We conducted a self-administered survey to determine the patient’s sex and whether they underwent cancer screening in 2019, and if not, the reason for the same. The primary outcome measure was the percentage of people who did not undergo cancer screening.Results: A total of 198 responses (108 male respondents) were obtained. Among them, 189 were valid responses (valid response rate 94.5%). One hundred and twenty-nine patients (68.2%, 76 male) had not undergone screening. The most common reasons provided were “I have regular regular clinic visits and do not think they are necessary” (N = 65, 50.3%), “I underwent a gastroscopy within 2 years, a colorectal camera examination within a few years, and a chest radiography within a year” (42.5%), and “I have a separate complete medical checkup” (N = 15, 11.6%). Of the 65 patients who responded that their cancer screenings were unnecessary, 42 patients (64.6%) had not undergone a gastroscopy within 2 years, a colorectal camera examination within a few years, or a chest radiography or examination within a year.Conclusion: Roughly half of the respondents who did not undergo cancer screening elected to abstain because they believed that regular hospital visits were sufficient. Encouraging patients who regularly visit medical institutions to receive cancer screening is therefore necessary.Keywords: cancer screening, internal medicine, non-consultation, rural medicine

Abstract

We report a Keldysh-like model for the electron transition rate in dielectrics under an intense circularly polarized laser. We assume a parabolic two-band system and the Houston function as the time-dependent wave function of the valence and conduction bands. Our formula reproduces the experimental result for the ratio of the excitation rate between linear and circular polarizations for $\alpha$-quartz. This formula can be easily introduced into simulations of nanofabrication using an intense circularly polarized laser.

Abstract

A fully first principles theory capable of treating strongly correlated solids remains the outstanding challenge of modern day materials science. This is exemplified by the transition metal oxides, prototypical Mott insulators, that remain insulating even in the absence of long range magnetic order. Capturing the non-magnetic insulating state of these materials presents a difficult challenge for any modern electronic structure theory. In this paper we demonstrate that reduced density matrix functional theory, in conjunction with the power functional, can successfully treat the non-magnetic insulating state of the transition metal oxides NiO and MnO. We show that the electronic spectrum retains a gap even in the absence of spin order. We further discuss the detailed way in which RDMFT performs for Mott insulators and band insulators, finding that for the latter occupation number minimization alone is required, but for the former full minimization over both occupation numbers and natural orbitals is essential.

Abstract

We theoretically investigate the dynamical Franz-Keldysh effect in femtosecond time resolution, that is, the time-dependent modulation of a dielectric function at around the band gap under an irradiation of an intense laser field. We develop a pump-probe formalism in two distinct approaches: first-principles simulation based on real-time time-dependent density functional theory and analytic consideration of a simple two-band model. We find that, while time-average modulation may be reasonably described by the static Franz-Keldysh theory, a remarkable phase shift is found to appear between the dielectric response and the applied electric field., Comment: 5 pages, 3 figures

Abstract

We calculate the energy deposition by very short laser pulses in SiO_2 (alpha-quartz) with a view to establishing systematics for predicting damage and nanoparticle production. The theoretical framework is time-dependent density functional theory, implemented by the real-time method in a multiscale representation. For the most realistic simulations we employ a meta-GGA Kohn-Sham potential similar to that of Becke and Johnson. We find that the deposited energy in the medium can be accurately modeled as a function of the local electromagnetic pulse fluence. The energy-deposition function can in turn be quite well fitted to the strong-field Keldysh formula for a range of intensities from below the melting threshold to well beyond the ablation threshold. We find reasonable agreement between the damage threshold and the energy required to melt the substrate. The ablation threshold estimated by the energy to convert the substrate to an atomic fluid is higher than the measurement, indicating significance of nonthermal nature of the process. Abstract

We calculate the dielectric response of excited crystalline silicon in electron thermal equilibrium by adiabatic time-dependent density functional theory (TDDFT) to model the response to irradiation by high-intensity laser pulses. The real part of the dielectric function is characterized by the strong negative behavior at low frequencies due to excited electron-hole pairs. The response agrees rather well with the numerical pump-probe calculations which simulate electronic excitations in nonequilibrium phase immediately after the laser pulse irradiation. The thermal response is also compared with the Drude model which includes electron effective mass and collision time as fitting parameters. We find that the extracted effective masses are in the range of 0.22-0.36 and lifetimes are in the range of 1-14 fs depending on the temperature. The short Drude lifetimes show that strong damping is possible in the adiabatic TDDFT, despite the absence of explicit electron-electron collisions., Comment: 19 pages, 10 figures. arXiv admin note: text overlap with arXiv:1303.3249

