Abstract

The measurement of muon energy is critical for many analyses in large Cherenkov detectors, particularly those that involve separating extraterrestrial neutrinos from the atmospheric neutrino background. Muon energy has traditionally been determined by measuring the specific energy loss (dE/dx) along the muon's path and relating the dE/dx to the muon energy. Because high-energy muons (E_mu > 1 TeV) lose energy randomly, the spread in dE/dx values is quite large, leading to a typical energy resolution of 0.29 in log10(E_mu) for a muon observed over a 1 km path length in the IceCube detector. In this paper, we present an improved method that uses a truncated mean and other techniques to determine the muon energy. The muon track is divided into separate segments with individual dE/dx values. The elimination of segments with the highest dE/dx results in an overall dE/dx that is more closely correlated to the muon energy. This method results in an energy resolution of 0.22 in log10(E_mu), which gives a 26% improvement. Abstract

In cosmic ray air showers, the muon lateral separation from the center of the shower is a measure of the transverse momentum that the muon parent acquired in the cosmic ray interaction. IceCube has observed cosmic ray interactions that produce muons laterally separated by up to 400 m from the shower core, a factor of 6 larger distance than previous measurements. These muons originate in high pT (> 2 GeV/c) interactions from the incident cosmic ray, or high-energy secondary interactions. The separation distribution shows a transition to a power law at large values, indicating the presence of a hard pT component that can be described by perturbative quantum chromodynamics. However, the rates and the zenith angle distributions of these events are not well reproduced with the cosmic ray models tested here, even those that include charm interactions. This discrepancy may be explained by a larger fraction of kaons and charmed particles than is currently incorporated in the simulations.

Abstract

IceTop, the surface component of the IceCube Neutrino Observatory at the South Pole, is an air shower array with an area of 1 km2. The detector allows a detailed exploration of the mass composition of primary cosmic rays in the energy range from about 100 TeV to 1 EeV by exploiting the correlation between the shower energy measured in IceTop and the energy deposited by muons in the deep ice. In this paper we report on the technical design, construction and installation, the trigger and data acquisition systems as well as the software framework for calibration, reconstruction and simulation. Abstract

In health education and behavior change interventions, process tracking monitors the delivery of an intervention and its receipt to the intended audience. A randomized controlled trial in the state of Bihar, India was conducted to help school teachers become tobacco free through appropriately designed intervention program and delivery system. We describe the results from process tracking of this intervention delivery. The intervention program was centred on six topics delivered in each school through 12 sessions over 6 successive months. The program deliverers recorded the process measures as total number of sessions and program-components implemented (fidelity); time spent conducting sessions (dose) and proportion of teachers attending at least one session (reach). The outcome measures (teachers' exposure to intervention messages and tobacco policy adoption) were assessed post-intervention. All 12 sessions were delivered in 33 out of 36 schools. Thirty-one schools implemented all six program components. In 18 schools, =95% of the teachers participated in one or more sessions. Thirty-three schools received 12 or more hours of dose. In 29 schools, 100% teachers reported exposure to all program messages. Tobacco policy was adopted by all schools. Thus, the intervention was generally delivered as planned and it had a positive impact on teachers and schools.

28. Difficulties in implementing an organized screening program for breast cancer in Brazil with emphasis on diagnostic methods.

Abstract

Introduction: Breast cancer is the most common type of cancer among women, and the leading cause of cancer deaths worldwide. Among early detection methods, screening by mammography has been used in most developed countries as gold standard. The goal of this study was to evaluate the difficulties and opportunities in implementing breast cancer screening in Brazil, with an emphasis on the diagnostic methods used according to stage distribution. Methods: Between 2007 and 2009, 248 women were diagnosed with breast cancer in the Barretos region. Most of these were interviewed in their homes using a questionnaire with sociodemographic and preventive breast cancer screening questions. All other data were obtained from Barretos Cancer Hospital (BCH) medical records. Results: The screening program conducted by BCH was responsible for 46.1% of diagnosed cases, with 30.1% of these referred from the private system and 23.8% from the public system. Among asymptomatic women screened by the BCH Screening Program 70.8% had clinical stage 0-I disease, compared with 58.1% in the private and 50% in the public systems. Monthly breast selfexamination was reported by 48.5% of the women. Clinical breast examinations were regularly performed by 88.9% of gynecologists in the private and 40.7% in the public health systems. Only 5.6% of the women reported difficulty in accessing mammography and this was most frequently due to fear of the disease or lack of knowledge about mammography in asymptomatic women. Conclusion: This breast cancer screening program resulted in a substantial number of patients presenting with clinical stage (CS) 0-I disease. The success of this program was due to intensive community interventions, free mammography, and the availability of health care and mammography close to patients' homes. [ABSTRACT FROM AUTHOR]

Abstract

The article focuses on the effects of androgens on menopause and the effectiveness of androgen therapies in the management of menopausal women. It states that dehydroepiandrosterone (DHEA), androstenedione and total testosterone begin to decline until menopause when the levels of sex hormones drop. It reveals that increase of androgens in comparison to estrogens creates increased risk for metabolic syndrome, cardiovascular disease and possibly breast cancer. Meanwhile, absolute reduction in the levels of androgen is associated with a loss of libido, sexual satisfaction, and personal well-being. It discloses that early studies show consistent effectiveness of androgens in alleviating libidinal problems but they may not be the appropriate treatments for menopausal-associated problems.

