Leishmaniasis is caused by protozoan parasites of the genus Leishmania and is a major health problem in various parts of the world. Cutaneous leishmaniasis (CL) occurs, among others, in unpredictable outbreaks after wars and disasters. After the last war in Iraq, the collapse of the health system led to the spread of infectious diseases, including CL. Between September 2016 and November 2017, all patients with confirmed CL having one or more skin lesion(s) were referred to a regional health center in Musol City within the Rabeea District. During this period, 1539 patients visited the clinic. A total of 190 patients were excluded from the study because of poor follow-up. The treatment success rate was 97.7% (1319/1349). Statistically significant associations were found between number of lesions and treatment failure (p = 0.0018; OR = 1.4430; CI = 1.1466-1.8161), number of doses and number of lesions (Pearson's correlation coefficient = 0.095; p = 0.001), and the lack of municipality services and number of doses used for the treatment (p = 0.008; OR = 1.0629; CI = 1.0158-1.1122). To conclude, the highest number of patients with CL in the city of Musol was recorded after the war in Iraq. The treatment success rate was high, which reflected the strict treatment and follow-up program. An urgent plan is needed to stop the spread of infection.

Orthopedic injuries can be misdiagnosed if physical examination is not feasible. The literature is abundant with different pattern of hand injuries. Carpometacarpal fracture dislocations constitute one of the rarest patterns of injuries of the hand. These injuries can often be missed because of the gross swelling and edema of the hand. The diagnosis of this unusual form of injury requires a high index of suspicion, vigilant examination and high-quality radiography. Apart from the routine antero-posterior and oblique radiographs, a lateral radiograph of the hand is essential for making an accurate diagnosis in suspected cases of carpometacarpal dislocations. Although many such case reports are described in the literature, we are reporting a rare case of dislocation involving all the five carpometacarpal joints which has never been reported in the literature till date.

Abstract Background Metabolic associated steatohepatitis (MASH) is metabolic disease that may progress to cirrhosis and hepatocellular carcinoma. Mouse models of diet-induced MASH, which is characterized by the high levels of fats, sugars, and cholesterol in diets, are commonly used in research. However, mouse models accurately reflecting the progression of MASH in humans remain to be established. Studies have explored the potential use of serological metabolites as biomarkers of MASH severity in relation to human MASH. Methods We performed a comparative analysis of three mouse models of diet-induced MASH in terms of phenotypic and metabolomic characteristics; MASH was induced using different diets: a high-fat diet; a Western diet; and a high-fat, high-cholesterol diet. Liver cirrhosis was diagnosed using standard clinical approaches (e.g., METAVIR score, hyaluronan level, and collagen deposition level). Mouse serum samples were subjected to nuclear magnetic resonance spectroscopy–based metabolomic profiling followed by bioinformatic analyses. Metabolomic analysis of a retrospective cohort of patients with hepatocellular carcinoma was performed; the corresponding cirrhosis scores were also evaluated. Results Using clinically relevant quantitative diagnostic methods, the severity of MASH was evaluated. Regarding metabolomics, the number of lipoprotein metabolites increased with both diet and MASH progression. Notably, the levels of very low-density lipoprotein (VLDL) and low-density lipoprotein (LDL) significantly increased with fibrosis progression. During the development of diet-induced MASH in mice, the strongest upregulation of expression was noted for VLDL receptor. Metabolomic analysis of a retrospective cohort of patients with cirrhosis indicated lipoproteins (e.g., VLDL and LDL) as predominant biomarkers of cirrhosis. Conclusions Our findings provide insight into the pathophysiology and metabolomics of experimental MASH and its relevance to human MASH. The observed upregulation of lipoprotein expression reveals a feedforward mechanism for MASH development that may be targeted for the development of noninvasive diagnosis.

