- Abstract
The Italian health system is quickly empowering the capacity of specific COVID-19 hospital admission (creating new COVID-19 emergency centres-hubs and new intensive care beds in the already existing hospitals) showing a great response in the first-line hospital-based treatment. The major aim of this brief viewpoint is to underline the underestimated role of second line home-based treatment (for positive or suspected patients isolated at home) that could avoid to overwhelm the first line, taking into account the contribution of the necessary social limitations-containment campaign. Particularly the condition of acute respiratory failure could be promptly detected for COVID-19 patients in home isolation without not necessary and potentially dangerous hospital accesses and checks, but using easy-to-use (also for elderly patients and caregivers), comfortable, low-cost, reliable remote monitoring devices, such as connected pulse oximeters that can send real-time vital signs and other key biomedical parameters (especially respiration and blood oxygen levels, body temperature and other parameters) to a hospital-based big data server-repository, where Artificial Intelligence algorithms-based interpretation systems can elaborate these clinical records alerting the physicians only when it is necessary (not too early and not too late) to evaluate if moving a patient from a second to a first line. The experience of biomedical engineering and informatics, strong in chronic care management, could functionally help clinicians in managing many patients and citizens during this strong and long pandemic. It is not possible to postpone the implementation of large remote monitoring programs providing digital devices centrally controlled. We hope that Italian and International scientific and clinical community could promptly introduce and strengthen this already available option. Nevertheless, much more work remains to be carried out for providing and sharing an optimized information content. In this paper we formulate, from a theoretical perspective, an optimization problem aimed to encourage the creation of a sub-network of patients which, being recently diagnosed, wish to deepen their knowledge about their pathologies with some other patients, whose clinical profile turn to be similar, and have already been followed up within specific, even alternative, care centers. We will focus on the hardness of the proposed problem and provide a Genetic Algorithm (GAbased) approach to seek faster approximated solutions.

Objectives: We studied the effect of Telehomecare (THC) in a group of Cystic Fibrosis (CF) patients. Specifically, in order to investigate the evolution of the clinical trend, we examined the monitoring activities of CF patients followed at home for a period of 4.5 years. Methods: Forced Expiratory Volume in the first second (FEV1) was monitored at home, with the aim of an early recognition of the relapses of pulmonary infections. FEV1 was monitored by Spirotel-MIR instrumentation for 4.5 years, using THC as a tool, in addition to the standard therapeutic protocol. 16 CF patients were followed by doctors specialized in the treatment, over a period of 4.5 years. As control group, we enrolled 16 CF patients with similar characteristics of age, degree of pulmonary involvement, bacterial colonization and O2 dependency. Results: While in the first 5 year report we showed statistically significant reduction in hospital admissions and a tendency over time towards a better stability of the respiratory function, here we provide an evidence of the significant different evolution between THCs and controls measured as the difference between the values of average annual FEV1 and those values observed during a starting reference year (i.e. 2011). Discussion: The obtained results in both quantitative and qualitative parameters of our work during these 10 years is positive. Data are encouraging concerning the possible role of Telemedicine as a tool for domestic assistance of patients affected by chronic diseases, such as CF.

New technologies are multiplying at an enormous speed and the produced data is not only massive but also complex. In fact, despite the abundance of tools to capture, process and share information (e.g. data) one cannot broadly assume the standard hypothesis that such data are identically and independently distributed (i.i.d.). As a result, proper handling of data is fundamental in order to convert the available observation in to useful information that leads to knowledge and suitable decision making. In this paper, we focus on network data. That is, we introduce the reader to a theoretical perspective concerning the knowledge mining of huge amount of relational information collected in all the network systems which are ubiquitous in our life. In this context, following a numerical evaluation we show the reader how different kind of information can provide a benefit for a typical machine learning problem i.e. classification. The main issue of our investigation is to provide a case where the accuracy of a classification model benefits when considering the additional information given by both network and dissimilarity features. Moreover, we treat a clinical example that will serve as running case for our analysis.

