The knife-cut sign is a distinctive manifestation of herpes simplex virus (HSV) type 1 or HSV type 2 infection that has been described in at least 10 immunocompromised patients. It appears as an extremely painful linear erosion or fissure in an intertriginous area such as the body folds beneath the breast, or within the abdomen, or in the inguinal region. Also, concurrent HSV infection at other mucocutaneous sites, or viscera, or both have been observed. The patients had medical conditions (at least 9 patients) and/or immunosuppressive drug therapy (6 patients). The diagnosis of HSV infection was confirmed by viral culture (8 patients), biopsy (4 patients), direct fluorescence antibody testing (3 patients), immunohistochemistry staining (2 patients), polymerase chain reaction (2 patients), or Western blot serologic assay (1 patient). Knife-cut sign-associated HSV infection is potentially fatal; three patients died. However, clinical improvement or complete healing occurred in the patients who received oral valacyclovir (1 patient), or intravenous acyclovir (2 patients), or intravenous acyclovir followed by foscarnet (1 patient). In summary, HSV infection associated with a positive the knife-cut sign is a potentially fatal variant of HSV infection that occurs in the intertriginous areas of immunocompromised patients and usually requires intravenous antiviral therapy.

This article aims to renew the discussion about meaning in the field of psychology. A model is presented that, contrary to the classical view of meaning as an entity taken for granted, explains the dynamics through which it comes to be constituted, opening itself to the possibility of being experienced, as a psychological reality. The autoethnographic analysis carried out by von Fircks (IBPS 53(4):632–643, 2023) is used as example to show how such a model enables an understanding of local phenomena through the comprehension of the semiotic dynamics underlying them. Objective: To develop and validate the Reflux Sign Assessment‐10 (RSA‐10) for documenting the physical findings of laryngopharyngeal reflux disease (LPRD). Methods: Patients with LPRD at the hypopharyngeal‐esophageal multichannel intraluminal impedance‐pH monitoring and asymptomatic individuals were consecutively recruited from two European hospitals. Three experienced otolaryngologists rated RSA‐10 in patients and controls for assessing internal validity. RSA‐10 was rated within a 7‐day period to assess test‐retest reliability. Internal consistency was measured using Cronbach's α in patients and controls. Convergent validity was evaluated through a correlation analysis between RSA‐10 and Reflux Finding Score (RFS). Interrater reliability was evaluated by comparing the RSA‐10 evaluations of the three otolaryngologists through Fleiss kappa. Pre‐ to posttreatment change of RSA‐10 was evaluated to assess responsiveness to change. The RSA‐10 thresholds were examined by receiver operating characteristic analysis. Results: Fifty‐five patients completed the pre‐ to posttreatment evaluations from January 2020 to December 2023. A total of 115 asymptomatic individuals completed the study. RSA‐10 reported high internal consistency reliability (α = 0.822) and test‐retest reliability (rs = 0.725). The RSA‐10 scores of patients were significantly higher than those of controls (p = 0.001), suggesting high internal validity. RSA‐10 was significantly correlated with the RFS (rs = 0.771). The interrater reliability was adequate for sub‐ and total RSA‐10 scores (k = 0.708). RSA‐10 significantly improved from baseline to 3‐month posttreatment (p = 0.001). An RSA‐10 > 13 may be suggestive of LPRD. Both RSA‐10 > 13 and Reflux Symptom Score‐12 > 11 were associated with a sensitivity of 92.7% and a specificity of 97.3%. Conclusion: The RSA‐10 is a reliable and valid clinical instrument for documenting the most prevalent laryngeal and extra‐laryngeal findings associated with LPRD. This study develops a biosemiotic framework for a descriptive phenomenology. We incorporate the set utterance-genre-lifeworld in biosemiotic theory by paralleling it with the Peircean-Uexküllean notions of sign, habit, and Umwelt (respectively). This framework for empirical semiotic studies aims to complement the concepts of affordance and scaffold, as applied in studies on learning. The paper also contributes to bridging Bakhtinian-Hallidayian-Habermasian views on utterance, genre, and lifeworld with biosemiotics. We exploit the possibility that biosemiotics offers to bring together hermeneutic and phenomenological analysis. We relate these views to integrated levels in a systemic framework for communication. Signs are seen as inter-dependent construction elements in utterances. Repeated use of utterances in shifting contexts generates shared recognizable kinds of communication, or genres. 