Abstract Kerr-AdS d+1 black holes for d ≥ 3 suffer from classical superradiant instabilities over a range of masses above extremality. We conjecture that these instabilities settle down into Grey Galaxies (GGs) — a new class of coarse-grained solutions to Einstein’s equations which we construct in d = 3. Grey Galaxies are made up of a black hole with critical angular velocity ω = 1 in the ‘centre’ of AdS, surrounded by a large flat disk of thermal bulk gas that revolves around the centre of AdS at the speed of light. The gas carries a finite fraction of the total energy, as its parametrically low energy density and large radius are inversely related. GGs exist at masses that extend all the way down to the unitarity bound. Their thermodynamics is that of a weakly interacting mix of Kerr-AdS black holes and the bulk gas. Their boundary stress tensor is the sum of a smooth ‘black hole’ contribution and a peaked gas contribution that is delta function localized around the equator of the boundary sphere in the large N limit. We also construct another class of solutions with the same charges; ‘Revolving Black Holes (RBHs)’. RBHs are macroscopically charged SO(d, 2) descendants of AdS-Kerr solutions, and consist of ω = 1 black holes revolving around the centre of AdS at a fixed radial location but in a quantum wave function in the angular directions. RBH solutions are marginally entropically subdominant to GG solutions and do not constitute the endpoint of the superradiant instability. Nonetheless, we argue that supersymmetric versions of these solutions have interesting implications for the spectrum of supersymmetric states in, e.g. N $$ \mathcal{N} $$ = 4 Yang-Mills theory.

Background Patients undergoing neoadjuvant chemoradiation for oesophageal cancer often experience dehydration from decreased fluid intake and increased losses. Despite frequent clinical visits during treatment, patients can still present with dehydration, suggesting the need for increased patient awareness and engagement around adverse event management at home. Evidence for benefits of self-monitoring may help motivate patients to engage proactively in their own care to improve their treatment experience.Methods We performed a randomised single-centre study of a urine colour self-monitoring card (UCC) during chemoradiation therapy for oesophageal cancer, compared with standard dietitian counselling. Primary outcome was self-efficacy as determined by the Self-Management Resource Centre Self-Efficacy for Managing Chronic Disease Scale (SMCD). Secondary outcomes included Burge thirst scores, Edmonton Symptom Assessment System scores (ESAS), patient-initiated hydrations, creatinine rise and satisfaction with the UCC.Results Thirty-five patients were randomised. UCC use was not associated with improved SMCD or ESAS scores compared with standard counselling. The card was highly rated by patients as a welcome tool for self-monitoring.Conclusions No beneficial effect on self-efficacy or dehydration markers with UCC use was demonstrated. The study nonetheless drew attention to several factors potentially hindering its use for effective self-care: the unexpected severity of other symptoms consuming patients’ attention, reduced sensitivity of urine colour due to chemotherapy, absence of active inquiry by the healthcare team and the inconvenient location of the UCC in wallet/purse. Urine colour monitoring in patients with oesophageal cancer to improve the patient experience during treatment warrants further study but supported by active healthcare provider inquiry, more accessible format of the card, and possibly home vital checks to increase its sensitivity in the clinical context.

Manufacturing titanium alloys with simultaneous enhancement in strength and ductility has motivated extensive research into various strategies for regulating the arrangement and texture of α and β phases. The present study explores a novel α + β titanium alloy, TIMETAL 639 (Ti-639), produced by replacing a portion of vanadium in Ti-64 with molybdenum. The low diffusivity and β-stabilizing effects of molybdenum help retain bimodal characteristics within solution heat-treated Ti-639 microstructures. EBSD and TEM were used to examine β-phase evolution after thermal processing and recrystallization of new globular α grains within pre-existing colonies in a depleted bimodal microstructure. These depleted bimodal colonies in solution heat-treated Ti-639 also led to lower misorientation spreads and dislocation densities within neighboring primary α grains. Quasi-static compression along the plate normal direction demonstrated the ability of the depleted bimodal microstructure to simultaneously enhance strength and ductility in Ti-639 (~90 MPa stronger, ~6% higher failure strain) versus identically processed Ti-64. Only one solution heat-treatment step (1 h at 900 °C) is needed to achieve these properties in Ti-639, whereas comparable properties in Ti-64 required prolonged aging heat treatment (24 h at 600 °C) after the same solution heat-treatment step, making Ti-639 a viable α + β alloy candidate.

