Resumo Contrapondo as teorias sobre as literaturas afro-brasileira, negro-brasileira e o romance moderno, retomamos as noções de autor implicado, pacto de leitura e leitor desconfiado para indicar na dissimulação machadiana o esvaziamento de poder do leitor idealmente branco. Sugerimos que a teoria do romance moderno europeu é insuficiente para abarcar os procedimentos narrativos de Machado, próprios de uma sociedade cujas relações étnicas se assentam sobre a escravidão. Vendo em Aires o último dos narradores machadianos indignos de confiança, indicamos as bases escravistas da cordialidade do conselheiro em suas anotações sobre a Abolição. Concluímos que o leitor desconfiado, diante do romance machadiano, recupera o pacto de confiança no autor real, cujos artifícios retóricos marcam a indignidade das vozes narrativas de nossas elites.

4. Prevalence of left internal mammary artery disease in patients undergoing coronary angiography for suspected coronary artery disease: A meta-analysis and meta-regression study

Left internal mammary artery (LIMA) to bypass left anterior descending artery has demonstrated to improve survival in multivessel coronary artery disease, but its routine angiography during index coronary angiography is seldom performed as LIMA is rarely diseased.A systematic literature review and meta-analysis was conducted using PubMed and Cochrane databases selecting the studies reporting prevalence of LIMA disease among patients undergoing coronary angiography and considered for CABG. Meta-regression analysis was performed to evaluate the prevalence of LIMA disease and the relative predictive value of canonical risk factors. After scrimmage, 9 studies for a total of 1365 patients were included.LIMA disease prevalence was 1.8 % (95 % CI; 1.2 %-2.8 %) in the entire cohort, whereas we reported a rate of subclavian artery disease of 7.6 % (95 % CI, 6–9.9 %). At univariate meta-regression analysis, only age (p = 0.031) and smoking habit (p = 0.035) were directly correlated with LIMA disease.In conclusion, LIMA angiographic assessment might be considered in selected patients that could undergo CABG, during index coronary angiography. However, prospective studies are needed to better evaluate the safety of routine selective LIMA angiography prior to CABG and whether the practice is associated with improved clinical outcomes among those individuals.

9. An Aminoglycoside-Sparing Regimen with Double Beta-Lactam to Successfully Treat Granulicatella adiacens Prosthetic Aortic Valve Endocarditis—Time to Change Paradigm?

(1) Background: Granulicatella adiacens is a former nutritionally variant streptococci (NVS). NVS infective endocarditis (IE) is generally characterized by a higher rate of morbidity and mortality, partially due to difficulties in choosing the most adequate microbiological culture method and the most effective treatment strategy, and partially due to higher rates of complications, such as heart failure, peripheral septic embolism, and peri-valvular abscess, as well as a higher rate of valve replacement. Depending on the affected valve (native valve endocarditisNVE, or prosthetic valve endocarditisPVE), the American Heart Association (AHA) 2015 treatment guidelines (GLs) suggest penicillin G, ampicillin, or ceftriaxone plus gentamicin (2 weeks for NVE and up to 6 weeks for PVE), while vancomycin alone may be a reasonable alternative in patients who are intolerant of β-lactam therapy. The European Society of Cardiology (ESC) 2023 GLs recommend treating NVE with penicillin G, ceftriaxone, or vancomycin for 6 weeks, suggesting combined with an aminoglycoside (AG) for at least the first 2 weeks only for PVE; likewise, the same recommendations for IE due to Enterococcus faecalis. (2) Methods: Starting from the case of a 51-year-old man with G. adiacens aortic bio-prosthesis IE who was successfully treated with aortic valve replacement combined with double beta-lactams, an AG-sparing regimen, we performed microbiology tests in order to validate this potential treatment change. (3) Results: As for E. faecalis IE, we found that the combination of ampicillin plus cephalosporines (like ceftriaxone or ceftobiprole) showed a synergistic effect in vitro, probably due to wider binding to penicillin-binding proteins (PBPs), thus contributing to enhanced bacterial killing and good clinical outcome, as well as avoiding the risk of nephrotoxicity due to AG association therapy. (4) Conclusions: Further studies are required to confirm this hypothesis, but double beta-lactams and an adequate sourcecontrol could be a choice in treating G. adiacens IE.

