Introduction: In the treatment of inoperable patients with severe aortic stenosis or at high surgical risk, transcatheter aortic valve implantation (TAVI) is applied as an alternative to surgical aortic valve replacement. In our study, it was aimed to evaluate the factors affecting the length of stay in hospital of TAVI cases. Method: After obtaining the approval of the ethics committee, the patients who received TAVI between 09.06.2012-19.01.2018 in the Interventional Cardiology unit of Dokuz Eylül University were retrospectively screened. A total of 243 patients were included in the study. Demographic data, preoperative accompanying diseases and echocardiographic results of the cases, the type of anesthesia applied, the duration of postoperative coronary intensive care, and a hospital stay were recorded. The analysis of the data was carried out in the SPSS 22.0 statistical package program, Results: 53.5% of the patients who underwent transcatheter aortic valve implantation received general anesthesia, 46.5% received local anesthesia and sedation. 93.8% of the patients were discharged upon recovery. There was a significant difference between the postoperative length of stay in the hospital and the type of anesthesia (p=0.008). It was shown that patients who underwent general anesthesia were discharged later than patients who underwent sedation. Discussion and Conclusion: Due to the high risks of surgery, it is of vital importance to shorten the length of stay in hospital in high-risk patients over 65 years of age who undergo TAVI.

Objectives: To the our best knowledge, there is no literature on the effectiveness of YouTube on extracorporeal membrane oxygenation (ECMO) practice. in our study, we aimed to evaluate the characteristics and medical aspects of videos on YouTube about ECMO. Materials and Methods: in internet media website YouTube.com search engine; the word ECMO was searched in December 2018 without any filter. the listed first 100 videos were classified according to count of like, dislike, origin of country and types of ECMO. ECMO videos were categorized under three main topics as, educational purpose, patient follow-up and weaning from ECMO. the materials were evaluated in terms of intelligibility using the suitability assessment of materials (SAM). User participation measurements were obtained for each video. Results: in our study, it was observed that the number of videos, content variety and duration of ECMO contents that were uploaded to YouTube after 2016 increased significantly. However, it was found that there was no significant relationship between date and duration of SAM and there was no significant correlation between the duration of the video and the SAM score. However, it was determined that there was a positively weak correlation between the scores of the videos and the number of views. Conclusion: Medical education is forming from didactic education to a system which is problem-based, media and visually aided, and effectively using all of the educational tools. During this period, the use of sites such as YouTube is increasing among medical education sources. Considering that, the discussions may be based on insufficient information quality, patient confidentiality and legal issues. Thus, we think that the necessity of double-blind peer eviews is one of the important points to discuss in terms of content, educational direction, patient confidentiality and legal procedures in social media related to medical education.

Objective:This study aimed to determine and compare the effects of two different gastric residual volume (GRV) thresholds, 200 mL and 400 mL, on target calories and gastrointestinal intolerance in intensive care patients receiving enteral nutrition.Materials and Methods:Prospective data of 56 patients over 18 years of age who were expected to stay in the intensive care unit for more than three days were included in the study. Patients were randomized into two groups as GRV threshold 200 mL (group 1) and GRV 400 mL (group 2). Enteral Measurement of GRV was done every six hours. The feeding was initiated at a rate of 20 mL/h and was increased by 10 mL/h to reach the goal rate.Results:There was no significant difference between two groups regarding gender, age, APACHE II score, body mass index, length of stay in intensive care unit, admission reason, and co-morbidities. There was also no significant difference in the incidence of GRV, vomiting, diarrhea and intolerance. Time to target calories was 24 hours in group 1 and 26 hours in group 2, and no significant difference was found between the two groups.Conclusion:In this study, there was no significant difference between 200 mL and 400 mL GRV regarding time to target calories and incidence of gastrointestinal complications in critical intensive care unit patients.

