Skin diseases differ in different countries, and within various regions of a country depending on social, economic, racial and environmental factors. Many patients have reported various patterns of skin diseases in different countries. It was a descriptive type of cross sectional study conducted at outpatient department (OPD) of Dhamrai upazilla health complex to know the pattern of skin diseases during the period of June 2018 to December 2018. Total 2000 patients at any age and sex who attended in the OPD of the hospital with different types of skin disorders/diseases were selected as study population. Among the patients 27.05% were less than 10 years of age and 56.04% were female, 45.05% patients were illiterate, 57.03% had monthly family oncome more than Tk 15000 and 40.89% were housewife, 20.01% were students. The diagnosis was made on clinical basis as an expert dermatologist. Lab. investigations were restricted to the cases where they carried diagnostic importance. Majority (57%) were suffering from some infectious among them 33.02% had parasitic infection, 28.3% suffering from fungal infection, 20.1% bacterial infection and 43% from some non-infectious skin diseases among them 37.79% had eczema, 17.87% had papulosquamous disease, 14.25% had acne, 14.01% had urticaria, 8.7% had vitiligo. Infectious skin diseases were the most common skin disease seen in our study, followed by dermatological infections. The emerging challenges for dermatologists are to prevent and reduce these skin diseases.

This cross sectional study of fifteen days’ duration was conducted among Married women attending in OPD at Dhamrai Dhaka to know Reproductive Health Practice of Married Women in the Rural Community with a sample size of 100 following purposive sampling technique using pretested semi-structured questionnaire by face to face interview method after taking informed consent. Descriptive type of Cross sectional study conducted in Jaintapur Upazila from the period of October-December 2017 to assess knowledge, attitude and practice of personal hygiene among selected rural people with a sample size of 308 using a semi-structured questionnaire following purposive sampling technique. Out of 308 respondents about three fourth of respondents were Muslims, 55.8% were male. More than half were in the age group 15-35 years, 85.7% were married, 44.5% were Illiterates. 30.4% were house wife and only 6% had their monthly income ≥Tk. 30,000. Almost 286(92.8%) respondents had knowledge on importance of hand washing, majority 244(79.2%) had knowledge about transmission of Communicable disease by Hand contamination. Regarding factors causes discouraging from hand washing about 123(39.9%) respondents forget to wash, 74(24%) feel Shortage of time and 70(22.8%) not known. Only 13% of respondents wore ring during cooking and food handling. Almost 290(94.2%) washed hand before eating, 298(96.8%) washed their hand after coming back from the toilet and 301(97.7%) respondents had habit of Cutting Nail. Majority of them 234 (78.5%) used soap for hand washing after coming back from latrine, only 1(0.3%) used liquid hand wash and others respectively. About (84.1%) discussed about hand washing with their children and family members and 238(77.3%) respondent’s children’s habit of hand washing of children before taking food & after toilet. More than half of respondents got information from Radio and Television, 79(25.6%) from health educator, 17(5.5%) from others. Awareness of general people about hand washing and its practices should be increased within earliest possible time through behavior Change Communication (BCC) programme of GO/NGO partnership should be planned with more emphasis on hand washing.

The current descriptive type of cross sectional study was conducted during 16th May to 31st May 2018 to assess awareness regarding personal hygiene and sanitation practices in Dhamrai, Dhaka with a sample size of 120 using interviewer administered semi-structured questionnaire employing convenience sampling technique. More than one third (35.9%) of the respondents were in age group 25-35 years and about 65% were female, 32.5% were housewives and 35.8% were found illiterate. source of drinking water was from71.67% tube well and 78.3% used sanitary latrine. More than half of the respondents (61.67%) were taking daily bath with soap and water, 61.66% cut their nails at leisure time and 51.67% wash hair with shampoo and water; while 59.67% washed their hands before meal and 76.67% after defecation with soap and water. It is revealed that 43.3% were aware about the transmission of diarrheoa through dirty nail and 73.3% were aware not to defecate on barefoot. Health education and comprehensive knowledge of proper personal hygiene and sanitation is essential in daily life and should be used to prevent the spread of infectious diseases.