Background: Medical assistance in dying (MAID) was legislatively enacted in Canada in June 2016. Most studies of patients who received MAID grouped patients with cancer and non-cancer diagnoses. Our goal was to analyze the characteristics of oncology patients who received MAID in a Canadian tertiary care hospital. Methods: We conducted a retrospective review of all patients with cancer who received MAID between June 2016 and July 2020 at London Health Sciences Centre (LHSC). We describe patients’ demographics, oncologic characteristics, symptoms, treatments, and palliative care involvement. Results: Ninety-two oncology patients received MAID. The median age was 72. The leading cancer diagnoses among these patients were lung, colorectal, and pancreatic. At the time of MAID request, 68% of patients had metastatic disease. Most patients (90%) had ECOG performance status of 3 or 4 before receiving MAID. Ninety-nine percent of patients had distressing symptoms at time of MAID request, most commonly pain. One-third of patients with metastatic or recurrent cancer received early palliative care. The median time interval between the first MAID assessment and receipt of MAID was 7 days. Interpretation: Most oncology patients who received MAID at LHSC had poor performance status and almost all had distressing symptoms. The median time interval between first MAID assessment and receipt of MAID was shorter than expected. Only one-third of patients with metastatic or recurrent cancer received early palliative care. Improving access to early palliative care is a priority in patients with advanced cancer. The high-velocity impact of powder particles in cold-spray additively manufactured (CSAM) parts creates intersplat boundaries with regions of high dislocation densities and sub-grain structures. Upon microstructure and mechanical characterization, CSAM Aluminum 6061 showed non-uniformity with spatial variation in the microstructure and mechanical properties, affecting the overall response of the additively manufactured parts. Post-processing treatments are conducted in as-printed samples to improve particle bonding, relieve residual stresses, and improve mechanical properties. In this work, we attempt to implement the effects of grain size and distribution of smaller grains along the intersplat boundaries using the grain size distribution function and powder size information to accurately predict the deformation response of cold-sprayed material using a mean-field viscoplastic self-consistent (VPSC) model. The incorporation of an intersplat boundary term in the VPSC model resulted in a stress–strain response closely matching the experimental findings, preventing the superficially high stresses observed due to Hall–Petch effects from ultra-fine-grain structures. Likewise, the results from the grain analysis showed the combined effects of grain size, orientation, and intersplat mechanisms that captured the stresses experienced and strain accommodated by individual grains.

Abstract We demonstrate that the known expressions for the thermal partition function of large N Chern-Simons matter theories admit a simple Hilbert space interpretation as the partition function of an associated ungauged large N matter theory with one additional condition: the Fock space of this associated theory is projected down to the subspace of its quantum singlets i.e. singlets under the Gauss law for Chern-Simons gauge theory. Via the Chern-Simons / WZW correspondence, the space of quantum singlets are equivalent to the space of WZW conformal blocks. One step in our demonstration involves recasting the Verlinde formula for the dimension of the space of conformal blocks in SU(N) k and U(N) k,k′ WZW theories into a simple and physically transparent form, which we also rederive by evaluating the partition function and superconformal index of pure Chern-Simons theory in the presence of Wilson lines. A particular consequence of the projection of the Fock space of Chern-Simons matter theories to quantum (or WZW) singlets is the ‘Bosonic Exclusion Principle’: the number of bosons occupying any single particle state is bounded above by the Chern-Simons level. The quantum singlet condition (unlike its Yang-Mills Gauss Law counterpart) has a nontrivial impact on thermodynamics even in the infinite volume limit. In this limit the projected Fock space partition function reduces to a product of partition functions, one for each single particle state. These single particle state partition functions are q-deformations of their free boson and free fermion counterparts and interpolate between these two special cases. We also propose a formula for the large N partition function that is valid for arbitrary finite volume of the spatial S 2 and not only at large volume.