33. Young children in foster care: multiple vulnerabilities and complex service needs.

Young children who have been removed from their biological families and placed in foster care are at significant risk for poor developmental outcomes. Their vulnerability is often the result of adverse biological and psychosocial influences: prenatal exposure to alcohol and other drugs, premature birth, abuse and neglect leading to foster placement, and failure to form adequate attachments to their primary caregivers. Children younger than 6 years form the largest group entering foster care, and remain longest in care. Meeting the complex needs of this vulnerable group of young children and their families presents extensive challenges for early intervention service systems. The purpose of the following discussion is to describe the foster care population and the kinds of medical conditions, mental health problems, and developmental disabilities experienced by young children in foster care, and to explore implications for intervention. By increasing their understanding of risk factors, vulnerabilities, and complex service needs, early childhood professionals can become effective advocates and provide services that ameliorate risk and optimize outcomes for these children and their families. Today, clinicians are challenged to address a woman's contraceptive needs during her reproductive and perimenopausal years and then provide her with a menopausal therapeutic option. This option should offer optimal symptom relief, noncontraceptive health benefits, and a good tolerability profile. The benefits of hormone replacement therapy include control of vasomotor symptoms, reduction of vulvovaginal atrophy, and protection against osteoporosis. Research also points to emerging hormone replacement therapy benefits such as protection against cardiovascular disease, colon cancer, and Alzheimer's disease. One of the primary considerations in the transition from oral contraceptive use to hormone replacement therapy is the tolerability profile of the progestin component of the hormone replacement therapy. Because progestin-related side effects are among the main reasons for discontinuation of hormone replacement therapy, the selection of a formulation that contains the same well-tolerated progestin as in the woman's oral contraceptive can be particularly important to successful use of hormone replacement therapy. Currently in the United States continuous combined hormone replacement therapy is available in 3 formulations and 1 continuous estrogen/intermittent progestin formulation. Although direct comparative trials are lacking, available data suggest that the new, continuous 17beta-estradiol/intermittent norgestimate hormone replacement therapy formulation may offer advantages over regimens that contain older progestins with metabolic disadvantages. Abstract

Oral contraceptives are one of the most highly effective forms of contraception and provide many short- and long-term noncontraceptive health benefits. They control menstrual cycle irregularities, such as breakthrough bleeding and amenorrhea, and are effective in treating dysfunctional uterine bleeding. In addition, for decades after oral contraceptive use is discontinued they are associated with substantial decreases in the risk of ovarian cancer (up to 80%) and of endometrial cancer (40%-50%), and nearly eliminate benign functional ovarian cysts. Long-term oral contraceptive use confers protection against benign breast disease and colorectal cancer, may help prevent rheumatoid arthritis, decreases ectopic pregnancy and hospitalizations for pelvic inflammatory disease, and helps preserve bone mineral density to reduce risk of fractures. Large bodies of evidence from extensive research have clarified the perceived association of oral contraceptive use with cardiovascular disease and with breast cancer. Findings indicate that there is no increased risk of myocardial infarction or stroke associated with oral contraceptive use in healthy, nonsmoking, normotensive women. Although there is a 3- to 4-fold increased risk of venous thromboembolism with current oral contraceptive use, the absolute risk is very small and is half that associated with pregnancy. Women of all reproductive ages, including perimenopausal women, can realize many health benefits through oral contraceptive use, including improved health status later in life. Abstract

Objective: Our purpose was to assess the feasibility of primed in situ labeling for analysis of prenatal diagnostic specimens.Study Design: Prenatal diagnostic specimens were chosen at random for analysis without knowledge of clinical indication. Primed in situ labeling with primers for chromosomes 18, 21, X, and Y was performed separate from conventional cytogenetic analyses. All clinical management considerations were based solely on conventional cytogenetic analyses.Results: Forty-one samples were analyzed by primed in situ labeling: 35 direct preparations of chorionic villi and 6 uncultured amniotic fluid samples. In all cases analysis confirmed the particular chromosome number determined by conventional cytogenetic analysis.Conclusions: Although conventional metaphase studies remain the standard for prenatal cytogenetic analyses, the preliminary feasibility study finds primed in situ labeling to be a rapid and reliable adjunctive diagnostic technique applicable for prenatal diagnosis in certain clinical situations. Further study is needed to assess the efficacy of primed in situ labeling in comparison to fluorescent in situ hybridization and conventional cytogenetic analyses for prenatal diagnoses. Abstract

Objective: Our purpose was to determine the risk of fetal mosaicism when placental mosaicism is found on chorionic villus sampling.Study Design: We present a case of mosaic trisomy 22 detected on chorionic villus sampling and subsequently found in the fetus. A review of comprehensive chorionic villus sampling studies with emphasis on follow-up for fetal mosaicism was conducted.Results: Among 13 studies reviewed, 469 cases of placental mosaicism are presented; fetal mosaicism was found in 50 (10.7%). Factors associated with fetal mosaicism are (1) mosaicism on mesenchymal core culture and (2) type of chromosome abnormality involved--specifically, marker chromosomes (26.7%) and common autosomal trisomies (19.0%). Amniocentesis predicted fetal genotype in 93% to 100% of cases of placental mosaicism, depending on the cell type in which mosaicism was diagnosed.Conclusions: Although mosaicism is usually confined to the placenta, the fetus is involved in about 10% cases.