- Abstract
Chronic diseases and conditions typically require long-term monitoring and treatment protocols both in traditional settings and in out-patient frameworks. The economic burden of chronic conditions is a key challenge and new and mobile technologies could offer good solutions. mHealth could be considered an evolution of ehealth and could be defined as the practice of medicine and public health supported by mobile communication devices. mHealth approach could overcome limitations linked with the traditional, restricted and highly expensive in-patient treatment of many chronic pathologies. Possible applications include stepped mHealth approach, where patients can be monitored and treated in their everyday contexts. Unfortunately, many barriers for the spread of mHealth are still present. Due the significant impact of psychosocial factors on disease evolution, psychotherapies have to be included into the chronic disease protocols. Existing psychological theories of health behavior change have to be adapted to the new technological contexts and requirements. In conclusion, clinical psychology and medicine have to face the "chronic care management" challenge in both traditional and mHealth settings.

12. Bioinformatic Integration of 'Omics' Data to Evaluate and Improve Laser Induced Neuroregeneration after SCI: an Overview

Spinal cord injury (SCI) counts about 17,000 new cases each year in the United States. Since axons lose the competence to regenerate in adult mammals, SCI can lead to permanent neurological damages with dramatic personal, social and economic impacts. The long-term deficit of SCI results first from the type of insult and then from the secondary phase that includes many pathophysiological events. Among these, inflammation and epigenetic factors play a crucial role in the recovery of neuron connections. Variations in epigenetic and immune contribution, in turn, depend on age and health status as well as to microbiota profile of individuals at the time of, or consequent to SCI. Indeed, gut microorganisms, highly influence the immune system, but also produce bioactive substances such as folates, butyrate and acetate that participate to the epigenetic processes. Interestingly, variations in the profile of microbiota and bioactive substances have been described in patients with neurologic intestine because of SCI. Recently, different approaches, including stem cell therapy, use of biomaterial and laser therapy, have been proposed for neuronal regeneration after SCI but they are still far from resolutive interventions. As the complexity of pathophysiological processes of the secondary phase can deeply condition the success of regeneration, we provide a landscape of microbiota-epigenetic-immune modulation of neurological recovery predisposition or prevention. We discuss most of data about epigenetics (microRNAs, circulating microRNAs and chromatin remodelling) after SCI in animal models as well as microbiome profile of patients with SCI. We also propose a bioinformatics approach to compare "comics" data (gut microbiome, circulating microRNAs and inflammatory profiles) of patiens with SCI before and after laser therapy to evaluate and improve laser induced neuroregeneration. 13. Epigenetic-immune-microbiota contribution to neural regeneration after spinal cord injury: an overview

Objectives: spinal cord injury (SCI) triggers a cascade of intrinsic pathophysiological events that influence the SCI long-term deficit and that should be therapeutically modulated. The aim is to identify biomarkers to favour neuronal recovery after SCI. Material and Methods: we reviewed the literature indexed in PubMed about SCI, SCI epigenetics, SCI microbiota and novel therapeutic strategies for SCI repair. Results: inflammation-epigenetics-microbiota vicious cycle affects recovery after SCI. The dysregulation of specific microRNAs and circulating microRNAs was found in spinal cord and in serum, respectively, after SCI thus circulating microRNAs are promising biomarkers for evaluating the severity of SCI, as demonstrated in animal models. SCI patients often display neurogenic intestine dysfunction including changes in gut microbiome composition with a significant reduction in butyrate producing bacteria. Butyrate is a potent anti-inflammatory agent, a histone deacetylase inhibitor and suppresses inflammation in the CNS probably reducing microglia-mediated neurotoxicity. In mice, induced gut dysbiosis exacerbates neurologic damage impairing recovery after SCI. To date, there is not a cure for SCI, however new regenerative approaches are recently suggested. In rats, valproic acid administration after SCI protects motoneurons through modulation of apoptotic pathways. The microRNA-based therapy, manipulating the expression of specific microRNAs can activate or block target genes involved in neuro-regeneration; advances in CNS microRNA delivery technologies able to cross the spinal cord blood barrier have been reached. In SC injured rats, passive cycling exercises modulated microRNAs and gene transcription favouring biochemical and cellular restore, and potentially damage recovery. Then again, regarding microbiota, probiotic-induced eubiosis improves loco-motor recovery in mice. New nutraceuticals have also been demonstrated to help in neuro-regeneration and SCI recovery. This is the case of Naringin, curcumin epigallocathechin-3-gallate, omega-3 polyunsaturated fatty acid docosahexaenoic acid (DHA) that play anti-oxidant, anti-inflammatory and anti-apoptotic rules.

