Thirty patients suffering from complicated urinary tract infections with underlying disease were treated with pipemidic acid, (PPA) at oral doses of 1.5 g/day for 14 days, and following results were obtained. 1. Urinary organisms isolated from the patient were Pseudomonas 10 strains, E. coli 6 strains, Klebsiella 3 strains, Enterococcus 2 strains, Proteus vulgaris 1 strain and Staphylococcus epidermidis 1 strain. Two patients had mixed infections. 2. The overall efficacy rate was 50%. 3. As for side effect, anorexia was observed in 2 patients and epigastralgia, sense of abdominal dilatation in I patient, respectively.

Pivmecillinam, a new semisynthetic penicillin antibiotic was administered orally 50 mg three times a day to patients with acute simple cystitis. The results obtained were as follows: 1) The clinical efficacy of pivmecillinam in 132 patients included excellent cases (72.7%), 30 moderate cases (23.5%) and 6 poor cases (4.5%), the overall effective rate being 95.5%. 2) Two patients of 148 receiving pivmecillinam exhibited side effects: one with headache and nausea, and one with slight increase of GOT. 3) Clinical usefulness of pivmecillinam in 136 patients was evaluated as useful in 124 cases (91.2%) by attending doctors.

A case of sarcoma botryoides of the bladder in infant is reported. A 4-year and 11-month-old girl visited our hospital on June 15, 1973, with macroscopic hematuria, high fever and dysuria as chief complaints. They were observed about one month before admission. Total cystectomy combined with ileal conduit and chemotherapy was performed, and she was discharged 38 days later. She has still good conditions without evidence of recurrence or metastasis 7 months after operation. Review of the cases reported in Japan and some discussions concerning sarcoma botryoides of the bladder were made.

症例1， 74歳.初診， 1975年3月11日.主訴，外尿道口部の尽痛。外尿道口部に外観尿道小阜を思わせるアズキ大の腫瘤を認めた。電気切除された腫瘤は，組織学的にはadenocarcinomaであった。さらに尿道全周にわたりTUR，TUECを行なった。 1979年7月他施設にて死亡した。死因，その他は不明であった。 症例2，63歳。初診， 1980年5月14日。主訴，外尿道口部の腫瘤。尿道全摘術+膀胱痩造設術を施行。組織学的にはsquamous cell carcinomaであった。術後， 下腹部と両ソケイ部に4000radのコバルト照射を行なった。 1980年11月末現在，再発，転移の徴候は認めていない。1975年より6年間に経験した2例の原発性尿道癌を, 若干の文献的考察を加えて報告した。, Primary carcinoma of the female urethra is uncommon. 尿管結石症患者180例を対象として, HSR-902(チアトン(R))の鎮痛効果臭化ブチルスコポラミンを対照薬剤として二重盲検法により検討した.HSR-902の疼痛改善度は77例中, 著明改善22例, 中等度改善21例であった.対照薬剤は84例中, 著明改善12例, 中等度改善31例であった.HSR-902は疼痛改善度や全般改善度で対照薬剤との間に有意差はなかったが, 有用度判定では対照薬剤に比して有意差を認め(P, The clinical efficacy and safety of HSR-902 (tiquidium bromide, Thiaton) in patients with spastic pain caused by ureteral stones were evaluated in a double blind comparative trial. A daily dose of 30 mg HSR-902 or 60 mg butylscopolamine bromide as a control drug was orally administered for 7 days. With either drug marked improvement of spastic pain was observed. The time to obtain relief from spastic pain and utility rating were significantly more excellent with HSR-902 than with butylscopolamine bromide. With all other parameters used no significant difference was observed between the two drugs. Mild adverse effects such as abdominal discomfort and constipation were observed in 2 of the 87 cases in the butylscopolamine bromide group but none of the 83 cases in the HSR-902 group. Taking efficacy and safety of the treatment into consideration, no significant difference was observed in usefulness between the two drugs, and we were able to confirm the usefulness of HSR-902 for relief from spastic pain caused by ureteral stones.

Three cases of metastatic ureteral tumor were reported. Case 1: A 71-year-old man was admitted on November 16, 1973 with a chief complaint of gross hematuria of 2 weeks duration. Retrograde pyelograms showed the characteristic filling defect of a neoplasm in the left lower ureter. Cystoscopy revealed a red bean sized papillary tumor on the right lateral wall. Left nephroureterectomy, left adrenalectomy and partial cystectomy were performed. The final diagnosis was ureteral and adrenal metastases from renal adenocarcinoma and transitional cell carcinoma (grade 1) of the bladder. Case 2: A 67-year-old man was admitted on August 9, 1973 complaining of hematuria and pollakisuria of 8 months duration. IVP showed bilateral non-visualizing kidneys and cystoscopy could not be done because of increased resistance of the posterior urethra. The blood urea nitrogen was 110 mg per cent and the creatinine was 14.8. Bilateral nephrostomy was performed. The final diagnosis was ureteral metastasis from mucinous forming adenocarcinoma of the prostate. Case 3: A 52-year-old woman treated with radical operation for carcinoma of the cervix at another hospital in 1966, was admitted on April 4, 1973 with a chief complaint of hem ateuia of 2 weeks duration. Retrograde pyelograms revealed the stricture of the right upper ureter. Right ureteroanastomosis and nephrostomy were performed. The final diagnosis was ureteral metastasis from epidermoid carcinoma of the cervix. Review of literatures and some discussion on metastatic ureteral tumor were done.