Background and study aims We assessed sessile serrated lesion detection rate (SSLDR) at a large academic medical center from 2008 to 2020 and modeled a local, aspirational target SSLDR. We also assessed SSLDRs among all gastroenterology fellows to better understand the relationship between SSLDRs and total colonoscopies performed. Patients and methods SSL-positive pathology results were flagged from a dataset composed of all screening colonoscopies for average-risk patients from 2008 to 2020. Unadjusted SSLDRs were calculated for individual endoscopists by year. A mixed effects logistic regression was used to estimate the log odds of SSL detection, with one model estimating division-wide predictors of SSL detection and a second model focused exclusively on colonoscopies performed by fellows. Model-adjusted SSLDRs were estimated for all 13 years and across both categories of all endoscopists and fellows only. Results Adjusted SSLDRs showed a consistent improvement in SSLDR from a low of 0.37 % (95 % confidence interval [CI]: 0.10–0.63) in 2008 to a high of 7.94 % (95 % CI: 6.34–9.54) in 2020. Among fellows only, the odds of SSL detection were significantly lower during their first year compared to their second year (OR: 0.80, 95 % CI: 0.66–0.98) but not significantly higher in their third year compared to their second year (OR: 1.09, 95 % CI: 0.85–1.4). Conclusions SSLDR increased steadily and significantly throughout our study period but variance among endoscopists persists. The peak SSLDR from 2020 of 7.94 % should serve as the local aspirational target for this division’s attendings and fellows but should be continuously reevaluated.

The “electron wind effect” has long been cited as a potential catalyst of solid-state transformations in metals, particularly when high current densities are involved. However, the literature exploring similar effects at lower current densities, such as those occurring during Gleeble thermomechanical simulation, remains scarce. The present work compares recrystallization activity in cold-rolled low-carbon steel during heat treatment by conventional furnace versus direct resistance heating (Gleeble). Multiple levels of cold work, annealing durations, and soak temperatures were examined, allowing for an in-depth comparison of recrystallization rates and activation energies between samples subjected to identical time–temperature profiles in the furnace and Gleeble. In addition to the expected increase in recrystallization behavior with the increases in temperature and cold-reduction levels, the use of the Gleeble system as the heating method resulted in faster initial microstructural transformation than a conventional furnace. The variability in recrystallized fractions persisted until the microstructures had saturated to their nearly fully recrystallized levels, at which point the microhardness and electron backscatter diffraction (EBSD) revealed convergence to equivalent behavior irrespective of the heating method. Analysis of the recrystallization kinetics by fitting to a JMAK relationship reflected the increased transformation activity during Gleeble treatment, with the value of the kinetic exponent also indicating greater grain growth activity at higher temperature.

Background Virtual care has emerged as an adjunctive response to challenges in rural health care, including maternity care, and use has accelerated during the coronavirus disease 2019 (COVID-19) pandemic. This gives rise to the need for a strategic plan for post-COVID-19 virtual maternity care in rural communities. To date, no provincial initiative has focused on understanding and documenting the needs of maternity care practitioners to provide virtual care. Methods Qualitative study, including virtual interviews and focus groups with rural primary maternity care providers and urban and rural specialists on perceptions of the utility of virtual maternity care pre- and post-COVID-19, and benefits and barriers of virtual care. Data were thematically analysed. Results In total, 82 health care providers participated in the study. Health care provider responses fell into three categories: Attributes of virtual care, barriers to virtual care and system interventions needed to optimize the provision of virtual perinatal care. Participants expressed a desire for use of virtual communication tools post-COVID-19, continued ability to use fee codes for virtual care and a need for more secure texting options. The benefits of tripartite consultations were noted by many participants; impacts of the transition to virtual care included additional workload and interrupted workflow. Concerns over the lack of physical examinations and challenges in building relationships with patients when providing virtual care were frequently noted. Conclusion Adapting the current implementation of virtual maternity care in British Columbia may be enhanced through several provider- and evidence-derived strategies, many of which are currently underway in BC. The results from this provincial survey will be used to focus further discussion on the characteristics of an optimal system to meet patient and provider needs within a rural context.

