4. Hip Sway in Patients With Hip Osteoarthritis During One-Leg Standing With a Focus on Time Series Data.

Abstract

This study aimed to investigate the hip sway and the relationship between the center of pressure (CoP) and kinematic parameters regarding the time series scaling component α in patients with hip osteoarthritis (OA) during a one-leg standing task. The scaling exponent α, SD, hip sway maximal acceleration change, and balance performance, which was measured using CoP parameters, were compared between hip OA and control groups during a one-leg standing task. The relationships between balance performance with CoP parameters and kinematic parameters were investigated with the regression analysis. In the hip OA group, the scaling exponent α was smaller in the medial-lateral direction, and the SD and maximal amount of change in hip sway acceleration were larger in the anterior-posterior direction in the hip OA group. In this group, the CoP parameters were significantly associated with α in the medial-lateral direction (negatively) and in the anterior-posterior direction (positively). In the hip OA group, hip sway adaptability in the medial-lateral direction was limited, while the anterior-posterior direction showed greater movement. [ABSTRACT FROM AUTHOR]

Abstract

[目的]本研究は,体幹および下肢の運動連鎖の観点から変形性膝関節症(膝OA)の発症・進行に関与する機能障害を明らかにするために,膝OA患者における椅子からの立ち上がり動作(STS)の運動学的分析を行った。[対象]膝OAと診断された女性17名の膝OA群と膝関節痛を有さない女性16名の対照群とした。[方法]課題動作は座面高が下腿長の高さの椅子からのSTSとした。3次元動作解析システムKinema Tracer(キッセイコムテック社製)を用いて各体節および下肢関節の角度を求めた。[結果]身体重心(COM)前方移動期における各体節の角速度の平均値には有意差が認められなかったが,COM上方移動期における膝関節伸展,足関節底屈の角速度平均値は対照群に比し,膝OA群が有意に小さかった。[結語]膝OA群のSTSにおいて,臀部離床後に体幹前傾で得られた速度を下肢に伝えることができず,適切な膝関節の関節運動および肢節のアライメント保持が難しくなっていることが示唆された。, [Purpose] The purpose of this study was to analyze the kinematics of sit-to-stand motion (STS) in subjects with knee osteoarthritis (knee OA) and to demonstrate the factor of impairment responsible for pathogenesis and progression of knee OA based on discussion from the standpoint of the kinetic chain of the thorax-pelvis-lower extremity. [Subjects] The subjects were 17 patients with knee OA and 16 age-matched asymptomatic controls. [Methods] Subjects performed STS from a chair with a seat adjusted to the height of the subject's lower leg. Kinematic data of the body segment and joint angles were collected using a motion analysis system (Kissei Comtec). [Results] There were no significant differences between joint angular velocity averages of the body segment in forward movement of center of mass (COM), but those of the knee and ankle in upward movement of COM were significantly smaller. [Conclusion] These results suggest that knee OA subjects can't transfer kinetic energy generated in trunk forward tilting to the lower extremity, making it difficult for them to keep appropriate knee movement and alignment.