Background: Occupational stress is one of the key factors in reducing staff productivity in any organization with physical and psychological impacts on employees. Nursing is among the most stressful professions. Therefore, this study aimed to assess the extent of perceived level occupational stress and its association with work-related and non-work related variables as causes of stress and work ability among nurses in a Public Medical College Hospital. Methods: A cross sectional study was done among nurses in Shaheed Suhrawardy Medical College Hospital (ShSMCH) dmi.ng July 2015-June 2016 among 197 nurses, selected through a simple random sampling. Face to face interview was conducted, using a pretested semi-structured questionnaire. Occupational stress and work ability were measured using Expanded Nurses Stress Scale Questionnaire (ENSS) and WorkAbility Index Questionnaire (WAIQ). Appropriate statistical analysis was performed to see the level of significance. Results: Majority (69.0%) of the study population were between the age of 21 to 39 years, 94.gG/o were female, 68.0% were married, 79.2% were Muslim, 84.3% had diploma, 91.4% fumily income were equal or more than Tk 40,000, 95.5% did clinical work. Some 25.0% worked in medicine ward, 22.8% worked at surgery ward and 90.9% worked more or less than 50 hours per week. Only 5.1% found to be high stress and 94.6% low stress. This descriptive type of cross- sectional study was conducted in Dhamrai, Dhaka among nursing mother with a sample size 120 using semi-structured questionnaire to determine the knowledge and practice regarding exclusive breast feeding irrespective of socio-demographic characteristics from January 2018 to February 2018 employing purposive sampling method. The percentage of mother-infant pair were highest within 20-30 years (65%) of age group. About 13.33% of mothers were illiterate, 33.33% had primary level and only 2.50% completed graduation; more than half (56.67%) of the mothers were housewives. It was revealed that 55.83% of the respondents had knowledge of exclusive breast feeding, however 42.50% of them gave only breast milk up to 6 months. The initiation of breast feeding within 1 hour was 47.50%, 32.50% within 1-2 hours. 8.83% after 2 hours, 9.16% of the respondents did not remember about the exact time and 2.50% did not breast fed their babies, the reason of not feeding breast milk was insufficient milk secretion. Premature complementary feeding was associated with an increased occurrence of vomiting (47.50%), diarrhea (59.16%), dyspnea (19.16%) but 55% of the respondents did not mentioned specific condition. One of the ten steps to successful breastfeeding is initiating breastfeeding within the first hour of delivery, the major barrier to achieving the recommendations of the Global Strategy for Infant and Young Child Feeding.

Background: Bangladesh had an official Family Planning (FP) programme beginning in 1960. Bangladesh has experienced a sevenfold increase in its Contraceptive Prevalence Rate (CPR) in less than forty years from 8% in 1975 to 62% in 2014. However, despite this progress, almost one-third of pregnancies are still unintended which may be attributed to unmet need for family planning and discontinuation and switching of methods after initiation of their use. To find out the acceptance of contraceptive devices among female in rural area of Bangladesh and its relation with their educational level. Methods : A cross sectional descriptive study of fifteen days’ duration was conducted among female in rural area of Bangladesh. Participants not willing to respond and pregnant women were excluded. Total of 120 women were selected as study participants. Every woman was interviewed face to face with pre-tested questionnaire after taking informed consent. Results: About 46.92% were age group 26 to 35 years and 11.11% were age group more than 36 years of age, 83.96% were Muslim and 14.81% were Hindu. Maximum (83.96%) were married. About 37.04% were upto class V and 1.23% were SSC passed. About 27.16% were class VI to X. About 60.49% duration of married life 1 to 10 years and 1.23% married life 30 to 40 years. 