Abstract We study four-point functions of scalars, conserved currents, and stress tensors in a conformal field theory, generated by a local contact term in the bulk dual description, in two different causal configurations. The first of these is the standard Regge configuration in which the chaos bound applies. The second is the ‘causally scattering configuration’ in which the correlator develops a bulk point singularity. We find an expression for the coefficient of the bulk point singularity in terms of the bulk S matrix of the bulk dual metric, gauge fields and scalars, and use it to determine the Regge scaling of the correlator on the causally scattering sheet in terms of the Regge growth of this S matrix. We then demonstrate that the Regge scaling on this sheet is governed by the same power as in the standard Regge configuration, and so is constrained by the chaos bound, which turns out to be violated unless the bulk flat space S matrix grows no faster than s 2 in the Regge limit. It follows that in the context of the AdS/CFT correspondence, the chaos bound applied to the boundary field theory implies that the S matrices of the dual bulk scalars, gauge fields, and gravitons obey the Classical Regge Growth (CRG) conjecture.

Background: Diabetes mellitus is a serious metabolic disorder of multiple etiologies manifested by chronic hyperglycemia. The type 2 diabetes mellitus is characterized as multifactorial genetic syndrome, induced by mutations of different genes and environmental factors. Circulating homocysteine, a non- essential amino acid containing sulfur, is a broad biochemical marker for health/disease status diagnostics. The aim of our study is to evaluate circulating serum homocysteine levels in type 2 diabetes mellitus patients. Method: A cohort of 197 individuals randomly identified for this study. Of those, 148 individuals were diagnosed by consultants as type 2 diabetes mellitus and the rest 49 volunteer were normal controls group. In clinical chemistry laboratory, serum samples were analyzed for serum homocysteine, fasting blood sugar, glycated hemoglobin, serum creatinine, blood urea, blood urea nitrogen and lipid profile. Results: There was a significant difference in the HbA1c and sugar level between type 2 diabetes mellitus patients and control group. There was non-significant (P-value=0.32), the serum level of homocysteine was (13.6 ± 4.8, 12.5 ± 5.0 and 12.7 ± 6.2) in T2DM glycemic (controlled, poorly controlled and un controlled), respectively and (11.4 ± 4.8) in control group. Conclusions: Serum level of homocysteine was not differed significantly in type 2 diabetes mellitus when compared with control group. According to ages of T2DM and control group, there was no significant difference in serum level of homocysteine. There was significant difference between male and female in the level of serum homocysteine in glycemic uncontrolled group.

Abstract We generalize previously obtained results for the (all orders in the ’t Hooft coupling) thermal free energy of bosonic and fermionic large N Chern-Simons theories with fundamental matter, to values of the chemical potential larger than quasiparticle thermal masses. Building on an analysis by Geracie, Goykhman and Son, we present a simple explicit formula for the occupation number for a quasiparticle state of any given energy and charge as a function of the temperature and chemical potential. This formula is a generalization to finite ’t Hooft coupling of the famous occupation number formula of Bose-Einstein statistics, and implies an exclusion principle for Chern-Simons coupled bosons: the total number of bosons occupying any particular state cannot exceed the Chern-Simons level. Specializing our results to zero temperature we construct the phase diagrams of these theories as a function of chemical potential and the UV parameters. At large enough chemical potential, all the bosonic theories we study transit into a compressible Bose condensed phase in which the runaway instability of free Bose condensates is stabilized by the bosonic exclusion principle. This novel Bose condensate is dual to — and reproduces the thermodynamics of — the fermionic Fermi sea.

Abstract We explicitly construct every kinematically allowed three particle graviton- graviton-P and photon-photon-P S-matrix in every dimension and for every choice of the little group representation of the massive particle P. We also explicitly construct the spacetime Lagrangian that generates each of these couplings. In the case of gravitons we demonstrate that this Lagrangian always involves (derivatives of ) two factors of the Riemann tensor, and so is always of fourth or higher order in derivatives. This result verifies one of the assumptions made in the recent preprint [1] while attempting to establish the rigidity of the Einstein tree level S-matrix within the space of local classical theories coupled to a collection of particles of bounded spin.