'Life-genre' is used in a zoo-communication context, to avoid anthropocentrism. Life-genre serves animals' life-functions. Genres make up a systemic network of communicational resources, along with the related concepts of event, affordance, and scaffold. Utterance, genre, and lifeworld have five aspects, constituting an integrative approach to communication: form, content, act, time, and space. Semiosis and positioning are processes that connect aspects and levels. Levels, aspects, and processes make up the framework as a system. While biosemiotics supports a phenomenological notion of life-genre, in turn, this notion also contributes to the development of the former, bridging a gap between organisms' sign experience on a micro-level and organisms' phenomenal lifeworld/Umwelt. Comparisons of lifeworld and Umwelt reveal that, although not identical, these are sufficiently similar to be perceived as an overall macro-level for signs and communication. Starting from a construal of utterance and genre as dynamic, dialogical, and reciprocal, genre is positioned as meso-level, mediating between signs in utterances and the lifeworld/Umwelt level. We propose genres, understood as semiotic scaffoldings built through affordances, as an analytical concept to capture meso-level phenomena. Scaffolding is determined by both ongoing events and (pre-)available genres. Events are manifestations of these scaffolding processes. Identifying animal and educational communication as two particularly relevant fields for future applications, we compare key socio-semiotic concepts to the notions of sign, habit, and Umwelt. The notions affordance and scaffolding are particularly important to think of learning as continuous with adaptation. An overall aim is to broaden as well as finetune semiotic phenomenology as a platform for empirical analyses of communication. The visible human body is composed of flesh and bones for the most part, yet an invisible orchestra of sensations and perceptions creates a virtual or phantom body that behaves like a shadow following every movement and gesture of its anatomical complement. This shadow becomes only "visible" to the individual when bodily integrity is affected, anatomically or cognitively. Phantom limbs have been known for a long time. They refer to the felt presence of a missing hand, leg, or other body part as if it was still in place. Reciprocally and of a supposedly cognitive origin, phantom extremities are reported by some patients that feel the virtual presence of a supernumerary limb – signifying anatomical "overcompleteness." However, other patients feel as one of their limbs does not belong to their body – signifying "foreignness". Various shades of the so-called body integrity identity disorder exemplify the assumed complex signification processes within the human body. The Peircean theory of signs and the Uexküllian concept of endosemiosis are combined to approach the still poorly understood phantom phenomena in light of representation and embodiment. There is high burden of long bone fractures in low- and middle-income countries (LMICs). Given a limited availability of fluoroscopy in LMICs, the Surgical Implant Generation Network (SIGN) developed two types of intramedullary nails: the SIGN standard nail and the SIGN Fin Nail. A limited number of studies have analyzed healing outcomes with the SIGN Fin Nail and the current study is the largest one to date. The purpose of this study is to compare outcomes between the SIGN standard nail and SIGN Fin Nails in adult femoral shaft fractures treated with a retrograde approach. A retrospective cohort study of adults with femoral shaft fractures was performed using the Sign Online Surgical Database (SOSD). The primary outcome was achieving full painless weight bearing and the secondary outcomes assessed were radiographic healing and infection. A propensity-score adjustment was performed for potential confounders and effect modification due to fracture location was tested using a Mantel-Haenszel test for heterogeneity. Of 19,928 adults with femoral shaft fractures, 2,912 (14.7%) had the required 6-month follow-up to be included. The overall propensity score weighted relative risk between the Fin and Standard Nail for achieving painless weight-bearing was 0.99, 95% CI [0.96–1.03] and for radiographical healing was 0.99, 95%CI [0.97–1.02]. The propensity score weighted relative risk for infection was 1.30, 95% [0.85–1.97]. Use of the Fin nail was also significantly associated with shorter surgery times (p < 0.005, effect size = 24 min). Sub-group analysis based on fracture location and injury cause demonstrated no change in relative risk. The Fin nail showed no change in relative risk in terms of achieving full painless weightbearing or radiographic healing compared to the standard nail for retrograde nailing of femoral shaft fractures in adults. The heterogeneous nature of the cohort and large sample size allow for generalizability and add to a growing base of literature supporting use of the Fin Nail for retrograde femoral nailing. Chinese