14. Influence of deflection of wheelchair seat on estimated shear force on the buttocks

Abstract

本研究の目的は,スピードの違いによる動作特性をバイオメカニクス解析することで,連続した起立から歩行までの動作において速く動作するための要因を調査することである.対象は,本研究課題に影響を及ぼすような疾患や障害を有さない8名の高齢男性とした.課題は,椅子から起立し5m離れた場所に設置されたブザーを取りに行くものとした.動作遂行に要する時間を規定し,速い動作と遅い動作を定義した.運動学的データは,6台のカメラとVicon512を用いて収集した.動力学的データはキスラー社製床反力計2台を用いて収集した.その結果,速い動作における一歩目遊脚期踵接地時の床反力前後成分は,遅い動作と比較して有意に高値を示した.立脚側股関節角度と膝関節角度は,一歩目遊脚側足趾離床期と踵接地期において速い動作で有意に屈曲角度が大きかった.身体重心は,遅い動作と比較して速い動作において滑らかに移動した.得られた結果は,起立動作,歩行開始動作を個々に分析しても得られなかったであろう.今後更なる研究により,高齢者や障害を持つ者の転倒を予防し,健康増進に役立つ結果が得られるものと思われる., The purpose of this study was to investigate the kinematic and kinetic characteristics of the two speed phases of a motion and to determine the pertinent factors in the sequence of quickly standing up and initiating gait. Eight elderly male subjects, who had no disease and/or impairment that would affect the task in this study, were recruited. The test task was to stand up from a chair, walk 5 m, and then pick up a telephone on the table. We defined the difference in setting time as faster motion and slower motion. The kinematic data was obtained using a six camera Vicon 512 motion capture system. The kinetic data was obtained by using two Kistler force plates. As a result, the anterior-posterior ground reaction force was larger in the faster motion than in the slower motion at first heel strike. As for the hip and knee angle, each angle was significantly larger for the first toe off and heel strike in the faster motion. The center of gravity of the whole body moved more smoothly in the faster motion than in the slower motion. The moment of the knee joint was 1.7 times larger in the faster motion than in the slower motion. The observed effects could not be obtained by analyzing each motion of standing up and initiating gait individually. The authors believe the results in the current study will significantly help to promote the health status of the elderly and/or physically handicapped and prevent them from falling in activities of daily living.

This study suggested that from the viewpoint of preventing decubitus ulcers in wheelchair users who need to sit in a wheelchair for long periods, it is important to coordinate and re-examine the deflection of the seat and the height of the relative backrest in relation to the top of the spinal kyposis.

35. Effects of Variation in Cushion Thickness on the Sit-to-Stand Motion of Elderly People

Abstract

This study was done to investigate whether cushion thickness affected elderly people during the sit-to-stand motion (STS motion). Fourteen elderly subjects aged 79 or over were recruited from the outpatients at a rehabilitation clinic. Their STS motions were evaluated and analyzed using a digital video camera and analyzing software. As the thickness of the cushion increased, it was difficult to identify the pelvic movement during STS motion, although the trunk was inclined more anteriorily and the load to the knee extensors was greater. Furthermore, three of the fourteen subjects had difficulties performing STS motion when the pelvis was contoured by a thick cushion. These results suggests that if an older person has a knee extensor strength below an appropriate level and/or the cushion thickness is enough large to contour the subject’s pelvis, the risk of falling during STS motion is higher.

Abstract

Purpose: The purposes of this study were to construct a real-time acceleration gait analysis system equipped with software to analyse real-time trunk acceleration during walking and to examine the intra-rater and inter-rater reliabilities of the this system.Methods: This system has been comprised of an accelerometer, an acceleration amplifier, a transmitter, two foot switches, a receiver and a personal computer installed with the real-time acceleration analysis software. The acceleration signals received were analysed using the real-time acceleration analysis software, and gait parameters were calculated. The subjects were 20 healthy individuals and two raters. The intra-rater and inter-rater reliabilities of the measurement results obtained from this system were examined by performing intraclass correlation coefficients (ICC) and Bland–Altman analysis.Results: The intra-rater and inter-rater ICCs ranged from 0.61 to 0.92 in any gait parameters. In the Bland–Altman analysis, neither fixed nor proportional bias was found in any of the gait parameters.Conclusions: From the ICC and Bland–Altman analysis results, the gait measurement using this system clearly demonstrates that the intra-rater and inter-rater measurements had good reproducibility. Owing to this system, we can improve the clinical efficiency of gait analysis and gait training for physiotherapy.Implication for RehabilitationThis study focused on the advantage of a gait analysis method using an accelerometer and constructed a gait analysis system that calculates real-time gait parameters from trunk acceleration measurements during walking. The gait analysis using this system has good intra-rater and inter-rater reliabilities, and using this system can improve the clinical efficiency of gait analysis and gait training.