The article discusses the literary works Os sertões (1902), by Euclides da Cunha, A Bagaceira (1928), by José Américo de Almeida, O Quinze (1930) by Rachel de Queiroz, and Pedra Bonita (1938), by José Lins do Rêgo, aiming to identify points in common, such as the compositional violence of the sertanejo’s formal structure: hunger and the environment – natural and social – hostile. The study allows delimiting issues around life in the hinterland that, denounced in Euclidean work, are fictionalized by Almeidiano, Queiroziano and Reguiano novels, observing the relationship between literature and the context of its creation. The approach makes it possible to glimpse the literature as a way of provoking reflections, other perceptions in the face of the reality of violence with which individuals have become accustomed, and may contribute to changing this situation, especially when it is verified, day by day, the increasing number of its victims.

Abstract Background Perceived stress is related to poor diet quality and unhealthy dietary patterns in women of reproductive age. Eating competence represents a variety of contextual skills reflecting a comfortable and flexible approach to eating and is associated with diet quality and health related behavior. In non-pregnant samples, perceived stress is negatively associated with eating competence. Given that pregnancy and the postpartum period can be periods of high stress, we hypothesized that higher stress in pregnancy would result in lower pregnancy eating competence. Methods Women (n = 296, mean BMIbaseline pregnancy = 26.3 ± SD 6.0) in the Pregnancy Eating Attributes Study (PEAS) were recruited from the Chapel Hill, North Carolina area. Perceived stress was assessed using the Perceived Stress Scale and eating competence using the ecSatter Inventory at their first trimester and 6-month postpartum visits. We used a mixed effect model to assess the effect of stress by time on eating competence, controlling for baseline pregnancy BMI, race and ethnicity, poverty to income ratio, and WIC status. 19. Physical therapy interventions to manage pain in Parkinson’s disease: A systematic review

Purpose Pain is a common non-motor symptom in Parkinson’s disease (PD), affecting about 40% of this population. Non-drug treatments and physical exercises, including physical therapy, are recommended options to ease PD pain. However, there are gaps in the literature regarding the treatment of this symptom, as well as few clinical trials assessing possible physical therapy interventions to manage PD pain. Hence, the objective of this study was to verify and analyse the physical therapy treatments available in the literature to manage pain in PD patients. Methods A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), searching the PEDro, PubMed, SciELO, and Science Direct databases. Results After applying the eligibility criteria, nine clinical trials remained, whose total sample comprised 242 individuals with PD (Hoehn and Yahr stage 1 to 4), submitted to physical therapy resources and methods such as hydrotherapy, physical therapy exercises, gait training with body weight support, electroacupuncture, and massage. Conclusions All the articles had positive physical therapy results in treating pain in people with PD. It is concluded that land and aquatic physical therapy methods improve the levels of pain in PD patients. However, further studies are needed with larger samples, who should be followed up to verify the duration of the effects.

In this paper, we describe the case of a patient admitted to our hospital because of a brain abscess due to Streptococcus intermedius. The management of brain abscess is challenging given the limited potential drug options with effective penetration into both the central nervous system and the abscess capsule to achieve adequate therapeutic concentrations. Due to the high anti-streptococcal activity of ceftobiprole and the availability of ceftobiprole therapeutic drug monitoring in our hospital, we decided to treat the patient with ceftobiprole. To maximize the antimicrobial effect of ceftobiprole, we chose a prolonged intravenous infusion, and we monitored its concentrations in both plasma and cerebrospinal fluid.

ABSTRACT Seed vigor is one of main attributes of the physiological quality from seed lot. Thus, identify which mechanisms are used by seeds of higher vigor that favor the formation of seedlings with better performance is a key aspect in agriculture. The enzyme activity of phytase and alpha-amylase favor the availability of phosphorus and sugars, respectively, during germination and can be determinants of seed vigor. Thus, the objective of the present work was to identify the association of phytase and alpha-amylase enzymes with the vigor of a seed lot of common beans. The results showed a positive correlation between seed vigor and the enzymes evaluated. Based on the findings, it can be concluded that seeds with high vigor have a high capacity to form seedlings with better performance, because they present high enzymatic activity. Also, in seed lots of the same genotype, they can be used to determine seed vigor during germination.

