Objective: To investigate the effect of disk training and tape application on diminished proprioception after Lateral Ankle Sprain (LAS) in active populations.\ud Eligibility criteria: Only clinical trials investigating the effectiveness of disk training and ankle tape on proprioception deficits following LAS by assessing JPS or kinaesthesia were included. \ud Information sources: Electronic databases of PubMed, MEDLINE, SPORTDiscus, CINAHL, Web of Science, Cochrane and PEDro were searched. The main search was conducted in February 2022.\ud Risk of bias: The physiotherapy Evidence Database (PEDro) scale was utilised to assess the methodological quality of each article.\ud Result: The search yielded six studies investigating the effects of disk training and four studies investigating the effects of inelastic tape. Five articles showed a significant improvement on JPS after disk training. However, no difference across different intervention groups was observed in one study. Only one trial presented an immediate significant improvement when taped, while another study showed that external ankle supports significantly improved JPS after 2 weeks and after 2 months. Three of four studies found no immediate improvement on proprioceptive deficits by the use of tape.\ud Conclusions: This review found that disk training may improve impaired proprioception after LAS, whilst the efficacy of inelastic tape was not evident due to methodological quality of the few available studies. Further studies are needed to establish whether these interventions can be used clinically with sufficient evidence.

Nevertheless in breast conserving surgery, the dissection of inferior part of breast mound will cause severe deformity, which affects the patient’s quality of life adversely. We have used an infra-mammary adipofascial tissue to repair the defect, and breast mound could be reconstructed concurrently in primary breast cancer surgery. From September 2009 to June 2012, we performed the infra-mammary adipofascial tissue repair (IATR) on 25 patients with breast cancer in inferior site of their breast mound. Surgeon makes the skin incision on the infra-mammary line and performs the quadrantectomy as planned. After verifying its negative surgical margin pathologically, we mobilize the adipofascial fat tissue associated with myofascia of the serratus anterior muscle from the infra-mammary area. This tongue-shaped flap is reflected back, and fills the defect of breast mound. We performed routinely examination, mammography, and ultra-sound echogram to the IATR patients who visited our hospital after 1 year from the end of treatment. We, furthermore, performed the QOL questionnaire (QOL-ACD and QOL-ACD-B) to them and retrospectively compared their data to that from the other breast surgery patients. All patients with IATR were treated with irradiation, and no image findings of a partial or total necrosis of the flap recognized 1 year after examination. They produced good cosmetic results; however, they could not elevate the QOL score on the whole.

Delirium is a common and serious acute neuropsychiatric syndrome characterized by inattention and global cognitive dysfunction. Delirium is associated with higher morbidity, higher mortality and longer hospitalization, but its aetiology remains unclear. We successfully treated five cases of delirium within 1 day with ramelteon, a novel selective melatonin receptor agonist. This suggests that correction of the circadian rhythm disturbance, one of the main symptoms of delirium, plays a crucial role in its treatment and sheds new light on a therapeutic strategy for treatment of delirium.

Reported results of coarse-grained molecular dynamics simulations rationalize the effect of water on the phase-segregated morphology of Nafion ionomers. We analyzed density maps and radial distribution functions and correlated them with domain structures, distributions of protogenic side chains, and water transport properties. The mesoscopic structures exhibit spongelike morphologies. Hydrophilic domains of water, protons, and anionic side chains form a random three-dimensional network, which is embedded in a matrix of hydrophobic backbone aggregates. Sizes of hydrophilic domains increase from 1 to 3 nm upon water uptake. At low water content, hydrophilic domains are roughly spherical and poorly connected. At higher water content, they convert into elongated cylindrical shapes with high connectivity. Further structural analysis provides a reasonable estimate of the percolation threshold. Radial distribution functions from coarse-grained and atomistic molecular dynamics models exhibit a good agreement. Water cluster size distributions from coarse-grained molecular dynamics and dissipative particle dynamics are consistent with small angle x-ray scattering data. Moreover, we calculated the water diffusivity by molecular dynamics methods and corroborated the results by comparison with pulsed field gradient NMR.

BACKGROUND Patients with HER2-positive breast cancer, who received paclitaxel (P) followed by epirubicin at 75 mg/m2, fluorouracil, and cyclophosphamide (FEC75) and concurrent trastuzumab (Trastuzumab Group) show good cardiac tolerability. We assessed left ventricular ejection fraction (LVEF) of these patients regularly, and compared with that of HER2-negative breast cancer patients who administered P followed by FEC100 (Standard Group), and followed for more one year, and address the longer-term issues in LVEF. OBJECTIVE & METHODS We routinely assessed LVEF, at the time of initiation, after P, after FEC, and after 1 year, and compared them between 49 patients in Trastuzumab Group and 45 patients in Standard Group. RESULTS In Trastuzumab Group, LVEF was reduced from the initial level (63.1%) to 60.4 at ``After FEC'' (p= 0.007), but had recovered to 60.9% at ``After 1 year''. A two-way repeated-measures ANOVA demonstrated a significant decline in LVEF level along the time course (p< 0.002), but there was no interaction revealed between time course of LVEF and treatment with or without trastuzumab (p= 0.834). In addition, there was no significant difference between groups (p= 0.386)CONCLUSIONS: P followed by FEC75 with concurrent trastuzumab could provide enough evidence of cardiac safety.