86.42% were use contraceptive device, among them 34.43% were use condom and 3.28% were used IUD. About 43.33% respondents suffer from irregular menstruation and 25% from menorrhagia, 20.84% weight gain. About 46.66% respondents had 2 children and 4.16% had no child. Conclusion: A renewed commitment from government bodies and independent organizations is needed to implement and monitor family planning strategies in order to ensure the adherence to and provision of the most appropriate contraceptive method for couples. Background: Now a days, there is increased demand and progress in the nursing profession. Along with these stress among the nurses has also increased.Objective: Purpose of the study was to assess the extent of perceived occupational stress and its association with work-related and non-work related causes among nurses.Materials and method: This Cross-sectional study was carried out among 197 nurses of Shaheed Suhrawardy Medical College hospital (ShSMCH), Dhaka, Bangladesh, from the period of 1st July 2015 to 30th June 2016. Sampling method was simple random sampling. Data was collected by face to face interview with the help of semi-structured questionnaire and finally all data were analyzed using SPSS software version 21.Results: It was found that 69% nurses were between the age of 21 to 39 years, 94.9% were female, 68% were married, 79.2% were Muslim, 84.3% had diploma, family income of 91.4% were equal to more than 40,000 Tk. and 95.5% used to do clinical work. Only 5.1% found to experience high stress. Statistically significant difference was found between sex, occupational stress and religion, educational status, and type of work (p < 0.05). Few nurses suffered from high stress but high level of stress were more in male, non Muslim, post graduate nurse and those who were engaged in administrative work.Conclusion: Nurse Managers should take appropriate actions to decrease stress helping their nurses to work efficiently and effectively.Delta Med Col J. Perception on breast cancer influences the help seeking pattern of breast cancer patients. Negative perception causes delay in help seeking. Outcome or survival rate of breast cancer has a relation on perception. A cross sectional study was done during July 2015-June 2016, to assess the perception on breast cancer and to find out the help seeking pattern and extent of help seeking time among breast cancer patients. A total of 200 patients were selected following the convenient method of sampling from National Institute of Cancer Research and Hospital. Dhaka. Face to face interviews were taken from the diagnosed breast cancer patients admitted in the hospital. Forty five percent patients were illiterate and 83% were house wife. The mean age was 42 and mean monthly family income was around 9000 taka. Two thirds of the respondents were present with locally advance stage (stage III) and three fourth of the patients identified the lump as 1st symptom. Four of 10 patients first sought help to homeopathy for their cancer diagnosis and treatment. Mean help seeking time was 11 months. Nine out of 10 patients were agreed with the statement, ‘I was healthy’. Maximum respondents (81.0%) agreed with the statement, ‘wait and see what would be happened’. Results showed that there was a relationship between delay help seeking and the statement of perception. There were significant associations between the perception on expensive treatment, perception on ‘I am healthy’, perception on ‘lump related with menstruation’, perception on ‘wait and see what would be happened’, perception on ‘feared to do operation, it may cause death’ and perception on family income and the delay in help seeking. Along with perception on breast cancer use of alternative medicine, lack of social support, health care delivery system, economic factor etc. influence the pattern and delay in help seeking. This was a cross sectional descriptive study conducted to assess Health Status of Waste pickers collecting waste from private waste bins or dumpsters along streets & some landfill sites of Dhaka City Corporation using a semistructured questionnaire employing purposive sampling technique with a sample size of 200 by face to face interview from January to December 2018. Most of the respondents (45%) belonged to age group within 20 to 30 years of age and 55% were male. About 45% of respondents completed primary education and half of the respondents got married and three fourth of respondents absolutely depended on waste pickings and only 28% have other sources of income where 20% had monthly income less than 3000 taka. Three fourth of respondents lived in hut in a slum area and used kacha latrine. Almost all were used supply water for consumption and More than half of the respondents (119, 59.4%) suffered from injury. There is therefore a need to facilitate improvement in their working conditions and raise awareness on their health status.