Obesity is now one of the most critical and demanding public health condition due to the correlation with many medical and psychological comorbidities, such as cardiovascular, orthopedic, pneumological, endocrinological, psychopathological complications, above all the type 2 diabetes. Obesity traditionally needs long and expensive treatments in a chronic care management approach. So clinical research has to develop, test and validate cheaper rehabilitation programs. For this reason, we developed the DIABESITY study, the design of a mHealth integrated platform to promote the empowerment of patients in self-monitoring and successfully managing their pathological conditions (focusing on obesity and type 2 diabetes) through the use of mobile devices. In this paper we report this study by discussing the following two important aspects of DIABESITY. (i) Dietary mHealth tools for home-patients; (ii) Measures to capture the psychological factors and processes which mediate change of behavior and affect initiation and maintenance phases.

Obesity is one of the most important medical and public health problems of our time: it increases the risk of many health complications such as type 2 diabetes, needs long-lasting treatment for effective results and involves high public and private costs. Therefore, it is imperative that enduring and low-cost clinical programs are developed and evaluated. As reported in several studies, ICT may be valid alternatives to reduce costs and improve adherence to prescribed treatment. Nevertheless few studies have tested a long-term intervention addressed to the behavior change and to measure the weight loss of obese subjects. For this reason, we developed the DIABESITY study, the design of a mHealth integrated platform for empowering diabetic and obese citizens in self-monitoring, and self-management through the use of mobile devices, monitors and treatment protocols. In this paper we focus on the following three important aspects of DIABESITY. i) Dietary mHealth tools for home-patients; ii) Application and analysis of psychological factors and processes which mediate change of behavior and affect initiation and maintenance phases; iii) Employment of social networks for patients and clinicians. Currently, this study involves 14 international partners chosen amongst hospitals, universities and ICT companies which will strictly collaborate by contributing with their own specific skills. The effectiveness of DIABESITY compared with usual care (hospital-based treatment) will be provided in a randomized controlled trial with a 24-month follow-up. In particular, here we report primary and secondary clinical outcomes with the basic statistical procedures which will be used for this evaluation.

- Abstract
Since 2001, in the Cystic Fibrosis Center of the Pediatric Hospital Bambino Gesù in Rome, we use telemedicine for monitoring of our patients. While in our first published works reporting this experience, we showed statistically significant reduction in hospital admissions and a tendency over time towards a better stability of the respiratory function for telehomecare (THC) patients, here we focus on the trend of the Forced Expiratory Volume in the first second (FEV1). In particular, we investigate the evolution of the clinical trend of the FEV1 index, by monitoring the activities of home patients from 2011 to 2014. THC is applied in addition to the standard therapeutic protocol by following 16 Cystic Fibrosis (CF) patients with specialized doctors. Our results show that THC patients improve their FEV1 values with a trend which can be considered significantly better than the one reported by the control group.

- Abstract
Chronic diseases and conditions typically require long-term monitoring and treatment protocols both in traditional settings and in out-patient frameworks. The economic burden of chronic conditions is a key challenge and new and mobile technologies could offer good solutions. mHealth could be considered an evolution of ehealth and could be defined as the practice of medicine and public health supported by mobile communication devices. mHealth approach could overcome limitations linked with the traditional, restricted and highly expensive in-patient treatment of many chronic pathologies. Possible applications include stepped mHealth approach, where patients can be monitored and treated in their everyday contexts. Unfortunately, many barriers for the spread of mHealth are still present. Due the significant impact of psychosocial factors on disease evolution, psychotherapies have to be included into the chronic disease protocols. Existing psychological theories of health behavior change have to be adapted to the new technological contexts and requirements. In conclusion, clinical psychology and medicine have to face the "chronic care management" challenge in both traditional and mHealth settings.

The Institute physicians have the daily need of consulting images coming from various modalities. The high volume of archived material and the need of retrieving and displaying new and past images and clinical information has motivated the development of a Picture Archiving and Communication System (PACS) for the automatic management of images and clinical data, related not only to the Radiology Department, but also to the Radiotherapy Department for 3D virtual simulation, to remote teleconsulting, and in the following to all the wards, ambulatories and labs.