Parsing the effect of slip-twin interactions on the strain rate and thermal sensitivities of Magnesium (Mg) alloys has been a challenging endeavor for scientists preoccupied with the mechanical behavior of hexagonal close-packed alloys, especially those with great latent economic potential such as Mg. One of the main barriers is the travail entailed in fitting the various stress–strain behaviors at different temperatures, strain rates, loading directions applied to different starting textures. Taking on this task for two different Mg alloys presenting different textures and as such various levels of slip-twin interactions were modeled using visco-plastic self-consistent (VPSC) code. A recently developed routine that captures dislocation transmutation by twinning interfaces on strain hardening within the twin lamellae was employed. While the strong texture was exemplified by traditional rolled AZ31 Mg alloys, the weak texture was represented by ZEK100 Mg alloy sheets. The transmutation model incorporated within a dislocation density based hardening model showed enhanced flexibility in predicting the complex strain rate and thermal sensitive behavior of Mg textures’ response to various mechanical loading schemes.

During oncogenesis, alterations in driver genes called driver alterations (DAs) modulate the transcriptome, methylome and proteome through oncogenic signaling pathways. These modulatory effects of any DA may be analyzed by examining differentially expressed mRNAs (DEMs), differentially methylated genes (DMGs) and differentially expressed proteins (DEPs) between tumor samples with and without that DA. We aimed to analyze these modulations with 12 common driver genes in Isocitrate Dehydrogenase 1 wildtype glioblastomas (IDH1-W-GBs). Using Cbioportal, groups of tumor samples with and without DAs in these 12 genes were generated from the IDH1-W-GBs available from “The Cancer Genomics Atlas Firehose Legacy Study Group” (TCGA-FL-SG) on Glioblastomas (GBs). For all 12 genes, samples with and without DAs were compared for DEMs, DMGs and DEPs. We found that DAs in PTEN were unassociated with any DEM or DMG in contrast to DAs in all other drivers, which were associated with several DEMs and DMGs. This contrasting PTEN-related property of being unassociated with differential gene expression or methylation in IDH1-W-GBs was unaffected by concurrent DAs in other common drivers or by the types of DAs affecting PTEN. From the lists of DEMs and DMGs associated with some common drivers other than PTEN, enriched gene ontology terms and insights into the co-regulatory effects of these drivers on the transcriptome were obtained. The findings from this study can improve our understanding of the molecular mechanisms underlying gliomagenesis with potential therapeutic benefits.

Abstract We study the space of all kinematically allowed four photon and four graviton S-matrices, polynomial in scattering momenta. We demonstrate that this space is the permutation invariant sector of a module over the ring of polynomials of the Mandelstam invariants s, t and u. We construct these modules for every value of the spacetime dimension D, and so explicitly count and parameterize the most general four photon and four graviton S-matrix at any given derivative order. We also explicitly list the local Lagrangians that give rise to these S-matrices. We then conjecture that the Regge growth of S-matrices in all physically acceptable classical theories is bounded by s 2 at fixed t. A four parameter subset of the polynomial photon S-matrices constructed above satisfies this Regge criterion. For gravitons, on the other hand, no polynomial addition to the Einstein S-matrix obeys this bound for D ≤ 6. For D ≥ 7 there is a single six derivative polynomial Lagrangian consistent with our conjectured Regge growth bound. Our conjecture thus implies that the Einstein four graviton S-matrix does not admit any physically acceptable polynomial modifications for D ≤ 6. A preliminary analysis also suggests that every finite sum of pole exchange contributions to four graviton scattering also violates our conjectured Regge growth bound, at least when D ≤ 6, even when the exchanged particles have low spin.

