3. Team formulation in intellectual disability services

Abstract

The systematic review (Paper 1) explores the evidence base for team formulation in intellectual disability settings. The systematic review aimed to (1) synthesise and evaluate how team formulation is defined and implemented in intellectual disability (ID) practice and (2) analyse team formulation outcomes for ID practice. The review suggested that there is no standard definition of team formulation in ID practice and psychologists use different terms to label team formulation. The review also showed that there is no specific way team formulation was applied, a common factor was a structured and collaborative approach using frameworks and protocols. Overall, there is a moderate evidence base for the outcomes for team formulation in ID practice. The empirical study (Paper 2) examines team formulation in ID. The study uses a mixed- methods design to evaluate the effectiveness of team formulation meetings. The quantitative phase measures the impact of team formulation on carer empathy, emotions, therapeutic optimism within staff that support people with an intellectual disability. Twenty staff were asked to complete three questionnaires pre-formulation and post formulation attendance. Statistical analysis shows no significant changes in staff empathy, therapeutic optimism, attachment security or emotional responses to challenging behaviour. The qualitative phase of the study involved semi-structure interviews to explore staff experiences of team formulation within the service. Twelve staff were asked a series of open-ended questions about team formulation meetings and the data was analysed using thematic analysis. These findings are discussed in more detail. The final paper (Paper 3) provides a critical reflection of the author’s research process. This includes a discussion of the decisions behind the systematic review and empirical study as well as the methods, challenges and learning.

Abstract

Background Transcutaneous electrical nerve stimulation (TENS) is an established method for pain relief. But electrical TENS currents are also a source of electromagnetic interference (EMI). Thus, TENS is considered to be contraindicated in implantable cardioverter-defibrillator (ICD) patients. However, data might be outdated due to considerable advances in ICD and cardiac resynchronization therapy (CRT) filtering and noise protection algorithm technologies. The aim of this pilot safety study was to re-evaluate the safety of TENS in patients with modern ICDs. Methods and results One hundred and seven patients equipped with 55 different models of ICD/CRT with defibrillators from 4 manufacturers underwent a standardized test protocol including TENS at the cervical spine and the thorax, at 2 stimulation modes—high-frequency TENS (80 Hz) and burst-mode TENS (2 Hz). Potential interference monitoring included continuous documentation of ECG Lead II, intracardiac electrograms and the marker channel. Electromagnetic interference was detected in 17 of 107 patients (15.9%). Most frequent were: interpretations as a premature ventricular beats (VS/S) in 15 patients (14%), noise reversion in 5 (4.6%) which resulted in temporary asynchronous pacing in 3 (2.8%), interpretation as ventricular tachycardia/ventricular fibrillation in 2 (1.9%), and premature atrial beat in 2 (1.9%) patients. Electromagnetic interference occurrence was influenced by position (chest, P < 0.01), higher current intensity (P < 0.01), and manufacturer (P = 0.012). Conclusion Overall, only intermittent and minor EMI were detected. Prior to the use of TENS in patients with ICDs, they should undergo testing under the supervision of a cardiac device specialist. 35. The Calm Curriculum : Theories that inform practice

Abstract

This capstone project examined theories and literature regarding social and emotional learning and self-regulation, and aimed to provide a link in how they support, and translate into the practice strategies of a curriculum already developed called, the CALM Curriculum (Sidhu & Elliott, 2013). The review of the literature of this project was guided by the following two questions: What is the theoretical framework that informs the CALM Curriculum, and, how do these theories translate into practice when implementing the CALM Curriculum? I developed the CALM Curriculum in response to the increase in referrals for children with self-regulation issues in the community of Chilliwack, B.C., and to help educators conceptualize self-regulation through a workshop and a curriculum that provided training and concrete practical strategies for the classroom. In this paper I provide the theory and literature regarding social and emotional learning that focus on the domains of biological, emotional, cognitive, social, and prosocial self-regulation that make up the CALM Curriculum. The CALM Workshop, presented in Chapter Three connects the research to the practice strategies of the CALM Curriculum and helps solidify educator’s conceptualization of self-regulation. The findings of this project revealed that the CALM Curriculum is substantiated by solid theoretical underpinnings and current research. Recommendations for future research include a CALM 2 grounded in theory and research and including the motivational domain of self-regulation.