BackgroundDiet quality may be an important area of focus for promoting cognitive health; however, the association between diet quality and cognitive function among Hispanics/Latinos remains largely unexamined. We hypothesized that a healthier diet quality will be associated with better cognitive function in middle-aged and older Hispanics/Latinos.ObjectiveThe objective of this study was to examine associations between the Alternate Healthy Eating Index (AHEI-2010), a measure of diet quality, and cognitive function in middle-aged and older Hispanics/Latinos.MethodsData from the Hispanic Community Health Study/Study of Latinos (HCHS/SOL) Visit 1 (2008-2011) were used (n = 8461; ages 45-74 y). Cognitive function was assessed with tests of verbal learning and memory, verbal fluency, and processing speed; a global cognition score was derived by summing the z scores of individual tests. Dietary intake was assessed via two 24-h recalls. Total AHEI-2010 score was categorized into quintiles (higher quintiles indicating healthier diet). Linear regression models were used to examine associations between AHEI-2010 quintiles and cognitive function adjusting for sociodemographic characteristics, daily energy intake, type 2 diabetes, smoking, and depressive symptoms.ResultsCompared with the lowest quintile, in the second to fourth AHEI-2010 quintiles, global cognition scores were significantly higher by 0.28, 0.52, and 0.48 units (P-trend = 0.042). In the second to fifth AHEI-2010 quintiles, verbal learning scores were significantly higher by 0.60, 0.62, 0.92, and 0.88 units, and verbal memory scores were higher by 0.33, 0.40, 0.52, and 0.46 units (P-trend = 0.020 and 0.007, respectively). No associations were observed between the AHEI-2010 and verbal fluency or processing speed (P-trend = 0.49 and 0.84, respectively). Among AHEI-2010 components, adequate consumption of vegetables, alcohol, and whole fruits were each associated with better cognitive function.ConclusionsAn overall healthier diet quality was associated with better global cognition, verbal learning, and verbal memory in middle-aged and older Hispanics/Latinos.

O objetivo do presente estudo foi avaliar a qualidade fisiológica de sementes de feijão com diferentes tamanhos, identificando a relação do tamanho da semente com a mobilização e utilização de reservas. Foram utilizadas sementes da cultivar comercial IPR-88 Uirapuru e Iapar 81 padronizadas e posteriormente, determinadas três frações a partir de pesagem individual, sendo: sementes pequenas, médias e grandes. A qualidade fisiológica das três frações foi determinada pelo teste de germinação e pelos testes de vigor: envelhecimento acelerado e teste de desempenho de plântulas (comprimento). A mobilização e utilização de reservas foi mensurada aos cinco dias de hidratação das sementes, onde foram avaliadas massa seca, taxa de mobilização de reservas, taxa de redução de reservas e eficiência de uso das reservas. Os resultados demonstraram que não houve diferença entre as frações na porcentagem de germinação e envelhecimento acelerado. Considerando os parâmetros que avaliaram a mobilização e utilização de reservas, verificou-se que sementes de maior tamanho formam plântulas de maior massa seca, no entanto, tem menor taxa de mobilização de reservas e menor eficiência de uso de suas reservas. Desta forma, evidencia-se que o tamanho da semente influencia a mobilização e utilização de reservas, de maneira que sementes da fração média e pequena apresentam maior eficiência de uso de reservas até os cinco dias de germinação.

Purpose Aquatic intervention seems to influence the motor skills of people with Parkinson’s disease (PD). However, little is known about the transference of these motor skills to the land environment, as well as the use of the therapeutic properties of water. The study aim was to identify the aquatic intervention strategies considering the properties of water for the development of motor skills in PD. Methods With the PRISMA and Cochrane checklist, a systematic review was conducted involving 7 databases with randomized and nonrandomized studies from 2010 to July 2020. The prescription and description of aquatic physical exercise data were extracted and summarized, and the use of physical and thermal properties of heated water was analysed. Results Overall, 13 studies involving 307 participants (Hoehn and Yahr scale 1–4) met the inclusion criteria. In accordance with the Jadad scale, 7 studies were classified as low-quality (score 0–2), whereas 6 were categorized as high-quality (score 3). All the selected studies concentrated on aquatic physical exercise programs developed by the researchers who focused on transferring motor skills – such as mobility and functional reach, balance, motor aspects, activities of daily living, quality of life, and gait – to the land environment. Conclusions Aquatic intervention strategies can be adequately prescribed as beneficial to motor skill training in PD. The analysis of the physical and thermal properties of heated water is useful to systematize prescription parameters depending on the characteristics of the PD patient and the heated aquatic environment.