Emergency obstetric hysterectomy is removal of part or whole of the uterus done to save the life of a mother in complications in the intra partum or post-partum period. This study was done to assess the cases of emergency obstetric hysterectomy (EOH) in view to find out the indications, the patient profile, the risk factors, the morbidity and mortality associated with the procedure. It was a retrospective analysis of 40 cases of obstetric hysterectomy performed in Faridpur Medical College Hospital (FMCH) during the period of August 2012 to July 2014. Patients were evaluated from history and clinical observation records regarding maternal age, parity, indication of operation, associated risk factors, surgical procedure and complications during hospital stay. In maximum patient with EOH the overall incidence is in the age group of 25-29 years (42.5%) and parity of 2nd-3rd. The most common complication was ruptured uterus 50% and PPH 12.5%. Among the risk factors the most common was previous caesarean section (55%) then grand multipara (15%). Out of 40 cases 40% needed total hysterectomy and 60% was managed by subtotal hysterectomy. In most cases complications were hemorrhage and 100% patient needed blood transfusion among which 50% in the post-operative period. All the patients were discharged after treatment with no history of maternal death or referral to higher centers. Emergency obstetric hysterectomy is a lifesaving procedure when decision is taken at proper time, done rapidly by expert surgeon along with availability of blood and safe anesthesia. Placenta previa is a life threatening pregnancy complication where placenta partially or completely covers the internal cervical os causing serious adverse consequence for both mother and baby. History of cesarean delivery is an important risk factor for placenta previa. Which have a increased chance of cesarean hysterectomy and bladder injury because of an associated accrete syndrome. The objective of this study is to analyze the outcome of placenta previa with history of cesarean section. This is a prospective study done in Faridpur Medical College Hospital, Bangladesh from 01.01.2013 up to 31.12.14. Total 18 (study group) patients had undergone cesarean delivery with placenta previa and history of previous cesarean section. Among 18 patients maternal age range was 25- 40, parity 1-4, emergency hysterectomy was done in 11, average blood transfusion 2-8 and other morbidity like urinary bladder injury was 4. Background: Taking protective equipment is important for poultry workers as they are at risk of poultry hazard. Use of personal protective equipment (PPE) is essential for preventing & controlling diseases experienced by poultry processing workers.Aims and Objectives: To assess the awareness about personal protective equipments and health problems among poultry workers.Method and materials: Descriptive cross-sectional study was conducted under the guidance of the Department of Community Medicine, National Institute of Preventive and Social Medicine (NIPSOM), Dhaka, from March to June 2006 on 100 poultry workers who were selected purposively from six poultry farms from the area of Gazipur districts. Informed consent was obtained and structured questionnaire was used to collect data through face to face interview. Data was edited, processed and analyzed both manually and used of SPSS version 10.Result: Mean age was 27.49% with SD ± 9.84 years, highest age range 59% was 20-30 years. Regarding level of education & economic status revealed that 43% were illiterate and 72% earned 2000-3000 taka monthly. 20% had highest work experience of 6-8 years. 82% of the respondents thought that personal protection is necessary during working in poultry. 79% of the respondents knew the name of PPE. Preeclampsia is a pregnancy specific, multi-system disorder of unknown etiology characterized by new onset of elevated blood pressure & proteinuria after 20 weeks of gestation. Globally preeclampsia and other hypertensive disorders of pregnancy are a leading cause of maternal and infant illness and death. But the exact pathophysiology is yet to be explored. It was a case control study and was conducted during the period of January 2010 - December 2010 in the department of Obs & Gynae DMCH and dept. of Biochemistry of BSMMU. The main objective of the study was to evaluate the association of serum ferritin and iron in preeclampsia & eclampsia. A total 100 pregnant women were included in this study. Of them 50 preeclamptic or eclamptic, nonanaemic patients not in labour (26-40weeks) were taken as case and 50 normotensive pregnant women were taken as control. Mean Serum ferritin level in case and control group was 100.03 ± 123.52 ?gm/L and 31.53 ± 20.86 ?gm/L respectively which is highly significant (P< 0.001). Out of 50 cases ferritin level was raised in 10 cases (20%). In 80% cases ferritin level was below the cut-off value that is normal or below normal