Abstract

High levels of human immunodeficiency virus (HIV) DNA in peripheral blood mononuclear cells (PBMCs), and specifically within CD14+ blood monocytes, have been found in HIV-infected individuals with neurocognitive impairment and dementia. The failure of highly active antiretroviral therapy (HAART) to eliminate cognitive dysfunction in HIV may be secondary to persistence of HIV-infected PBMCs which cross the blood-brain barrier, leading to perivascular inflammation and neuronal injury. This study assessed brain cortical thickness relative to HIV DNA levels and identified, we believe for the first time, a neuroimaging correlate of detectable PBMC HIV DNA in subjects with undetectable HIV RNA. Cortical thickness was compared between age- and education-matched groups of older (>40 years) HIV-seropositive subjects on HAART who had detectable (N = 9) and undetectable (N = 10) PBMC HIV DNA. Statistical testing revealed highly significant (P < 0.001) cortical thinning associated with detectable HIV DNA. The largest regions affected were in bilateral insula, orbitofrontal and temporal cortices, right superior frontal cortex, and right caudal anterior cingulate. Cortical thinning correlated significantly with a measure of psychomotor speed. The areas of reduced cortical thickness are key nodes in cognitive and emotional processing networks and may be etiologically important in HIV-related neurological deficits.

40. Characteristics of activated monocyte phenotype support R5-tropic human immunodeficiency virus.

Abstract

Background: Microbial translocation has been recognized as an important factor in monocyte activation and contributing to AIDS pathogenesis with elevated plasma lipopolysaccharide (LPS) levels, as a marker for microbial translocation, seen in advanced HIV disease. Therefore, the current study was undertaken to assess monocyte activation in vitro by LPS and to determine its impact on monocyte phenotype. Methods: Monocytes from non-HIV-infected donors were analyzed for CD14, CD16, CD69, TNFα, and CCR5 by flow cytometry pre- and post-stimulation with LPS. In-vitro cultures were then set up to expose non-activated and activated monocytes to R5-, X4-, and dual (R5/X4)-tropic viruses; and the amount of HIV present on the cells was assayed. Results: Non-HIV-infected monocytes, after LPS stimulation, were confirmed to have an activated phenotype with increase in CD16 and CD69 surface expressions (p < 0.05). The activation phenotype was supported by increase in TNFα production, p < 0.05. The activated monocytes had increased surface CCR5 (from 21% to 98%; p = 0.05); and were found to have more R5-tropic virus than non-activated monocytes (p < 0.05). Conclusions: Following activation by LPS, non-HIV-infected monocytes were found to have increase in surface CCR5. These activated monocytes, when exposed to R5-tropic virus, were found to have more virus compared to non-activated monocytes. The significance of the findings could lie in explaining how microbial translocation plays a role in HIV progression; and possibly promoting CCR5-directed strategies in treating HIV. Abstract

HIV-associated dementia (HAD) is not firmly established in patients with circulating recombinant form (CRF) 01_AE HIV-1. In this study, we compared neuropsychological performance among 15 Thai individuals with HAD, 15 Thai individuals without HAD, and 30 HIV-negative control subjects. HIV-1 participants were highly active anti-retroviral therapy naive and matched by age, education, and CD4 count. Neuropsychological testing abnormalities were identified in most cognitive domains among HAD vs HIV-negative participants, confirming the presence of HAD in CRF01_AE.

Abstract

Monocytes and macrophages serve as HIV-1 reservoirs and may indirectly lead to HIV-1-associated dementia via neurotoxic cytokine/chemokine production. It remains unknown if peripheral monocytes and macrophages are responsible for the presence of circulating and cerebral spinal fluid cytokine/chemokine. The purpose of this evaluation was to determine the relationship between inflammatory and chemoattractant cytokine/chemokine in the periphery and the CNS among individuals with HIV-1-associated dementia and normal cognition. To accomplish this, we utilized specimens from the Hawaii Aging with HIV Cohort to assay plasma, cerebral spinal fluid, and cultured peripheral monocyte and macrophage supernatant cytokine/chemokines from individuals with HIV-1-associated dementia and normal cognition by ELISA, relative real-time PCR, and protein macroarrays. To further characterize the activated cells that may be responsible for HIV-1-associated dementia, inverse-PCR was used to identify sites of viral integration. Different mediators of inflammation, and chemoattraction from monocyte and macrophage supernatants, plasma, and cerebral spinal fluid were identified in HIV-1-associated dementia versus normal cognition. The data suggest unique pathways leading to cytokine/chemokine release in the periphery versus the brain region. This may have implications in delineating a cause and effect in HIV-1-associated dementia pathogenesis.