Introdução: A Profilaxia Pós-Exposição (PEP) de risco à infecção pelo Vírus da Imunodeficiência Humana (HIV) é fornecida no Brasil com o objetivo de ampliar as formas de intervenção para evitar novas infecções pelo HIV, sendo necessário iniciar o uso em período inferior a 72 horas após a exposição sexual, uma vez que é considerada uma emergência médica. Conforme Protocolo Clinico e Diretrizes Terapêuticas, o esquema preferencial em adultos constitui-se de 1 comprimido de Dolutegravir 50mg+ Tenofovir 300mg +Lamivudina 300mg por 28 dias. O farmacêutico é um profissional estratégico dentro dos serviços de saúde e possui suas atividades clínicas, incluindo a prescrição de medicamentos, respaldadas através de legislações específicas como a Resolução do Conselho Federal de Farmácia 713/2021.Objetivos: Descrever a experiência da implantação do serviço de prescrição farmacêutica de PEP em uma instituição pública de Porto Alegre. Métodos: Nesse protocolo, são elegíveis pessoas que atendam os seguintes requisitos: maiores de 18 anos de idade, que tenham tido relação sexual consentida desprotegida ou com rompimento de preservativo, sem contraindicação ao esquema preferencial e sorologia negativa para o HIV. A coleta de dados foi realizada com uso de dados secundários obtidos de prontuários farmacêuticos, fichas de atendimento e banco de dados do Centro de Testagem e Aconselhamento (CTA) de usuários que acessaram o serviço para realização de PEP no período de 01 de dezembro de 2021 até 06 de julho de 2022.Resultados: Desde então, 86 usuários já foram atendidos. Apenas uma pessoa apresentou intercorrências durante o tratamento ocasionadas por efeitos adversos requerendo atendimento médico. Não houve necessidade de interrupção da PEP em nenhuma das pessoas prescritas , não houve relato de soroconversão do HIV e mais de 50% dos usuários atendidos retornaram ao CTA para testagem após conclusão do uso dos medicamento. O monitoramento dos pacientes está sendo feito através de indicadores como o retorno para realização do teste rápido de HIV , avaliação de efeitos adversos e recorrência ao uso de PEP , o qual sugere abordagem de indicação para Profilaxia PréExposição (PrEP).Conclusão: A iniciativa, que é pioneira no estado do Rio Grande do Sul, contribui para consolidação da prescrição farmacêutica nos serviços públicos e para aumentar o acesso da população às ações de prevenção do HIV

35. Improvement of Balance, Motor Aspects, and Activities of Daily Living in Parkinson’s Disease after a Sequential Multimodal Aquatic- and Land-Based Intervention Program

Parkinson’s disease (PD) is a neurodegenerative, with heterogeneous clinical conditions and motor changes that reduce functioning. Postural instability is one of the motor aspects of disease progression, with a potential increase in the risk of falls, consequently affecting the activities of daily living (ADL). The objective of this study was to verify the influence of a multimodal intervention program (MIP) sequentially applied in aquatic- (AEs) and land-based environments (LEs) on balance, postural control, motor activities, and ADL in people with PD. It is an interventional clinical study with patients in stages 1 to 4 in the Hoehn and Yahr scale, assessed with Berg Balance Scale (BBS), Mini-Balance Evaluation System Test (Mini-BESTest), Unified Parkinson’s Disease Rating Scale (UPDRS) II and III, Dynamic Gait Index (DGI), and quiet stance (QS) analysis in a force platform. The MIP was conducted sequentially with aquatic- (AIs) and land-based interventions (LIs) for 12 weeks each, twice a week, each session lasting 1 hour, and a 12-week interval between interventions. The comparison analysis was made with Friedman ANOVA, and the multiple comparisons with Wilcoxon signed-rank, Bonferroni correction, and effect size (r). The sample comprised 18 people with PD (66.83±11.74 years). The AI and the full intervention (FI) had a large effect according to BBS. With Mini-BESTest, the LI and FI had a large effect. According to UPDRS II, the MIP improved ADL after LI, with a medium effect, and the motor aspects of UPDRS III improved after LI and FI, with a large effect. DGI was not sensitive in the analyses, with a ceiling effect after FI. No differences were identified in QS analyses. This research identified improved balance, ADL, and motor aspects in people with PD after sequential MIP in AI and LI, indicating that land-based and aquatic interventions are complementary and advantageous to people with PD.