but in 100% of controls had ferritin level below the cut off value. Removal of uterine myoma during caesarean section (CS) is not commonly done due to fear of severe haemorrhage which may lead to hysterectomy. Contradicting the previous belief many studies have been showing that myomectomy during caesarean section is a safe procedure without significant increase of risk. The objective of this study is to analyze the safety and clinical outcome of caesarean myomectomy. This is a prospective study carried out in different hospitals of Faridpur district, Bangladesh from 01.01.10 up to 31.12.12. Total 16 (study group) patients had undergone caesarean myomectomy. Operation time, peroperative blood loss, post operative complications and length of hospital staying was compared with that of 32 women (control group) with caesarean section alone. The result shows that peroperative blood loss was average 350±100 ml in study group which is 50 ml more than control group. Operation time was 20 minutes more and length of hospital staying was 1 day more than control group. There was no post partum haemorrhage, no hysterectomy was done at the time of CS and there was no maternal or perinatal mortality. Objective: Aim of the study is to identify changes that occur in renal function in eclampsia patients and to determine the effect of impaired renal function on pregnancy outcome Materials and Methods: This cross-sectional study on the impairment of renal function in eclamptic patients was carried out in the Eclampsia ward of Dhaka Medical College Hospital from July 2009 to December 2009. Study group comprised of 50 diagnosed cases of eclampsia and 50 patients with uncomplicated pregnancy was taken as control. Results: In this study 30% of the eclamptic patients were found to have impairment of renal function. Incidence of impaired renal function was significantly increased among the patients > 25 years of age. Multigravida eclamptic patients were found to be more prone to develop impaired renal function compared to primigravida patients. Blood pressure was significantly increased and platelet count was significantly low in the impaired renal function group of eclamptic patients compared to the normal renal function group. Incidence of foetal complications was significantly higher among the patients with impaired renal function. After abdominal surgery some patient's needs relaparotomy for complications developed postoperatively. Aim of this study was to determine the risk factors causing re-laparotomy, the indications, management and outcomes of relaparotomy admitting in Obstetrics and Gynaecology department in Faridpur Medical College Hospital, a tertiary referral hospital where all complicated patients are referred for management from surrounding districts hospital. It was a cross sectional study done during the period of January 2011 to December 2011, All the cases among which relaparotomy were done after primary surgery were recorded using a protocol prepared for the study. Data was analyzed. Total 6304 patients were admitted in emergency way in Obstetrics and Gynaecology department of Faridpur Medical College Hospital. Among them 1864 patients undergone emergency operations, here 15 patients needed re-laparotomy. The incidence of re-laparotomy was 0.80%. Indications of relaparotomy included internal hemorrhage, postpartum hemorrhage (PPH), retained foreign body, incorrect diagnosis at first laparotomy, intraabdominal collection of pus and urine, Rectus sheath haematoma and burst abdomen. Often more than one procedure was needed to manage the cases. Re-laparotomy causes much morbidity & mortality with increase in hospital stay and cost. Careful selection of cases for primary operation, expertise of the surgeon, good surgical technique and careful postoperative follow-up can reduce the need for re-laparotomy. Fuzzy systems generally work based on expert knowledge base. Fuzzy expert knowledge base incorporates human knowledge through fuzzy rules and fuzzy membership functions. In designing fuzzy models, a major difficulty in the identification of an optimized fuzzy rules as well as membership function shape and type of an individual rule. In fact, it is difficult and time consuming for an expert to define a complete rule set for a complex system having a large number of parameters. In this paper, we proposed a flexible encoding method for evolutionary algorithm to discover parameters of fuzzy rule, design of a suitable fuzzy models and controller for a particular environment. In Evolutionary Fuzzy System (EFS), evolutionary algorithms are adapted for finding the optimal fuzzy rule sets including the number of rules inside it and selecting the membership function shape and type of each individual rule in two different ways respectively. The benefits of this methodology are illustrated for the modeling and control of nonlinear system that shows better performance than existing fuzzy expert systems.
