Abstract

A study was conducted to ascertain the success of a pay-for-performance scheme introduced in England in 2004 to improve the quality of clinical care. Results revealed that an improvement was observed in clinical care for chronic conditions included in the scheme but not much improvement was noticed in cases where conditions were not included in the scheme incentives.

Abstract

Patient preferences have recently been highlighted as a potential threat to the validity of randomised controlled trials (RCTs). Although there have been significant methodological and statistical developments in relation to these issues, comparatively little attention has been paid to the development of a conceptual model concerning preferences and their effects on decision-making. This article describes the development of such a model, which was undertaken in parallel with a systematic review of the empirical data concerning preference effects. The model describes the concept of preference in terms of theoretical concepts from the psychological and economics literature, and describes a preliminary model of the development and operation of preferences in the context of RCTs. The paper then examines the implications of the model for informed consent and recruitment procedures. Key issues for future research are also outlined. Abstract

To help account for and address observed variations in medical practice, evaluations of 'appropriateness' have sought to supplement incomplete evidence with professional opinion. This article contributes to an understanding and refinement of the construct of appropriateness by discussing how it has been defined and applied in studies of health care in general and prescribing in particular. We suggest that appropriateness is the outcome of a process of decision-making that maximises net individual health gains within society's available resources. This definition distinguishes between (in)appropriate prescribing, as an outcome, and (ir)rational prescribing as a process. To assess appropriateness, we advocate combining explicit criteria with independent review in cases of uncertainty and disagreement. Refinements based on reviews using implicit criteria should draw on shared professional knowledge and post hoc state the process followed as explicitly as possible. The Medication Appropriateness Index is shown to provide a solid foundation for identifying dimensions of prescribing appropriateness. Key words - appropriateness, health care, prescribing, rational

Abstract

A structural model of job satisfaction and quitting intentions is estimated using data from a survey of general practitioners in the UK. Previous research has used reduced form models, making the interpretation of coefficients problematic. The use of a structural recursive model helps to clarify the relationships between intentions to quit, overall job satisfaction, domains of job satisfaction and personal and job characteristics. Job and personal characteristics have a direct effect on job satisfaction in addition to their indirect impact through job satisfaction domains. Job satisfaction domains have a direct effect on intentions to quit, in addition to their effect via overall job satisfaction. The structural approach provides a richer interpretation of the role and effect of job characteristics on job satisfaction and intentions to quit than is found in previous research. This is particularly relevant in some public sector labour markets, where the opportunity to alter wages to compensate for the relative advantages and disadvantages of jobs is limited because of national wage bargaining.