Obesity is now one of the most critical and demanding public health condition due to the correlation with many medical and psychological comorbidities, such as cardiovascular, orthopedic, pneumological, endocrinological, psychopathological complications, above all the type 2 diabetes. Obesity traditionally needs long and expensive treatments in a chronic care management approach. So clinical research has to develop, test and validate cheaper rehabilitation programs. For this reason, we developed the DIABESITY study, the design of a mHealth integrated platform to promote the empowerment of patients in self-monitoring and successfully managing their pathological conditions (focusing on obesity and type 2 diabetes) through the use of mobile devices. In this paper we report this study by discussing the following two important aspects of DIABESITY. (i) Dietary mHealth tools for home-patients; (ii) Measures to capture the psychological factors and processes which mediate change of behavior and affect initiation and maintenance phases.

Social media, online social networks and apps for smartphones and tablets are changing the way we communicate. According to a recent Pew Research Center survey, 73% of Internet users among US adults engage in social networking to access, create, and share contents (Duggan and Smith, 2013). The number of smartphone users is growing worldwide [56% of American adults are currently smartphone owners (Smith, 2013)], and millions of applications (most of them related to social media or other communication tools) are available on the Google Play or iTunes store. The increased prevalence of chronic (and non-communicable) diseases in high-income countries is today largely attributable to the convergence of an aging population with the persistence of several risk factors, including physical inactivity, use of tobacco and alcohol, high blood pressure and cholesterol, stress, depression, and overweight and obesity. Many of these risk factors can be mitigated by health interventions and education, and communication tools could support healthy lifestyle and behavior change. In the past years there was an increasing interest in the use of digital technologies to support these changes because they contribute to enhance levels of surveillance over behaviors and have the potential to provide acceptable and cost-effective interventions by transferring treatment, rehabilitation and prevention of a condition to self-care in the community (Castelnuovo et al., 2010). Social media and smartphone-based applications are now changing how people interact with the healthcare and public health systems (Santoro, 2013; Santoro and Quintaliani, 2013). The participatory, interactive nature of social media platforms allows for information to be generated and shared in a viral fashion, and provide new mechanisms to foster engagement and partnership with consumers, to change their behaviors and to fight against unhealthy lifestyles. More than 100,000 health related apps are also available in the market allowing people to record, track, and analyze vital signs and physical health data over time, to obtain feedback and general information about the disease they suffer from, and to receive alerts to remind them to take their medications or to measure their blood glucose levels. Due to their possible implications in public health a growing number of scientists suggests to incorporate social media and mobile health in health promotion and healthcare programs (Burke-Garcia and Scally, 2014).

Wearable devices have a significant impact on society, and recent advancements in modern sensor technologies are opening up new possibilities for healthcare applications. Continuous vital sign monitoring using Internet of Things solutions can be a crucial tool for emergency management, reducing risks in rescue operations and ensuring the safety of workers. The massive amounts of data, high network traffic, and computational demands of a typical monitoring application can be challenging to manage with traditional infrastructure. Cloud computing provides a solution with its built-in resilience and elasticity capabilities. This study presents a Cloud-based monitoring architecture for remote vital sign tracking of paramedics and medical workers through the use of a mobile wearable device. The system monitors vital signs such as electrocardiograms and breathing patterns during work sessions, and it is able to manage real-time alarm events to a personnel management center. In this study, 900 paramedics and emergency workers were monitored using wearable devices over a period of 12 months. Data from these devices were collected, processed via Cloud infrastructure, and analyzed to assess the system's reliability and scalability. The results showed a significant improvement in worker safety and operational efficiency. 42. Ten Years of Experience With a Telemedicine Platform Dedicated to Health Care Personnel: Implementation Report.

- Abstract
Background: Telemedicine, a term that encompasses several applications and tasks, generally involves the remote management and treatment of patients by physicians. It is known as transversal telemedicine when practiced among health care professionals (HCPs)., Objective: We describe the experience of implementing our telemedicine Eumeda platform for HCPs over the last 10 years., Methods: A web-based informatics platform was developed that had continuously updated hypertext created using advanced technology and the following features: security, data insertion, dedicated software for image analysis, and the ability to export data for statistical surveys. Customizable files called "modules" were designed and built for different fields of medicine, mainly in the ophthalmology subspecialty. Each module was used by HCPs with different authorization profiles., Implementation (results): Twelve representative modules for different projects are presented in this manuscript. These modules evolved over time, with varying degrees of interconnectivity, including the participation of a number of centers in 19 cities across Italy. The number of HCP operators involved in each single module ranged from 6 to 114 (average 21.8, SD 28.5). Data related to 2574 participants were inserted across all the modules. The average percentage of completed text/image fields in the 12 modules was 65.7%. All modules were evaluated in terms of access, acceptability, and medical efficacy. In their final evaluation, the participants judged the modules to be useful and efficient for clinical use., Conclusions: Our results demonstrate the usefulness of the telemedicine platform for HCPs in terms of improved knowledge in medicine, patient care, scientific research, teaching, and the choice of therapies. 46. Telepathology in emerging countries pilot project between Italy and Egypt.