A recent case–control study with single-photon emission computed tomography (SPECT) using 99mTc-ethyl cysteinate dimer could not find the specific features of regional cerebral blood flow (rCBF), which might be due to the heterogeneity of PBS. We analyzed the brain images of PBS corresponding to the clinical features by studying PBS subgroups.Methods: This study contributes to elucidating the pathophysiology of PBS by evaluating regional brain perfusion on SPECT and its clinical features. We performed SPECT using 99mTc-ethyl cysteinate dimer in 44 patients with PBS. The SPECT images were analyzed qualitatively and quantitatively.Results: Asymmetrical rCBF patterns were detected, corresponding to symptom laterality. Patients with PBS with right-side symptoms showed right-side-predominant rCBF asymmetry in the parietal region and left-side-predominant rCBF asymmetry in the thalamus, and vice versa. Moreover, the analysis of the association between rCBF and patient behaviors revealed that patients who blamed their dentists for their symptoms tended to have a symmetrical rCBF pattern.Conclusion: Patients with PBS showed blood flow imbalance in the thalamus and parietal region corresponding to symptom laterality. 17. Dielectric response of laser-excited silicon

Abstract

We calculate the dielectric response of crystalline silicon following irradiation by a high-intensity laser pulse, modeling the dynamics by time-dependent density functional theory (TDDFT). The pump-probe measurements are numerically simulated by solving the time-dependent Kohn-Sham equation with the pump and probe fields included as external fields. As expected, the excited silicon shows features of a particle-hole plasma in its response. We compare the calculated response with a thermal model and with a simple Drude model. The thermal model requires only a static DFT calculation to prepare electronically excited matter and agrees rather well with the TDDFT for the same particle-hole density. The Drude model with two fitted parameters (electron effective mass and collision time) also shows fair agreement at the lower excitation energies; the fitted effective masses are consistent with carrier-band dispersions. The extracted Drude lifetimes range from 6 fs at weak pumping fields to much lower values at high fields. However, we find that the Drude model does not give a good fit to the imaginary dielectric function at the highest fields. Comparing the thermal model with the Drude, we find that the extracted lifetimes are in the same range, 1-13 fs depending on the temperature. These short Drude lifetimes show that strong damping is possible in the TDDFT, despite the absence of electron scattering. One significant difference between the TDDFT response and the other models is that the response to the probe pulse depends on the polarization of the pump pulse. We also find that the imaginary part of the dielectric function can be negative, particularly for the parallel polarization of pump and probe fields., Comment: 24 pages, 14 figures

19. Perceptual training of English /r/ and /l/ for Japanese adults, adolescents and children

Abstract

Although phoneme perception and production in second-language (L2) learners has been the focus of much research over the past few decades, the learning mechanisms and the factors that influence them are still far from understood. The purpose of this PhD research is to cast some light on the mechanisms underlying the learning of L2 phonemes and the effects of age. Japanese speakers who have problems in perception and production of the English /r/-/l/ contrast participated in three perceptual training studies. The first study examined whether Japanese adults can improve their perception and production with identification and discrimination training. The results demonstrated that both identification and discrimination training methods improved their perception and production of the English /r/-/l/ contrast, but the combination of two different training methods did not have additive effects. The second study investigated how age affects the learning of the English /r/-/l/ contrast at phonetic and phonological levels of perception. The results demonstrated that Japanese adults are disadvantaged in improving their phonetic perception of the English /r/-/l/ contrast due to their relatively fossilised brain plasticity and their developed L1 phonetic units. On the other hand, younger learners are able to improve both phonetic and phonological perception of the contrast. This may be attributed to their greater brain plasticity and less interference from undeveloped L1 phonetic categories. Finally, the third study examined how Japanese speakers improve production of English /r/ and /l/ through perceptual training, and found that perceptual training transferred to production ability in both identifiability and acoustic realisations. The improvement in production seemed to be attributable to perceptual learning. There may be a common underlying ability for perception and production, although the acoustic dimensions which link the two may be different between individuals. The theoretical implications for understanding learning mechanisms and age effects are discussed.

20. Doping induced metal-insulator phase transition in NiO

Abstract

The insulator to metal phase transition in NiO is studied within the framework of reduced density matrix functional theory and density functional theory. We find that the spectral density obtained using reduced density matrix functional theory is in good agreement with experiments both undoped as well as doped NiO. We find that the physical description of the hole-doping induced phase transition \emph{qualitatively differs} depending on whether NiO is calculated within density functional theory or reduced density matrix functional. In the former case the underlying mechanism of the phase transition is identified to be a rigid shift of chemical potential, while in the latter case a redistribution of the spectral weight drives the transition. These latter results are found to be in good agreement with both experiments and previous many-body calculations.

Abstract

The time-dependent density functional theory (TDDFT) is the leading computationally feasible theory to treat excitations by strong electromagnetic fields. Here the theory is applied to coherent optical phonon generation produced by intense laser pulses. We examine the process in the crystalline semimetal antimony (Sb), where nonadiabatic coupling is very important. This material is of particular interest because it exhibits strong phonon coupling and optical phonons of different symmetries can be observed. The TDDFT is able to account for a number of qualitative features of the observed coherent phonons, despite its unsatisfactory performance on reproducing the observed dielectric functions of Sb. A simple dielectric model for nonadiabatic coherent phonon generation is also examined and compared with the TDDFT calculations., Comment: 19 pages, 11 figures. Abstract