Abstract We study the theory of a single fundamental fermion and boson coupled to Chern-Simons theory at leading order in the large N limit. Utilizing recent progress in understanding the Higgsed phase in Chern-Simons-Matter theories, we compute the quantum effective potential that is exact to all orders in the ’t Hooft coupling for the lightest scalar operator of this theory at finite temperature. Specializing to the zero temperature limit we use this potential to determine the phase diagram of the large N N $$ \mathcal{N} $$ = 2 supersymmetric theory with this field content. This intricate two dimensional phase diagram has four topological phases that are separated by lines of first and second order phase transitions and includes special conformal points at which the infrared dynamics is governed by Chern-Simons theory coupled respectively to free bosons, Gross-Neveu fermions, and to a theory of Wilson-Fisher bosons plus free fermions. We also describe the vacuum structure of the most general N $$ \mathcal{N} $$ = 1 supersymmetric theory with one fundamental boson and one fundamental fermion coupled to an SU(N ) Chern-Simons gauge field, at arbitrary values of the ’t Hooft coupling.

Abstract We study large N 2+1 dimensional fermions in the fundamental representation of an SU(N) k Chern Simons gauge group in the presence of a uniform background magnetic field for the U (1) global symmetry of this theory. The magnetic field modifies the Schwinger Dyson equation for the propagator in an interesting way; the product between the self energy and the Greens function is replaced by a Moyal star product. Employing a basis of functions previously used in the study of non-commutative solitons, we are able to exactly solve the Schwinger Dyson equation and so determine the fermion propagator. The propagator has a series of poles (and no other singularities) whose locations yield a spectrum of single particle energies at arbitrary t’ Hooft coupling and chemical potential. The usual free fermion Landau levels spectrum is shifted and broadened out; we compute the shifts and widths of these levels at arbitrary t’Hooft coupling. As a check on our results we independently solve for the propagators of the conjecturally dual theory of Chern Simons gauged large N fundamental Wilson Fisher bosons also in a background magnetic field but this time only at zero chemical potential. The spectrum of single particle states of the bosonic theory precisely agrees with those of the fermionic theory under Bose-Fermi duality.

The availability of labelled training data is one of the practical obstacles towards wide application of machine learning models in medicine. Here the authors develop a weakly supervised deep learning model for the classification of aortic malformations using unlabelled cardiac MRI sequences from the UK biobank.

Background: Tuberculous meningitis (TBM) is the most lethal form of tuberculosis with a mortality of ~50% in those co-infected with HIV-1. Current antibiotic regimens are based on those known to be effective in pulmonary TB and do not account for the differing ability of the drugs to penetrate the central nervous system (CNS). The host immune response drives pathology in TBM, yet effective host-directed therapies are scarce. There is sufficient data to suggest that higher doses of rifampicin (RIF), additional linezolid (LZD) and adjunctive aspirin (ASA) will be beneficial in TBM yet rigorous investigation of the safety of these interventions in the context of HIV associated TBM is required. We hypothesise that increased dose RIF, LZD and ASA used in combination and in addition to standard of care for the first 56 days of treatment with be safe and tolerated in HIV-1 infected people with TBM. Methods: In an open-label randomised parallel study, up to 100 participants will receive either; i) standard of care (n=40, control arm), ii) standard of care plus increased dose RIF (35mg/kg) and LZD (1200mg OD for 28 days, 600mg OD for 28 days) (n=30, experimental arm 1), or iii) as per experimental arm 1 plus additional ASA 1000mg OD (n=30, experimental arm 2). After 56 days participants will continue standard treatment as per national guidelines. The primary endpoint is death and the occurrence of solicited treatment-related adverse events at 56 days. In a planned pharmacokinetic (PK) sub-study we aim to assess PK/pharmacodynamic (PD) of oral vs IV rifampicin, describe LZD and RIF PK and cerebrospinal fluid concentrations, explore PK/PD relationships, and investigate drug-drug interactions between LZD and RIF. Safety and pharmacokinetic data from this study will inform a planned phase III study of intensified therapy in TBM. Clinicaltrials.gov registration: NCT03927313 (25/04/2019)
