient cohort. Grading of the quality of TME as reported by local histopathologists can therefore be used for outcome analysis. Quality control of TME grading is not warranted provided the histopathologist is adequately trained.

Abstract

Lake sediments contain valuable information about past volcanic and seismic events that have affected the lake catchment, and they provide unique records of the recurrence interval and magnitude of such events. This study uses a multilake and multiproxy analytical approach to obtain reliable and high-resolution records of past natural catastrophes from similar to 600-yr-old annually laminated (varved) lake sediment sequences extracted from two lakes, Villarrica and Calafquen, in the volcanically and seismically active Chilean Lake District. Using a combination of micro-X-ray fluorescence (mu XRF) scanning, microfacies analysis, grain-size analysis, color analysis, and magnetic-susceptibility measurements, we detect and characterize four different types of event deposits (lacustrine turbidites, tephra-fall layers, runoff cryptotephras, and lahar deposits) and produce a revised eruption record for Villarrica Volcano, which is unprecedented in its continuity and temporal resolution. Glass geochemistry and mineralogy also reveal deposits of eruptions from the more remote Carran-Los Venados volcanic complex, Quetrupillan Volcano, and the Huanquihue Group in the studied lake sediments. Time-series analysis shows 112 eruptions with a volcanic explosivity index (VEI) >= 2 from Villarrica Volcano in the last similar to 600 yr, of which at least 22 also produced lahars. This significantly expands our knowledge of the eruptive frequency of the volcano in this time window, compared to the previously known eruptive history from historical records. The last VEI >= 2 eruption of Villarrica Volcano occurred in 1991. Based on the last similar to 500 yr, for which we have a complete record from both lakes, we estimate the probability of the occurrence of future eruptions from Villarrica Volcano and statistically demonstrate that the probability of a 22 yr repose period (anno 2013) without VEI >= 2 eruptions is <= 1.7%. Abstract

Seven particles captured by the Stardust Interstellar Dust Collector and returned to Earth for laboratory analysis have features consistent with an origin in the contemporary interstellar dust stream. More than 50 spacecraft debris particles were also identified. The interstellar dust candidates are readily distinguished from debris impacts on the basis of elemental composition and/or impact trajectory. The seven candidate interstellar particles are diverse in elemental composition, crystal structure, and size. The presence of crystalline grains and multiple iron-bearing phases, including sulfide, in some particles indicates that individual interstellar particles diverge from any one representative model of interstellar dust inferred from astronomical observations and theory.

Abstract

Micro-X-ray fluorescence (μ-XRF) is a rapidly evolving analytical technique which allows visualising the trace level metal distributions within a specimen in an essentially nondestructive manner. At second generation synchrotron radiation sources, detection limits at the sub-parts per million level can be obtained with micrometer resolution, while at third generation sources the spatial resolution can be better than 100 nm. Consequently, the analysis of metals within biological systems using micro- and nano-X-ray fluorescence imaging is a quickly developing field of research. Since X-ray fluorescence is a scanning technique, the elemental distribution within the sample should not change during analysis. Biological samples pose challenges in this context due to their high water content. A dehydration procedure is commonly used for sample preparation enabling an analysis of the sample under ambient temperature conditions. Unfortunately, a potential change in elemental redistribution during the sample preparation is difficult to verify experimentally and therefore cannot be excluded completely. Creating a cryogenic sample environment allowing an analysis of the sample under cryogenic condition is an attractive alternative but not available on a routine basis. In this article, we make a comparison between the elemental distributions obtained by micro-SR-XRF within a chemically fixed and a cryogenically frozen Daphnia magna, a model organism to study the environmental impact of metals. In what follows, we explore the potential of a dual detector setup for investigating a full ecotoxicological experiment. Next to conventional 2D analysis, dual detector X-ray fluorescence cryotomography is illustrated and the potential of its coupling with laboratory absorption micro-CT for investigating the tissue-specific elemental distributions within this model organism is highlighted.