hieroglyphic writing has developed ways of recording meanings that differ from those in alphabetic systems, which generates interest in the hieroglyph as an object of research. This study complements the semiotic theory, which is usually applied to alphabetic systems, while looking for a complementary way to describe the ambiguous semiotics of Chinese writing. The study relied on a cross-disciplinary approach, i.e., the theory of semiosis in synthesis with the cultural-historical concept and a tertiary artifact as a link between them. A tertiary artifact corresponds to the semiotic sign-symbol, which is an implicit valuable textual category that cannot be manifested in a specific sign. The theory of artifacts was used in conjunction with a semiotic approach to the analysis of Chinese writing, both at the level of individual signs and at the text level. Diachronically, Chinese hieroglyphic signs tended towards symbolic semantics as they evolved, especially in cases where object nomination was motivated. As a synchronic example, the authors used a compositionally complex text of Wang Wei’s poem Magnolia Valley (辛夷坞), which illustrates the symbol both implicitly and explicitly. The analysis proved that semiotic ideas applied to alphabetic languages can be applied to Chinese writing but with the following caveat: in Chinese text, the implicit semantics of the symbol, being associated with a visually vivid imagery, hints at the symbol hidden behind the lines. Unlike letters, the hieroglyph possesses an additional technical potential for constructing an explicit series of images, thus contributing to a more effective comprehension of the main idea and morals, the emergence of which is regulated by its symbolic mode.

Evidence is steadily mounting that attribute-based models offer a more accurate description of intertemporal choices than traditional alternative-based models. Among the attribute-based models, the tradeoff model offers the broadest coverage of research findings, but at the cost of considerable complexity: There now are various instantiations of the model dealing with partially overlapping universes of choice options and preference patterns. Moreover, there are reports of preference patterns in intertemporal decisions about monetary losses that contradict all attribute-based models proposed so far. Taking stock of these core challenges, and all other evidence, we develop an account of intertemporal choice, the unified tradeoff model, that is simpler, yet more comprehensive, than all currently available versions of the tradeoff model taken together. It borrows extensively from its predecessors, but it introduces a new element, time bias, that enables it to accommodate an extraordinarily broad range of preference patterns, and also generate new predictions that contradict all existing models of intertemporal choice. We report four studies that test and confirm its predictions regarding delay, interval, sign, and magnitude dependence in choices between single-dated outcomes, and a fifth study that tests and confirms its predictions regarding the relation between delay preference in choices that only involve single-dated payments and duration preference in choices that also involve sequences of payments. Having subjected the unified tradeoff model to an elevated risk of disconfirmation, we discuss its parsimony and scope in relation to yet other phenomena, most notably, preference patterns in consumption decisions, the final frontier for attribute-based models. The present reflection is based on the idea that all matter is subject to movement and therefore is also subject to change. Thus, the question arises if is possible to consider the text as matter, it can behave in the same way, that is, show itself conditioned by the processes of alteration, change and subordinate to human experience. According to the above, it would be necessary to contemplate the possibility of a relative stability in the textual structures and the knowledge they support and review concepts that point to certain textual statutes that are unalterable in the face of the need of culture to preserve regulations, values and consensus, for example: textual longevity. This would explain the phenomenon of change in languages, narratives and later cultural transformations and that under a structural linguistic paradigm would be difficult to clarify. In modern conditions, one of the key concepts of the communication space is media, which actualises the dependence of the semiotic organisation of the text on the format properties of the communication channel. Accordingly, the tools for creating a media text, the method and medium of its dissemination in the communication space are undergoing transformation. The 'mosaicism' of the media environment is manifested in the change of the fundamental ideological guidelines and principles of creating a modern media message: the focus is shifting from the word to