Abstract

Abiraterone acetate, a drug that blocks endogenous androgen synthesis, plus prednisone is indicated for metastatic castration-resistant prostate cancer. We evaluated the clinical benefit of abiraterone acetate plus prednisone with androgen-deprivation therapy in patients with newly diagnosed, metastatic, castration-sensitive prostate cancer. In this double-blind, placebo-controlled, phase 3 trial, we randomly assigned 1199 patients to receive either androgen-deprivation therapy plus abiraterone acetate (1000 mg daily, given once daily as four 250-mg tablets) plus prednisone (5 mg daily) (the abiraterone group) or androgen-deprivation therapy plus dual placebos (the placebo group). The two primary end points were overall survival and radiographic progression-free survival. Abstract

Music information retrieval (MIR) is “a multidisciplinary research endeavor that strives to develop innovative content‐based searching schemes, novel interfaces, and evolving networked delivery mechanisms in an effort to make the world's vast store of music accessible to all.” MIR was born from computational musicology in the 1960s and has since grown to have links with music cognition and audio engineering, a dedicated annual conference (ISMIR) and an annual evaluation campaign (MIREX). MIR combines machine learning with expert human knowledge to use digital music data – images of music scores, “symbolic” data such as MIDI files, audio, and metadata about musical items – for information retrieval, classification and estimation, or sequence labeling. This chapter gives a brief history of MIR, introduces classical MIR tasks from optical music recognition to music recommendation systems, and outlines some of the key questions and directions for future developments in MIR.

Abstract

The two annotated bibliographies present in this publication document and feature pertinent discussions toward the activity of modeling the social edition, first exploring reading devices, tools and social media issues and, second, social networking tools for professional readers in the Humanities. In this work, which is published conjointly with the LLC piece "Toward Modeling the Social Edition: An Approach to Understanding the Electronic Scholarly Edition in the Context of New and Emerging Social Media," we consider a typology of electronic scholarly editions adjacent to activities common to humanities scholars who engage texts as expert readers, noting therein that many methods of engagement both reflect the interrelated nature of long-standing professional reading strategies and are social in nature; extending this framework, the next steps in the scholarly edition’s development in its incorporation of social media functionality reflect the importance of traditional humanistic activities and workflows, and include collaboration, incorporating contributions by its readers and re-visioning the role of the editor away from that of ultimate authority and more toward that of facilitator of reader involvement.

35. Wait times for prostate cancer treatment and patient perceptions of care in Canada: a mixed-methods report.

Abstract

Background Access to cancer care is a significant concern for Canadians. Prolonged delays between cancer diagnosis and treatment have been associated with anxiety, stress, and perceived powerlessness for patients and their family members. Longer wait times can also be associated with poorer prognosis, although the evidence is inconclusive. Here, we report national wait times for radiation therapy and surgery for localized prostate cancer (pca) and the effect of wait time on patient perceptions of their care. Results Treatment wait times showed substantial interprovincial variation. The longest 90th percentile wait times for radiation therapy and surgery were, respectively, 40 days and 105 days. In all provinces, waits for radiation therapy were longer for pca patients than for patients with breast, colorectal, or lung cancer. In the focus groups and interviews conducted with 47 men treated for pca, many participants did not perceive that wait times for treatment were prolonged. Those who experienced delays between diagnosis and treatment voiced issues with a lack of communication about when they would receive treatment and a lack of support or information to make an informed decision about treatment. Minimizing treatment delays was an aspect of the cancer journey that participants would like to change because of the stress it caused. Conclusions Although wait time statistics are useful, a review of cancer control in Canada cannot be considered complete unless an effort is made to give voice to the experiences of individuals with cancer. 47. Prognostic variables in adult Wilms tumour.

Abstract

Objective: To identify outcomes and prognostic variables that predict survival outcomes in adult Wilms tumour patients. Methods: We collected data on 128 patients with adult Wilms tumour treated between 1973 and 2006. Six cases from our 2 Canadian centres have not been previously reported. We collected data on the remaining 122 patients from published case reports or case series. Analyzed factors included age, sex, favourable or unfavourable histopathology, clinical stage (I, II, III or IV) and chemotherapy and radiotherapy received. The outcomes studied included overall survival (OS) and disease-specific survival (DSS). Univariate analysis with KaplanDSMeier actuarial methodology and multivariate analyses with Cox regression were used to determine outcomes and predictive clinical factors. Results: The patients' mean age was 26 (range 15DS73) years. After a mean follow-up of 54 (range 2DS240) months, the OS and DSS of the entire cohort were both 68%. Favourable histopathology predicted superior OS and DSS (both p < 0.001). Higher clinical stage predicted inferior OS and DSS (both p < 0.001). Conclusion: Adult Wilms tumour has a poorer prognosis than pediatric Wilms tumour. In adults with Wilms tumour, more aggressive patient- and tumour-specific surveillance and adjunctive therapies than those advocated by pediatric National Wilms Tumor Study guidelines may be warranted, especially in patients with an unfavourable histopathology and higher clinical stage. Abstract

Objective: To review resources available to aid family physicians in their care of prostate cancer patients and develop an algorithm to summarize these findings.Sources Of Information: MEDLINE, EMBASE, and relevant website search. All relevant guidelines were level III evidence.Main Message: Improved screening and treatment of patients with prostate cancer is resulting in an increasing number of survivors. These men require ongoing monitoring, a responsibility that is largely falling to family physicians. We review the expected prostate-specific antigen (PSA) responses to different prostate cancer treatment modalities and provide an appropriate schedule of follow-up and monitoring techniques for prostate cancer patients.Conclusion: In light of the paucity of resources for family physicians in their ongoing care of prostate cancer patients, we present an algorithm, primarily based on PSA kinetics, for practical use in the continuing care of these patients.