Objective This study aimed to determine the levels of herbal product use among couples in vitro fertilisation (IVF) treatment in Turkey and the status of questioning in terms of herbal product usage. Methods After ethics committee approval, the study included 257 people, which were the couples receiving treatment in Dokuz Eylul University IVF units from 1st August, 2018, to 28th February, 2019. The couples were given 16-questions survey form to determine their herbal product use. Vitamin and herbal product use in the past 3 months was separately questioned. Results Significant differences were identified between women and men only in terms of age group. The rate of answering yes to the question about herbal product use in the study group was 13.6%, whereas 40.8% participants marked at least 1 item on the list of herbal products. Most people stated that they used herbal products by increasing the amounts in daily consumption. The top 5 products included onion, garlic, thyme, cinnamon and carob. Of the female patients being treated with IVF, 62.5% used vitamin supplements and 48.6% used herbal products. Of the men undergoing the same treatment (partners of the female participants), 37.5% used vitamin supplements and 51.4% used herbal products. Conclusion Asking questions to patients receiving infertility treatment or anaesthesia is important in terms of medication interactions and treatment success. For a laborious and costly treatment, such as IVF, detailed history should be taken and herbal product used and cessation times must be searched in detail.

Purpose: This study aims to compare the perceptions of nurses and families on the needs of the relatives of the patients in Intensive Care Unit (ICU). Methods This cross-sectional study was conducted in the ICU of a university hospital. The study comprised 213 critical care patients’ relatives and 54 nurses working in the same ICU. Data were collected using the Turkish version of Critical Care Family Needs Inventory (CCFNI) and a questionnaire on the characteristics of the participants. The difference between the perceptions of families and nurses was analyzed using Student t-test. Results: CCFNI’s assurance/proximity subscale mean scores ranked first among bothpatients and nurses. The item “To be assured the best care possible is being given to the patient” was the top priority for both groups. Mean assurance/proximity and information dimensions of relatives were significantly higher compared to nurses (p p Conclusion The needs of the relatives of patients are underestimated by nurses. This inhibited the performance of ICU nurses in line with the holistic care approach. Educational objectives that include the needs of ICU patients’ relatives should be incorporated into the undergraduate and in-service training of nurses. Policies should be established to create space and time for effective relative-nurse communication.

BACKGROUND/AIMS Bicuspid aortic valve disease may cause both aortic stenosis and regurgitation; therefore, it manifests as different symptoms. Aortic stenosis in patients with bicuspid aortic valve exhibits ambiguous hemodynamic effects; moreover, it may remain asymptomatically until the effective orifice area is narrowed below 1.5-2 cm(2). In this study, we aimed to evaluate 100 articles related to bicuspid aortic valve that have been most frequently cited in the literature. MATERIAL and METHODS The study was performed using advanced mode of the search engine server Institute for Scientific Information Web of Science. For this purpose, the words TS=bicuspid or TI=bicuspid were reviewed. The search was carried out, and the 100 most cited articles were determined. The total and annual cite count for each article and information about the authors and the journals were determined via WOS and PubMed. RESULTS The mean cite count of the 100 most cited articles was found to be 238.10 +/- 227.48. The annual cite counts of the studies varied from 4 to 185, and the mean value was calculated as 20.93 +/- 25.25. The first 3 subjects were clinical BAV (41%), pathology of BAV (15%), and genetics of BAV (15%). There was no statistically significant difference between the continent of the corresponding author and overall and annual cite counts. In addition, no statistically significant difference was observed between the journal's continent and overall and annual cite counts (p>.05). CONCLUSION This study is the first in the literature to identify the 100 most cited articles related to BAV. The majority of these studies were based on clinical evaluation of BAV. We found an increase in the number of studies on percutaneous transcatheter interventions over the last 5 years.