We apply the coupled dynamics of time-dependent density functional theory and Maxwell equations to the interaction of intense laser pulses with crystalline silicon. As a function of electromagnetic field intensity, we see several regions in the response. At the lowest intensities, the pulse is reflected and transmitted in accord with the dielectric response, and the characteristics of the energy deposition is consistent with two-photon absorption. The absorption process begins to deviate from that at laser intensities ~ 10^13 W/cm^2, where the energy deposited is of the order of 1 eV per atom. Changes in the reflectivity are seen as a function of intensity. When it passes a threshold of about 3 \times 1012 W/cm2, there is a small decrease. At higher intensities, above 2 \times 10^13 W/cm^2, the reflectivity increases strongly. This behavior can be understood qualitatively in a model treating the excited electron-hole pairs as a plasma., Comment: 27 pages; 11 figures

Abstract

We show that time-dependent density functional theory (TDDFT) is applicable to coherent optical phonon generation by intense laser pulses in solids. The two mechanisms invoked in phenomenological theories, namely impulsively stimulated Raman scattering and displacive excitation, are present in the TDDFT. Taking the example of crystalline Si, we find that the theory reproduces the phenomena observed experimentally: dependence on polarization, strong growth at the direct band gap, and the change of phase from below to above the band gap. We conclude that the TDDFT offers a predictive ab initio framework to treat coherent optical phonon generation., Comment: 5 pages, 5 figures

In this study, we assessed the outcomes of treatment and attempted to identify factors that could account for pain remission in patients with AO.Patients and methods: Data for 165 patients diagnosed with AO from June 2015 to August 2017 were retrospectively reviewed. The patients’ sex, age, duration of pain, and psychiatric history were collected, along with information on pain intensity, depressive status, and catastrophizing scores. Responses at 4 and 16 weeks from the start of treatment were observed. The associations between potentially associated factors and outcome were investigated using Bayesian model averaging.Results: A 30% reduction in pain was reported by 38 patients (46.3%) at 4 weeks and by 54 patients (65.9%) at 16 weeks. It is thought to be a rare disease in psychiatry, but many patients are visiting dental clinics seeking treatment to remove a foreign body. Even though the features of oral cenesthopathy might be different between a psychiatric clinic and a dental clinic, there has been no clinico-statistical study from dentists. In this study, we report a clinico-statistical study of patients with oral cenesthopathy in dentistry. Methods: This is a retrospective chart review of 606 outpatients with oral cenesthopathy in Tokyo Medical and Dental University from April 2010 through to March 2015. Results: A total of 159 male and 447 female patients were included in this study. The mean age was 62.08 years, and female patients were older than male patients. The trigger of the dental treatment and the acute phase of depression at the onset were significantly related (p=0.037). Only 128 patients (36%) had clinically significant improvement after 6 months of pharmacotherapy. No history of psychiatric disorders (odds ratio [OR] 0.479 [95% confidence interval {CI}: 0.262–0.875], p=0.017) and longer duration of illness (>18 months) (OR 2.626 [95% CI: 1.437–4.799], p=0.002) were significant factors for clinical outcomes. Conclusion: Patients with oral cenesthopathy in our clinic were predominantly elderly female patients. Dental treatment in the acute phase of depression might be a risk factor for oral cenesthopathy. Therefore, comprehending the situation of psychiatric disorder and obtaining adequate informed consent might be required to prevent the trouble concerning oral cenesthopathy. 30. Short-term outcomes after EX-PRESS implantation versus trabeculectomy alone in patients with neovascular glaucoma

Yoichiro Shinohara, Hideo Akiyama, Mikiya Magori, Shoji Kishi Department of Ophthalmology, Gunma University Graduate School of Medicine, Maebashi, Gunma, Japan Background: The purpose of this study was to investigate postoperative clinical outcomes in patients with neovascular glaucoma (NVG) who underwent trabeculectomy or EX-PRESS implantation and to identify predictors of successful EX-PRESS implantation. Methods: The study designed as a retrospective observational analysis and was set in a single tertiary center. Eighty-nine patients (89 eyes) with NVG, were treated using trabeculectomy alone (Trab group; n=39) or EX-PRESS implantation (EX group; n=50). All patients with a history of glaucoma surgery were excluded. The main outcome measures were postoperative intraocular pressure (IOP), visual acuity (VA), number of medications, complications, additional therapy, and success rate. Results: The mean follow-up duration was 4.6 months in the Trab group and 4.2 months in the EX group. There was no significant difference in IOP between the groups except at 1-month follow-up (P=0.045). The number of patients with early postoperative hypotony, a shallow anterior chamber, hyphema, or anterior chamber irrigation was significantly lower in the EX group than in the Trab group (P=0.016, 0.008, 0.019, and 0.014, respectively). The other outcomes, ie, VA, number of medications, and success rate, were similar between the two groups. In the EX group, the success rate was significantly lower in 15 patients with a history of vitrectomy compared to that in 35 patients without a history of vitrectomy (P=0.047). Conclusion: Implantation of an EX-PRESS device may be more effective and safer than trabeculectomy alone in patients with NVG. However, a history of vitrectomy may affect the surgical success rate when using EX-PRESS. Keywords: trabeculectomy, EX-PRESS, neovascular glaucoma, surgery, tube shunt