visualisation, sign, symbol, which are combined in a clip, fragmented presentation of information. The rapid development of interactive media and the creolisation of mass media discourse not only contribute to the creation of new cultural artefacts, but also new forms of precedent that require scientific understanding. The purpose of the study is to carry out a philosophical and cultural analysis of the phenomenon of precedent in the non-verbal practices of the contemporary media space. Research methodology: method of theoretical analysis, which allowed to present a scientific interpretation of precedent in the context of non-verbal practices of our time; descriptive method, which was used to describe the structural, semantic and functional features of the use of non-verbal precedent phenomena in the modern communication space; method of ontological analysis, which was used to study the essence and nature of precedent in the non-verbal dimension of the media space; hermeneutic method, which allowed to interpret and understand the f The generalisations presented in the article have led to the conclusion that the clip and 'mosaic' nature of the media environment actualises such non-verbal means of communication as visualisation, sign, symbol, which have reference, imperative, and set a model for the communicative act. It is proved that a precedent phenomenon of this order is distinguished from these phenomena by its special emotional and cognitive significance. In other words, attempts to differentiate this concept are based on the understanding and recognition of different degrees of 'rootedness' of a precedent text in the mind, and, consequently, different degrees of its culture-creating potential. The practical significance of the study is to substantiate the connection between the phenomenon of precedent in the non-verbal practices of modern mass media and the cognitive experience of the public consciousness. Deleuze (1925–1995), in the early 1980s, adopts Peirce's (1839–1914) semiotics in order to classify the signs that the images of the cinema display. Aiming at insufflating the Peircean principles with the movement that animates the images of cinema, he provides Peirce's triadic logic with a new category – Zeroness – which stands for the semiotic movement of cinematic images. Deleuze's new category has impacts on the main domains of Peirce's philosophy. Accordingly, our inquiry will focus on the irradiation of Zeroness over the (a) system of categories, the (b) Peircean phenomenology (phaneroscopy), the (c) generative categorial role, the (d) semiotic effectiveness, the (e) doctrine of continuity, and the (f) logic of relatives. This article will show that for Deleuze nearly like more recent Peircean scholarship: (a) zeroth state holds a categorial status, (b) some phenomenon instantiates zeroth state, (c) zeroth state plays a generative role regarding Peirce's categories, (d) zeroth state as a phenomenological category is semiotically effective, (e) zeroth state and the doctrine of continuity, and (f) zeroth state is a "medad." In order to assess these topics, we recover Deleuze's advances on Peirce's philosophy and confront them with the Peircean studies on the zeroth state. Finally, we will see that Deleuze, far from being a Peircean scholar, developed the importance of the zeroth state for Peirce's semiotics in advance to the subsequent Peircean scholarship. This article aims to address the theories of Peirce and Freud with the objective of clarifying what is called ‘ethical conduct’. We do not establish a one-to-one correspondence between both theories, given that they have different concerns and paths, but we start from the conception that the isolated individual, enclosed in his dogmatic belief, whose neurotized behavior defines him, is a source of ignorance, in terms of the lack of social thinking or in terms of ignoring the cunning of their unconscious. Therefore, we use texts from the so-called first pragmatism, given that Peirce's discussion allows comparison between the problematic individual in Freud. As for the psychoanalyst, we worked on the texts of his Complete Works, selecting those that suited our intention, that is, that served to bring together two ways of conceiving the isolated individual. Indeed, for Peirce, the individual must yield to the opinion of the community of beings who think through signs, thus assuming a logical, rational attitude, and in accordance with the scientific method. In turn, the focus on psychoanalysis directs our attention to the unproductive individual, incapable of assuming a mature attitude, guided by the reality principle. Briefly, this is the individual trapped in the web of libido development in the pre-genital stage and without overcoming the Oedipus complex, a breakthrough that would make him ready to exist as a person, acting according to the principle of reality and knowing his limitations. This essay discusses the notion of 'semiotic