Abstract

The drug rifabutin may prevent Mycobacterium avium (M. avium) complex infections in some AIDS patients. This infection strikes many AIDS patients and causes considerable complications. Researchers evaluated the effectiveness of rifabutin in preventing M. avium complex infections in two studies. All the patients studied had AIDS and CD4 cell counts less than 200 per cubic millimeter. A total of 1,146 patients were randomized to take either 300 milligram of rifabutin each day or a placebo, or inactive substance. They were followed for several years. In both studies, only 8% to 9% of the patients taking rifabutin developed an M. avium complex infection, compared to 17% to 18% of those taking a placebo. Survival rates were essentially similar in the two studies.

Abstract

There appears to be a relationship between early opportunistic infections in HIV patients and a drop in the percentage of CD4 cells. This was demonstrated in 103 HIV-infected patients who were receiving immunotherapy with an inactivated form of HIV. A drop in the percentage of CD4 cells occurred sooner and more rapidly in the patients who developed an early opportunistic infection such as oral Candida infection or oral hairy leukoplakia. Patients with the greatest decline had 5 times the risk of these infections.

Abstract

Interferon (IFN)-alpha production and accessory cell (AC) activity may be independent of each other. Previous studies have shown significant drops in IFN-alpha precede the onset of opportunistic infections in HIV patients and have also suggested that ACs may produce IFN-alpha which then activates natural killer (NK) cells to destroy virally-infected cells. Because NK activity remains normal until the late stages of AIDS however, the role of ACs in IFN-alpha production has been questioned. ACs were removed from the NK cells of healthy donors, and the depleted NK cells were mixed with peripheral blood mononuclear cells (PBMC) from either HIV patients or healthy donors. PBMCs from most HIV-infected donors were producing low levels of IFN-alpha. Despite this deficiency, the ACs from HIV patients were still able to function. This separation of IFN-alpha production and AC function suggests that these processes are either controlled by different cell populations or that they are independently controlled by the same cells.

Abstract

Hyponatremia is a disturbance in the body's metabolism resulting in a low concentration of sodium in the blood. Hyponatremia may be caused by abnormal secretion of the antidiuretic hormone (ADH) by the adrenal gland, in possible combination with an increased intake of water and decreased removal of water by the kidneys. Hyponatremia causes confusion and tiredness, and can lead to convulsions and coma. The prognosis of patients with AIDS and hyponatremia is poor. The incidence, clinical significance, and prognosis of hyponatremia were studied in 96 patients with AIDS-related complex or AIDS. Hyponatremia was seen in 31 percent (30 out of 96) of the patients with AIDS-related complex or AIDS. Twenty out of 57 patients (35 percent) developed hyponatremia outside of the hospital when the they were being treated on an outpatient basis, and 10 out of 39 (27 percent) developed hyponatremia when they were in the hospital. Patients with hyponatremia had a greater frequency of opportunistic infections, including Pneumocystis carinii pneumonia and cytomegalovirus infections. The death rate of AIDS patients with hyponatremia was 70 percent, which was twice that of patients without hyponatremia. The length of time that 50 percent of patients survived after diagnosis of HIV infection was 11.5 months for those with hyponatremia compared with 39 months for those without hyponatremia. The length of time after the development of hyponatremia that 50 percent of patients survived was 4.5 months. Two patients developed Addison's disease (a serious disease caused by partial or complete failure of the adrenal gland), and 15 had high levels of sodium in their urine but did not have any evidence of problems with their kidneys or adrenal glands. The cause of the high levels of urinary sodium was not determined. This study showed that hyponatremia commonly occurs in patients with AIDS-related complex and AIDS, and is associated with an increased incidence of complications and death. Hyponatremia is not a symptom characteristic of the terminal stages of AIDS, since it also occurred in patients with AIDS-related complex. Abstract

We describe five patients with HIV-2 infection (four antiretroviral-experienced and one antiretroviral-naive) treated with a regimen containing raltegravir. All responded to treatment as demonstrated by viral load and CD4+ T-cell count monitoring. Our series confirms the clinical effectiveness of raltegravir in HIV-2-infected patients when given with other antiretrovirals to which the virus is susceptible.

Abstract

Optimal HIV vaccines should elicit CD8+ T cells specific for HIV proteins presented on MHC class I products, because these T cells contribute to host resistance to viruses. We had previously found that the targeting of antigen to dendritic cells (DCs) in mice efficiently induces CD8+ T cell responses. To extend this finding to humans, we introduced the HIV p24 gag protein into a mAb that targets DEC-205/CD205, an endocytic receptor of DC5. We then assessed cross-presentation, which is the processing of nonreplicating internalized antigen onto MHC class I for recognition by CD8+ T cells. Low doses of αDEC-gag, but not control 19-gag, stimulated proliferation and IFN-γ production by CD8+ T cells isolated from the blood of HIV-infected donors. αCD205 fusion mAb was more effective for cross-presentation than αCD209/DC-SIGN, another abundant DC uptake receptor. Presentation was diverse, because we identified eight different gag peptides that were recognized via DEC-205 in 11 individuals studied consecutively. Our results, based on humans with highly polymorphic MHC products, reveal that DC5 and DEC-205 can cross-present several different peptides from a single protein. Because of the consistency in eliciting CD8+ T cell responses, these data support the testing of αDEC-205 fusion mAb as a protein-based vaccine. [ABSTRACT FROM AUTHOR]

Abstract

Chronically HIV-1-infected patients fail to contain their viremia despite high frequencies of HIV-1-specific, IFN-γ-producing CD8 T cells. However, these cells are known to exhibit both phenotypic and functional defects. We tested if mature dendritic cells (DC) could correct defective HIV-1 gag-specific T cell responses and if responses to other viral antigens were comparably affected. The circulating gag-specific CD8 T cells in fresh blood reliably produced IFN-γ but lacked IL-2 and high perforin levels and failed to expand significantly during culture with mature DC presenting HIV-1 gag peptides. In contrast, CD8 T cells from long-term nonprogressors contained gag-specific IFN-γ and IL-2 double producers, and the numbers of IFN-γ producers expanded ∼15-fold during culture with DC. DC from chronically infected patients could expand IFN-γ- and IL-2-producing cells specific for influenza, cytomegalovirus and Epstein Barr virus, and the expansions were comparable to those in healthy donors. When the proliferative capacity of CD8 T cells from progressor patients was assessed by CFSE dilution, proliferation to other viral antigens was more vigorous than to HIV-1 gag. Therefore, monocyte-derived DC from HIV patients present viral antigens effectively, but there is a selective inability to expand CD8 IFN-γ-producing and IFN-γ and IL-2 double-producing T cells when challenged with HIV-1 gag.
