Abstract

This study investigates the application and evaluation of existing indirect methods, namely point-based registration techniques, for the estimation and compensation of observed motion included in the 2-D image plane of contrast-enhanced ultrasound (CEUS)cine-loops recorded for the characterization and diagnosis of focal liver lesions (FLLs). The value of applying motion compensation in the challenging modality of CEUS is to assist in the quantification of the perfusion dynamics of an FLL in relation to its parenchyma, allowing for a potentially accurate diagnostic suggestion. Towards this end, this study also proposes a novel quantitative multi-level framework for evaluating the quantification of FLLs, which to the best of our knowledge remains undefined, notwithstanding many relevant studies. Following quantitative evaluation of 19 indirect algorithms and configurations, while also considering the requirement for computational efficiency, our results suggest that the “compact and real-time descriptor” (CARD)is the optimal indirect motion compensation method in CEUS. © 2019 World Federation for Ultrasound in Medicine & Biology

29. Fast semi-automatic segmentation of focal liver lesions in contrast-enhanced ultrasound, based on a probabilistic model

Abstract

Assessment of focal liver lesions (FLLs) in contrast-enhanced ultrasound requires the delineation of the FLL in at least one frame of the acquired data, which is currently performed manually by experienced radiologists. Such a task leads to subjective results, is time-consuming and prone to misinterpretation and human error. This paper describes an attempt to improve this clinical practice by proposing a novel fast two-step method to automate the FLL segmentation, initialised only by a single seed point. Firstly, rectangular force functions are used to improve the accuracy and computational efficiency of an active ellipse model for approximating the FLL shape. Then, a novel probabilistic boundary refinement method is used to iteratively classify boundary pixels rapidly. The proposed method allows for faster and easier assessment of FLLs, whilst requiring less interaction, but producing results comparably consistent with manual delineations, and hence increasing the confidence of radiologists when making a diagnosis. Quantitative evaluation based on real clinical data, from two different European countries reflecting true clinical practice, demonstrates the value of the proposed method. © 2015 Informa UK Limited, trading as Taylor & Francis Group.

Abstract

We present here the first update of the 2013 EFSUMB (European Federation of Societies for Ultrasound in Medicine and Biology) Guidelines and Recommendations on the clinical use of elastography with a focus on the assessment of diffuse liver disease. The short version provides clinical information about the practical use of elastography equipment and interpretation of results in the assessment of diffuse liver disease and analyzes the main findings based on published studies, stressing the evidence from meta-analyses. The role of elastography in different etiologies of liver disease and in several clinical scenarios is also discussed. All of the recommendations are judged with regard to their evidence-based strength according to the Oxford Centre for Evidence-Based Medicine Levels of Evidence. This updated document is intended to act as a reference and to provide a practical guide for both beginners and advanced clinical users.

Item does not contain fulltext, BACKGROUND AND AIMS: EUS-guided FNA or biopsy sampling is widely practiced. Optimal sonographic visualization of the needle is critical for image-guided interventions. Of the several commercially available needles, bench-top testing and direct comparison of these needles have not been done to reveal their inherent echogenicity. The aims are to provide bench-top data that can be used to guide clinical applications and to promote future device research and development. METHODS: Descriptive bench-top testing and comparison of 8 commonly used EUS-FNA needles (all size 22 gauge): SonoTip Pro Control (Medi-Globe); Expect Slimline (Boston Scientific); EchoTip, EchoTip Ultra, EchoTip ProCore High Definition (Cook Medical); ClearView (Conmed); EZ Shot 2 (Olympus); and BNX (Beacon Endoscopic), and 2 new prototype needles, SonoCoat (Medi-Globe), coated by echogenic polymers made by Encapson. Blinded evaluation of standardized and unedited videos by 43 EUS endoscopists and 17 radiologists specialized in GI US examination who were unfamiliar with EUS needle devices. RESULTS: There was no significant difference in the ratings and rankings of these needles between endosonographers and radiologists. Overall, 1 prototype needle was rated as the best, ranking 10% to 40% higher than all other needles (P < .01). Among the commercially available needles, the EchoTip Ultra needle and the ClearView needle were top choices. The EZ Shot 2 needle was ranked statistically lower than other needles (30%-75% worse, P < .001). CONCLUSIONS: All FNA needles have their inherent and different echogenicities, and these differences are similarly recognized by EUS endoscopists and radiologists. Needles with polymeric coating from the entire shaft to the needle tip may offer better echogenicity.

33. Automatic identification and localisation of potential malignancies in contrast-enhanced ultrasound liver scans using spatio-temporal features

Abstract

The identification and localisation of a focal liver lesion (FLL) in Contrast-Enhanced Ultrasound (CEUS) video sequences is crucial for liver cancer diagnosis, treatment planning and follow-up management. Currently, localisation and classification of FLLs between benign and malignant cases in CEUS are routinely performed manually by radiologists, in order to proceed with making a diagnosis, leading to subjective results, prone to misinterpretation and human error. This paper describes a methodology to assist clinicians who regularly perform these tasks, by discharging benign FLL cases and localise potential malignancies in a fully automatic manner by exploiting the perfusion dynamics of a CEUS video. The proposed framework uses local variations of intensity to distinguish between hyper- and hypo-enhancing regions and then analyse their spatial configuration to identify potentially malignant cases. Automatic localisation of the potential malignancy on the image plane is then addressed by clustering, using Expectation-Maximisation for Gaussian Mixture Models. A novel feature that combines description of local dynamic behaviour with spatial proximity is used in this process. Quantitative evaluation, on real clinical data from a retrospective multicentre study, demonstrates the value of the proposed method. © Springer International Publishing Switzerland 2014.