Este relato de experiência objetiva descrever o processo de construção e avaliação de um recurso educacional digital que utiliza o Storytelling para trabalhar os aspectos da humanização no processo de formação do Técnico em Enfermagem. Para a construção do recurso educacional digital utilizou-se a Design Science Research como guia para elaboração e avaliação da proposta. O processo de construção e avaliação durou de julho a novembro de 2020. Foram produzidas duas histórias, um vídeo de orientação aos professores avaliadores e duas atividades de fixação de conteúdo, tendo como abordagem a humanização. A avaliação da do recurso educacional digital foi realizado por 19 professores de Cursos Técnico em Enfermagem. Considera-se que o Storytelling é uma metodologia viável e relevante no processo de ensino-aprendizagem dos cursos Técnicos em Enfermagem no que tange desenvolver a temática de humanização, podendo ser explorada em outras áreas do conhecimento. Este artigo toma como objetos os relatos de Hans Staden, André Thevet, Jean de Léry e Anthony Knivet sobre suas passagens, no século XVI, pelas terras do que viria a ser o Brasil. Analisando comparativamente tais textos, observamos como as diferentes realidades histórico-culturais em concorrência na Europa moldaram perspectivas ora convergentes ora conflitantes sobre um mesmo tema: o indígena, seus hábitos e costumes. Para a análise, mobilizamos as ideias de topoi ou tópicas do discurso, baseando-nos em Hansen (2013; 2021), assim como a noção de lutas de representação, teorizada por Chartier (1991; 2002), articulando-as a estudos sobre a história e mentalidade coloniais, entre outros. Concluímos que, nos discursos dos viajantes, o lugar comum do indígena é sede de argumentação em prol do grupo ao qual pertence o enunciador do discurso, que lança mão de opiniões pré-concebidas e aceitas pela sua comunidade como verdadeiras.

ABSTRACT Previous studies have investigated the associations between the vaginal microbiome and preterm birth, with the aim of determining whether differences in community patterns meaningfully alter risk and could therefore be the target of intervention. We report on vaginal microbial analysis of a nested case-control subset of the Pregnancy, Infection, and Nutrition (PIN) Study, including 464 White women (375 term birth and 89 spontaneous preterm birth, sPTB) and 360 Black women (276 term birth and 84 sPTB). We found that the microbiome of Black women has higher alpha-diversity, higher abundance of Lactobacillus iners, and lower abundance of Lactobacillus crispatus. However, among women who douche, there were no significant differences in microbiome by race. The sPTB-associated microbiome exhibited a lower abundance of L. crispatus, while alpha diversity and L. iners were not significantly associated with sPTB. For each order of magnitude increase in the normalized relative abundance of L. crispatus, multivariable adjusted odds of sPTB decreased by approximately 20% (odds ratio, 0.81; 95% confidence interval, 0.70, 0.94). When we considered the impact of douching, associations between the microbiome and sPTB were limited to women who do not douche. We also observed strong intercorrelations between a range of maternal factors, including poverty, education, marital status, age, douching, and race, with microbiome effect sizes in the range of 1.8 to 5.2% in univariate models. Therefore, race may simply be a proxy for other socially driven factors that differentiate microbiome community structures. Future work will continue to refine reliable microbial biomarkers for preterm birth across diverse cohorts. IMPORTANCE Approximately 10% of all pregnancies in the United States end in preterm birth, and over 14% of pregnancies end in preterm birth among Black women. Knowledge on the associations between vaginal microbiome and preterm birth is important for understanding the potential cause and assessing risk of preterm birth. Our study is one of the largest studies performed to date to investigate the associations between vaginal microbiome and spontaneous preterm birth (sPTB), with stratified design for Black and White women. We found that the vaginal microbiome was different between Black and White women. The vaginal microbiome was associated with sPTB, and a lower abundance of L. crispatus increased the risk of sPTB independent of racial differences in microbial community structures. Furthermore, we also found that vaginal douching obscured the associations between vaginal microbiome, race, and preterm birth, suggesting that vaginal douching is an important factor to consider in future studies.