development in child development' and highlights potential concerns for 'psychologism' when semiotics turns into cognitive semiotics. The notion of 'semiotic development in child development' indicates a transdisciplinary approach involving both semiotics, the general study of meaning and signs, and child psychology. This, however, invites the criticism of committing the fallacy of psychologism. Piaget was aware of this dilemma when developing his theory of the semiotic function as a united capacity in children's cognitive development. Sonesson's proposal of a general definition of signs in meaning-making is suggested to, at some points, meet the dilemma with psychologism in studies of children's semiotic development. Starting from a phenomenological point of view in semiotics and integrating Piaget's theory on cognitive development and meaning-making meet the study of subjectivity in intersubjectivity. On the one hand, the sign as a theoretical object is not reducible to any given psychological process or processes; on the other hand, sign meaning can only exist if there are beings (consciousnesses) capable of grasping and using signs. Introduction: Gallbladder polyps are more common than is commonly believed. They encompass any lesions that protrude or rise from the surface of the gallbladder mucosa. Materials: We treated 112 patients with gallbladder polyps, all of whom underwent laparoscopic cholecystectomy. Results: Most frequently, polyps were localized in the body of the gallbladder (60.7%). Polyps were found in the gallbladder fundus in 33 patients (29.5%), and in the neck of the gallbladder in 12 patients (10.7%). In cases of multiple polyps, the location was determined by the largest polyp. The location of the polyp in specific anatomical regions of the gallbladder had no significant bearing on clinical symptoms. After cholecystectomy and histological examination of the polyps, we identified 7 patients with adenocarcinoma (6.2%). Differentiated adenocarcinoma predominated (4 patients). Severe pain in the right upper quadrant of the abdomen was less common in patients with adenocarcinoma (p = 0.05). For adenoma, pain that occurred after eating was predominant (p = 0.021) and accompanied by positive Ortner (p = 0.055) and Murphy (p = 0.005) symptoms. Patients with adenoma more frequently reported a loss of appetite (p = 0.017) and a deterioration in well-being (p = 0.005). Conclusions: The location of the polyp in different anatomical areas of the gallbladder results in varying clinical symptoms. This study aims to describe and analyze the career decision-making of the first author in the context of life trajectory using autoethnographic trajectory equifinality modeling (Auto-TEM). The life story and analysis of the first author indicate that career decision-making is a constant pre-constructive process in the life trajectory of an individual. This process is a semiotic and trans-action one between the individual and society. In other words, career decision-making is an issue of the cultural psychology of dynamic semiosis, which is concerned with understanding the manner in which the life trajectory of a person promotes or inhibits socio-culturalism and not only a subject of matching and self-esteem. Alternatively, we emphasize that career decision-making for an individual is an act of meaning with a unique ontological aspect. We propose the value of Auto-TEM as a qualitative method for describing such a dynamic career decision-making process. This article examines the conceptual structure of psychoanalytic thinker Jacques Lacan's thoughts on language. It is stated by many researchers and thinkers that interdisciplinary interaction is the basis of this perspective, called Lacanian theory. Lacan especially brought the terms langage, langue and parole, which linguist Ferdinand de Saussure used to describe linguistic skill and the social and speaking parts of language, into his theory and gave them meaning at a conceptual level. Lacan, who also discussed Saussure's concept of the sign, developed a theoretical approach regarding the function of the signifier in the establishment of the subject. Another linguist who was influenced by Lacan is Roman Jakobson. By associating Jakobson's concepts of metaphor and metonymy with Freud's concepts of condensation and displacement, respectively, Lacan pointed out the existence of some functional phenomena between psychological processes and language and speech. Thus, with the support he receives from linguistic theories, Lacan develops concepts such as subject and other, which are widely discussed in the field of psychoanalysis and philosophy. Especially these two concepts have led to important discussions in modern philosophy and contributed to the emergence of new philosophical movements. In this article, the author presents a multimodal discourse analysis of a student-produced video, drawing upon Jacques Derrida's theorization of