- Abstract
Pathological examination includes gross & microscopic examinations at different magnification. Through the steps of examination, we obtain many images that can be used for telepathology. Telepathology is the practice of pathology at a distance, viewing images on a monitor rather than directly through a light microscope. It can be used for primary diagnosis, second opinion, quality assurance and distance learning. Telepathology is classified into Static, Dynamic, Hybrid and Whole Slide Imaging (WSI). We have a successful experience in Egypt in applying the static & dynamic techniques in a pilot project between the Italian Hospital in Cairo (NPO) and the Civico Hospital in Palermo. This project began in 2003 and continued till now. From the second year 2004, Ospedale S. Giovanni e Paolo Hospital in Venice, Charing Cross Hospital in London and the University of Pittsburgh Medical Center Health System (UPMC) in the USA participated actively in our project. During the past five years we consulted on many problematic pathological cases with these different specialized pathological centers in Italy, UK & USA. In addition to the highly specialized scientific value of consulting on the cases and exchanging knowledge, we saved a lot of time and money and succeeded in providing our patients with a better medical service.We are now in the process of establishing a Digital Telepathology Center (DTC) in the pathology department, Cairo University, using the latest technique of telepathology which is Whole Slide Imaging (WSI). We believe that it will help us to improve and extend diagnosis for our difficult pathological cases and will facilitate increased E-learning opportunities for staff and students both in Egypt and in the longer term in the wider Eastern Mediterranean.

The paper presents an interregional telemedicine project between hospitals from two countries that encourage good economic cooperation. It uses modern technology and diverse human resources that contribute to a better accomplishment of the goals of the project and insures continuity of the services after the ending of the project. It instantiates the vision of e-Europe regarding access to healthcare services and the increasing mobility of patients.

- Abstract
The aim of this paper was to describe exhaustively, but concisely, the matter of the health technology assessment. According to health politicians at present, the modernisation of the health care system is a continuous task of the health policy of governments. The core of this modernisation: to have information about the needs of the population and the patients, to ensure immaterial and material conditions of health care, to organise and direct the process of health care in the interests of the favourable results of the outcome of care activities. The fundamental conditions of the modernisation to update the health information systems are harmonised on an international level. The main goal of the bilateral program is: to develop and introduce health informatics telemedicine systems for transmission and integration of information systems and health data between structures in Hungary and Italy. The program contemplates doing problem-oriented electronic patients record structures with a flexible, interactive and high operation-speed knowledge basis network for health institutes of both countries.

- Abstract
Telematics and information technology are the base on which it will be possible to build an integrated health information system to support population and improve their quality of life. This system should be based on record linkage of all data based on the interactions of the patients with the health structures, such as general practitioners, specialists, health institutes and hospitals, pharmacies, etc. The record linkage can provide the connection and integration of various records, thanks to the use of telematic technology (either urban or geographical local networks, such as the Internet) and electronic data cards. Particular emphasis should be placed on the introduction of smart cards, such as portable health cards, which will contain a standardized data set and will be sufficient to access different databases found in various health services. The inter-operability of the social-health records (including multimedia types) and the smart cards (which are one of the most important prerequisites for the homogenization and wide diffusion of these cards at an European level) should be strongly taken into consideration. In this framework a project is going to be developed aiming towards the integration of various data bases distributed territorially, from the reading of the software and the updating of the smart cards to the complete management of the patients' evaluation records, to the quality of the services offered and to the health planning. The applications developed will support epidemiological investigation software and data analysis. The inter-connection of all the databases of the various structures involved will take place through a coordination center, the most important system of which we will call "record linkage" or "integrated database". Smart cards will be distributed to a sample group of possible users and the necessary smart card management tools will be installed in all the structures involved. All the final users (the patients) in the whole network of services involved will be monitored for the duration of the project. The system users will also include general practitioners, social workers, physicians, health operators, pharmacists, laboratory workers and administrative personnel of the municipality and of the health structures concerned.