WOS: 000388699700011, PubMed ID: 27909608, Objective: In this study, two enteral nutrition protocols with different gastric residual volumes (GRVs) and different monitoring intervals were compared with respect to gastrointestinal intolerance findings in intensive care unit (ICU) patients. Methods: The study was carried out prospectively in 60 patients in the anaesthesiology and reanimation ICU under mechanical ventilation support, who were scheduled to take enteral feeding. Patients were sequentially divided into two groups: Group 1, GRV threshold of 100 mL, and monitoring interval of 4 hours, and Group 2, GRV threshold of 200 mL, monitoring interval of 8 hours. To test the significant difference between the groups, Student's t test, chi-square text and Fisher exact test were used. Results: In Group 1, 3.3% vomiting, 6.6% diarrhoea was observed; in Group 2, 16.6% vomiting, 10% diarrhoea. In terms of total intolerance (vomiting and/or diarrhoea) of the two groups, the incidence was significantly higher in Group 2 (33.3%) than in Group 1 (10%) (p=0.02). Conclusion: According to the results of the study, a lower gastrointestinal intolerance rate was detected in the GRV threshold 100 mL, monitoring interval for 4 hours protocol (Group 1) than in GRV threshold 200 mL, monitoring interval for 8 hours protocol (Group 2); Group 1 may be preferred renovation.

Methyl bromide (CH3Br) is a halogenated aliphatic hydrocarbon that may cause acute and chronic toxicities. We describe a case of a 44-year-old male patient who developed toxic brain syndrome (TBS) and central nervous system (CNS) toxicity after exposure to CH3Br by inhalation. Toxicity began with progressive nervousness, dysarthria and coordination disorder. The complaints on admission to the hospital were speech defect, balance disorder, consciousness disorder and involuntary movements. The patient was treated symptomatically in the intensive care unit (ICU), and organic reasons were excluded. Findings in the magnetic resonance imaging were considered secondary demyelination related to systemic intoxication. Because of the CH3Br, alkylates the crucial sulfhydryl-containing enzymes, N-acetylcysteine was used as a source of sulfhydryl groups for the treatment of the patient. He was hospitalised for nearly 1.5 months in the ICU.

Yoğun bakım ünitelerinin karmaşık ortamında hasta yakınlarının gereksinimleri düşük öncelikli olarak görülebilir. Diğer yandan hastalarının durumlarının ciddi ve değişken olmasından dolayı hasta yakınlarının stres düzeyi oldukça yüksektir. Bu çalışmanın amacı, yoğun bakım hasta yakınlarının gereksinimlerini ölçen Yoğun Bakım Ünitesindeki Hastaların Yakınları için Gereksinim Ölçeği'nin (Critical Care Family Need Inventory - CCFNI) Türkçe'ye uyarlanması ve ölçeğin psikometrik özelliklerinin değerlendirilmesidir. Çalışma bir devlet hastanesinin yoğun bakım biriminde yatan 191 hastanın yakınları ile yürütülmüştür. Ölçeğin kapsam geçerliliği uzman görüşleri ile değerlendirilmiş, yapı geçerliliği açıklayıcı faktör analizi (AFA) ile incelenmiştir. İç tutarlılığın belirlenmesinde Cronbach alfa katsayısı kullanılmıştır. Türkçe'ye uyarlanan ölçeğin kapsam geçerlilik oranı kabul edilebilir en düşük düzeyden daha yüksek bulunmuştur. Ölçeğin yapı geçerliliği için AFA kulanılarak faktör yapısı ortaya konulmuştur. Tüm ölçek için 0.93 olan Cronbach alfa katsayısının, alt ölçekler için 0.80'den yüksek olması ölçeğin Türkçe versiyonunun güvenilirliğini kanıtlamaktadır. Türkçe'ye uyarlanan orjinal ölçekteki gereksinimlerin tüm boyutlarını uygun bir şekilde yansıtmaktadır. The translated inventory has a content validity ratio higher than the minimum acceptable level. Its construct validity was established by the EFA. Cronbach's alpha coeff icient for the entire scale was 0.93 and higher than 0.80 for subscales, thus demonstrating the translated version's reliability. The Turkish adaptation appropriately refl ects all dimensions of needs in the original CCFNI, and its psychometric properties were acceptable. The revised tool could be useful for helping critical care healthcare workers provide services in a holistic approach and for policymakers to improve quality of service.