différance. He analyzes the film as a signifying chain, drawing specifically upon the differing and deferring aspects of différance in order to conceptualize the movement of signification in the video. The focus of his analysis is on how différance, as a constitutive force, splits and divides attempts to discursively construct a present through coordination of signifiers from across multiple modes. His application of différance illustrates how the present, as constructed in the focal video, is ill-defined and always blurred since presently occurring, visible, and audible signifiers in the video do not signify in and of themselves, but rather refer to past signifiers and anticipate future signifiers for their constitution as they engage the dynamic and complex operations of différance. This analysis adds to approaches to multimodal discourse analysis of student-produced videos by accounting for the interaction of visual, actional, bodily, and spoken signifiers, as well as the pedagogical implications for understanding how discursive agencies act upon student video-composers. This paper has tried to answer to a question: Is it important to use signs, symbols and pictograms to create a graphic and visual language, to communicate between people all over the world? And the answer is YES and will be explained in this paper. These graphic representations can be found in universities and schools, hospitals, airports, museums, libraries, network communication, business centres etc. But why are they so important? Because they inform, guide, allow, prohibit or warn depending on the situation. In our actual world visual communication without written language has become necessary. The main direction of the study was to present/design signage maps, as training models using signs, symbols, colours and pictograms.

The article attempts to combine the structural and comparative historical methods of symbol research. This is its scientific novelty, since modern symbolology is dominated by studies of a structural and semiotic nature and there is a clear lack of works that address the issue of the symbol's dependence on specific cultural and historical epochs. The author clarifies the concepts of the denoting and denoted symbol in the context of the ideas of linguistic conceptology and narratology, and also explores the problem of the conditionality of the meaning of the symbol and the means of its embodiment in the text by the artistic method and the type of narrative corresponding to it. The article proves that in realist prose the symbolic image is multifunctional. Firstly, it appears as an element of a landscape or interior description. Secondly, it performs a conceptual function, correlating its deep meaning with the main idea of the work. Thirdly, it plays a role in the development of the plot, in particular, it anticipates a significant and often unexpected turn within it. Fourthly, if it functions in the character's speech, it becomes one of the means of characterization, since the ability to perceive, comprehend and experience the symbolic meaning of an image is always evidence of the beginning of the character's spiritual insight. As for the symbolist, in particular ornamental, prose with its characteristic weakening of the plot, the symbol here has a completely different meaning. Firstly, it performs the function of a leitmotif - a figurative characteristic of the character, which is constantly repeated, because it expresses their essence. At the formal level, the function of symbols-leitmotifs is to organize the coherence and integrity of a literary text, which in realistic prose is ensured by the cause-and-effect relationships between the events that organize the plot. The author emphasises that these differences are due to the peculiarities of the worldview on which the respective artistic methods are based.

The study deals with discourse emblems treated as recognizable identifying signs of communication participants. They occupy a certain place in a general system of semiotic interpretation of reality - they are indices characterizing the pragmatic vector of a sign, according to Ch. Morris. The difference between emblems and allegories or symbols consists in their main quality - the former should be immediately recognized by the people who belong to a certain culture whereas the latter may be deciphered. Emblems as a system constitute an outer form of culture. Discourse emblems are inherent in any type of communication and may be regarded as discourse type indices. Their linguistic and cultural essence is to show communication partners as belonging to the joint community. Emblems in a personal habitual discourse demonstrate a restricted communicative distance, in an institutional discourse they indicate the group status of communicative partners, and in an artistic discourse they show a multiplex extensible variety of meanings contained in the text or any art object having an aesthetic value.
